The aim of this study was to coat theophylline-loaded spherical beads with pH-sensitive polymer blends to control the resulting drug release kinetics. Various mixtures of ethylcellulose (water-insoluble) and Eudragit L (methacrylic-acid-ethyl-acrylate-copolymer; water-insoluble/water-soluble below/above pH 5.5) were used as coating materials. Two types of theophylline cores were studied:  pure drug matrixes and theophylline-layered sugar cores. Importantly, the type of core significantly affected the resulting drug release patterns. Interestingly, not only the slope, but also the shape of the release curves was altered, indicating changes in the underlying mass transport mechanisms, despite of the identical composition of the polymeric coatings. The observed differences could be explained based on the physicochemical properties of the film coatings and the swelling behavior of the beads upon exposure to the release media. Using this knowledge the development/optimization of this type of drug delivery system can be facilitated and the safety of the pharmacotherapies be improved.

Alkyl- and arylmanganese reagents react with o-chloro or o-bromoaryl ketones to give the substituted ketones in high yields. The cross-coupling reaction is performed under mild conditions (−60 to +40 °C, 30 min to 4 h) and takes place with excellent chemoselectivity.

Two vibrating tube densimeters have been used to measure the densities of two mixtures&sbd;water + diethanolamine + methanol and water + N-methyldiethanolamine + methanol&sbd;in the temperature range (283 to 353) K. The alkanolamines and methanol contents are respectively (between 20 and 50 and between 20 and 60) mass %. The experimental results have been correlated using the Redlich−Kister equation for the excess volume. A set of parameters for the Redlich−Kister equation has been determined using the densities of pure compounds and of binary systems available in the literature. The methodology has been extended to density calculations dealing with hybrid solvents.

Isothermal vapor−liquid equilibrium (VLE) data are reported for the binary systems methanol + 2,2‘-diethanolamine and water + 2,2‘-diethanolamine and for the ternary system methanol + 2,2‘-diethanolamine + water. Moreover, excess enthalpies (H^E) were measured for both binary systems with 2,2‘-diethanolamine. The data were measured by means of a computer operated static apparatus and isothermal flow calorimetry, respectively. For the pure component 2,2‘-diethanolamine, also vapor pressure data were measured using the dynamic ebulliometric method. The experimental binary data were correlated using linear or even quadratic temperature-dependent NRTL interaction parameters. The ternary data were used to validate the capability of g^E models such as NRTL for the prediction of multicomponent mixtures on the basis of binary information.

Summary The efficacy of ciprofloxacin alone and in combination with azlocillin was compared with that of azlocillin plus tobramycin in a rat model of aortic valve endocarditis due toPseudomonas aeruginosa. MICs against the infecting strain of ciprofloxacin, azlocillin and tobramycin were 0.125, 8, and 0.5 mg/l, respectively. Antimicrobials were administered 24 h after bacterial challenge and for six days. Mean peak/trough serum levels for ciprofloxacin (50 mg/kg i.v. q 12 h), azlocillin (500 mg/kg i.v. q 12 h) and tobramycin (6.5 mg/kg i.v. q 12 h) were: 10.5/0.2, 386/<16, and 6.2/<0.6 mg/l, respectively. Ciprofloxacin alone was more effective than the combination azlocillin-tobramycin in increasing survival (p<0.05), sterilizing blood (p<0.05) and valves (p<0.001), and in reducing bacterial titers in vegetations (p<0.001). Ciprofloxacin-azlocillin combination was not more effective than ciprofloxacin alone. Drug resistance was not encountered in post-treatment isolates with any therapy regimen.

La folliculite décalvante (FD) est une dermatose inflammatoire chronique du cuir chevelu menant progressivement à une alopécie cicatricielle. La PDT est rapportée dans la littérature comme alternative thérapeutique aux traitements systémiques. Il s’agit d’un traitement simple d’application mais dont l’effet principal est la douleur au cours de l’illumination. L’efficacité et la tolérance du traitement pourraient être améliorées par les systèmes d’illumination textiles à basse irradiance.

La MPV est une maladie rare correspondant à un adénocarcinome intra-épidermique. Elle atteint préférentiellement les femmes de plus de 70 ans. Elle se manifeste par des lésions érythémateuses souvent prurigineuses et/ou douloureuses, parfois difficilement identifiables. Le traitement de première intention est la chirurgie, qui induit souvent, compte tenu de la localisation, des complications esthétiques et fonctionnelles. Le taux de récidive post-opératoire est élevé (12-61 %). Ces observations mettent en lumière la nécessité de traitements alternatifs, non invasifs, notamment chez des patientes âgées et fragiles. La PDT s’est montrée efficace, avec un taux de récidive d’environ 30 % rapporté dans la littérature. Cependant, la PDT conventionnelle n’est pas adaptée à l’anatomie vulvaire dont l’illumination homogène et exhaustive est difficile, et provoque souvent des douleurs importantes notamment en zone génitale. Un dispositif lumineux textile Fluxmedicare ® a été commercialisé récemment pour le traitement des kératoses actiniques du cuir chevelu. De façon similaire, un dispositif lumineux textile Pagetex® a été conçu pour le traitement des MPV. Ce traitement est quasiment indolore grâce à sa basse irradiance. Son caractère flexible et conformable délivre une illumination homogène sur la surface à traiter. Le traitement consiste en l’application de méthyl aminolévulinate en occlusion pendant 30minutes, suivie d’une illumination de 2h30 à une fluence de 12J/cm2, et fait l’objet d’un essai clinique en cours au CHU de Lille (NCT03713203). Nous rapportons les résultats de la première patiente inclue dans cet essai.

Les kératoses actiniques (KA) sont des lésions pré-cancéreuses chroniques fréquentes chez le sujet âgé. La photothérapie dynamique (PDT) est un traitement efficace des KA ciblant des grandes surfaces cutanées telles que le cuir chevelu où de nombreuses KA forment les « champs de cancérisation ». La PDT conventionnelle (C-PDT) pose la problématique de la douleur pendant les séances. Nous avons conçu un tissu lumineux (PDT textile) (T-PDT, dispositif Phos-Istos®) composé de fibres optiques reliées à une source qui diffuse une lumière homogène sur la surface à traiter. Notre objectif principal était de comparer l’efficacité de la T-PDT à la C-PDT en termes d’efficacité à 3 mois. Un de nos objectifs secondaires était d’évaluer la douleur avec une échelle visuelle analogique (EVA 1 à 10) pour chaque zone traitée.

La prothèse MOOVIS a été introduite en 2012 pour le traitement de l’arthrose trapézo-métacarpienne (TMC). Nous rapportons les résultats cliniques et radiologiques de cet implant avec un recul moyen de 5 ans.
