152. Validación de la versión española: Expectativas y conocimiento recibido del paciente hospitalizado

Objetivo: evaluar la validez y la fiabilidad de la versión española del instrumento Expectativas y conocimiento recibido del paciente hospitalizado (ECPH/CRPH) en pacientes con osteoartritis intervenidos de artroplastia total de rodilla. Material y métodos: el proceso de validación siguió las recomendaciones internacionales: validez de contenido, validez de constructo, validez de criterio y fiabilidad. Un total de 248 pacientes que debían intervenirse de artroplastia total de rodilla en 5 hospitales públicos españoles contestaron la ECPH antes de ser hospitalizados y 220 pacientes contestaron el CRPH al darles el alta hospitalaria. Resultados: el análisis factorial confirmó las dimensiones de la escala original para el conocimiento recibido, pero no en la escala para las expectativas. La correlación de las dimensiones de la ECPH con la Escala de Opinión de la Salud de Krantz (KHOS) y la correlación de las dimensiones de la CRPH con la Escala de Acceso a la Información (AKS) indicaron la validez de criterio mostrando unas relaciones positivas. Los resultados señalaron buena fiabilidad con valores de alfa Cronbach elevados. Conclusión: el proceso de validación indica que la versión española del ECPH/CRPH es un instrumento válido y fiable para medir las expectativas y el conocimiento recibido en pacientes quirúrgicos en el contexto español.

- Abstract
Este Trabajo de Fin de Grado es una propuesta de intervención dirigida hacia la escuela rural, más concretamente al Colegio Rural Agrupado (CRA) El Jalón. El CRA cuenta con aulas en las localidades de Medinaceli, Monteagudo de las Vicarias y la cabecera que se encuentra en Arcos de Jalón. En este caso trabajaremos en base al área de Ciencias Sociales adaptada a los cursos de quinto y sexto de educación primaria, en donde trabajaremos la Edad Media junto al entorno más cercano trabajando de manera conjunta. Por ello la propuesta se recoge dentro de un cuadernillo de actividades el cual poseerán los alumnos, llamado “Descubrimos la Edad Media”. Con esta propuesta pretendemos motivar al alumnado a trabajar en equipo, conocer su entorno más cercano y ampliar su conocimiento acerca de la historia, trabajando de manera interdisciplinar y realizando diferentes investigaciones a través de las cuales podrán ir completando su aprendizaje de manera cooperativa. Todo ello en base a la Ley Orgánica para la Mejora de la Calidad Educativa para los alumnos de sexto y la Ley Orgánica de Modificación de la LOE para los alumnos de quinto., Grado en Educación Primaria

Este Trabajo Final de Grado, presenta en primer lugar una investigación sobre la historia de los CRA, su definición, su contexto, su origen, los tipos de escuelas rurales, sus características, su estructura, su organización, sus ventajas y desventajas. Además de tratar el origen, la definición y la finalidad de las aulas multisensoriales. Esto se debe a que se pretende diseñar una propuesta de intervención didáctica adaptada a la escuela rural, en el CRA “La Huecha”, en la provincia de Zaragoza. Dicha propuesta trabaja a través de actividades sensoriales sostenibles, para no solo mejorar el desarrollo integral de todo tipo de alumnado, sino que también conocer y concienciar sobre los alumnos con Necesidades Educativas Especiales a toda la comunidad educativa. - Abstract
Existing epidemiological evidence regarding the potential role of (poly)phenol intake in prostate cancer (PCa) risk is scarce and, in the case of flavonoids, it has been suggested that their intake may increase PCa risk. We investigated the associations between the intake of the total and individual classes and subclasses of (poly)phenols and the risk of PCa, including clinically relevant subtypes. The European Prospective Investigation into Cancer and Nutrition (EPIC) cohort included 131,425 adult men from seven European countries. (Poly)phenol intake at baseline was assessed by combining validated center/country-specific dietary questionnaires and the Phenol-Explorer database. Multivariable-adjusted Cox proportional hazards models were used to estimate the hazard ratios (HR) and 95% confidence intervals (CI). In total, 6939 incident PCa cases (including 3501 low-grade and 710 high-grade, 2446 localized and 1268 advanced, and 914 fatal Pca cases) were identified during a mean follow-up of 14 years. No associations were observed between the total intake of (poly)phenols and the risk of PCa, either overall (HRlog2 = 0.99, 95% CI 0.94–1.04) or according to PCa subtype. Null associations were also found between all classes (phenolic acids, flavonoids, lignans, and stilbenes) and subclasses of (poly)phenol intake and the risk of PCa, overall and according to PCa subtype. The results of the current large prospective cohort study do not support any association between (poly)phenol intake and PCa incidence.

