Knee arthroscopy is the most common orthopedic procedure worldwide. While incidence of post-arthroscopy venous thromboembolic events (VTE) is low, treatment patterns and patient outcomes have not been described. Patients from the "Registro Informatizado Enfermedad TromboEmbolica" who had confirmed post-arthroscopy VTE were compared to patients with provoked, post bone-fracture, and to patients with unprovoked VTE. Baseline characteristics, presenting signs and symptoms, treatment and outcomes including recurrent VTE, bleeds or death were compared. The eHealth Eurocampus (ehealtheurocampus.eu) is an EU-funded Erasmus+ Strategic Partnership (2016-2019). It aims at preparing professionals able to cope with the challenge of “fostering a spirit of innovation in eHealth in Europe as the way forward to ensure better health and better and safer care for EU citizens, a more skilled workforce, more efficient and sustainable health and care systems, new business opportunities” (EC eHealth Action Plan 2012-2020). This project has a truly European perspective, developing teaching materials in collaboration between universities and testing them on a highly diverse set of students. Partners: 8 higher education institutions, which represent 7 technical schools (Barcelona School of Informatics/ Polytechnic School, Mallorca/ Polytech Engineer School, Montpellier/ ISIS Engineering School, Castres/ Glyndwr University, Wales/ Hochschule Ulm/ Computer Science Department, Cyprus University) and 3 medical schools (Faculty of Nursing and Physiotherapy, Mallorca/ Medical School of Cyprus/ Sant Joan de Déu University School of Nursing, Barcelona), a regional centre of technological development and entrepreneurship promotion (Bit Foundation, Mallorca), and a European Grouping of Territorial Cooperation (EGTC Pyrenees-Mediterranean). The consortium represents different health management systems. This variety will help to form adaptable and international professionals, aware of the European diversity in this area. Main objectives: • Improving the RELEVANCE of higher education in the field of information and communication technologies (ICT) applications for health (eHealth) • Enhancing the QUALITY of eHealth higher education through mobility and transnational cooperation • Fostering EMPLOYABILITY through curricula adaptation to labour market needs • Providing skills of [eHealth] ENTREPRENEURSHIP for students. Included actions: • The development of materials for 5 eHealth subjects: Innovation and Entrepreneurship (finished), IT for a Longer Independent Life (quite finished), Applications and Tools (quite finished), Robotics for Healthcare (on-going), and Graphics and Medical Imaging (on-going). • Summer Schools: Entrepreneurship (Castres, 2017), Longer Independent Life (Wrexham, 2018), Robotics and Imaging (Barcelona, 2019). Participants are students from the partner institutions, mixing engineers with health students. Summer Schools are used as a testbed of materials and innovative teaching methodologies. • Training Seminars: eHealth Teaching Challenges (Montpellier, 2017), Accessibility, Inclusion, Rehabilitation (Palma, 2018). For teachers and researchers from the consortium, exchanging best practices and knowledge. • Final conference: a public general presentation of the project results (Barcelona, 2018) • Digital platform to facilitate the distribution of the project outputs. The eHealth Eurocampus is a project in which, for the first time, the use of ICT in healthcare is dealt from very different perspectives and disciplines: education, entrepreneurship, assistive healthcare medicine, medical robotics, medical imaging, privacy and data security, ethics... One of its main goals is to create an original eHealth teaching-learning space where university professors, students, researchers, and clinicians will be able to share their experiences and knowledge with the aim of improving the skills and employability of graduates. - Abstract
Background: The event of X chromosome inactivation induced by XIST, which is physiologically observed in females, is retained in testicular germ cell tumors (TGCTs), as a result of a supernumerary X chromosome constitution. X chromosome inactivation also occurs in male germline, specifically during spermatogenesis. We aimed to analyze the promoter methylation status of XIST in a series of TGCT tissues, representative cell lines, and testicular parenchyma. Methods: Two independent cohorts were included, comprising a total of 413 TGCT samples, four (T)GCT cell lines, and 86 testicular parenchyma samples. The relative amount of methylated and demethylated XIST promoter fragments was assessed by quantitative methylation-specific PCR (qMSP) and more sensitive high-resolution melting (HRM) methylation analyses. Results: Seminomas showed a lower amount of methylated XIST fragments as compared to non-seminomas or normal testis (p < 0.0001), allowing for a good discrimination among these groups (area under the curve 0.83 and 0.81, respectively). Seminomas showed a significantly higher content of demethylated XIST as compared to non-seminomas. The percentage of demethylated XIST fragment in cell lines reflected their chromosomal constitution (number of extra X chromosomes). A novel and strong positive correlation between the Johnsen’s score and XIST demethylation was identified (r = 0.75, p < 0.0001). Conclusions: The X chromosome inactivation event and demethylated XIST promoter are promising biomarkers for TGCTs and for assessing spermatogenesis quality.

