Abstract

Sustained hyperglycemia in type 2 diabetes is associated with low levels of the endogenous antioxidant glutathione (GSH), which leads to oxidative stress and tissue damage. It is therefore important to investigate whether oral GSH supplementation would restore GSH levels, and also help improve redox state and glycaemia in diabetic individuals. We conducted a pragmatic clinical trial to assess effectiveness of oral GSH supplementation along with anti-diabetic treatment. 104 non-diabetic and 250 diabetic individuals were recruited. All the 250 diabetic individuals were on anti-diabetic therapy; of these 125 were given oral GSH supplementation additionally for a period of six months. Fasting and PP glucose and insulin, HbA1c, GSH and the oxidative DNA damage marker 8-OHdG were measured at the recruitment time and after three and six months of supplementation. GSH supplementation increased blood GSH and decreased 8-OHdG significantly within three months in diabetic individuals. It also reduced HbA1c within three months and stabilized it thereafter in diabetic population overall. Patients above 55 years benefited more as evidenced by a significant decrease in HbA1c and 8-OHdG and an increase in fasting insulin. Data suggest that GSH supplementation can be used as an adjunct therapy to anti-diabetic treatment to achieve better glycemic targets, especially in elderly population.Trial registeredThis study has been registered with Clinical Trials Registry-India (CTRI/2018/01/011257)Novelty bulletsOral GSH supplementation replenishes body stores of GSH and significantly reduces oxidative DNA damage in diabetic patients over six months.HbA1c is lowered at three months and is stabilized thereafter.A significant decrease in HbA1c and elevated fasting insulin was observed in people older than 55 years.

Abstract

We examine a number of probabilistic choice models in which people might form their beliefs to play their strategies in a game theoretic setting in order to propose alternative equilibrium concepts to the Nash equilibrium. In particular, we evaluate the Blavatskyy model, Returns Based Beliefs (RBB) model, Quantal Response Equilibrium (QRE) model, Boundedly Rational Nash Equilibrium (BRNE) model and the Utility Proportional Beliefs (UPB) model. We outline the foundational axioms for these models and fully explicate them in terms of probabilistic actions, probabilistic beliefs and their epistemic characterizations. We test the model predictions using empirical data and show which models perform better under which conditions. We also extend the Blavatskyy model which was developed to consider games with two actions to cases where there are three or more actions. We provide a nuanced understanding of how different types of probabilistic choice models might predict better than others.

Abstract

AIM: The issue of youth who are not engaged in education, employment or training has been a focus of policymakers for decades. Although interventions exist for these youth, they often measure success in ways that fail to capture what youth seek to gain. The project aims to address this gap by assessing youth-oriented outcomes for interventions targeting upcoming youth. Acknowledging the stigma attached to the deficit-based notion of not engaged in education, employment or training, hereafter we refer to 'upcoming youth', a term coined by youth partners on the project. This study asks what youth want to achieve by participating in an intervention for upcoming youth, with a view to guiding service and research design. METHODS: A mixed-methods discrete choice experiment will be conducted with youth engaged as partners. A qualitative (focus group) stage will be conducted to design discrete-choice experiment attributes and levels. The experiment will be piloted and administered online to approx. 500 youth (aged 14-29) across Canada to identify the outcomes that youth prioritize for interventions. Latent class analyses will then be conducted to explore clusters of outcomes that different groups of youth prioritize. CONCLUSIONS: From a strengths-based recovery-oriented framework, hearing the voices of the target population is important in designing and evaluating services. This youth-oriented research project will identify the intervention outcomes that are the highest priority for upcoming youth. Findings will inform the development, implementation and testing of interventions targeting relevant outcomes for youth who are not engaged in education, employment or training.

178. Structural health monitoring of railway tracks using IoT-based multi-robot system

Abstract

A multi-robot-based fault detection system for railway tracks is proposed to eliminate manual human visual inspection. A hardware prototype is designed to implement a master–slave robot mechanism capable of detecting rail surface defects, which include cracks, squats, corrugations, and rust. The system incorporates ultrasonic sensor inputs coupled with image processing using OpenCV and deep learning algorithms to classify the surface faults detected. The proposed Convolutional Neural Network (CNN) model fared better compared to the Artificial Neural Network (ANN), random forest, and Support Vector Machine (SVM) algorithms based on accuracy, R-squared value, F1 score, and Mean-Squared Error (MSE). To eliminate manual inspection, the location and status of the fault can be conveyed to a central location enabling immediate attention by utilizing GSM, GPS, and cloud storage-based technologies. The system is extended to a multi-robot framework designed to optimize energy utilization, increase the lifetime of individual robots, and improve the overall network throughput. Thus, the Low Energy Adaptive Clustering Hierarchy (LEACH) protocol is simulated using 100 robot nodes, and the corresponding performance metrics are obtained.

