Abstract

In the UK and the USA, social work academics must balance the requirements of research excellence against the time, expertise, and focus needed to ensure students are ready to practice safely and effectively with clients and community members service users and carers. However, there is variation to the extent to which academics have experience in both research and practice. This chapter presents baseline information needed to fully appreciate the nature of the problem by describing academics’ demographic characteristics, practice qualifications and experience, and research skills and activity, and concludes with a discussion of the key issues in terms of developing the future social work academic workforce.

179. Research activity among UK social work academics

Abstract

Summary The research activity of social work academics in the UK has been of interest and concern amongst academics and research funders. Multiple initiatives have been implemented to develop social work research activity, yet research by social work academics remains limited, hindered by lack of time, support infrastructures, funding and training. Through the use of a mixed-methods cross-sectional survey (N = 200) and follow-up individual interviews (N = 11), this study reports on the factors that were found to contribute to or impede the amount of time that social work academics reported spending on research. Findings The results from the survey indicated that 73% of respondents were research active. Bivariate analysis revealed that academics spent less time on research and teaching, and more time on administration than expected by their employing universities. Multivariate analysis found that less time spent on administration and teaching, more university supports, and being from a pre-1992 university predicted more time spent on research. Applications The findings indicate that the administrative burdens associated with teaching and assessment in social work education result in academics struggling to fit research into their busy lives, despite initiatives to raise the profile and productivity of social work research. Research support infrastructures and strategies should be reviewed in light of such findings.

182. Physician induced demand for C-sections: does the convenience incentive matter?

Abstract

This paper investigates whther physicians induce demand for C-sections in the days preceding leisure periods. I reject that doctors are meaningfully increasing the number of primary cesareans to accommodate their own preferences for control of deliveries around holiday Mondays. Using a sample of more than 1,300,000 deliveries, I can estimate that the induced demand due to convenience is close to zero. A small significant effect is found for women having had a previous C-section. While I replicate previous results of lower C-section rates on leisure periods, I show that they are due to the way doctors schedule planned cesareans rather than to an induced demand for reason of physicians' convenience. If induced demand occurs, the decision takes place in the labour room and is not planned in advance.

Abstract

En 2012, l'Ifsttar a souhaité débuter la phase d'inclusion de l'étude individuelle longitudinale du programme de recherche DEBATS. Cette phase d'inclusion s'est déroulée en deux étapes sur une période d'un an. La première étape a consisté à recruter des volontaires, riverains des aéroports Paris-Charles de Gaulle, Lyon Saint-Exupéry et Toulouse-Blagnac. La seconde étape a consisté en la passation d'un questionnaire en face-à-face et la réalisation de mesures physiologiques et acoustiques. Fin 2012, l'Ifsttar a confié à GfK ISL la première étape du protocole d'inclusion, c'est-à-dire le recrutement par téléphone de 1 236 volontaires acceptant de participer à l'enquête individuelle longitudinale. Le présent rapport décrit la méthodologie utilisée et les résultats obtenus par GfK ISL concernant la phase de recrutement téléphonique des 1 236 volontaires qui a été menée entre novembre 2012 et octobre 2013.

Abstract

This article proposes a new method to assess the health impact of populations exposed to fine particles (PM2.5) during their whole lifetime, which is suitable for comparative analysis of energy scenarios. The method takes into account the variation of particle concentrations over time as well as the evolution of population cohorts. Its capabilities are demonstrated for two pathways of European energy system development up to 2050: the Baseline (BL) and the Low Carbon, Maximum Renewable Power (LC-MRP). These pathways were combined with three sets of assumptions about emission control measures: Current Legislation (CLE), Fixed Emission Factors (FEFs), and the Maximum Technically Feasible Reductions (MTFRs). Analysis was carried out for 45 European countries. Average PM2.5 concentration over Europe in the LC-MRP/CLE scenario is reduced by 58% compared with the BL/FEF case. Health impacts (expressed in days of loss of life expectancy) decrease by 21%. For the LC-MRP/MTFR scenario the average PM2.5 concentration is reduced by 85% and the health impact by 34%. The methodology was developed within the framework of the EU's FP7 EnerGEO project and was implemented in the Platform of Integrated Assessment (PIA). The Platform enables performing health impact assessments for various energy scenarios.

