This study aimed to assess and compare the extent and pattern of psychosocial dysfunction and family burden in schizophrenia and obsessive-compulsive disorder, and to identify interrelationships between the two variables in these two disorders. First-degree relatives/spouses of 35 schizophrenic and 30 OCD patients were interviewed using the Dysfunction Analysis Questionnaire (DAQ) and the Family Burden Interview Schedule (FBIS). Global score and scores in vocational, personal, familial and cognitive areas on the DAQ, and global score, subjective score, and scores on items such as financial burden, disruption of family-routine, disruption of family leisure and disruption of family interactions on the FBIS were significantly higher in the schizophrenic group. Dysfunction in social area was comparable in two groups. OCD group showed a significant positive correlation between dysfunction and all areas of family burden except physical and mental health. Schizophrenic group showed a significant positive correlation between dysfunction and disruption of family interaction. The implications of these findings are discussed.

This study attempted to differentiate suicide attempters from completers based on their psycho-socio-demographic profile. Suicide attempters admitted to Govt. Medical College, Kozhikode during the period of 1st January to 31st December 2001 were evaluated using a specially designed proforma and for the same period, data of completers were collected using retrospective chart review. The parametric and non-parametric variables between these two groups were compared using appropriate statistics. Both attempters and completers in male gender were in the age of forties and females were in thirties. There was over representation of young females, married and housewives among the victims. In male and female victims, hanging was the commonest method followed by poisoning in males and self-immolation in females. Poisoning was the commonest mode in male attempters and drug over dose in females. Many of the differences in the psycho-socio-demographic profile of suicide attempters and completers reported from western countries could not be replicated in the present study. However comparison of our findings with studies from India and other developing countries shows many similarities.

The tetrapeptide Pro-Glu-Leu-Leu forms the 94-97 fragment of C globin in sea cucumber. 2% Butanediol dimethacrylate-cross linked polystyrene (2% BDDMA-PS), which had been optimized, was used for the synthesis of the tetrapeptide Pro-Glu-Leu-Leu. The peptide was synthesized by using Boc-amino acid strategy. The peptide purity was checked by RP-HPLC and the peptide was characterized by (1)H NMR spectroscopy and amino acid analysis. Conformation of the peptide was studied by 1D- and 2D- homonuclear (1)H NMR, in DMSO-d6 at 300K. The conformation of the synthetic tetrapeptide (extended backbone conformation) is not in agreement with that in C globin.

Self inflicted injury to the eyes including self-enucleation is a rare phenomenon. The authors describe a case of bilateral self-enucleation in a patient with paranoid schizophrenia. A review of literature shows that self-enucleation is correlated with moral, religious and psychodynamic paradigms and various unconscious mechanisms have been proposed to explain this unusual behaviour.

Background: Mortality and morbidity related to AIDS have decreased among HIV-infected patients taking highly active anti-retroviral therapy (HAART), but previous studies may have been confounded by other changes in treatment., Objective: To assess the benefit of HAART in patients with advanced AIDS and anemia., Design: Prospective, multicenter cohort study., Setting: The Viral Activation Transfusion Study (VATS), with enrollment from August 1995 through July 1998 and follow-up through June 1999., Patients: 528 HIV-infected patients with cytomegalovirus (CMV) seropositivity or disease who were receiving a first red blood cell transfusion for anemia., Measurements: In a person-year analysis of follow-up before and after initiation of HAART, Poisson regression was used to calculate crude rate ratios and rate ratios adjusted for CD4 count, HIV RNA level, calendar period, time on study, sex, ethnicity, and injection drug use., Results: At baseline, patients had a median CD4(+) lymphocyte count of 0.015 x 10(9) cell/L, median plasma HIV RNA level of 4.8 log(10) copies/mL, and median hemoglobin concentration of 73 g/L. Use of HAART increased from 1% of active patients in January 1996 to 79% of active patients in January 1999. The crude death rate was 0.24 event/person-year among patients taking HAART and 0.88 event/person-year among those not taking HAART (rate ratio, 0.26; adjusted rate ratio, 0.38; P < 0.001 for both comparisons). Rates of non-CMV disease were 0.15 event/ person-year after HAART and 0.45 event/person-year before HAART (crude rate ratio, 0.34 [ P < 0.001]; adjusted rate ratio, 0.66 [ P < 0.05]). Rates of CMV disease were 0.10 event/person-year after HAART and 0.25 before HAART (crude rate ratio, 0.42 [ P < 0.01]; adjusted rate ratio, 1.01 [ P > 0.2]). Results were similar in patients with baseline CD4(+) lymphocyte counts less than 0.010 x 10(9) cells/L., Conclusions: The data support an independent reduction in mortality and opportunistic events attributable to HAART, even in patients with very advanced HIV disease. However, patients with CMV infection or disease may not have a reduction in new CMV events due to HAART.