- Abstract
A partir de la situación vivida a causa de la COVID-19 en la que tuvimos que adaptarnos a la docencia virtual, hemos descubierto el gran potencial que las herramientas disponibles en nuestro entorno nos ofrecen para poder mejorar la interacción con los alumnos y alumnas; los cuales demandan este tipo de herramientas, ya que al ser nativos tecnológicos se sienten más cómodos en su uso y les facilita un aprendizaje más dinámico. Este hecho nos llevó a solicitar dos proyectos de innovación de forma consecutiva (161 y 302) en los que se virtualizó el contenido práctico de las asignaturas de Parasitología e Inmunología de Grado, respectivamente. Dado el éxito de estas iniciativas, en este proyecto nos planteamos ampliar el uso de este tipo de herramientas en las dos asignaturas del Máster de Análisis Sanitarios en los que participan miembros de este proyecto. Al igual que en los casos anteriores, estos desarrollos se pondrán a disposición del alumnado del Máster, a través del Campus Virtual, para completar y facilitar su formación en las asignaturas de Diagnóstico Parasitológico y Micológico y Diagnóstico Inmunológico. Por otro lado, pensamos que estas herramientas podrían suponer una alternativa óptima para una posible futura docencia virtual, siendo también aplicable a los alumnos que por diferentes motivos no puedan asistir a la docencia presencial, evitando de esta forma, que nuestros alumnos y alumnas pierdan la experiencia práctica de los protocolos realizados en las diferentes técnicas de laboratorio. Dado que nos encontramos en la formación de alumnos de posgrado, en un Máster eminentemente profesionalizante, consideramos indispensable que adquieran una sólida base tanto en el diagnóstico de parásitos, como en la correcta realización de técnicas inmunológicas que posteriormente se completarán en su período práctico. La extraordinaria preservación de los restos dentales en el registro fósil supone una importante fuente de información sobre las características del espécimen al que pertenece. Con el fin de resolver la cuestión acerca de la existencia de una o dos especies dentro del género Kuseracolobus, se ha realizado un análisis enfocado en la variación que presentan las medidas de las cúspides molares. Para poder realizar la comparativa se ha estudiado una muestra de 30 ejemplares de una sola especie, Colobus guereza, y otra segunda muestra de 60 especímenes pertenecientes a dos especies diferentes Macaca fascicularis y Macaca mulatta. Con ello se estudiará el coeficiente de variación para una y para dos especies y podrá ser contrastado con los resultados de las cúspides molares de Kuseracolobus, para así esclarecer la existencia de una o más especies dentro de este género.

