154. Spatial analysis of areas likely to harbor American cutaneous leishmaniasis in Seropédica, Rio de Janeiro State, Brazil

The goal of the present study was to evaluate the spatial distribution of American cutaneous leishmaniasis (ACL) in Seropédica, Rio de Janeiro State, Brazil according to the favorability of occurrence. The Vista-SAGA Geo-environmental Analysis System (Sistema de Análise Geoambiental Vista-SAGA) was used to create a database (theme maps) and perform environmental analyses, signatures and combinations. The degrees of likelihood were obtained by evaluating soil use and plant cover; soil type and geomorphology (geo-environmental factors); and altitude and slope (topographic factors). The factors used in the analyses were selected based on results from the literature and according to the technical feasibility of their inclusion in the databases. Weights and grades were attributed to the factors and their categories, respectively, according to their relevance to the life cycle of parasites from genus Leishmania and parasite vectors. Based on the results of the environmental analyses, areas were classified and grouped to represent the spatial distribution of favorability. Areas were classified as very unfavorable, unfavorable, somewhat favorable, favorable and very favorable for ACL occurrence. The results of the signature analyses showed that the municipality included 6.81% very favorable areas, 65.14% favorable areas, 17.98% somewhat favorable areas, 2.39% unfavorable areas and 0.07% very unfavorable areas for ACL occurrence. The signature analyses of the results and theme maps were able to locate and characterize the ecosystem profiles in terms of risk of ACL occurrence, indicating that rural, peri-urban and urban areas of the municipality of Seropédica may be classified as favorable for the occurrence of this disease. The results of the present study may be used to support management decisions made by authorities responsible for ACL control and prevention in Seropédica. In addition, geoprocessing was found to be a low-cost alternative for data integration in areas where it is difficult to perform surveys.

- Abstract
Regenerative agriculture has recently attracted much attentiondue to its ability to provide a wide range of advantages to producers and the environment, (Schreefel et al., 2020). With this type of farming, the goal is to improve the health of the soil and increase its ability to support natural functions like animal habitat, water filtration, and food security (Rhodes, 2017).Regenerative agriculture differs from organic farming in the fact it emphasizes restoring soil health and the ecosystem that surrounds it, rather than focusing only on soil health and the problem of soil degradation. Beyond just producing goods, restorative agriculture aims to establish a strong and resilient farming system. This essay will examine the advantages of regenerative agriculture's ecological functions.

Determining SARS-CoV-2 immunity is critical to assess COVID-19 risk and the need for prevention and mitigation strategies. We measured SARS-CoV-2 Spike/Nucleocapsid seroprevalence and serum neutralizing activity against Wu01, BA.4/5 and BQ.1.1 in a convenience sample of 1,411 patients receiving medical treatment in the emergency departments of five university hospitals in North Rhine-Westphalia, Germany, in August/September 2022. 62% reported underlying medical conditions and 67.7% were vaccinated according to German COVID-19 vaccination recommendations (13.9% fully vaccinated, 54.3% one booster, 23.4% two boosters). We detected Spike-IgG in 95.6%, Nucleocapsid-IgG in 24.0%, and neutralization against Wu01, BA.4/5 and BQ.1.1 in 94.4%, 85.0%, and 73.8% of participants, respectively. Neutralization against BA.4/5 and BQ.1.1 was 5.6- and 23.4-fold lower compared to Wu01. Accuracy of S-IgG detection for determination of neutralizing activity against BQ.1.1 was reduced substantially. We explored previous vaccinations and infections as correlates of BQ.1.1 neutralization using multivariable and Bayesian network analyses. Given a rather moderate adherence to COVID-19 vaccination recommendations, this analysis highlights the need to improve vaccine-uptake to reduce the COVID-19 risk of immune evasive variants. The study was registered as clinical trial (DRKS00029414).