Plica neuropathica also known as plica polonica is an uncommon condition in which the hairs of the scalp in a localised area is compacted into irregularly twisted, irreversibly entangled plaits. A review of literature shows several predisposing factors figuring in plica neuropathica like kinky hair, use of shampoos, febrile illness and psychological disturbances. The authors describe a case of plica neuropathica in a paranoid schizophrenic patient who developed irreversible tangling of scalp hair.

The Viral Activation Transfusion Study (VATS) was a randomized trial that compared leukocyte-reduced transfusions with unfiltered red blood cell transfusions in HIV and cytomegalovirus (CMV) antibody-positive patients with anemia who were undergoing their first blood transfusion. The relations of the baseline qualitative and quantitative polymerase chain reaction (PCR) measures of plasma CMV viremia, HIV RNA, CD4(+) cell counts, and quality of life in these study subjects were examined. The 511 study subjects had a median CD4(+) cell count equal to 15 cells/mm3, and 110 (21.5%) had CMV viremia by qualitative assay. In multivariate models, frequency of positive qualitative CMV increased with decreasing CD4(+) cell counts (p =.04 trend), higher HIV RNA (p <.001), and a history of CMV disease (p <.001). Quantitative CMV PCR were performed on the 110 qualitative assay-positive study subjects. Median CMV viral load was 1780 copies/ml. In multivariate regression models, lower CD4(+) cell count (p =.03), and a history of CMV disease (p <.001) correlated with the level of CMV load. HIV RNA load and CMV load were not correlated. A lower Karnofsky score was associated with both the presence and quantity of CMV DNA.

Several platelet glycoprotein polymorphisms have been associated with an increased risk of myocardial infarction (MI) in studies that included predominantly men. In a population-based sample of 68 Caucasian women < 45 years old with non-fatal MI and 346 demographically similar control subjects, we found an increased risk of MI among women who possessed at least one copy of the glycoprotein IIb Ser843 allele compared with those lacking the Ser843 allele (odds ratio 1.85; 95% confidence interval = 1.03-3.33). The increased risk was present only in subgroups of women who smoked cigarettes, had hypercholesterolaemia or who had a family history of early onset MI. The Ser843 variant of glycoprotein IIb may be associated with an increased risk of MI in young women with other cardiovascular risk factors. Additional studies involving larger numbers of subjects are needed to confirm this preliminary finding.

Several platelet membrane glycoprotein polymorphisms have been identified as potential risk factors for cardiovascular disease. Recently a nucleotide -5T/C dimorphism in the translation initiation site (Kozak sequence) of the platelet glycoprotein Ibalpha (GPIbalpha) gene was associated with increased platelet surface levels of the GPIb-IX-V receptor complex. The role of this GPIbalpha Kozak sequence polymorphism in the occurrence of arterial thrombotic disease is unknown. We performed genotype analysis of the Kozak sequence polymorphism of GPIbalpha in a population-based study of 18- to 44-year-old women with nonfatal myocardial infarction (MI) (n = 78), nonfatal stroke (n = 106), and 384 demographically similar female control subjects. Analysis of -5T/C genotypes revealed that at least one copy of the C allele was present in 14.1% of MI cases, 23.6% of stroke cases, and 23.7% of controls. The age-adjusted odds ratio for MI in women carrying at least one copy of the C allele was 0.53 (95% confidence interval [CI] 0.27-1.05). The age-adjusted odds ratio for stroke in women carrying at least one copy of the C allele was 0.99 (95% CI 0.59-1.65). Analyses stratified by stroke type (ischemic, hemorrhagic) yielded similar results. In conclusion, young women carrying the C allele of the Kozak sequence polymorphism of GPIbalpha are not at increased risk of MI or stroke. Paradoxically, the C allele may even be associated with a reduced risk of MI in this population. This finding requires further study.