- Abstract
Current (high throughput omics-based) data support the model that human (malignant) germ cell tumors are not initiated by somatic mutations, but, instead through a defined locked epigenetic status, representative of their cell of origin. This elegantly explains the role of both genetic susceptibility as well as environmental factors in the pathogenesis, referred to as ‘genvironment’. Moreover, it could also explain various epidemiological findings, including the rising incidence of this type of cancer in Western societies. In addition, it allows for identification of clinically relevant and informative biomarkers both for diagnosis and follow-up of individual patients. The current status of these findings will be discussed, including the use of high throughput DNA methylation profiling for determination of differentially methylated regions (DMRs) as well as chromosomal copy number variation (CNV). - Abstract
Background Limited data exist about the clinical presentation, ideal therapy and outcomes of patients with hereditary hemorrhagic telangiectasia (HHT) who develop venous thromboembolism (VTE). Methods We used the data in the RIETE Registry to assess the clinical characteristics, therapeutic approaches and clinical outcomes during the course of anticoagulant therapy in patients with HHT according to initial presentation as pulmonary embolism (PE) or deep venous thrombosis (DVT). Results Of 51,375 patients with acute VTE enrolled in RIETE from February 2009 to January 2019, 23 (0.04%) had HHT: 14 (61%) initially presented with PE and 9 (39%) with DVT alone. Almost half (47.8%) of the patients with VTE had a risk factor for VTE. Most PE and DVT patients received low-molecular-weight heparin for initial (71 and 100%, respectively) and long-term therapy (54 and 67%, respectively). During anticoagulation for VTE, the rate of bleeding events (major 2, non-major 6) far outweighed the rate of VTE recurrences (recurrent DVT 1): 50.1 bleeds per 100 patient-years (95%CI: 21.6-98.7) vs. 6.26 recurrences (95%CI: 0.31-30.9; p = 0.020). One major and three non-major bleeding were epistaxis. No patient died of bleeding. One patient died shortly after being diagnosed with acute PE. Conclusions During anticoagulation for VTE in HHT patients, there were more bleeding events than VTE recurrences. Most bleeding episodes were non-major epistaxis.

BackgroundCurrent guidelines suggest treating cancer patients with incidental pulmonary embolism (PE) similarly to those with clinically suspected and confirmed PE. However, the natural history of these presentations has not been thoroughly compared.MethodsWe used the data from the RIETE (Registro Informatizado de Enfermedad TromboEmbólica) registry to compare the 3-month outcomes in patients with active cancer and incidental PE versus those with clinically suspected and confirmed PE. The primary outcome was 90-day all-cause mortality. Secondary outcomes were PE-related mortality, symptomatic PE recurrences and major bleeding.ResultsFrom July 2012 to January 2019, 946 cancer patients with incidental asymptomatic PE and 2274 with clinically suspected and confirmed PE were enrolled. Most patients (95% versus 90%) received low-molecular-weight heparin therapy. During the first 90 days, 598 patients died, including 42 from PE. Patients with incidental PE had a lower all-cause mortality rate than those with suspected and confirmed PE (11% versus 22%; OR 0.43, 95% CI 0.34–0.54). Results were consistent for PE-related mortality (0.3% versus 1.7%; OR 0.18, 95% CI 0.06–0.59). Multivariable analysis confirmed that patients with incidental PE were at lower risk of death (adjusted OR 0.43, 95% CI 0.34–0.56). Overall, 29 (0.9%) patients developed symptomatic PE recurrences, and 122 (3.8%) had major bleeding. There were no significant differences in PE recurrences (OR 0.62, 95% CI 0.25–1.54) or major bleeding (OR 0.78, 95% CI 0.51–1.18).ConclusionsCancer patients with incidental PE had a lower mortality rate than those with clinically suspected and confirmed PE. Further studies are required to validate these findings, and to explore optimal management strategies in these patients.

