Abstract

Purpose Methods Results Conclusion Level of Evidence Similar to the anterior cruciate ligament, the femoral footprint of the posterior cruciate ligament (PCL) is composed of different fibre areas, possibly having distinct biomechanical functions. The aim of this study was to determine the role of different fibre areas of the femoral footprint of the PCL in restraining posterior tibial translation (PTT).A sequential cutting study was performed on eight fresh‐frozen human knee specimens, utilizing a six‐degrees‐of‐freedom robotic test setup. The femoral attachment of the PCL was divided into 15 areas, which were sequentially cut from the bone in a randomized sequence. After determining the native knee kinematics, a displacement‐controlled protocol was performed replaying the native motion, while constantly measuring the force. The reduction of the restraining force presented the percentage contribution of each cut, according to the principle of superposition.The PCL was found to contribute 29 ± 16% in 0°, 51 ± 24% in 30°, 60 ± 22% in 60° and 55 ± 18% in 90°, to restricting a PTT. The fibre areas contributing the most were located at the proximal border of the PCL footprint, away from the cartilage, and directly adjacent to the medial intercondylar ridge (p < 0.05). Of these, one fibre area showed the highest contribution at all flexion angles. This area was located at the posterior half of the medial intercondylar ridge. No clear assignment of the areas to either the anterolateral or posteromedial bundle was possible.An area towards the proximal and posterior part of the femoral PCL footprint was found to significantly restrain a posterior tibial force. Based on the data of this testing setup, a PCL graft positioned at the identified area may best mimic the part of the native PCL, which bears the most load in resisting a PTT force.No evidence level (laboratory study). Abstract

Abstract Background Surgical microvascular decompression (MVD) is the curative treatment for hemifacial spasm (HFS). Monitoring MVD by recording the lateral spread response (LSR) intraoperatively can predict a successful clinical outcome. However, the rate of the LSR varies between trials, and the reason for this variation is unclear. The aim of our trial is to evaluate the rate of the LSR after intubation following treatment with succinylcholine, vecuronium, or no muscle relaxant. Methods and design This trial is a prospective randomised controlled trial of 96 patients with HFS (ASA status I or II) undergoing MVD under general anaesthesia. Patients are randomised to receive succinylcholine, vecuronium, or no muscle relaxant before intubation. Intraoperative LSR will be recorded until dural opening. The primary outcome of this study is the rate of the LSR, and the secondary outcomes are post-intubation pharyngolaryngeal symptoms, the rate of difficult intubations, the rate of adverse haemodynamic events and the relationship between the measurement of LSR or not, and clinical success rates at 30 days after surgery. Discussion This study aims to evaluate the impact of muscle relaxants on the rate of the LSR, and the study may provide evidence supporting the use of muscle relaxants before intubation in patients with HFS undergoing MVD surgery. Abstract

Abstract Background Spontaneous intracerebral hemorrhage is a disease with high morbidity, high disability rate, high mortality, and high economic burden. Whether patients can benefit from surgical evacuation of hematomas is still controversial, especially for those with moderate-volume hematomas in the basal ganglia. This study is designed to compare the efficacy of endoscopic surgery and conservative treatment for the moderate-volume hematoma in spontaneous basal ganglia hemorrhage. Methods Patients meet the criteria will be randomized into the endoscopic surgery group (endoscopic surgery for hematoma evacuation and the best medical treatment) or the conservative treatment group (the best medical treatment). Patients will be followed up at 1, 3, and 6 months after initial treatment. The primary outcomes include the Extended Glasgow Outcome Scale and the Modified Rankin Scale. The secondary outcomes consist of the National Institutes of Health Stroke Scale and the mortality. The Barthel Index(BI) will also be evaluated. The sample size is 100 patients. Discussion The ECMOH trial is a randomized controlled trial designed to evaluate if endoscopic surgery is better than conservative treatment for patients with moderate-volume hematomas in the basal ganglia. Abstract