Aims We tested the hypothesis that epicardial adipose tissue (EAT) quantification improves the prediction of the presence of obstructive coronary artery disease (CAD) in patients presenting with acute chest pain to the emergency department. Methods and results Within this prospective observational cohort study, we included 657 consecutive patients (mean age 58.06 ± 18.04 years, 53% male) presenting to the emergency department with acute chest pain suggestive of acute coronary syndrome between December 2018 and August 2020. Patients with ST-elevation myocardial infarction, haemodynamic instability, or known CAD were excluded. As part of the initial workup, we performed bedside echocardiography for quantification of EAT thickness by a dedicated study physician, blinded to all patient characteristics. Treating physicians remained unaware of the results of the EAT assessment. The primary endpoint was defined as the presence of obstructive CAD, as detected in subsequent invasive coronary angiography. Patients reaching the primary endpoint had significantly more EAT than patients without obstructive CAD (7.90 ± 2.56 mm vs. 3.96 ± 1.91 mm, P < 0.0001). In a multivariable regression analysis, a 1 mm increase in EAT thickness was associated with a nearby two-fold increased odds of the presence of obstructive CAD [1.87 (1.64–2.12), P < 0.0001]. Adding EAT to a multivariable model of the GRACE score, cardiac biomarkers and traditional risk factors significantly improved the area under the receiver operating characteristic curve (0.759–0.901, P < 0.0001). Conclusion Epicardial adipose tissue strongly and independently predicts the presence of obstructive CAD in patients presenting with acute chest pain to the emergency department. Our results suggest that the assessment of EAT may improve diagnostic algorithms of patients with acute chest pain.

Videolaryngoscopy (VL) is a promising tool to provide a safe airway during cardiopulmonary resuscitation (CPR) and to ensure early reoxygenation. Using data from the German Resuscitation Registry, we investigated the outcome of non-traumatic out-of-hospital cardiac arrest (OHCA) patients treated with VL versus direct laryngoscopy (DL) for airway management.We analysed retrospective data of 14,387 patients from 1 January 2018 until 31 December 2021 (VL group, n = 2201; DL group, n = 12186). Primary endpoint was discharge with cerebral performance categories one and two (CPC1/2). Secondary endpoints were the rate of return of spontaneous circulation (ROSC), hospital admission, hospital admission with ongoing cardiopulmonary resuscitation, 30-day survival/ hospital discharge and airway management complications. We used multivariate binary logistic regression analysis to identify the effects on outcome of known influencing variables and of VL vs DL.The multivariate regression model revealed that VL was an independent predictor of CPC1/2 survival (OR = 1.34, 95% CI = 1.12-1.61, p = 0.002) and of hospital discharge/30-day survival (OR = 1.26, 95% CI = 1.08-1.47, p=0,004).VL for endotracheal intubation (ETI) at OHCA was associated with better neurological outcome in patients with ROSC. Therefore, the use of VL for OHCA offers a promising perspective. The effect of four levels of inclusion (0; 450; 900 and 1,350g T-1) of Ractopamine hydrochloride was assessed concerning weight gain, feed conversion, dry matter intake, carcass traits and quality of castrated male cattle meat in confinement. Forty Nellore steers were used, with an average age of 26 months and initial average weight of 423.4±2.7kg, in a randomized block experimental design with four treatments and ten replications. The diet was fixed with the ratio of forage to concentrate dry matter of 75.3:24.7. A Linear positive effect observed was the inclusion of Ractopamine on daily weight gain and linear negative effect on feed conversion, highlighting the improvements with the increasing inclusion of Ractopamine hydrochloride. In relation to carcass traits, the linear effect was negative for fat thickness and no differences were found regarding the hot carcass weight ; carcass yield; area, width and depth of rib eye area of the Longissimus dorsi muscle, and noble courts. In relation to dry matter intake, the comparison of the treatments demonstrated that Ractopamine didn't influence negatively, which highlights its positive effect on the animal performance. The use of Ractopamine improves performance and decreases de amount of superficial fat in male nellore carcass in confinement.

