We have studied the magnetic and electrical properties of LaySr1-yTi0.9Fe0.1O3-δ films for compositions where y=0, 0.2, 0.5, and 0.7. All the films exhibited room temperature ferromagnetism with a magnetic moment ranging from 0.7 μB/Fe to 0.2 μB/Fe. The SrTi0.9Fe0.1O3-δ (y=0) sample is an insulator with a small polaron like temperature dependence of resistivity. On the other hand y=0.2 and y=0.5 films exhibited a metallic type of resistivity which can be described by a power law. The largest magnetic moment was observed in the most resistive member (y=0) of the LaySr1-yTi0.9Fe0.1O3-δ family. This is in contrast to the carrier mediated magnetism models. The origin of the magnetism in highly resistive y=0 film could be attributed to the mixed valence state of Fe. On the other hand, La doped sample (y>0) exhibited features of carrier mediated magnetism as well as the mixed valence magnetism. The origin of the observed magnetism in all films has been discussed. - Abstract
Global inequalities in access to health care are reflected in differences in cancer survival. The CONCORD programme was designed to assess worldwide differences and trends in population-based cancer survival. In this population-based study, we aimed to estimate survival inequalities globally for several subtypes of childhood leukaemia. Cancer registries participating in CONCORD were asked to submit tumour registrations for all children aged 0-14 years who were diagnosed with leukaemia between Jan 1, 1995, and Dec 31, 2009, and followed up until Dec 31, 2009. Haematological malignancies were defined by morphology codes in the International Classification of Diseases for Oncology, third revision. We excluded data from registries from which the data were judged to be less reliable, or included only lymphomas, and data from countries in which data for fewer than ten children were available for analysis. We also excluded records because of a missing date of birth, diagnosis, or last known vital status. We estimated 5-year net survival (ie, the probability of surviving at least 5 years after diagnosis, after controlling for deaths from other causes [background mortality]) for children by calendar period of diagnosis (1995-99, 2000-04, and 2005-09), sex, and age at diagnosis (<1, 1-4, 5-9, and 10-14 years, inclusive) using appropriate life tables. We estimated age-standardised net survival for international comparison of survival trends for precursor-cell acute lymphoblastic leukaemia (ALL) and acute myeloid leukaemia (AML). We analysed data from 89 828 children from 198 registries in 53 countries. During 1995-99, 5-year age-standardised net survival for all lymphoid leukaemias combined ranged from 10·6% (95% CI 3·1-18·2) in the Chinese registries to 86·8% (81·6-92·0) in Austria. International differences in 5-year survival for childhood leukaemia were still large as recently as 2005-09, when age-standardised survival for lymphoid leukaemias ranged from 52·4% (95% CI 42

- Abstract
After changing optical AGN type from 1.9 to 1 in 1984, the AGN Mrk 1018 recently reverted back to its type 1.9 state. Our ongoing monitoring now reveals that the AGN has halted its dramatic dimming, reaching a minimum around October 2016. The minimum was followed by an outburst rising with ~0.25 U-band mag/month. The rebrightening lasted at least until February 2017, as confirmed by joint Chandra and Hubble observations. Monitoring was resumed in July 2017 after the source emerged from sunblock, at which point the AGN was found only ~0.4 mag brighter than its minimum. The intermittent outburst was accompanied by the appearance of a red wing asymmetry in broad-line shape, indicative of an inhomogeneous broad-line region. The current flickering brightness of Mrk 1018 following its rapid fading either suggests that the source has reignited, remains variable at a low level, or may continue dimming over the next few years. Distinguishing between these possibilities requires continuous multiwavelength monitoring. © ESO, 2017.