Spent lead paste is the main component in lead-acid batteries reaching end of life. It contains about 55% lead sulphate and 35% lead dioxide, as well as minor amounts of lead oxide. It is necessary to recycle spent lead paste with minimal pollution and low energy consumption instead of the conventional smelting method. In this study, a novel approach involving hydrometallurgical desulphurisation and thermal degradation is developed to recover lead as PbO products from spent lead acid batteries. First, the desulphurisation effects and phase compositions of products with different transforming agents were compared, and the optimum conditions using (NH4)2CO3 as a transforming agent were determined. Effect of pre-harvest continuous light with different red/blue ratio on photon flux density (R/B ratio) on reducing nitrate accumulation was studied by growing lettuce (Lactuca sativa L.) under continuous illumination delivered by light-emitting diodes (LEDs). Results show that nitrate concentration decreased by 1648.0–2061.1 mg kg−1 in leaf blade and 962.9–2090.3 mg kg−1 in petiole, accompanied by a dramatic increase in soluble sugar content. Compared with monochromatic red light treatment, the decrease in nitrate concentration and increase in soluble sugar content in lettuce under mixed red and blue light were more pronounced. The lowest nitrate concentration was observed in the treatment with R/B ratio of 4. It's concluded that pre-harvest exposure to 48 h continuous LED light could effectively reduce nitrate accumulation in lettuce and this process is strongly affected by R/B ratio of light. Human health worldwide is being threatened by excessive nitrate intake through the dietary pathway, i.e. vegetables and drinking water, and the former is the primary source (about 80%) for large quantitative ingestion and the biological characteristics of nitrate accumulation, especially for leafy vegetables. In this paper, four kinds of organic nutrient solutions (biogas slurry dilution or biogas slurry dilution with added glycine) were developed to culture lettuce in order to lower the nitrate concentrations in leafy tissue. Two experiments were designed to investigate the effects of soil disturbance and organic P (Na- phytate and lecithin) on mycorrhiza formation and symbiotic efficiency of AMF associated with maize in greenhouse. The results showed that P forms did not affect the root colonization of BEG167 and Glomus versifome (G.v) with maize. G.v produced more extraradical fungal biomass than BEG167 when KH2PO4 and lecithin were applied. The treatments with lecithin and KH2PO4 had higher maize biomass than those supplied with Na-phytate. BEG 167 and G.v differed in maize biomass contribution when Na-phytate was applied. Abstract

As the first neuronal element in the auditory pathway, spiral ganglion neurons are responsible for the initial signal encoding of complex sound information. Experiments presented in this dissertation explore the physiological basis of coding capacity in the murine postnatal spiral ganglion using electrophysiological and immunocytochemical techniques. Three separate and complete studies are presented. First, heterogeneity within each tonotopic region of the ganglion was characterized by the distribution patterns of calretinin and calbindin, two widely used markers for neuronal subpopulations (Liu and Davis, 2014). Second, the developmental establishment of diverse firing patterns in the spiral ganglion was investigated in embryonic cochlear epithelial cells in which neurogenic transcription factors were overexpressed (Nishimura et al., 2014). Finally, recordings made from acute cochlear whole-mounts preparations were made to assess the diverse intrinsic firing features in comparison to invading action potentials and passively-propagated synaptic potentials (Liu and Davis, in preparation). Results from these studies suggest that the physiological heterogeneity within the spiral ganglion may contribute to a population code essential for conveying the rich content of auditory information.

Abstract

Purpose: Primary hyperparathyroidism (PHPT) is a common endocrine disorder and is usually diagnosed in adults. PHPT due to ectopic parathyroid adenoma in adolescents is rare. Methods: We describe the case of a 15-year-old boy with PHPT due to ectopic parathyroid adenoma. A review of the literature of PHPT in adolescents was performed, focusing on etiology, clinical presentation, preoperative localization methods, pathology, and treatment. Results: The patient was successfully treated with surgery and was followed up for 5 years with no signs or symptoms of hyperparathyroidism. By reviewing the literature, only seven cases of PHPT associated with ectopic parathyroid lesions in adolescents have been reported. Parathyroidectomy is the only known curative treatment. Accurate preoperative localization of the target lesion is critical. Conclusions: This study should raise awareness of the diagnosis and treatment of PHPT due to ectopic parathyroid adenoma/carcinoma in adolescents. [ABSTRACT FROM AUTHOR]

Desulfurized of lead paste in waste lead-acid batteries was researched using (NH4)2CO3 and NH4HCO3,respectively. The effects of reaction time, temperature, the molar ratio of desulfurization agent to lead sulfate and the ratio of liquid volume-to-solid mass on desulfurization of lead paste were examined. The optimum reaction conditions were determined. The results show that the desulfurization rate can reach 95. 22% under the conditions of reaction time of 60 min, temperature of 50 °C, mole ratio of 1.2/1, and liquid volume-to-solid mass of 5/1, using (NH4)2CO3 as desulfurizing agent. 186. Hybrid receptive field network for small object detection on drone view

