Abstract

Cutaneous T-cell lymphomas (CTCL) are a group of non-Hodgkin lymphomas that typically present in the skin but can progress to systemic involvement. The optimal treatment for patients who relapse from or are refractory to systemic chemotherapy remains unclear. Romidepsin is a potent, class-I selective histone deacetylase inhibitor approved for the treatment of patients with CTCL who have had ≥1 prior systemic therapy. Here, we present a subanalysis of two phase-2 trials (NCT00106431, NCT00007345) of romidepsin in patients with CTCL who had prior treatment with systemic chemotherapy. Patients with prior chemotherapy were able to achieve durable responses to romidepsin, and response rates were similar to those in patients who were chemotherapy naïve. Overall, no new safety signals emerged in patients who had received prior chemotherapy. The data presented here suggest that romidepsin is safe and effective in patients with CTCL who received prior systemic chemotherapy. [ABSTRACT FROM AUTHOR]

Abstract

Nicotinamide adenine dinucleotide (NAD+) extends longevity in experimental organisms, raising interest in its impact on human health. De novo NAD+biosynthesis from tryptophan is evolutionarily conserved yet considered supplanted among higher species by biosynthesis from nicotinamide (NAM). Here we show that a bottleneck enzyme in de novo biosynthesis, quinolinate phosphoribosyltransferase (QPRT), defends renal NAD+and mediates resistance to acute kidney injury (AKI). Following murine AKI, renal NAD+fell, quinolinate rose, and QPRT declined. QPRT+/−mice exhibited higher quinolinate, lower NAD+, and higher AKI susceptibility. Metabolomics suggested an elevated urinary quinolinate/tryptophan ratio (uQ/T) as an indicator of reduced QPRT. Elevated uQ/T predicted AKI and other adverse outcomes in critically ill patients. A phase 1 placebo-controlled study of oral NAM demonstrated a dose-related increase in circulating NAD+metabolites. NAM was well tolerated and was associated with less AKI. Therefore, impaired NAD+biosynthesis may be a feature of high-risk hospitalizations for which NAD+augmentation could be beneficial.

186. Rehabilitating the consequentialist view of moral responsibility

Abstract

In this thesis, I defend a consequentialist view of moral responsibility, according to which justifiably (and permissibly) holding people morally responsible in a way that makes them suffer (by outwardly blaming them, punishing them, and so forth) requires -- and only requires -- that there is no alternative available that would produce better overall consequences than holding them morally responsible, where the suffering of wrongdoers is just as bad a consequence as the suffering of anyone else (all else being equal). In chapter 1, I argue that this type of view avoids a crucial objection to previous versions of consequentialism about moral responsibility. In chapter 2, I develop the view further and defend it against the objection that it intuitively seems as though it is at least sometimes permissible to blame and punish guilty people even when doing so would fail to maximize beneficial consequences. This defense primarily takes the form of an error theory for that intuition. Drawing upon a wide variety of empirical work, I argue that we have reason to doubt that the psychological mechanisms that generate that intuition are accurately representing the states of affairs to which that intuition is a response, and that we should therefore doubt the reliability of that intuition. In chapter 3, I extend this error theory to account for another intuitive objection to consequentialism: that it always seems wrong to punish the innocent or to punish the guilty disproportionately relative to their crime, even when doing so would maximize beneficial consequences. In chapter 4, I argue against practical objections to consequentialism about moral responsibility, including the claims that it is psychologically impracticable, that it requires the solution of an apparently insurmountable collective action problem, and that any ways of dealing with these first two problems would be fundamentally unfair. Chapter 5 summarizes and concludes.