Abstract

International audience; This paper addresses Trace-Based Reasoning (TBR) by using Case-Based Reasoning (CBR) as a descriptive framework. TBR is a reasoning paradigm in which inferences are made on specific objects called traces. Traces are sequential records of events observed and stored during an interactive process. We report two contributions. First, we propose a review of the current researches related to TBR. Then, we compare CBR and TBR. From this comparison, we show that the exploitation of traces instead of cases as knowledge sources raises very specific challenges. More precisely, new methods for defining similarity measures and for performing adaptation of traces are required. These new methods have to take into account the sequential properties of traces. In the discussion, we emphasis the benefits of using traces as a knowledge container in a reasoning process and we pinpoint promising applications of TBR.

Abstract

Background Chloroquine (CQ) was the main malaria therapy worldwide from the 1940s until the 1990s. Following the emergence of CQ-resistant Plasmodium falciparum, most African countries discontinued the use of CQ, and now promote artemisinin-based combination therapy as the first-line treatment. This change was generally initiated during the last decade in West and Central Africa. The aim of this study is to describe the changes in CQ susceptibility in this African region, using travellers returning from this region as a sentinel system. Methods The study was conducted by the Malaria National Reference Centre, France. The database collated the pfcrtK76T molecular marker for CQ susceptibility and the in vitro response to CQ of parasites from travellers’ isolates returning from Senegal, Mali, Ivory Coast or Cameroon. As a proxy of drug pressure, data regarding CQ intake in febrile children were collated for the study period. Logistic regression models were used to detect trends in the proportions of CQ resistant isolates. Results A total of 2874 parasite isolates were genotyped between 2000–2011. The prevalence of the pfcrt76T mutant genotype significantly decreased for Senegal (from 78% to 47%), Ivory Coast (from 63% to 37%), Cameroon (from 90% to 59%) and remained stable for Mali. The geometric mean of the 50% inhibitory concentration (IC50) of CQ in vitro susceptibility and the proportion of resistant isolates (defining resistance as an IC50 value > 100 nM) significantly decreased for Senegal (from 86 nM (59%) to 39 nM (25%)), Mali (from 84 nM (50%) to 51 nM (31%)), Ivory Coast (from 75 nM (59%) to 29 nM (16%)) and Cameroon (from 181 nM (75%) to 51 nM (37%)). Both analyses (molecular and in vitro susceptibility) were performed for the 2004–2011 period, after the four countries had officially discontinued CQ and showed an accelerated decline of the resistant isolates for the four countries. Meanwhile, CQ use among children significantly deceased in this region (fixed effects slope = −0.3, p -3). Conclusions An increase in CQ susceptibility following official withdrawal of the drug was observed in travellers returning from West and Central African countries. The same trends were observed for molecular and in vitro analysis between 2004-2011and they correlated to the decrease of the drug pressure.

Abstract

International audience; OBJECTIVES: The study objectives were to describe the investigation and management of an imipenem-resistant Acinetobacter baumannii outbreak that occurred in the 15-bed ICU of a tertiary care teaching hospital (Brest, France), during the summer 2008. PATIENTS AND METHODS: Patients harboring an imipenem-resistant A. baumannii strain were defined as case patients. We described case occurrence and steps taken to control the outbreak: contact isolation, reinforcement of hygiene procedures, unit shutdown decision, unit disinfection, and reopening. We also made a case control study and a cost analysis of the outbreak management. RESULTS: During a 10-day period, five patients were positive for a single clone of imipenem-resistant oxa-23 A. baumannii. Four patients presented with ventilation-acquired pneumonia and one was asymptomatic. The first two patients died one day after the first swab which led to the identification of A. baumannii. No additional case was noted in the ICU or in other hospital units after deciding to close the ICU. The cost of outbreak management was estimated at 264,553 euros. The case control study identified several factors associated with infection or colonization: length of stay in the ICU, chronic respiratory disease, number of previous antibiotic classes used, duration of ventilation, prone position, echocardiography, and presence of a nasogastric tube. CONCLUSION: This outbreak occurred during the summer period requiring the shutdown of the ICU and inducing a considerable cost. Rapid reactions of the ICU staff during the outbreak enabled to limit the epidemic.