Abstract

Drone-based small object detection is of great significance in practical applications such as military actions, disaster rescue, transportation, etc. However, the severe scale differences in objects captured by drones and lack of detail information for small-scale objects make drone-based small object detection a formidable challenge. To address these issues, we first develop a mathematical model to explore how changing receptive fields impacts the polynomial fitting results. Subsequently, based on the obtained conclusions, we propose a simple but effective Hybrid Receptive Field Network (HRFNet), whose modules include Hybrid Feature Augmentation (HFA), Hybrid Feature Pyramid (HFP) and Dual Scale Head (DSH). Specifically, HFA employs parallel dilated convolution kernels of different sizes to extend shallow features with different receptive fields, committed to improving the multi-scale adaptability of the network; HFP enhances the perception of small objects by capturing contextual information across layers, while DSH reconstructs the original prediction head utilizing a set of high-resolution features and ultrahigh-resolution features. In addition, in order to train HRFNet, the corresponding dual-scale loss function is designed. Finally, comprehensive evaluation results on public benchmarks such as VisDrone-DET and TinyPerson demonstrate the robustness of the proposed method. Most impressively, the proposed HRFNet achieves a mAP of 51.0 on VisDrone-DET with 29.3M parameters, which outperforms the extant state-of-the-art detectors. HRFNet also performs excellently in complex scenarios captured by drones, achieving the best performance on the CS-Drone dataset we built.

Abstract

Light intensity plays a critical regulatory role in determining plant yield and quality, and energy consumption in plant factory with artificial light (PFAL). The effects of different light intensities (200, 300, 400, 500, and 600 µmol·m−2·s−1) of light-emitting diodes (LED) red and blue light with ratio 4:1 on growth, quality, nitrogen and amino acid metabolisms of ryegrass were investigated. The results showed the tiller number and shoot fresh weight in ryegrass were increased, while plant height and stem diameter remained stable with the improvement of light intensity. The highest levels of soluble protein, free amino acid, and DPPH free radical clearance rate in photon flux density (PPFD) at 500 µmol·m−2·s−1, and then significantly decreased with light intensity increased. The highest soluble sugar content was found at PPFD of 600 µmol·m−2·s−1, while malondialdehyde (MDA) content remained stable in all treatments. As the light intensity increased, nitrate content decreased, while nitrate reductase (NR) activity and the ammonium content increased. As light intensity was increased from 200 to 600 µmol·m−2·s−1, nitrite reductase (NiR) activity first decreased and then increased. Moreover, glutamate and cysteine contents increased first and then decreased, while glutamate synthase (GOGAT), glutamine synthase (GS), glutamate dehydrogenase (GDH), and cysteine synthases (CS) activities increased continuously with light intensity. Light intensity was significantly correlated with the parameters of growth, quality, nitrogen and amino acid metabolisms in ryegrass. Abstract

For more plant species to be suitable for plant factory production, their optimal light regimes need to be optimized. We evaluated the effects of light quality, light intensity, and photoperiod on the growth and yield of cherry radish grown under red plus blue LEDs in a controlled environment. Radish plants were cultivated under two light qualities with different red:blue ratios (1R:1B, 2R:1B) at three light intensities (180, 240, 300 μmol m−2 s−1) or two photoperiods (12 h/12 h, 16 h/8 h), respectively. The light quality 2R:1B increased root diameter, root volume, and the biomass of shoot and root compared to light quality 1R:1B under a light intensity of 240 and 300 μmol m−2 s−1, but the growth differences between 1R:1B and 2R:1B were significant when the light intensity was 240 μmol m−2 s−1. New leaf chlorophyll content, root growth indices and the root-shoot ratio increased with light intensity. Cherry radish only formed storage roots with commercial value when light intensity was equal to or over 240 μmol m−2 s−1. The root diameter, root volume, root-shoot ratio, and the biomass of shoot and root of plants grown in the 2R:1B treatment was significantly higher than those in the 1R:1B treatment under the 16 h/8 h photoperiod. However, no significant difference was observed in the 12 h/12 h photoperiod. These results indicated that light regime in combination with a light intensity between 240 and 300 μmol m−2 s−1, the light quality 2R:1B, and a 16 h/8 h photoperiod produced appropriate growth of cherry radish in plant factory settings using an LED light source. In conclusion, the production of commercial storage roots in cherry radish is primarily dependent on light intensity, followed by light quality and photoperiod. 194. Primary trigeminal neuralgia is associated with posterior fossa crowdedness: A prospective case-control study.