Abstract

Iodic acid (HIO) is known to form aerosol particles in coastal marine regions, but predicted nucleation and growth rates are lacking. Using the CERN CLOUD (Cosmics Leaving Outdoor Droplets) chamber, we find that the nucleation rates of HIOparticles are rapid, even exceeding sulfuric acid-ammonia rates under similar conditions. We also find that ion-induced nucleation involves IOand the sequential addition of HIOand that it proceeds at the kinetic limit below +10°C. In contrast, neutral nucleation involves the repeated sequential addition of iodous acid (HIO) followed by HIO, showing that HIOplays a key stabilizing role. Freshly formed particles are composed almost entirely of HIO, which drives rapid particle growth at the kinetic limit. Our measurements indicate that iodine oxoacid particle formation can compete with sulfuric acid in pristine regions of the atmosphere.

Abstract

Introduction: Young people with a first episode of psychosis can achieve full remission with prompt treatment. Throughout Canada, early psychosis intervention programs are implementing policies to ensure timely delivery of services. One of Canada's first early intervention services, the Prevention and Early Intervention for Psychosis program, set the guideline that all youth referred should receive an appointment within 72 hours. The availability of early intervention programs has increased significantly but the standards these programs have adopted to ensure timely delivery of services remains unknown. Objectives: This project aims to identify the policies and practices in early intervention programs that ensure timely delivery of services. Secondly, the project aims to understand the level of awareness of the 72-hour recommendation and the level of adoption of this recommendation. Thirdly, the project aims to identify the factors that facilitate and hinder a program's ability to reach and maintain their benchmarks for timely delivery of services. Methods: Participants included 17 service delivery providers from four early intervention programs located in socio-culturally distinct regions in Canada. Participants completed a survey about their program's service delivery policies and practices. We led individual semi-structured interviews with seven service providers to identify the barriers and facilitators to delivering timely care. We conducted frequency analyses of the survey data and thematic analysis of the interviews to identify emerging themes. Results: Forty-one percent of survey respondents indicated that their program implemented formal policies to minimize the delay to the first appointment, with benchmarks ranging from 72 hours to 12 weeks. The majority of program managers interviewed were aware of the 72-hour benchmark, voiced satisfaction with standards, and felt that establishing standards was helpful to delivering quality services. Average time between referral and first appointment ranged from 10 days to 12 weeks; however, more than half of survey respondents were unaware of the average delay in their program. Notable barriers to implementation included patient non-responsiveness, insufficient staffing, and missing patient contact information from referrals. The service providers reported engaged staff, flexible schedules, and team-based care as facilitators to meeting service delivery benchmarks. Conclusions: Benchmarks such as the 72-hour recommendation are an excellent step in improving timeliness of delivery of early intervention services. Common barriers to meeting benchmarks, such as patient adherence and staff resources may be difficult to overcome; however, implementing standardized referral forms and processes, increasing staff engagement, providing flexible schedules, and encouraging team-based care could improve timely delivery of services. 195. A socio-legal understanding of state compensation for silicosis in India

Abstract

This thesis explores the emergence of government compensation for silicosis in India by interrogating two main ideas: first, that compensation for workplace disease is an outcome of a shifting relationship between capital, labour and the state on the proper locus of risk and responsibility for worker welfare. Second, labour law, as it developed in both Britain and the colonies excluded large sections of the workforce, often in order to facilitate resource extraction. It narrates the history of employer compensation for silicosis in South Africa and Britain, exposing its contingent, limited coverage and explicit exclusion of Black workers as well as non-industrial workers in fragmented or artisanal production. It uses this insight on exclusion to demonstrate how state compensation in India became possible as a result of lacunae in labour law’s very origins. ‘Informal’ workers, always outside labour law’s ambit, therefore were forced to use the language of rights and the public interest before the Supreme Court of India to advance their demands. This found form in the Public Interest Litigation, a form of legal petition, that variously constructed workers as a community facing unfair working conditions; residents suffering from air pollution; and finally, as victims of a human rights violation. These shifting constructions were played out along the backdrop of advancing marketization in the global political economy, as well as the Supreme Court’s own role in dismantling formal labour law protections at the same time. This thesis situates the development of state compensation for silicosis within these developments, demonstrating that the rise of human rights to advance worker protection was concomitant with a broader assault on the working classes, within which the worker-employer relation has been reconstituted as a citizen-state relation.