La computación cuántica tiene un inmenso potencial para resolver problemas clásicamente intratables aprovechando las propiedades únicas de los cúbits. Sin embargo, la escalabilidad de las arquitecturas cuánticas sigue siendo un desafío significativo. Para abordar este problema, se proponen arquitecturas cuánticas de múltiples núcleos. No obstante, la realización de dichas arquitecturas plantea múltiples desafíos en hardware, algoritmos y la interfaz entre ellos. En particular, uno de estos desafíos es cómo particionar de manera óptima los algoritmos para que se ajusten dentro de los múltiples núcleos. Esta tesis presenta un enfoque novedoso para la partición escalable de circuitos en arquitecturas cuánticas de múltiples núcleos utilizando Aprendizaje Profundo Reforzado. El objetivo es superar a los algoritmos metaheurísticos existentes, como el algoritmo de particionamiento de FGP-rOEE, en términos de precisión y escalabilidad. Esta investigación contribuye al avance tanto de la computación cuántica como de las técnicas de particionamiento de gráficos, ofreciendo nuevos conocimientos sobre la optimización de los sistemas cuánticos. Al abordar los desafíos asociados con la escalabilidad de las computadoras cuánticas, abrimos el camino para su implementación práctica en la resolución de problemas computacionalmente desafiantes., Quantum computing holds immense potential for solving classically intractable problems by leveraging the unique properties of qubits. However, the scalability of quantum architectures remains a significant challenge. To address this issue, multi-core quantum architectures are proposed. Yet, the realization of such multi-core architectures poses multiple challenges in hardware, algorithms, and the interface between them. In particular, one of these challenges is how to optimally partition the algorithms to fit within the cores of a multi-core quantum computer. This thesis presents a novel approach for scalable circuit partitioning on multi-core quantum architectures using Deep Reinforcement Learning. The objective is to surpass existing meta-heuristic algorithms, such as FGP-rOEE's partitioning algorithm, in terms of accuracy and scalability. This research contributes to the advancement of both quantum computing and graph partitioning techniques, offering new insights into the optimization of quantum systems. By addressing the challenges associated with scaling quantum computers, we pave the way for their practical implementation in solving computationally challenging problems.

163. Estudio de la variabilidad molecular en poblaciones mediterráneas del cangrejo marino Liocarcinus depurator

[EN] The marine crab Liocarcinus depurator is a species that inhabits the muddy bottoms of the shelf and the upper part of the continental slope of the Iberian Peninsula coasts and shows a certain commercial interest. Adults do not apparently show large-scale displacements, but their larvae, which are epipelagic, can be transported great distances by surface ocean currents. However, these same currents, by generating eddies and gyres, can produce oceanographic barriers that hinder or even prevent gene flow between populations. For this reason, L. depurator is a good model species to analyse the effect of oceanographic fronts on population connectivity. For this study, the molecular variability of a fragment of the Cytochrome Oxidase subunit I (COI) mitochondrial gene was analysed in six Iberian peninsular Mediterranean populations sampled in 2021: West of the Alboran Sea, Alicante, Valencia, Ebro Delta, Central and North of Catalonia. The 91 sequences obtained belonged to 19 different haplotypes. All of them were classified into two well-defined haplogroups: ATL, dominant in Atlantic-influenced waters, and MED, in Mediterranean-influenced waters. A previously undetected haplotype (Ldep_77) was found in the western Mediterranean, but it was found in Greek waters. For the COI gene, there seemed to be a differentiation between the samples from the Alboran Sea and those from the Levantine/Catalan area. In addition, when analysing the distribution of haplotypes in our populations, it was observed that the effect of the Almeria-Oran Front was significant, confirming the importance of this oceanographic barrier in reducing gene flow. This information is relevant for the proper definition of MPAs (Marine Protected Areas) as well as for the management of fishery resources, [ES] El cangrejo marino Liocarcinus depurator es una especie que habita en los fondos fangosos de la plataforma y de la parte superior del talud continental de las costas peninsulares ibéricas y que presenta interés comercial. Los adultos no tienen grandes desplazamientos, pero sus larvas, epipelágicas, pueden ser desplazadas a grandes distancias mediante las corrientes marinas superficiales. No obstante, estas mismas corrientes, al generar remolinos y giros, pueden producir barreras oceanográficas que dificultan o incluso impiden el flujo génico entre las poblaciones. Por esta razón, L. depurator es una buena especie modelo para analizar el efecto de los frentes oceanográficos sobre la conectividad poblacional. Para este estudio se analizó la variabilidad molecular de un fragmento del gen mitocondrial Citocromo Oxidasa subunidad I (COI) en seis poblaciones del Mediterráneo peninsular muestreadas en el año 2021: Oeste del Mar de Alborán, Alicante, Valencia, Delta del Ebro, Centro y Norte de Catalunya. Las 91 secuencias obtenidas pertenecían a 19 haplotipos diferentes. Todas ellas se clasificaron en dos haplogrupos bien definidos: ATL, dominante en aguas de influencia atlántica, y MED, en las de influencia mediterránea. Se encontró un haplotipo (Ldep_77) no detectado previamente en el Mediterráneo occidental, pero sí en aguas de Grecia. Para el gen COI, parecía existir una diferenciación entre las muestras del mar de Alborán y las de la zona levantino/catalana. - Abstract
Menarche marks the beginning of female reproductive cycle and constitutes an important milestone of female growth. Age at menarche varies according to genetic and socio-environmental factors, so it stands as an interesting indicator of biological wellbeing of the population. Based on an initial sample of 445 women from Valparaiso, born in 1851-1895, we examine the impact of socio-environmental factors on age of menarche in a historical context characterized by a strong GDP growth and intense, although uneven, urban development. Although results indicate a tendency for menarche to present itself at increasingly younger ages, this tendency is not statistically significant (p > .05). Factors like rural/urban origin, size of the household and intra-household stress do not seem to affect age of menarche in the sample, characterized by a low socio-economic status. Urban penalty phenomena, typical of early processes of modernization, could be partially explaining the results obtained., La menarquia señala el inicio del periodo reproductivo y constituye un importante marcador del crecimiento humano femenino. La edad en la que se presenta depende de la interacción de factores genéticos y socioambientales, por lo que resulta un interesante indicador del bienestar biológico de las poblaciones. Con una muestra inicial de 445 mujeres residentes en Valparaíso y nacidas entre los años 1851-1895, evaluamos la incidencia de factores socioambientales en la edad de la menarquia en un contexto histórico caracterizado por un fuerte incremento del Producto Interno Bruto y un intenso, pero inarmónico desarrollo urbano. Aunque los resultados muestran una tendencia al adelanto de la edad de menarquia, este no resulta estadísticamente significativo (p > .05). El origen rural/urbano, el tamaño familiar y el estrés intrafamiliar no parecen incidir en la edad de menarquia de la población de la muestra. El fenómeno de penalización urbana podría explicar en parte los resultados.