Despite US Public Health Service (USPHS) recommendations for antimicrobial prophylaxis for patients with advanced human immunodeficiency virus (HIV) disease, the proportion of patients who receive prophylaxis is not known. We measured the prevalence of antimicrobial prophylaxis use, and treatment for HIV wasting at baseline among 531 patients with advanced HIV disease enrolled in a multicenter randomized trial of red blood cell transfusion. Use of antimicrobial prophylaxis and treatment for wasting in the 30 days before enrollment was ascertained in patients eligible for primary prophylaxis, secondary prophylaxis, or both, according to USPHS guidelines. There was high utilization of primary and secondary Pneumocystis carinii pneumonia prophylaxis, variability in primary Mycobacterium avium complex prophylaxis by center, and low use of primary cytomegalovirus prophylaxis. Treatment of wasting was more common in white than nonwhite patients and in patients with HIV disease who lived in the region west of the Mississippi River of the United States versus those whose lived in the eastern region.

Risperidone is an atypical antipsychotic with broad spectrum of antipsychotic activity and lower potential for extrapyramidal side effects at therapeutic doses. This case report illustrates the development of tardive dyskinesia with therapeutic dose of risperidone in a paranoid schizophrenic patient who was not on any antipsychotic medication previously.

Several studies of patients infected with human immunodeficiency virus (HIV) type 1 have suggested that women have lower plasma HIV-1 RNA levels than men, even when controlling for CD4 T cell levels. A cross-sectional analysis was performed in 494 patients (21% of whom were women) who enrolled in a prospective study of anemic HIV-1-infected patients requiring transfusion. The median CD4 T cell count and plasma HIV-1 RNA levels were 15 cells/microL and 4.83 log(10) copies/mL (67,350 copies/mL), respectively. In unadjusted analyses, women had slightly higher mean log HIV-1 RNA titers than men (0.19 log(10) higher copies/mL; 95% confidence interval, -0.05 to 0.44; P=.11). Adjustment for CD4 T cell count, race or ethnicity, injection drug use, and age yielded a smaller sex difference (0.13 log(10) copies/mL higher in women; P=.28). In this population of patients with very advanced HIV disease, there is no evidence that women have lower HIV-1 RNA levels than men.

Background and Purpose: A number of studies have examined the relationship between genetic platelet glycoprotein variants and early-onset atherothrombotic disease, particularly acute myocardial infarction. Data on the association of these genetic susceptibility markers with ischemic stroke are more limited, and their role in hemorrhagic stroke has not been previously examined., Methods: We performed genotype analysis for 5 common diallelic platelet glycoprotein polymorphisms in a population-based study of 78 white women aged <45 years with arterial stroke (36 ischemic cases and 42 hemorrhagic cases) and 346 demographically similar control subjects., Results: The 807T variant of glycoprotein Ia was associated with a 2-fold increased risk of ischemic stroke (age-adjusted odds ratio [OR]=2.24; 95% CI=0.99 to 5.06). The Met(145) allele of glycoprotein Ibalpha was associated with a trend toward an increased risk of ischemic stroke that was more pronounced in the homozygous state (OR=10.36), but the CI is extremely wide because of the small numbers of subjects (95% CI=1.43 to 79.34). Homozygosity for the Ser(843) allele of the glycoprotein IIb was associated with an approximately 5-fold increased risk of ischemic stroke among subgroups of women who carried a diagnosis of hypertension or diabetes (OR=4.51; 95% CI=1.01 to 20.13) or had elevated plasma homocysteine levels (OR=5.94; 95% CI=1.53 to 23.05). The genotype distributions for all 5 platelet glycoprotein polymorphisms were similar among hemorrhagic stroke cases and controls., Conclusions: Several inherited platelet glycoprotein variants may be associated with an increased risk of ischemic stroke in young women. These associations seemed to be confined to women with other cardiovascular risk factors. Additional studies involving larger numbers of subjects are needed to confirm these preliminary findings.

