159. Effect of travel distance and rurality of residence on initial surveillance for hepatocellular carcinoma in VA primary care patient with cirrhosis

Abstract

Objective: To determine the relationship between travel distance and surveillance for hepatocellular carcinoma among veterans with cirrhosis. Data Sources: Veterans Health Administration (VHA) inpatient and outpatient administrative data were linked to geocoded enrollee files. CMS-VHA merged data were used to assess receipt of Medicare-financed non-VA imaging. Study Design: A retrospective cohort of US veterans diagnosed with cirrhosis between 2009 and 2015 was examined. First available abdominal imaging following the diagnosis of cirrhosis was analyzed separately as a function of travel distance to the nearest VA medical center (VAMC) and to the patient's assigned VA primary care provider. Veterans with dual use of Medicare and VA services were also examined for receipt of imaging outside of the VA. Principal Findings: Veterans who resided more than 30 miles from the nearest VAMC were less likely to receive any imaging for HCC surveillance. Among dual users, increased travel distance between the patient's residence and nearest VAMC was associated with an increased likelihood of receiving any abdominal imaging at non-VA facilities. Conclusion: Increased travel distance to the nearest VA medical center reduces the likelihood of receiving imaging for HCC surveillance in cirrhotic veterans. Future efforts should focus on reducing geographic barriers to HCC surveillance. 167. Manifestation of the Verwey Transition in the Tunneling Spectra of Magnetite Nanocrystals

Abstract

Tunneling transport measurements performed on single particles and on arrays of Fe3O4 (magnetite) nanocrystals provide strong evidence for the existence of the Verwey metal-insulator transition at the nanoscale. The resistance measurements on nanocrystal arrays show an abrupt increase of the resistance around 100 K, consistent with the Verwey transition, while the current-voltage characteristics exhibit a sharp transition from an insulator gap to a peak structure around zero bias voltage. The tunneling spectra obtained on isolated particles using a Scanning Tunneling Microscope reveal an insulator-like gap structure in the density of states below the transition temperature that gradually disappeared with increasing temperature, transforming to a small peak structure at the Fermi energy. Abstract

Photoluminescence excitation spectroscopy and scanning tunneling spectroscopy are used to study the electronic states in CdSe quantum rods that manifest a transition from a zero dimensional to a one dimensional quantum confined structure. Both optical and tunneling spectra show that the level structure depends primarily on the rod diameter and not on length. With increasing diameter, the band-gap and the excited state level spacings shift to the red. Abstract

Current imaging scanning tunneling microscopy is used to observe the electronic wavefunctions in InAs/ZnSe core/shell nanocrystals. Images taken at a bias corresponding to the s conduction band state show that it is localized in the central core region, while images at higher bias probing the p state reveal that it extends to the shell. This is supported by optical and tunneling spectroscopy data demonstrating that the s-p gap closes upon shell growth. Abstract

Tunneling spectroscopy of InAs nanocrystals deposited on graphite was measured using scanning tunneling microscopy, in a double-barrier tunnel-junction configuration. The effect of the junction symmetry on the tunneling spectra is studied experimentally and modeled theoretically. When the tip is retracted, we observe resonant tunneling through the nanocrystal states without charging. This is in contrast to previous measurements on similar nanocrystals anchored to gold by linker molecules, where charging took place. Charging is regained upon reducing the tip-nanocrystal distance, making the junctions more symmetric. The effect of voltage distribution between the junctions on the measured spectra is also discussed., Comment: submitted

Abstract

Data assimilation is the process of finding the best estimate of the current state of a system. In numerical weather prediction (NWP) this system is the atmosphere and oceans. In most operational weather forecasting centres variational data assimilation is performed using a different set of variables from the actual model variables. The transformation of variables simplifies the problem by assuming that the errors in the transformed variables are uncorrelated The validity of this hypothesis is key to the accuracy of the data assimilation. Recently a potential vorticity (PV) based set of variables has been proposed. These new variables are thought to exploit more accurately important dynamical properties of the atmosphere. Here we present new results, obtained with a simplified 1-D shallow water model, comparing the PV-based variables to the vorticity-based variables currently used at operational weather forecasting centres, including the Met Office.