Urban trees contribute to decarbonisation. However, the removal of trees may release the stored carbon within them. In Hong Kong, Acacia confusa and Leucaena leucocephala are being removed, but with unknown carbon impacts. This industry-academia-government collaborative research aimed to: (1) report the amount of carbon stored inside Acacia confusa and Leucaena leucocephala harvested from slopes; (2) estimate the carbon storage of a tree using dendrometric measurements; and (3) explain the variation in carbon content percentage of urban trees with respect to tree species, part of the tree, and position within the part. 10 Acacia confusa and 10 Leucaena leucocephala were harvested during March–September, 2023. Each tree was weighed for carbon storage estimation. Results showed that Acacia confusa had higher mean wood volume, biomass and carbon storage than Leucaena leucocephala. The mean carbon content of the analysed samples (45.53−52.58%) were mostly significantly different from 50%. But the difference may become insignificant depending on how volatile carbon loss was controlled. Diameter at breast height (mm) was a significant predictor of carbon storage (kg) for both Acacia. confusa (β = 0.9574) and Leucaena leucocephala (β = 0.3909). Significant interaction between tree species and tree part on carbon content percentage was confirmed. This research demonstrated the impacts of past arboricultural decisions on present decarbonisation plans.

Purpose:\ud To evaluate the dependency of the\ud 129\ud Xe-red blood\ud cell (RBC) chemical shift on blood oxygenation, and to use\ud this relation for noninvasive measurement of pulmonary blood\ud oxygenation in vivo with hyperpolarized\ud 129\ud Xe NMR.\ud Methods:\ud Hyperpolarized\ud 129\ud Xe was equilibrated with blood\ud samples of varying oxygenation in vitro, and NMR was performed\ud at 1.5 T and 3 T. Dynamic in vivo NMR during breath hold apnea\ud was performed at 3 T on two healthy volunteers following inhala-\ud tion of hyperpolarized\ud 129\ud Xe.\ud Results:\ud The\ud 129\ud Xe chemical shift in RBCs was found to increase\ud nonlinearly with blood oxygenation at 1.5 T and 3 T. During breath\ud hold apnea, the\ud 129\ud Xe chemical shift in RBCs exhibited a periodic\ud time modulation and showed a net decrease in chemical shift of\ud ~\ud 1 ppm over a 35 s breath hold, corresponding to a decrease of\ud 7–10 % in RBC oxygenation. The\ud 129\ud Xe-RBC signal amplitude\ud showed a modulation with the same frequency as the\ud 129\ud Xe-RBC\ud chemical shift.\ud Conclusion:\ud The feasibility of using the\ud 129\ud Xe-RBC chemical shift\ud to measure pulmonary blood oxygenation in vivo has been dem-\ud onstrated. Correlation between\ud 129\ud Xe-RBC signal and\ud 129\ud Xe-RBC\ud chemical shift modulations in the lung warrants further investiga-\ud tion, with the aim to better quantify temporal blood oxygenation\ud changes in the cardiopulmonary vascular circuit.

166. Exploring personality dimensions that influence practice and performance of a simulated laparoscopic task in the objective structured clinical examination

- Abstract
BACKGROUND: Surgical educators have encouraged the investigation of individual differences in aptitude and personality in surgical performance. An individual personality difference that has been shown to influence laparoscopic performance under time pressure is movement specific reinvestment. Movement specific reinvestment has 2 dimensions, movement self-consciousness (MS-C) (i.e., the propensity to consciously monitor movements) and conscious motor processing (CMP) (i.e., the propensity to consciously control movements), which have been shown to differentially influence laparoscopic performance in practice but have yet to be investigated in the context of psychological stress (e.g., the objective structured clinical examination [OSCE]). OBJECTIVE: This study investigated the role of individual differences in propensity for MS-C and CMP in practice of a fundamental laparoscopic skill and in laparoscopic performance during the OSCE. Furthermore, this study examined whether individual differences during practice of a fundamental laparoscopic skill were predictive of laparoscopic performance during the OSCE. METHODS: Overall, 77 final-year undergraduate medical students completed the movement specific reinvestment scale, an assessment tool that quantifies the propensity for MS-C and CMP. Participants were trained to proficiency on a fundamental laparoscopic skill. The number of trials to reach proficiency was measured, and completion times were recorded during early practice, later practice, and the OSCE. RESULTS: There was a trend for CMP to be negatively associated with the number of trials to reach proficiency (p = 0.064). A higher propensity for CMP was associated with fewer trials to reach proficiency (β = -0.70, p = 0.023). CMP and MS-C did not significantly predict completion times in the OSCE (p > 0.05). Completion times in early practice (β = 0.05, p = 0.016) and later practice (β = 0.47, p < 0.001) and number of trials to reach proficiency (β = 0.23, p = 0.003) significantly predicted completion times in the OSCE. CONCLUSION: It appears that a higher propensity for CMP predicts faster rates of learning of a fundamental laparoscopic skill. Furthermore, laparoscopic performance during practice is indicative of laparoscopic performance in the challenging conditions of the OSCE. The lack of association between the 2 dimensions of movement specific reinvestment and performance during the OSCE is explained using the theory of reinvestment as a framework. Overall, consideration of personality differences and individual differences in ability during practice could help inform the development of individualized surgical training programs.

