158. Nuclear halo measurements for accurate prediction of field size factor in a Varian ProBeam proton PBS system.

Abstract

Purpose: For pencil‐beam scanning proton therapy systems, in‐air non‐Gaussian halo can significantly impact output at small field sizes and low energies. Since the low‐intensity tail of spot profile (halo) is not necessarily modeled in treatment planning systems (TPSs), this can potentially lead to significant differences in patient dose distribution. In this work, we report such impact for a Varian ProBeam system. Methods: We use a pair magnification technique to measure two‐dimensional (2D) spot profiles of protons from 70 to 242 MeV at the treatment isocenter and 30 cm upstream of the isocenter. Measurements are made with both Gafchromic film and a scintillator detector coupled to a CCD camera (IBA Lynx). Spot profiles are measured down to 0.01% of their maximum intensity. Field size factors (FSFs) are compared among calculation using measured 2D profiles, calculation using a clinical treatment planning algorithm (Raystation 8A clinical Monte Carlo), and a CC04 small‐volume ion chamber. FSFs were measured for square fields of proton energies ranging from 70 to 242 MeV. Results: All film and Lynx measurements agree within 1 mm for full width at half maximum beam intensity. The measured radial spot profiles disagree with simple Gaussian approximations, which are used for modeling in the TPS. FSF measurements show the magnitude of disagreements between beam output in reality and in the TPS without modeling halo. We found that the clinical TPS overestimated output by as much as 6% for small field sizes of 2 cm at the lowest energy of 70 MeV while the film and Lynx measurements agreed within 4% and 1%, respectively, for this FSF. Conclusions: If the in‐air halo for low‐energy proton beams is not fully modeled by the TPS, this could potentially lead to under‐dosing small, shallow treatment volumes in PBS treatment plans. Abstract

Purpose: To study if abdominal compression can reduce breathing motion and mitigate interplay effect in pencil beam scanning proton therapy (PBSPT) treatment of liver tumors in order to better spare healthy liver volumes compared with photon therapy. Methods: Ten patients, six having large tumors initially treated with IMRT and four having small tumors treated with SBRT, were replanned for PBSPT. ITV and beam-specific PTVs based on 4D-CT were used to ensure target coverage in PBSPT. The use of an abdominal compression belt and volumetric repainting was investigated to mitigate the interplay effect between breathing motion and PBSPT dynamic delivery. An in-house Matlab script has been developed to simulate this interplay effect. The dose is computed on each phase individually by sorting all spots according to their simulated delivery timing. The final dose distribution is then obtained by accumulating all dose maps to a reference phase. Results: For equivalent target coverage PBSPT reduced average healthy liver dose by 9.5% of the prescription dose compared with IMRT/SBRT. Abdominal compression of 113.2±42.2 mmHg was effective for all 10 patients and reduced average motion by 2.25 mm. As a result, the average ITV volume decreased from 128.2% to 123.1% of CTV volume. Similarly, the average beam-specific PTV volume decreased from 193.2% to 183.3%. For 8 of the 10 patients, the average motion was reduced below 5 mm, and up to 3 repainting were sufficient to mitigate interplay. For the other two patients with larger residual motion, 4-5 repainting were needed. Conclusion: We recommend evaluation of the 4DCT motion histogram following simulation and the interplay effect following treatment planning in order to personalize the use of compression and volumetric repainting for each patient. Abdominal compression enables safe and more effective PBS treatment of liver tumors by reduction of motion and interplay effect.

Abstract

I.Introduction and purpose Proton therapy offers unique dosimetric advantages over photon therapy to improve healthy tissue sparing and potentially treatment outcome. Delivery by pencil beam scanning (PBS) enables highly conformal dose distributions by using multiple, intensity-modulated spots throughout the target. However, organ motion may severely degrade treatment quality. First, motion modifies the densities seen by the protons, and thus their range, which can cause unacceptable distortions of the dose distributions. Second, beam scanning may interfere with breathing motion, leading to the so-called interplay effect. This study aims to evaluate the impact of breathing motion and the interplay effect for 10 liver patients. In addition, the use of an abdominal compression belt has been investigated to mitigate deterioration of the treatment outcome. II.Material and methods The interplay effect with PBS has been studied in-silico in ten liver patients. Motion information is extracted from the 4D-CT scan, composed of 8 images of the patient at different phases of the breathing cycle. The PBS plans have been optimized on the average CT with Eclipse employing a beam-specific PTV (BSPTV) to take into account range uncertainties and motion. PBS spots are then binned with respect to the motion phases using the delivery timing. This allowed us to generate one partial plan for each phase. The dose is then computed on each CT phase with its corresponding plan, representing the dose that is actually delivered during each phase. The Morphon deformable registration algorithm is used to accumulate the 8 partial dose maps in a reference phase. This accumulated dose can be compared to the original treatment plan to assess the impact of the organ motion and interplay effect. The use of an abdominal compression belt can reduce the amplitude of organ motion and mitigate the degradation of the dose distribution. Abstract

This work introduces an approach for the synthesis of optimal covert sensor–actuator attackers in the context of discrete-event systems (DES). Building upon existing works on optimal supervisor synthesis, on one hand, and the base model construction in the transformation of covert sensor–actuator attacker synthesis to supervisor synthesis, on the other hand, we show how the optimal covert sensor–actuator attacker synthesis problem could be transformed to the optimal supervisor synthesis problem, thus generalizing its logic counterpart. We consider several different types of optimization objectives, such as (1) minimization of attack energy cost, and (2) minimization of time cost to cause damage infliction; for the latter one, asynchronous event firings and concurrent event firings at the plant are both considered. We provide the necessary and sufficient conditions for the existence of an optimal attacker for both the energy minimization case and damage infliction time minimization case. Abstract

In this work, we investigate the problem of privacy-preserving supervisory control against an external passive intruder via co-synthesis of a dynamic mask, an edit function, and a supervisor. We attempt to achieve the following goals: (1) the system secret cannot be inferred by the intruder, i.e., opacity of secrets against the intruder, and the existence of the dynamic mask and the edit function should not be discovered by the intruder, i.e., covertness of dynamic mask and edit function against the intruder; (2) some safety and nonblockingness requirement should be satisfied. We assume the intruder can eavesdrop both the sensing information generated by the sensors and the control commands issued to the actuators. Our approach is to model the co-synthesis problem as a distributed supervisor synthesis problem in the Ramadge–Wonham supervisory control framework, and we propose an incremental synthesis heuristic to incrementally synthesize a dynamic mask, an edit function and a supervisor. We present an algorithm for multiplying an arbitrary mby rmatrix Xby a rby rheaps of pieces matrix M, making it possible to calculate the resulting matrix in worst case time complexity O(mr),rather than O(mr2)which is required when using the matrix multiplication definition. We also give an algorithm for multiplying Mby an arbitrary rby nmatrix Xwith worst case time complexity O(nr).Finally, we consider a variant of the standard heaps of pieces model, and present an improved matrix multiplication algorithm for this variant as well.