Abstract

Objective: Local drug delivery aims to minimize systemic toxicity by preventing off-target effects; however, injection parameters influencing depot formation of injectable gels have yet to be thoroughly studied. We explored the effects of needle characteristics, injection depth, rate, volume, and polymer concentration on gel ethanol distribution in both tissue and phantoms. Methods: The polymer ethyl cellulose (EC) was added to ethanol to form an injectable gel to ablate cervical precancer and cancer. Tissue mimicking phantoms composed of 1% agarose dissolved in deionized water were used to establish overall trends between various injection parameters and the resulting gel distribution. Additional experiments were performed in excised swine cervices with a CT-imageable injectate formulation, which enabled visualization of the distribution without tissue sectioning. Results: Needle type and injection rate had minimal impact on gel distribution, while needle depths ≥13 mm yielded significantly larger distributions. Needle gauge and EC concentration impacted injection pressure with maximum gel distribution achieved when the pressure was 70–250 kPa. Injection volumes ≤3 mL of 6% EC-ethanol minimized fluid leakage away from the injection site. Results guided the development of a speculum-compatible hand-held injector to deliver gel ethanol into the cervix. Conclusion: Needle depth, gauge, and polymer concentration are critical to consider when delivering injectable gels. Significance: This study addressed key questions related to the impact of injection-based parameters on gel distribution at a scale relevant to human applications including: 1) how best to deliver EC-ethanol into the cervix and 2) general insights about injection protocols relevant to the delivery of injectable gels in tissue.

Abstract

Introduction. Some medical centers and surgeons require patients to stop smoking cigarettes prior to elective orthopaedic surgeries in an effort to decrease surgical complications. Given higher rates of smoking among rural individuals, rural patients may be disproportionately impacted by these requirements. We assessed the perceptions and experiences of rural-residing Veterans and clinicians related to this requirement. Methods. We conducted qualitative semistructured one-on-one interviews of 26 rural-residing veterans, 10 VA orthopaedic surgery staff (from two Veterans Integrated Services Networks), 24 PCPs who serve rural veterans (14 VA; 10 non-VA), and 4 VA pharmacists. Using the knowledge, attitudes, and behavior framework, we performed conventional content analysis. Results. We found three primary themes across respondents: (1) knowledge of and the evidence base for the requirement varied widely; (2) strong personal attitudes toward the requirement; and (3) implementation and possible implications of this requirement. All surgery staff reported knowledge of requirements at their institution. VA PCPs reported knowledge of requirements but typically could not recall specifics. Most patients were unaware. The majority of respondents felt this requirement could increase motivation to quit smoking. Some PCPs felt a more thorough explanation of smoking-related complications would result in increased quit attempts. About half of all patients reported belief that the requirement was reasonable regardless of initial awareness. Respondents expressed little concern that the requirement might increase rural-urban disparities. Most PCPs and patients felt that there should be exceptions for allowing surgery, while surgical staff disagreed. Discussion. Most respondents thought elective surgery was a good motivator to quit smoking; but patients, PCPs, and surgical staff differed on whether there should be exceptions to the requirement that patients quit preoperatively. Future efforts to augment perioperative smoking cessation may benefit from improving coordination across services and educating patients more about the benefits of quitting. [ABSTRACT FROM AUTHOR]

Abstract

Objective: School-based nutrition education can promote the development of skills, such as food label reading, that can contribute to making healthier food choices. The purpose of this study was to assess the reliability of a Russian language version of the previously validated Food Label Literacy for Applied Nutrition Knowledge (FLLANK) questionnaire and also to evaluate the impact of school-based nutrition education programmes on nutrition knowledge and food label-reading skills in Russian adolescents and young adults. Methods: Two nutrition education study programmes were administered to two separate groups of students--at a middle school (n?=?23) and a university (n?=?17). The study programmes consisted of a single 2-hour classroom session and emphasised basic nutrition and food components. The FLLANK questionnaire was administered to both groups of students before and after the intervention to assess change in food label-reading skills. Cronbach's a was calculated for the post-test scores to assess the instrument's internal consistency. Results: Middle school students increased their mean FLLANK scores from 54% to 95% (p?<0.01). University students increased their scores from 46% to 91% (p?<0.01).