Abstract

Neurovascular conflict (NVC) has been postulated to be the underlying cause of trigeminal neuralgia (TN). Does the posterior fossa crowdedness increase the chance of NVC? The aim of this study was to quantitatively measure the posterior fossa crowdedness in patients with TN and to perform a comparison with healthy controls. We conducted a prospective case-control study of 46 patients diagnosed with primary TN and 46 sex- and age-matched healthy controls. All subjects underwent high-resolution three-dimensional MRI, and the 3D Slicer software was used to measure the posterior fossa volume (PFV) and hindbrain volume (HBV). The posterior fossa crowdedness index (PFCI) was calculated as HBV/PFV × 100%. The results showed that patients with TN had a larger HBV (155.4 ± 23.2 cm 3 versus 152.9 ± 13.5 cm 3 , P = .16) and a smaller PFV (182.7 ± 18.3 cm 3 versus 186.1 ± 11.7 cm 3 , P = .42) as compared to control subjects, but these values were not significantly different. The mean PFCI was significantly higher in patients with TN than in controls (85.1% ± 3.4% versus 82.2% ± 5.3%; P = .03). Women had a more crowded posterior fossa than men (85.8% ± 2.1% versus 84.1% ± 2.6%; P = .023) in patients with TN. The correlation analysis showed that a higher PFCI was associated with younger age (P = .02), woman (P = .014), and TN disease (P = .001). From this study, we conclude that patients with TN have a more crowded posterior fossa than healthy subjects. Women, younger age and TN disease are associated with a higher PFCI. The adhesion G-protein-coupled receptor GPR133 (ADGRD1) supports growth of the brain malignancy glioblastoma. How the extracellular interactome of GPR133 in glioblastoma modulates signaling remains unknown. Here, we use affinity proteomics to identify the transmembrane protein PTK7 as an extracellular binding partner of GPR133 in glioblastoma. PTK7 binds the autoproteolytically generated N-terminal fragment of GPR133 and its expression in trans increases GPR133 signaling. This effect requires the intramolecular cleavage of GPR133 and PTK7's anchoring in the plasma membrane. PTK7's allosteric action on GPR133 signaling is additive with but topographically distinct from orthosteric activation by soluble peptide mimicking the endogenous tethered Stachel agonist. GPR133 and PTK7 are expressed in adjacent cells in glioblastoma, where their knockdown phenocopies each other. We propose that this ligand-receptor interaction is relevant to the pathogenesis of glioblastoma and possibly other physiological processes in healthy tissues. Abstract

We introduce a pioneering approach that integrates pathology imaging with transcriptomics and proteomics to identify predictive histology features associated with critical clinical outcomes in cancer. We utilize 2,755 H&E-stained histopathological slides from 657 patients across 6 cancer types from CPTAC. Our models effectively recapitulate distinctions readily made by human pathologists: tumor vs. normal (AUROC = 0.995) and tissue-of-origin (AUROC = 0.979). We further investigate predictive power on tasks not normally performed from H&E alone, including TP53 prediction and pathologic stage. Importantly, we describe predictive morphologies not previously utilized in a clinical setting. The incorporation of transcriptomics and proteomics identifies pathway-level signatures and cellular processes driving predictive histology features. Model generalizability and interpretability is confirmed using TCGA. We propose a classification system for these tasks, and suggest potential clinical applications for this integrated human and machine learning approach. Abstract

We describe a patient presented with sequential events of hemifacial spasm, cerebral infarction and fatal subarachnoid hemorrhage. All of them are seemingly separate entities. Radiological examination revealed that the cause was vertebrobasilar dolichoectasia (VBD) coexisting with a giant saccular aneurysm of basilar artery. VBD is a dilatation and elongation of vertebral artery (VA) and basilar artery (BA) which is of different mechanism from saccular aneurysm. The condition is very rare and its progression may result in compression of brainstem and cranial nerves, ischemic and/or hemorrhagic stroke. The treatment of such condition is difficult and further research on the risk stratification of VBD for predicting stroke. Close monitoring and aggressive surgical interventions might be needed for high-risk patients.