- Abstract
The aim of this study was to assess the effects of a mixture of four dietary fibers on obese rats. Four groups of male Wistar rats were fed with either standard chow (STD) or cafeteria diet (CAF) and were orally supplemented with either fibre mixture (2 g kg−1 of body weight) (STD+F or CAF+F groups) or vehicle (STD+VH or CAF+VH groups). We studied a wide number of biometric, biochemical, transcriptomic, metagenomic and metabolomic variables and applied an integrative multivariate approach based on multiple factor analysis and Pearson's correlation analysis. A significant reduction in body weight, adiposity, HbA1c and HDL-cholesterol serum levels, and colon MPO activity was observed, whereas cecal weight and small intestine length:weight ratio were significantly increased in F-treated groups compared to control animals. CAF+F rats displayed a significant enhancement in energy expenditure, fat oxidation and fresh stool weight, and a significant reduction in adiponectin and LPS serum levels, compared to control group. Animals in STD+F group showed reduced serum LDL-cholesterol levels and a significant reduction in total cholesterol levels in the liver compared to STF+VH group. The intervention effect was reflected at the metabolomic (i.e., production of short-chain fatty acids, phenolic acids, and amino acids), metagenomic (i.e., modulation of Ruminococcus and Lactobacillus genus) and transcriptomic (i.e., expression of tight junctions and proteolysis) levels. Altogether, our integrative multi-omics approach highlights the potential of supplementation with a mixture of fibers to ameliorate the impairments triggered by obesity in terms of adiposity, metabolic profile, and intestinal health.