In vitro laboratory and animal studies demonstrate a synergistic role for the combination of vancomycin and antistaphylococcal ß-lactams for methicillin-resistant Staphylococcus aureus (MRSA) bacteremia. Prospective clinical data are lacking. Methods. In this open-label, multicenter, clinical trial, adults with MRSA bacteremia received vancomycin 1.5 g intravenously twice daily and were randomly assigned (1:1) to receive intravenous flucloxacillin 2 g every 6 hours for 7 days (combination group) or no additional therapy (standard therapy group). Participants were stratified by hospital and randomized in permuted blocks of variable size. Randomization codes were kept in sealed, sequentially numbered, opaque envelopes. The primary outcome was the duration of MRSA bacteremia in days. Results. We randomly assigned 60 patients to receive vancomycin (n = 29), or vancomycin plus flucloxacillin (n = 31). The mean duration of bacteremia was 3.00 days in the standard therapy group and 1.94 days in the combination group. According to a negative binomial model, the mean time to resolution of bacteremia in the combination group was 65% (95% confidence interval, 41%-102%; P =. 06) that in the standard therapy group. There was no difference in the secondary end points of 28- and 90-day mortality, metastatic infection, nephrotoxicity, or hepatotoxicity. Conclusions. Combining an antistaphylococcal ß-lactam with vancomycin may shorten the duration of MRSA bacteremia. Nongenomic effect of testosterone on chloride secretion in cultured rat efferent duct epithelia. Am J Physiol Cell Physiol 280: C 1160-C 1167, 2001.--Short-circuit current ([I.sub.sc]) technique was used to investigate the role of testosterone in the regulation of chloride secretion in cultured rat efferent duct epithelia. Among the steroids tested, only testosterone, and to a lesser extent, 5[Alpha]-dihydrotestosterone (5[Alpha]-DHT), reduced the basal and forskolin-induced [I.sub.sc] in cultured rat efferent duct epithelia when added to the apical bathing solution. Indomethacin, a 3[Alpha]-hydroxysteroid dehydrogenase, did not affect the inhibitory effect of 5[Alpha]-DHT. The effect of testosterone occurred within 10-20 s upon application and was dose dependent with apparent [IC.sub.50] value of 1 [micro]M. The effect was abolished by removal of [Cl.sup.-] but not [MATHEMATICAL EXPRESSION NOT REPRODUCIBLE IN ASCII] from the normal Krebs-Henseleit solution, suggesting that testosterone mainly inhibited [Cl.sup.-] secretion. The efferent duct was found to be most sensitive to testosterone, while the caput and the cauda epididymidis were only mildly sensitive. Cyproterone acetate, a steroidal antiandrogen, or flutamide, a nonsteroidal antiandrogen, did not block the effect of testosterone on the forskolin-induced [I.sub.sc], nor did protein synthesis inhibitors, cycloheximide, or actinomycin D. However, pertussis toxin, a [G.sub.i] protein inhibitor, attenuated the inhibition of forskolin-induced [I.sub.sc] by testosterone. Testosterone caused a dose-dependent inhibition of forskolin-induced rise in cAMP in efferent duct cells. It is suggested that the rapid effect of testosterone was mediated through a membrane receptor that is negatively coupled to adenylate cyclase via [G.sub.i] protein. The role of nongenomic action of testosterone in the regulation of electrolyte and fluid transport in the efferent duct is discussed. testosterone; chloride secretion; efferent duct

Worldwide data for cancer survival are scarce. We aimed to initiate worldwide surveillance of cancer survival by central analysis of population-based registry data, as a metric of the eff ectiveness of health systems, and to inform global policy on cancer control. Methods Individual tumour records were submitted by 279 population-based cancer registries in 67 countries for 25·7 million adults (age 15–99 years) and 75 000 children (age 0–14 years) diagnosed with cancer during 1995–2009 and followed up to Dec 31, 2009, or later. We looked at cancers of the stomach, colon, rectum, liver, lung, breast (women), cervix, ovary, and prostate in adults, and adult and childhood leukaemia. Standardised