CD4(+) T cells from patients with human immunodeficiency virus (HIV) infection undergo apoptosis at an increased rate, which leads to their depletion during disease progression. Both the Fas-Receptor (Fas-R) and interleukin-1beta (IL-1beta)-converting enzyme (ICE; caspase 1) appear to play a role in the mechanism of apoptosis of CD4(+) lymphocytes. Although Fas-R is upregulated on both CD4(+) and CD8(+) cells in HIV-infected patients, results from our laboratory and others indicate that, in patients with advanced disease, CD4(+) cells preferentially express ICE. Protease inhibitors have successfully halted the progression of HIV disease and increased CD4(+) T counts. In this study, we examined the effect of protease inhibitors on Fas-R (CD95), ICE (caspase 1) expression, apoptosis, and cell death in CD4(+) T cells of (1) HIV-infected patients who were receiving protease inhibitors, and (2) normal and patient CD4(+) T cells cultured with a protease inhibitor in vitro. Fifteen patients with advanced HIV disease on treatment showed dramatically decreased CD4(+) T-cell ICE expression, diminished apoptosis, and increased numbers of CD4(+) cells within 6 weeks of institution of protease inhibitor therapy, and before down-modulation of Fas-R (CD95) expression was evident. To determine the role of HIV infection, we studied the effect of ritonavir, a protease inhibitor, on normal and patient cells in vitro. Stimulated and unstimulated normal CD4(+) T cells, cultured with protease inhibitor, demonstrated markedly decreased apoptosis and ICE expression (P =. 01). While Fas-R expression was not significantly altered during short-term culture by such treatment, Fas-Ligand (Fas-L) membrane expression of phytohemagglutinin (PHA)-stimulated blood lymphocytes was decreased by protease inhibitor. In the presence of ritonavir, CD4(+) T cells from HIV-infected patients showed similar changes in ICE intracellular levels without alteration of Fas expression. In conclusion, protease inhibitors appear to decrease CD4(+) T-cell ICE expression and apoptosis before they affect Fas-R expression in HIV-infected patients. This action was independent of HIV infection, as similar effects were seen in CD4(+) T cells from normal controls. Some of the benefit of protease inhibitors may be related to modification of programmed cell death, which increases CD4(+) T-cell number. Whether this is due to directly to the changes effected in the caspase system remains to be determined.