La indiscutible relevancia del espacio en «Casa tomada» de Julio Cortázar proviene, fundamentalmente, de su naturaleza a la vez física y psíquica. En realidad, la casa es un correlato de la psique del narrador, hasta el punto de que los ruidos invasores encarnan su pulsión incestuosa. Este artículo recoge la correspondencia entre las partes de la casa y las partes de la psique según las dos tópicas freudianas y ofrece una propuesta inadvertida por la crítica: la sección trasera pertenece al superyó, la sección delantera al ego, y la habitación de Irene al ello. Esta nueva interpretación se apoya en el análisis cuatro aspectos obviados por la bibliografía existente: (1) los mecanismos defensivos del narrador, (2) el origen de los ruidos invasores, (3) el significado de los gritos nocturnos de Irene y (4) las diferencias entre los dos personajes en cuanto a la voz narrativa. Un análisis espacial que tenga en cuenta todos estos elementos resuelve en gran medida la ambigüedad del cuento.

173. Quantitative plasma profiling by 1H NMR-based metabolomics: impact of sample treatment

Introduction: There is evidence that sample treatment of blood-based biosamples may affect integral signals in nuclear magnetic resonance-based metabolomics. The presence of macromolecules in plasma/serum samples makes investigating low-molecular-weight metabolites challenging. It is particularly relevant in the targeted approach, in which absolute concentrations of selected metabolites are often quantified based on the area of integral signals. Since there are a few treatments of plasma/serum samples for quantitative analysis without a universally accepted method, this topic remains of interest for future research. Methods: In this work, targeted metabolomic profiling of 43 metabolites was performed on pooled plasma to compare four methodologies consisting of Carr-Purcell-Meiboom-Gill (CPMG) editing, ultrafiltration, protein precipitation with methanol, and glycerophospholipid solid-phase extraction (g-SPE) for phospholipid removal; prior to NMR metabolomics analysis. The effect of the sample treatments on the metabolite concentrations was evaluated using a permutation test of multiclass and pairwise Fisher scores. Results: Results showed that methanol precipitation and ultrafiltration had a higher number of metabolites with coefficient of variation (CV) values above 20%. G-SPE and CPMG editing demonstrated better precision for most of the metabolites analyzed. However, differential quantification performance between procedures were metabolite-dependent. For example, pairwise comparisons showed that methanol precipitation and CPMG editing were suitable for quantifying citrate, while g-SPE showed better results for 2-hydroxybutyrate and tryptophan. Discussion: There are alterations in the absolute concentration of various metabolites that are dependent on the procedure. Considering these alterations is essential before proceeding with the quantification of treatment-sensitive metabolites in biological samples for improving biomarker discovery and biological interpretations. The study demonstrated that g-SPE and CPMG editing are effective methods for removing proteins and phospholipids from plasma samples for quantitative NMR analysis of metabolites. However, careful consideration should be given to the specific metabolites of interest and their susceptibility to the sample treatment procedures. These findings contribute to the development of optimized sample preparation protocols for metabolomics studies using NMR spectroscopy

Research coupling human nutrition and sustainability concerns is a rapidly developing field, which is essential to guide governments’ policies. This critical and comprehensive review analyzes indicators and approaches to “sustainable healthy diets” published in the literature since this discipline’s emergence a few years ago, identifying robust gauges and highlighting the flaws of the most commonly used models. The reviewed studies largely focus on one or two domains such as greenhouse gas emissions or water use, while overlooking potential impact shifts to other sectors or resources. The present study covers a comprehensive set of indicators from the health, environmental and socio-economic viewpoints. This assessment concludes that in order to identify the best food option in sustainability assessments and nutrition analysis of diets, some aspects such as the classification and disaggregation of food groups, the impacts of the rates of local food consumption and seasonality, preservation methods, agrobiodiversity and organic food and different production systems, together with consequences for low-income countries, require further analysis and consideration.

