Abstract

Tyrosine kinase inhibitors are used as first‐line treatment for non‐small cell lung cancer (NSCLC) patients harboring driver mutations in EGFR, ALK, ROS1, and BRAF. Currently, standard molecular testing approaches help identify single genes for such targetable driver mutations in NSCLC; however, next‐generation sequencing (NGS)‐based genetic profiling provides a more comprehensive approach and is hence strongly recommended. This case study aimed to highlight the benefits of NGS‐based tests for the diagnosis of complex EGFR L858R mutations. A patient was diagnosed with stage IVB NSCLC using a government‐approved in vitro diagnostic test and was noted to have a high programmed death‐ligand 1 tumor proportion score. This patient was treated with pembrolizumab monotherapy followed by cisplatin and pemetrexed owing to the lack of actionable driver gene mutations, including EGFR mutations. After treatment failure, a sample harvested from the same transbronchial lung biopsy specimen (formalin‐fixed and paraffin‐embedded) used for the initial EGFR test was subjected to NGS‐based broad genetic profiling. The NGS‐based test identified an EGFR L858R‐K860I cis doublet mutation; however, neither of these mutations was identified upon initial molecular testing. The patient was then successfully treated with a third‐generation EGFR‐tyrosine kinase inhibitor, osimertinib. In this study, we delved deeper into the realm of L858R and K860I mutations in NSCLC and discuss the potential causes underlying our initial negative diagnosis. Furthermore, this study highlighted the additional benefits of replacing typical molecular tests with NGS‐based broad profiling approaches. Abstract

BACKGROUND: Successful restoration of epicardial coronary artery patency by primary percutaneous coronary intervention (PPCI) for ST-elevation myocardial infarction (STEMI) does not always lead to adequate reperfusion at the microvascular level. AIMS: This study sought to investigate the association between lipid-rich coronary plaque identified by near-infrared spectroscopy combined with intravascular ultrasound (NIRS-IVUS) and microvascular obstruction (MVO) detected by cardiac magnetic resonance imaging (MRI) after PPCI for STEMI. METHODS: We investigated 120 patients with STEMI undergoing PPCI. NIRS-IVUS was used to measure the maximum lipid core burden index in 4 mm (maxLCBI4 mm) in the infarct-related lesions before PPCI. Delayed contrast-enhanced cardiac MRI was performed to evaluate MVO one week after PPCI. RESULTS: MVO was identified in 40 (33%) patients. MaxLCBI4 mm in the infarct-related lesion was significantly larger in the MVO group compared with the no-MVO group (median [interquartile range]: 745 [522-853] vs 515 [349-698], p600 was the optimal cut-off value to predict MVO (Youden index=0.44 and area under the curve=0.71) with a sensitivity of 75% and a specificity of 69%. CONCLUSIONS: Lipid content measured by NIRS in the infarct-related lesions was associated with the occurrence of MVO after PPCI in STEMI.

Abstract

Substances containing two π‐congested aromatic systems are attractive targets in synthetic studies as well as efforts designed to explore the unique properties that originate from π‐congestion. Since the time of the computational studies by Schleyer and Warner, the concept of stacked aromaticity created by the encounter of two antiaromatic rings has received much attention. A structure containing π‐congestion between two antiaromatic norcorrole rings was prepared by Shinokubo et al. In contrast, questions about what happens when two aromatic rings are closely stacked have remained unanswered. Specifically, the electronic consequences of stacking of two aromatic π‐conjugated systems have not been fully evaluated. To the best of our knowledge, only one computational study has been performed by Herges, the results of which suggest that [2.2]paracyclophane, possessing short distance (<3.00 Å) between two aromatic ring planes, has a paratropic ring current between two the benzene rings. This observation is consistent with the conclusion that stacked antiaromaticity exists within the π‐congested space between aromatic rings. Herein, we investigated the stacked antiaromaticity of highly π‐congested anthracene dimers using anthracenophane, which possesses a short interplane carbon–carbon distance of ca. 2.80 Å. The absorption spectrum of this substance contains a weak broad band from 450 to 550 nm that is attributed to the HOMO–LUMO transition. These properties exist in planar cyclooctatetraene derivatives that have 8π‐electron antiaromaticity. The results of nucleus‐independent chemical shift and anisotropy of the induced current density calculations indicate that a relatively weak antiaromatic character exists between the central six‐membered rings of the two anthracene moieties in anthracenophane. In addition, an attempt to enhance the stacked antiaromaticity of anthracenophane using pressure to enhance π‐congestion was unsuccessful. [ABSTRACT FROM AUTHOR]