We report on the performance of the capacitive gap-sensing system of the Gravitational Reference Sensor on board the LISA Pathfinder spacecraft. From in-flight measurements, the system has demonstrated a performance, down to 1 mHz, that is ranging between 0.7 and \ud 1.8\ud \ud \ud aF\ud \ud Hz\ud −\ud 1\ud /\ud 2\ud . That translates into a sensing noise of the test mass motion within 1.2 and \ud 2.4\ud \ud nm\ud \ud Hz\ud −\ud 1\ud /\ud 2\ud in displacement and within 83 and \ud 170\ud \ud nrad\ud \ud Hz\ud −\ud 1\ud /\ud 2\ud in rotation. This matches the performance goals for LISA Pathfinder, and it allows the successful implementation of the gravitational waves observatory LISA. A \ud 1\ud /\ud f\ud tail has been observed for frequencies below 1 mHz, the tail has been investigated in detail with dedicated in-flight measurements, and a model is presented in the paper. A projection of such noise to frequencies below 0.1 mHz shows that an improvement of performance at those frequencies is desirable for the next generation of gravitational reference sensors for space-borne gravitational waves observation.

International audience; INTRODUCTION:Patients with arterial disease receiving antiplatelet agents may develop venous thromboembolism (VTE) and need anticoagulant therapy, although concomitant use of these drugs may increase bleeding risk. We analyzed RIETE data and compared clinical outcomes depending on decision to discontinue or maintain antiplatelet therapy at VTE diagnosis.METHODS:Consecutive patients with acute VTE were enrolled in RIETE. Only patients receiving antiplatelet therapy at baseline were included in this analysis. Primary outcomes were: rate of subsequent ischemic events, major bleeding or death during anticoagulation course.RESULTS:1178 patients who received antiplatelet drugs at VTE diagnosis were included. Antiplatelet therapy was discontinued in 62% of patients. Thyroid carcinoma is the most common endocrine neoplasia. Differentiated thyroid carcinomas (DTCs) represent the majority of cases, which usually follow an indolent clinical course with low mortality rates. The authors describe two cases of well DTC without classic histological poor prognosis features, presenting as extensive and unresectable osteolytic bone metastases. DTCs are considered harmless tumours, due to their benign and silent behaviour. The authors want to underline the importance of clinical awareness during follow-up in cases of DTC, which can be aggressive in presentation and behaviour. Timely identification and diagnosis of these tumours are essential for prompt treatment initiation and improvement of overall survival.

We assessed the real-life use of direct oral anticoagulants (DOACs) in patients with venous thromboembolism (VTE) and exclusion criteria for randomized trials. From 2013 to 2016, 3,578 of 18,853 patients (19%) had exclusion criteria. Irrespective of which anticoagulant was chosen, they had more VTE recurrences (hazard ratio (HR): 3.10; 95% confidence interval (CI): 2.47-3.88), major bleeds (HR: 4.10; 95% CI: 3.38-4.96), and deaths (HR: 9.47; 95% CI: 8.46-10.6) than those without exclusion criteria. During initial therapy, no patient with exclusion criteria on DOACs (n = 115) recurred, but those on rivaroxaban bled less often (adjusted HR: 0.18; 95% CI: 0.04-0.79) than those on unfractionated heparin (n = 224) and similar to those (n = 3,172) on low-molecular-weight (LMWH) heparin. For long-term therapy, patients on rivaroxaban (n = 151) had nonsignificantly fewer VTE recurrences (adjusted HR: 0.74; 95% CI: 0.08-1.32) and major bleeds (adjusted HR: 0.41; 95% CI: 0.15-1.15) than those on LMWH (n = 2,071). The efficacy and safety of DOACs were similar to standard therapy.