Este estudo foi conduzido com o objetivo de avaliar os níveis de fitase em rações para suínos em crescimento selecionados para deposição de carne. Foram utilizados 64 suínos mestiços (32 machos castrados e 32 fêmeas), híbridos comerciais, com peso médio inicial de 23,311±2,254 kg, distribuídos em delineamento de blocos casualizados, com quatro tratamentos, oito repetições e dois animais (um macho e uma fêmea) por unidade experimental. Os tratamentos foram constituídos por uma ração controle de origem vegetal, sem inclusão de fosfato bicálcico e fitase, contendo 0,100% de fósforo disponível; e de outras três rações obtidas pela inclusão de 300, 600 e 1200 unidades de fitase (UFT kg-1) à ração controle. Os níveis de fitase influenciaram de forma linear o ganho de peso diário e consumo de ração, aumentando até o nível de 293 e 307 UFT kg-1 de ração, respectivamente, a partir do qual permaneceram em um platô. A conversão alimentar diminuiu até 255 UFT kg-1 de ração, a qual permaneceu em um platô. Os níveis de fitase não influenciaram os teores de cinzas e fósforo no metacarpo dos animais. Houve efeito linear decrescente dos tratamentos sobre o resultado bioquímico de fosfatase alcalina. Os melhores resultados de ganho de peso, consumo de ração e conversão alimentar foram obtidos, respectivamente, com os níveis de 293, 307 e 255 UFT kg-1 de ração, em suínos de alto potencial genético para deposição de carne na fase dos 55 aos 90 dias de idade.

Zusammenfassung Einleitung Mit weit mehr als 1400 stationär behandelten COVID-19-Patienten ist die Universitätsmedizin Essen der größte COVID-19-Versorger der Region Rhein-Ruhr. Wir präsentieren die Daten unserer Patienten aus den ersten 12 Monaten der Pandemie und die hieraus entstandenen praktischen Konzepte. Methode Retrospektive Analyse aller 1396 stationären COVID-19-Patienten, die zwischen dem 1. März 2020 und 28. Februar 2021 versorgt wurden, im Hinblick auf Komorbiditäten, Überleben und Komplikationen im Verlauf. Es erfolgte ein Gruppenvergleich zwischen Patienten auf Normalstation und Überwachungs-/ Intensivstation. Ergebnisse Bei einer Gesamtmortalität von 19,8 % (277/1396) starben 10,6 % (93/877) der Patienten auf Normalstation und 35,5 % (184/519) der Patienten auf Intensiv- und Überwachungsstationen im klinischen Verlauf. Hierbei waren ein Alter über 60 Jahre, Adipositas, maschinelle Beatmung, NO-Therapie, ECMO-Therapie sowie akutes Nierenversagen und Apoplex im Therapieverlauf unabhängige Prädiktoren für Mortalität. Fazit Die Mortalität unseres Kollektivs auf Normal- bzw. Intensivstationen liegt im Rahmen international publizierter Daten. Sowohl die hohe Rate von Komplikationen bei schwerem Verlauf als auch die große Bedeutung einfacher Komorbiditäten kann eindrücklich gezeigt werden. Das mittlere Alter der Patienten ist mit 60 Jahren auf Normalstation und 63 Jahren auf Intensivstationen überraschend niedrig. Maximaler Patienten- und Personalschutz, eine rasche und effektive Teststrategie im Rahmen der primären Triage, standardisierte Abläufe von der Notaufnahme bis zur Intensivstation sowie eine dynamische tagesaktuelle Anpassung der Ressourcen können eine hohe Versorgungsqualität, auch während der Pandemie, sichern.

- Abstract
This article outlines the activities of the research network "Festival Performance as a State of Encounter", which was funded by the Arts and Humanities Research Council as part of the Beyond Text strategic programme. The network was formulated in 2008, and a range of different events were organized over the course of two years to explore the concept of relational performance within the context of popular music festivals. One of the central aims of the network was to bring into dialogue scholars from a range of disciplines within the performing arts and creative industries and industry professionals and practitioners working on the festival circuit. The network provided a meeting place for industry-academy collaboration that prompted genuine exchange and knowledge transfer across sectors and challenged assumptions about the role and value of expertise and experience in relation to research processes. The article examines the notion of encounter and co-creation not only as a method of practice in festival performance but also as a methodology for facilitating fruitful conversation and dynamic interaction between stakeholders with a shared interest in understanding the deep impact of embodied participation in festival spaces. The experiment was conducted to evaluate five nutritional plans based on sequences of standardized ileal digestible lysine: 0.90-0.80-0.70, 1.00-0.90-0.80, 1.10-1.00-0.90, 1.20-1.10-1.00, and 1.30-1.20-1.10% fed to gilts from 60 to 99, 129 to 100, and 130 to 148 days of age, respectively. Eighty commercial hybrid gilts, selected for lean gain, with initial weight of 23.46±0.27kg were allotted in a randomized block design, with five treatments, eight replicates, and two pigs per experimental unit. No effect (P>0.05) of the nutritional plans was verified on daily feed intake, daily weight gain and feed conversion. The nutritional plans had no influence (P>0.05) on any of the carcass traits evaluated (carcass yield, meat amount, and meat yield). The nutritional plan of 0.90-0.80-0.70% standardized ileal digestible lysine fed to gilts from 60 to 99, 100 to 129, and 130 to 148 days of age, respectively, meets the standardized ileal digestible lysine requirements of gilts from 60 to 148 days of age.