quality control procedures were applied; errors were corrected by the registry concerned. We estimated 5-year net survival, adjusted for background mortality in every country or region by age (single year), sex, and calendar year, and by race or ethnic origin in some countries. Estimates were age-standardised with the International Cancer Survival Standard weights. Findings 5-year survival from colon, rectal, and breast cancers has increased steadily in most developed countries. For patients diagnosed during 2005–09, survival for colon and rectal cancer reached 60% or more in 22 countries around the world; for breast cancer, 5-year survival rose to 85% or higher in 17 countries worldwide. Liver and lung cancer remain lethal in all nations: for both cancers, 5-year survival is below 20% everywhere in Europe, in the range 15–19% in North America, and as low as 7–9% in Mongolia and Thailand. Striking rises in 5-year survival from prostate cancer have occurred in many countries: survival rose by 10–20% between 1995–99 and 2005–09 in 22 countries in South America, Asia, and Europe, but survival still varies widely around the world, from less than 60% in Bulgaria and Thailand to 95% or more in Brazil, Puerto Rico, and the USA. For cervical cancer, national estimates of 5-year survival range from less than 50% to more than 70%; regional variations are much wider, and improvements between 1995–99 and 2005–09 have generally been slight. For women diagnosed with ovarian cancer in 2005–09, 5-year survival was 40% or higher only in Ecuador, the USA, and 17 countries in Asia and Europe. 5-year survival for stomach cancer in 2005–09 was high (54–58%) in Japan and South Korea, compared with less than 40% in other countries. By contrast, 5-year survival from adult leukaemia in Japan and South Korea (18–23%) is lower than in most other countries. 5-year survival from childhood acute lymphoblastic leukaemia is less than 60% in several countries, but as high as 90% in Canada and four European countries, which suggests major defi ciencies in the management of a largely curable disease. Interpretation International comparison of survival trends reveals very wide diff erences that are likely to be attributable to diff erences in access to early diagnosis and optimum treatment. - Abstract
The practice of an infectious diseases (ID) physician is evolving. A contemporary understanding of the frequency and variety of patients and syndromes seen by ID services has implications for training, service development and setting research priorities. We performed a 2-week prospective survey of formal ID physician activities related to direct inpatient care, encompassing 53 hospitals throughout Australia, New Zealand and Singapore, and documented 1722 inpatient interactions. Infections involving the skin and soft tissue, respiratory tract and bone/joints together accounted for 49% of all consultations. Suspected/confirmed pathogens were primarily bacterial (60%), rather than viral (6%), fungal (4%), mycobacterial (2%) or parasitic (1%). Staphylococcus aureus was implicated in 409 (24%) episodes, approximately four times more frequently than the next most common pathogen. The frequency of healthcare-related infections (35%), immunosuppression (21%), diabetes mellitus (19%), prosthesis-related infections (13%), multiresistant pathogens (13%) and non-infectious diagnoses (9%) was high, although consultation characteristics varied between geographical settings and hospital types. Our study highlights the diversity of inpatient-related ID activities and should direct future teaching and research. Okur‐Chung syndrome is a neurodevelopmental condition attributed to germline