Abacavir (1592U89) is a nucleoside analog reverse transcriptase inhibitor that has been demonstrated to have selective activity against human immunodeficiency virus (HIV) in vitro and favorable safety profiles in mice and monkeys. A phase I study was conducted to evaluate the safety and pharmacokinetics of abacavir following oral administration of single escalating doses (100, 300, 600, 900, and 1,200 mg) to HIV-infected adults. In this double-blind, placebo-controlled study, subjects with baseline CD4+ cell counts ranging from < 50 to 713 cells per mm3 (median, 315 cells per mm3) were randomly assigned to receive abacavir (n = 12) or placebo (n = 6). The bioavailability of the caplet formulation relative to that of the oral solution was also assessed with the 300-mg dose. Abacavir was well tolerated by all subjects; mild to moderate asthenia, abdominal pain, headache, diarrhea, and dyspepsia were the most frequently reported adverse events, and these were not dose related. No significant clinical or laboratory abnormalities were observed throughout the study. All doses resulted in mean abacavir concentrations in plasma that exceeded the mean 50% inhibitory concentration (IC50) for clinical HIV isolates in vitro (0.07 microgram/ml) for almost 3 h. Abacavir was rapidly absorbed following oral administration, with the time to the peak concentration in plasma occurring at 1.0 to 1.7 h postdosing. Mean maximum concentrations in plasma (Cmax) and the area under the plasma concentration-time curve from time zero to infinity (AUC0-infinity) increased slightly more than proportionally from 100 to 600 mg (from 0.6 to 4.7 micrograms/ml for Cmax; from 1.0 to 15.7 micrograms.h/ml for AUC0-infinity) but increased proportionally from 600 to 1,200 mg (from 4.7 to 9.6 micrograms/ml for Cmax; from 15.7 to 32.8 micrograms.h/ml for AUC0-infinity. The elimination of abacavir from plasma was rapid, with an apparent elimination half-life of 0.9 to 1.7 h. Abacavir was well absorbed, with a relative bioavailability of the caplet formulation of 96% versus that of an oral solution (drug substance in water). In conclusion, this study showed that abacavir is safe and is well tolerated by HIV-infected subjects and demonstrated predictable pharmacokinetic characteristics when it was administered as single oral doses ranging from 100 to 1,200 mg.

We compared the efficacy of a 400-mg once-weekly dosage versus a 200-mg daily dosage of fluconazole for the prevention of deep fungal infections in a multicenter, randomized, double-blind trial of 636 human immunodeficiency virus-infected patients to determine if a less intensive fluconazole regimen could prevent these serious but relatively infrequent complications of AIDS. In the intent-to-treat analysis, a deep fungal infection developed in 17 subjects (5.5%) randomly assigned to daily fluconazole treatment and in 24 (7.7%) given weekly fluconazole during 74 weeks of follow-up (risk difference, 2.2%; 95% confidence interval [CI], -1.7% to 6.1%). Thrush occurred twice as frequently in the weekly versus daily fluconazole recipients (hazard ratio, 0.59; 95% CI, 0.40-0.89), and in a subset of patients evaluated, fluconazole resistance was infrequent. Fluconazole administered once weekly is effective in reducing deep fungal infections in patients with AIDS, but this dosage is less effective than the 200-mg-daily dosage in preventing thrush.

This prospective study was aimed at a qualitative and quantitative analysis of life events in depression and mania and to find out any correlation between the severity of life events and magnitude of illness. Forty two patients with first episode mania and thirty patients with first episode depression, satisfied the DSM III R Diagnostic Criteria were interviewed using Presumptive Stressful Life Events Scale, Hamilton Depression Rating Scale for depression, Modified Manic State Rating Scale for mania and a questionnaire concerning sociodemographic and clinical profile. Life events were experienced by 87% of depressives and 81% of manics. There was no difference in the quantitative and qualitative profile of life events except that desirable life events and events like major physical illness or injury were significantly higher in depressives. Financial loss/problems were the commonest events experienced by the two groups. There was no con-elation between the severity of life events and magnitude of illness. The implications of these findings are discussed in Indian context.

This prospective study was conducted to assess the prevalence of substance abuse comorbidity and its impact on psycho pathology and pattern of remission in mania. Hundred consecutive inpatients with a diagnosis of manic episode were followed up for three months using structured clinical interview schedule for DSM III R (SCID-P), DSM IIIR check list for mania. Bech-Rafaelsan Mania Rating Scale and a questionnaire concerning sociodemographic and clinical profile. The prevalence of substance abuse comorbidity was 52%. Substance abusers were significantly younger, had early age of onset of mood disorder, unmarried, unemployed, had more dysphoric and irritate mood states and grandiose and persecutory delusions. Risk factor analysis showed substance abuse as being consistently associated with poor outcome in mania.