The use of DNA damage as marker of oxidative stress, metabolic dysfunction and age-related diseases is debated. The present study aimed at assessing the level of DNA damage (evaluated as DNA strand-breaks, endogenous and oxidatively-induced DNA damage) in a group of older subjects with intestinal permeability enrolled within the MaPLE (Gut and Blood Microbiomics for Studying the Effect of a Polyphenol-Rich Dietary Pattern on Intestinal Permeability in the Elderly) intervention trial, to evaluate its association with clinical, metabolic and dietary markers. DNA damage in peripheral blood mononuclear cells was assessed by the comet assay in 49 older subjects participating in the study. Clinical and metabolic markers, markers of inflammation, vascular function and intestinal permeability were determined in serum. Food intake was estimated by weighted food diaries. On the whole, a trend towards higher levels of DNA damage was observed in men compared to women (p = 0.071). A positive association between DNA damage and clinical/metabolic markers (e.g., uric acid, lipid profile) and an inverse association with dietary markers (e.g., vitamin C, E, B6, folates) were found and differed based on sex. By considering the importance of DNA stability during aging, the results obtained on sex differences and the potential role of dietary and metabolic factors on DNA damage underline the need for further investigations in a larger group of older adults to confirm the associations found and to promote preventive strategies.

Injection molding is widely used to produce plastic components with large lot size. However, guaranteeing consistency and quality of parts in injection molding is challenging. Failures occur due to variation during injection cycles. Thus, real-time detection of failures will have a high impact on quality and productivity. This article is focused on Smart tHermoplastic injectION (SHION), a cloud-based Digital Twin supported by AI-based control of process parameters. Process parameters and their interrelationship with quality failure were studied and used to generate models for real-time prediction of part quality. Two injection manufacturing lines in industry were chosen for data acquisition, implementation, and validation of the Digital Twin. While SHION successfully predicted faulty products in real time, adoption of traditional Cloud-centric Internet of Things approaches poses unforeseen practical challenges such as exposure to risk of losing data due to network issues and prohibitive cost of regularly transferring a large amount data to Cloud services.

Objetivo: Determinar el impacto que ha tenido la pandemia COVID-19 en profesionales sanitarios, en términos de cambios laborales y de salud mental. Método: 1-Estudio transversal y 2-Estudio longitudinal prospectivo sobre profesionales de la salud que trabajaron de forma directa con pacientes afectados por la pandemia COVID-19 durante el periodo comprendido entre marzo-junio de 2020 en un hospital de tercer nivel de Barcelona. Se recogieron datos basales (julio-noviembre de 2020) y de seguimiento (noviembre de 2020-marzo de 2021). Se recogieron datos relacionados con el puesto de trabajo y hábitos tóxicos con cuestionarios ad hoc, y de estrés percibido, estrés postraumático, depresión y ansiedad mediante cuestionarios validados. Resultados: En el grupo basal (estudio transversal) participaron 90 profesionales, 76% enfermeras. Más de un tercio estuvo de baja o tomó fármacos para gestionar el estrés. La mitad de los fumadores aumentó el consumo de tabaco, y uno de cada 5 aumentó el consumo de alcohol. Se asocia de forma significativa el cambio de turno habitual con la depresión, el perfil profesional con la ansiedad, y el estrés percibido con la edad. En el estudio longitudinal, el grupo de seguimiento (n=64) muestra niveles elevados de estrés y ansiedad, que se mantienen o incluso aumentan en el tiempo de forma significativa. Conclusiones: Los cambios estructurales acontecidos en el hospital durante la primera ola de la pandemia COVID-19 tuvieron un fuerte impacto en los profesionales, presentando muchos de ellos un aumento en los hábitos tóxicos, así como una salud mental alterada que se mantiene en el tiempo.

Among the various (poly)phenols that are being sold as such or as part of a more complex mixture, hydroxytyrosol (HT) is the only one that bears a European Food Safety Authority health claim. Therefore, several HT-based products are being developed and sold and it becomes necessary to evaluate its accessibility following ingestion. Twenty-one volunteers were recruited for a randomized, crossover, placebo-controlled, and double-blind intervention study. We performed a Latin square design: after one-week washout, i.e. olive-free diet, subjects were randomly assigned to the placebo (maltodextrin), 5, or 25 mg/day HT group. Twenty-four hour urine samples were collected after the intervention week, and baseline urines were collected the week before the study and during periods of washout. The results show that HT given as the foremost component of a nutraceutical preparation is bioavailable and is recovered in the urine chiefly as sulphate-3′.