Avaliou-se o efeito do fornecimento de monensina sódica no desempenho de vacas leiteiras em fase inicial de lactação, cuja alimentação, em sua dieta total no cocho, compôs-se em 81,4% de silagem de milho e 18,6% de concentrado, em que se fez uso de maior proporção de silagem de milho de que concentrado. Foram utilizadas 14 vacas multíparas 7/8 Holandês, aos 20±10 dias pós-parto aproximadamente, com média de 22±0,5kg vaca-1 dia-1 de leite, em delineamento de blocos casualizados, adotando-se o esquema de parcelas subdivididas no tempo, em modelo de medidas repetidas. Os tratamentos nas parcelas foram diferenciados pela presença (200mg animal-1dia-1) ou ausência da monensina na ração e, nas subparcelas, pelos diferentes períodos de avaliação. Não se observaram diferenças no consumo em matéria seca da dieta, conversão alimentar, produção de leite in natura e corrigida e peso vivo nas médias diárias de cada período. No entanto, a produção de sólidos totais foi superior para os tratamentos com o ionóforo, sinalizando que a administração da monensina sódica para vacas no início de lactação modifica o rendimento em produção de leite.

This study aimed to verify the effect of age of dam on the performance of male and female Nellore calves, using the following variables: average daily gain (ADG), adjusted weight for 205 days of age (W205), and number of days to reach 160 kg (D160). Information were collected from a commercial herd consisting of 1,122 calves and 1,009 heifers and their mothers. To classify animals with similar performance based on the cows’ calving orders (age of dam), the multivariate cluster analysis was adopted through the complete linkage hierarchical method. The best performance was observed in the calves of cows in their sixth calving at most; for heifers, the best performance was seen in those born to cows in their eighth calving at most. Cows in their eighth calving should be discarded.

Background The ongoing COVID-19 pandemic remains a major challenge for worldwide health care systems and in particular emergency medicine. An early and safe triage in the emergency department (ED) is especially crucial for proper therapy. Clinical symptoms of COVID-19 comprise those of many common diseases; thus, differential diagnosis remains challenging. Method We performed a retrospective study of 314 ED patients presenting with conceivable COVID-19 symptoms during the first wave in Germany. All were tested for COVID-19 with SARS-Cov-2-nasopharyngeal swabs. Forty-seven patients were positive. We analyzed the 267 COVID-19 negative patients for their main diagnosis and compared COVID-19 patients with COVID-19 negative respiratory infections for differences in laboratory parameters, symptoms, and vital signs. Results Among the 267 COVID-19 negative patients, 42.7% had respiratory, 14.2% had other infectious, and 11.2% had cardiovascular diseases. Further, 9.0% and 6.7% had oncological and gastroenterological diagnoses, respectively. Compared to COVID-19 negative airway infections, COVID-19 patients showed less dyspnea (OR 0.440; p = 0.024) but more dysgeusia (OR 7.631; p = 0.005). Their hospital stay was significantly longer (9.0 vs. 5.6 days; p = 0.014), and their mortality significantly higher (OR 3.979; p = 0.014). Conclusion For many common ED diagnoses, COVID-19 should be considered a differential diagnosis. COVID-19 cannot be distinguished from COVID-19 negative respiratory infections by clinical signs, symptoms, or laboratory results. When hospitalization is necessary, the clinical course of COVID-19 airway infections seems to be more severe compared to other respiratory infections.