Hundred consecutive suicide attempters admitted in the Medical Intensive Care Unit(MlCU) of Christian Medical College, Vellore during the period December 1991 to December 1992 were evaluated in detail with respect to all psycho-socio-demographic variables. Instruments used included a specially designed sociodemographic proforma, Gurmeet Singh's Presumptive Stressful Life Event Scale, Suicide Intent Questionnaire and DSM III R Diagnostic Criteria. Young males constituted major part of the sample. Organophosphorus poisoning was the commonest mode of suicide attempt. More than 90% had psychiatric diagnosis. Adjustment disorder was the commonest psychiatric diagnosis followed by major depression and alcohol abuse/dependence. Many of the risk factors reported earlier was found to be operating in older age group only. Among males the commonest mode of attempt was organophosphorus poisoning whereas in females it was drug over dose and native poisoning. The implication of these findings are discussed in the context of prevention and further management strategies.

The chemokine receptors CXCR4 and CCR5 have been identified as major coreceptors for HIV-1 entry into CD4+ T cells. The majority of primary HIV-1 isolates in early disease use CCR5 as a coreceptor, whereas during disease progression with the emergence of syncytium-inducing viruses, CXCR4 is also used. We performed a cross-sectional study in which we evaluated the expression of two HIV-1 coreceptors, CCR5 and CXCR4, in whole blood samples taken from HIV-1-infected and uninfected individuals. We demonstrate that CXCR4 on CD4+ and CD8+ T cells, and CD14+ monocytes is significantly down-regulated, and CCR5 expression on CD4+ T cells is up-regulated in HIV-infected individuals compared with uninfected controls. Coreceptor expression correlated with the level of cellular activation in vivo in both HIV-infected and uninfected individuals, with CXCR4 being expressed predominantly on quiescent (HLA-DR-) T cells and CCR5 being expressed predominantly on activated (HLA-DR+) T cells. Lower expression of CXCR4 and higher expression of CCR5 on CD4+ T cells correlated with advancing disease. In addition, a tendency for greater activation of CXCR4+CD4+ T cells in patients with advanced disease was observed. Patients who harbored syncytium-inducing viruses, however, could not be distinguished from those who harbored nonsyncytium-inducing viruses based on the level of CD4+ T cell activation or chemokine receptor expression.

We report a case of crossed aphasia following a vascular insult of the right hemisphere in a middle aged dextral man with subsequent evolution to pure word deafness. Development of pure word deafness during recovery from crossed Wernickes aphasia is an extremely rare clinical entity. Probably this is the first Indian report of this entity.

This case report describes the induction of extrapyramidal symptoms with fluoxetine, which is reported to be a rare phenomenon. A dopamine blockade at the nigrostriatal level primarily mediated by serotonin has been proposed as the probable mechanism.

Neuroleptic induced neurological side effects were compared over a period of one year (1991-1992) in 45 schizophrenics and age and sex matched 42 mood disorder patients, diagnosed as per DSM-III-R criteria. Prevalence of dystonia was equally common in both groups. Pseudoparkinsonism was significantly high in female mood disorder patients, akathisia in middle aged mood disorder patients and tradive dyskinesia in mood disorder patients. Factors like age of onset, time of onset and mean dose of antipsychotics (chlorpromazine equivalent) did not show any significant difference. The findings are discussed in relation to their practical application and suggestions for future studies are outlined.

We describe a case of isolated Candida albicans arthritis in a non-intravenous drug-abusing, human immunodeficiency virus positive patient. This presentation is extremely unusual without other systemic features or risk factors for candidemia, as Candida septic arthritis normally reflects disseminated infection. In terms of diagnosis, this case also highlights the potential utility of magnetic resonance imaging for early detection of osteomyelitis.

We report a case of Guillain-Barré syndrome (GBS) associated with Rocky Mountain spotted fever (RMSF). The patient initially presented with fever, rash, and an altered mental status, and he responded to therapy with intravenous doxycycline; serological data confirmed RMSF. Concurrent physical findings and results of cerebrospinal fluid analysis and nerve conduction studies were compatible with GBS. Although neurological complications of RMSF are common, no known associations between RMSF and GBS were found in the literature. An association between possible infection with Rickettsia conorii and GBS was reported in a French study in 1968. To our knowledge, this is the first reported association between RMSF and GBS.