CSNK2A1 pathogenic missense variants. We present 8 unreported subjects with the above syndrome, who have recognizable dysmorphism, varying degrees of developmental delay and multisystem involvement. Together with 6 previously reported cases, we present a case series of 7 female and 7 male subjects, highlighting the recognizable facial features of the syndrome (microcephaly, hypertelorism, epicanthic fold, ptosis, arched eyebrows, low set ears, ear fold abnormality, broad nasal bridge and round face) as well as frequently occurring clinical features including neurodevelopmental delay (93%), gastrointestinal (57%), musculoskeletal (57%) and immunological (43%) abnormalities. The variants reported in this study are evolutionary conserved and absent in the normal population. We observed that theCSNK2A1 gene is relatively intolerant to missense genetic changes, and most variants are within the protein kinase domain. All except 1 variant reported in this cohort are spatially located on the binding pocket of the holoenzyme. We further provide key recommendations on the management of Okur‐Chung syndrome. To conclude, this is the second case series on Okur‐Chung syndrome, and an in‐depth review of the phenotypic features and genomic findings of the condition with suggestions on clinical management. There have been few recent studies demonstrating a definitive association between the transmission of airborne infections and the ventilation of buildings. The severe acute respiratory syndrome (SARS) epidemic in 2003 and current concerns about the risk of an avian influenza (H5N1) pandemic, have made a review of this area timely. We searched the major literature databases between 1960 and 2005, and then screened titles and abstracts, and finally selected 40 original studies based on a set of criteria. We established a review panel comprising medical and engineering experts in the fields of microbiology, medicine, epidemiology, indoor air quality, building ventilation, etc. Most panel members had experience with research into the 2003 SARS epidemic. The panel systematically assessed 40 original studies through both individual assessment and a 2-day face-to-face consensus meeting. Ten of 40 studies reviewed were considered to be conclusive with regard to the association between building ventilation and the transmission of airborne infection. There is strong and sufficient evidence to demonstrate the association between ventilation, air movements in buildings and the transmission/spread of infectious diseases such as measles, tuberculosis, chickenpox, influenza, smallpox and SARS. There is insufficient data to specify and quantify the minimum ventilation requirements in hospitals, schools, offices, homes and isolation rooms in relation to spread of infectious diseases via the airborne route. Aims To estimate recent secular changes in the incidence and prevalence of diabetes and pre-diabetes among Hong Kong Chinese adults, and thus show possible future trends for developing mainland China. Methods Based on a complete census of the public sector health records of 6.4 million people from 2006 to 2014, diabetes cases were ascertained using different methods including the World Health Organization ( WHO) 2011 guidelines (HbA
1c , fasting plasma glucose and glucose tolerance test), American Diabetes Association ( ADA) 2015 guidelines (plus random plasma glucose), and additionally recorded diagnosis codes and medication dispensation. Pre-diabetes was defined using ADA 2015 guidelines. Results We identified 697 201 people with diabetes (54.2% were incident cases); and 1 229 731 people with diabetes or pre-diabetes. In 2014, the overall incidence of diabetes was 9.46 per 1000 person-years [95% confidence interval ( CI): 9.38 to 9.54], and overall prevalence was 10.29% (95% CI: 10.27% to 10.32%). Incidence of diabetes decreased significantly from 2007 to 2014 (quadratic trend, P < 0.001). From 2006 to 2014, the prevalence of diabetes increased significantly in both sexes and across all age groups (quadratic trend, P < 0.001). The overall incidence of pre-diabetes in 2014 was 18.88 per 1000 person-years (95% CI: 18.76 to 18.99), and the overall prevalence of pre-diabetes was 8.90% (95% CI: 8.87% to 8.92%). Conclusions Similar to other developed western and Asian populations, diabetes (and pre-diabetes) incidence in Hong Kong Chinese appeared to have stabilized and there have been small declines during the period of observation. Ageing and survivorship will likely drive a continued increase in the prevalence of diabetes and pre-diabetes, albeit with a decelerating growth rate if past trends persist.