"European cities have experienced an intense growth in tourism and other related mobilities. This pressure on urban centers has increased the public’s perception of the need to design more sustainable tourism policies that deal with the various associated dilemmas. This article compares the tourism policies of eleven European cities considering what governments have introduced, or avoided, in their tourism policy documents. The objective is to observe the extent to which these actions directly related to sustainability are being incorporated. The results indicate that local tourism policies are moving away from a model of public action based only on considering the economic impact of tourism and are starting to work with a wider range of associated impacts. In this scenario, the ideas of balance and sustainability find a much clearer accommodation. This research presents a pre‑crisis tourism framework that can be challenged during post‑Covid19 transitions."

The main objective of Activity 3 (Models specification and implementation) of GRAPEVINE project is to define several models for the identification of diseases and plagues, and the way to mitigate their effects. These models represent: a physical model about the behaviour and evolution of diseases / plagues, identification of the diseases / plagues in crops, accurate weather forecasting, crops phenology and a deep learning model for disease / plague classification and decision making. This deliverable (in the first version v1) focuses on defining the basic models (phenological, disease and meteorological) that will be used in the implementation of the other tasks of Activity 3. Introduction Liposarcomas are malignant neoplasms that originate in fatty mesenchymal tissue and are the most common histological subtype of soft tissue sarcomas. They account for 1% of solid tumors in adults and are located mainly in the extremities, with the retroperitoneum being the second most frequent location. Given the rarity and aggressiveness of these tumors, it is advisable to establish a multidisciplinary approach towards them in order to improve the prognosis of patients. Clinical case Patient who was operated upon on a scheduled and outpatient basis for a left inguinal hernia. During surgery, a preperitoneal tumor was identified that was removed and revealed a high-grade dedifferentiated liposarcoma. In a second surgical act, a tumor mass of about 10 centimeters in length was found, lodged in the left pelvis and contacting sigma, which extended into the inguinal duct and scrotal pouch. Finally, compartment surgery and a left orchiectomy with end-to-end colo-sigmoid anastomosis were performed. Background healthy dietary patterns have been associated with lower risk for age-related cognitive decline. However, little is known about the specific role of dietary fibre on cognitive decline in older adults. Objective this study aimed to examine the association between dietary fibre and cognitive decline in older adults and to assess the influence of genetic, lifestyle and clinical characteristics in this association. Design and participants the Invecchiare in Chianti, aging in the Chianti area study is a cohort study of community-dwelling older adults from Italy. Cognitive function, dietary and clinical data were collected at baseline and years 3, 6, 9 and 15. Our study comprised 848 participants aged ≥ 65 years (56% female) with 2,038 observations. Main outcome and measures cognitive decline was defined as a decrease ≥3 units in the Mini-Mental State Examination score during consecutive visits. Hazard ratios for cognitive decline were estimated using time-dependent Cox regression models. Results energy-adjusted fibre intake was not associated with cognitive decline during the 15-years follow-up (P > 0.05). However, fibre intake showed a significant interaction with Apolipoprotein E (APOE) haplotype for cognitive decline (P = 0.02). In participants with APOE-ɛ4 haplotype, an increase in 5 g/d of fibre intake was significantly associated with a 30% lower risk for cognitive decline. No association was observed in participants with APOE-ɛ2 and APOE-ɛ3 haplotypes. Conclusions and relevance dietary fibre intake was not associated with cognitive decline amongst older adults for 15 years of follow-up. Nonetheless, older subjects with APOE-ɛ4 haplotype may benefit from higher fibre intakes based on the reduced risk for cognitive decline in this high-risk group.
