Purpose. Critically important to effectively treating and managing sexual offending is the identification or validation of an offender's deviant sexual interests as the nature of their sexual interests is what demarcates repetitive sexual offenders from non-offenders and lower risk offenders. As an alternative or verification to self-report or phallometric measures, focus has turned to attention-based measures. These measures assess sexual content-induced delay (SCID), a specific form of attentional bias associated with preferred sexual content (images or text). Viewing time (VT) and choice reaction time (CRT) were developed and utilized to assess sexual interest via SCID (Geer & Bellard, 1996) and examine the measures' clinical utility via estimates of sensitivity and specificity. Method. Participants were 44 youth non-sexual offenders, 60 university students, and 22 adult sexual offenders. Differences between groups were examined on various sub-scores and receiver operator characteristic curves provided information on clinical utility. Results. The VT and CRT measures produced subtest scores with high reliability in all three samples. There were significant differences in VT between the adult sexual offenders and the youth non-sexual offenders, but not between the youth non-sexual offenders and the university students. Some of the VT subtests demonstrated good clinical utility in their ability to differentiate adult heterosexual sexual offenders from non-sexual offenders (e.g., area under the curve (AUC) = 0.87 female mature images, 0.88 male child images). Interestingly, the VT and CRT measures provided significantly different results. Conclusion. The results of this study provide further evidence that measures of SCID are accurate and are useful as indications of sexual interest. PurposeThe aims of this study were twofold: to investigate if a compensatory head posture due to nystagmus causes long-term neck problems or adversely effects quality of life and to survey ophthalmologists on their opinions and management of these patients.MethodsA case-controlled study was carried out to assess the range of neck movements in patients with compensatory head posture due to congenital nystagmus. Exclusion criteria included known neck problems, vertebrobasilar insufficiency, and age less than 16 years. Neck movements were assessed using an inclinometer. Quality of life and disability was assessed using the American Academy of Orthopaedic Surgeons' Cervical Spine Questionnaire. One hundred and fifty consultant ophthalmologists throughout the United Kingdom were surveyed via a postal questionnaire.ResultsThe range of motion in these patients (n=20) was limited when compared to matched controls especially for lateral flexion (P=0.001) and extension (P=0.003). However, despite limited movement, patients did not perceive a disability and there was no adverse effect on quality of life. In all 55% of ophthalmologists believed compensatory head posture due to nystagmus leads to long-term neck problems. About 46% had personal experience of patients with long-term neck problems due to nystagmus. Basic modifications included doubling the thickness of the spring steel used In the penile ring, the use or larger strain gauges with a greater strain range, and a different method for protecting the gauge assembly. Four pedophiles, one sado-masochist, and one rapist were treated by varying different aspects of a single paradigm of masturbatory conditioning. In the basic paradigm, subjects masturbate to ejaculation while speaking their fantasies aloud. Weekly blocks of five trials with a deviant theme are alternated with weekly blocks where the theme is nondeviant. Evaluation sessions to assess levels of response to deviant and nondeviant slide stimuli are typically conducted later in each treatment day. The usual result is an increase in nondeviant arousal with a concomitant decrease in deviant arousal.Variations in the masturbation procedure included shortening the treatment week from 5 days to 2 or 3, alternating themes on a daily rather than weekly basis, and omitting the use of deviant imagery. In the evaluation sessions variations included the preparation of individually tailored audiotaped fantasies precisely expressive of the subjects' interests, the use of videotaped simulations of rape, assault, and consenting intercourse, and obtaining numerous measures of the subjects' cognitive changes during the treatment process. All six subjects showed the effect of decreasing their deviant sexual arousal and increasing their nondeviant arousal. The results of a prospective study to assess the value of padding eyes following cataract surgery are reported. A group of eyes dressed with a petroleum jelly mesh, gauze pad, and cartella shield were compared with a group dressed only with a cartella shield. Study of the eyes and lids before and after operation, including the taking of lid margin and fornix swabs for culture, suggested that there was no significant difference in the bacterial presence between the two groups. The eyes dressed with a cartella shield alone appeared less prone to discharge. The relative merits of different types of dressing are discussed. [ABSTRACT FROM PUBLISHER]