Objective: The significance of pneumatosis intestinalis in adults with AIDS is unknown. Adults with AIDS are predisposed to pneumatosis intestinalis. The purpose of this study is to determine the clinical significance and to characterize the imaging appearance of this finding in patients with AIDS., Materials and Methods: In just over 3 years at our institution, pneumatosis intestinalis in adults with AIDS was detected in six cases. Medical records of those cases were retrospectively analyzed for clinical history, laboratory data, and surgical and pathologic findings. CT scans and abdominal radiographs were analyzed for location and severity of pneumatosis, presence of portal venous gas, pneumoperitoneum, and cystic versus linear gas., Results: Five of the six patients with AIDS-associated pneumatosis intestinalis were managed conservatively or medically without surgical procedures and were discharged from the hospital with resolved or decreasing gastrointestinal complaints. The other patient underwent an exploratory laparotomy because of free intraperitoneal and retroperitoneal air; however, no bowel perforation was found at surgery., Conclusion: Pneumatosis intestinalis is a late-stage phenomenon in adult patients with AIDS that characteristically involves the cecum or right colon. Pneumatosis in patients with AIDS may be an indolent abnormality and does not necessarily constitute a surgical emergency or a sign of impending bowel necrosis.

We have examined the changing demographics of adult intussusception and implicate human immunodeficiency virus (HIV)- and acquired immune deficiency syndrome (AIDS)-associated gastrointestinal pathology as risk factors for intussusception in young adults. The clinical index of suspicion for intussusception should be raised for an HIV-positive young adult with intermittent crampy abdominal pain. Over a 10-year period, eight cases of adult intussusception were diagnosed at our institution, and we reviewed the diagnostic computed tomography (CT) scans and records of these patients to correlate them with radiological studies, clinical history, surgical findings, laboratory studies, pathologic analysis, and outcome. Three of the eight patients with adult intussusception had AIDS, all diagnosed by CT scans. Their average age was 41 years, whereas average age of the non-HIV-associated patients was 63. These findings suggest that HIV- and AIDS-associated gastrointestinal pathology provide lead points for intussusception and are significant risk factors for intussusception in young adults. We reviewed the five previously reported cases of AIDS and intussusception and conclude that intussusception should be a diagnostic consideration in an HIV-positive young adult with abdominal complaints. It is clear that AIDS-associated intussusception is a real clinical problem and that CT is an effective method of diagnosing it.

Fifteen prepubertal male rats (age 30 days) were divided into three groups of five each: Group 1, hypothyroid (Tx)--rats were thyroidectomized at day 30; Group 2, T4 (L-thyroxine) replacement therapy (Tx+T4)--rats were thyroidectomized at day 30 and treated daily i.m. with T4 (6 micrograms/100 g body weight/day) for 30 days from day 31 to day 60 post-thyroidectomy (age 90 days); Group 3, control--rats were sham-operated and treated with vehicle. The rats from all groups were killed on day 61 post-thyroidectomy or post-sham operation (age 91 days). The serum levels of testosterone, T4 and T3 decreased in the Tx group (p < 0.001). In the Tx+T4 group the levels of T4 and T3 were restored to control values, whereas testosterone levels remained lower than in the control group. Hypothyroidism caused various changes in the levels of epididymal phospholipids and neutral lipids. These were restored differentially or were altered further in the Tx+T4 group. The number and forward motility of spermatozoa, recovered from the cauda epididymis, were decreased significantly (p < 0.01) in the Tx group and were not restored in the Tx+T4 group. This study shows that chronic hypothyroidism, induced at prepuberty and continued for 60 days, causes various changes in the lipid composition of the caput and cauda epididymis and lowers the quality and quantity of spermatozoa in the cauda epididymis. T4 therapy for 30 days, especially during the postpubertal period, did not restore the quality or quantity of spermatozoa and caused differential changes in the levels of epididymal lipids, depending upon the region.(ABSTRACT TRUNCATED AT 250 WORDS)

