19131 Background: This study launched to identify predictive factors and discriminate subgroups according to risk of brain metastases in non-small-cell lung cancer (NSCLC) patients who have not neurological symptoms. Methods: We performed retrospective review of 446 NSCLC patients who underwent computed tomography (CT), magnetic resonance imaging (MRI) and bone scan at initial staging work-up between April 2003 and April 2007. Brain metastases were determined by MRI. Patients were stratified into groups according to number of risk factors identified in multivariate analysis. Multilevel likelihood ratio (LR) was calculated for each group. Results: Thirty-three patients had brain metastasis. Histopathology of adenocarcinoma, nodal stage ≥ 2 on CT and bone metastasis were predictive factors of brain metastasis in multivariate analysis.