Objective: Although the prevalence of intracranial lymphoma is high among patients with AIDS, current imaging techniques are not reliable for differentiating this tumor from other common nonneoplastic lesions, such as those seen in toxoplasmosis. The purpose of this study was to prospectively investigate the use of 201Tl single-photon emission computed tomography (SPECT) in identifying intracranial lymphoma in patients with AIDS., Subjects and Methods: Thirteen patients with AIDS and intracranial masses underwent 201Tl imaging with a three-headed SPECT camera. Sic of the 13 were subsequently proved to have lymphoma. Studies were interpreted prospectively as showing tumor if uptake of 201Tl was increased in the region where the lesion was seen on MR images. A lesion-to-nonlesion uptake ratio (counts/pixel) was calculated retrospectively., Results: The SPECT images of six patients were interpreted prospectively as showing no lymphoma. Uptake ratios in these six patients were 0.77-1.95 (mean, 1.45). In each, tumor was excluded as a final diagnosis (four had toxoplasmosis, one had progressive multifocal leukoencephalopathy, and one had venous angioma). Among the seven patients with SPECT images interpreted as showing lymphoma, six were later proved to have lymphoma (uptake ratio: mean, 3.65; range, 2.95-4.30; p < .005). The SPECT findings in the seventh patient were classified as false-positive for tumor on the basis of the prospective interpretation of the images; three concurrent infections were found at autopsy. The uptake ratio in this patient was low (1.81), suggesting that quantification might have diagnostic usefulness for improving accuracy., Conclusion: This preliminary study indicates that 201Tl SPECT might be a useful, noninvasive method for differentiating intracranial lymphoma from nonneoplastic lesions in patients with AIDS.

Lipopolysaccharide (LPS) in the cell wall of the P. aeruginosa is a major factor in the pathogenicity and virulence of this organism. Immunotype-specific serum antibodies to this LPS antigen (ALPS) are usually elevated with the onset of the bacteremia, act as opsonins, are protective in high levels, and are significantly associated with improved survival. In the present study, the ability of 11 patients with AIDS to mount a specific ALPS response with the onset of P. aeruginosa bacteria was evaluated prospectively. Of the 11 patients with AIDS only one had a substantial ALPS response, six mounted only a marginal ALPS response, and four had no ALPS response to the infecting strain of the P. aeruginosa. These data suggest most patients with AIDS do not exhibit a marked antigen-specific humoral response at the onset of P. aeruginosa bacteremia; this has important prognostic and therapeutic significance, as high ALPS titers are associated with survival.

Kaposi's sarcoma (KS) is common in individuals infected with the human immunodeficiency virus (HIV-1). Although KS is frequently indolent, it can also be aggressive and life-threatening, especially in patients with pulmonary involvement (PKS), who have poor survival rates when untreated. In an effort to develop treatment regimens for PKS that would prolong life or reduce clinical symptoms, we used combination chemotherapy to treat 18 patients who had AIDS and PKS; 13 (72%) of them had a history of previous opportunistic infections. Doxorubicin, bleomycin, vinblastine, vincristine, actinomycin D, and dacarbazine were used in 3-week cycles with concomitant zidovudine, zalcitabine (dideoxycytidine), or didanocine (dideoxyinosine). Antiviral therapy was continued with chemotherapy. A partial or complete response to chemotherapy was obtained in 15 of the 18 patients (83%), as characterized by clearing of infiltrates on chest films and resolution of dyspnea and cough. Only 2 patients had opportunistic infections during treatment. Median survival was 9 months; patients who received dose reductions in less than three cycles of chemotherapy survived more than 1 year. Most deaths were related to unresponsive PKS. These results indicate that patients with symptomatic PKS can be safely and effectively treated with combination chemotherapy while receiving myelosuppressive drugs such as zidovudine. Such patients receive substantial relief from dyspnea and cough. Survival for treated patients exceeds survival for untreated historical controls.
