Abstract

Abstract Background De-intensification of anti-cancer therapy without significantly affecting outcomes is an important goal. Omission of axillary surgery or breast radiation is considered a reasonable option in elderly patients with early-stage breast cancer and good prognostic factors. Data on avoidance of both axillary surgery and radiation therapy (RT) is scarce and inconclusive. Methods A retrospective cohort study comprising all women aged 70 years and older diagnosed with early, hormone receptor (HR) positive, HER2-negative breast cancer treated with breast-conserving surgery (BCS) without sentinel lymph node biopsy (SLNB) and RT in a large tertiary center (between 2016 and 2021). Data on patient and tumor characteristics as well as outcomes including local recurrence, loco-regional recurrence, distant metastases, and death were extracted. Disease free survival (DFS) was assessed by Kaplan-Meier analysis. The Cox proportional hazard regression model was performed to identify factors (demographic and clinical characteristics of the patients) that predict the disease recurrence or death. Results A total of 100 women were included, median age of patients was 81. All patients had clinically node-negative disease with a median tumor size was 13 mm. Five (5%) women had lymphovascular invasion. At a median follow-up of 3.9 years, there were 7 (7%) recurrences, 4 local, 2 local-regional, and one distant. The median DFS for the entire group was 42 months (11–128). Eight patients (8%) died, 5 of them for reasons unrelated to breast cancer (3 of unknown reason). Tumor size larger than 13 mm was associated with significantly worse DFS (HR = 4.02, 95% CI 1.08–14.99, p = 0.04). Conclusion Omission of both SLNB and adjuvant RT is feasible in elderly, early breast cancer patients with small luminal tumors.

Abstract

Abstract On October 7, 2023, a large-scale attack in southern Israel and the subsequent war resulted in extensive loss of life and injuries, with many individuals experiencing traumatic losses, such as family members or close friends being killed or kidnapped. This study aims to longitudinally examine its effects on mental health, specifically, clinical symptoms of anxiety, depression, and posttraumatic stress disorder (PTSD). We anticipated greater symptom severity among individuals who experienced traumatic loss, were forcibly displaced, or suffered income loss, as well as among women and members of ethnic minorities. This three-wave online survey study assessed mental health symptoms in a sample of 1052 individuals, aged 18–40, residing in conflict zones in Israel from February 2024 (day 1), through March 2024 (day 30), to May 2024 (day 90). Symptoms of anxiety, depression, and PTSD were measured using the Generalized Anxiety Disorder-7 (GAD-7), Patient Health Questionnaire-9 (PHQ-9), and Primary Care PTSD Screen (PC-PTSD). Abstract

Abstract Aims Immune checkpoint inhibitors (ICIs) have revolutionized cancer treatment outcomes. However, the response varies across different populations, and their use may lead to life-threatening cardiovascular (CV) events. While pre-treatment reduced left ventricular ejection fraction (LVEF) is considered a marker for high-risk cardiotoxicity and a contraindication for anthracycline and HER2-targeted therapies, there is limited evidence on the safety and efficacy of ICIs therapy in patients presenting with pre-treatment reduced LVEF. The study aims to evaluate the safety and efficacy of ICIs therapy in patients with pre-treatment reduced LVEF. Methods Retrospective single center cohort of patients treated with ICIs therapy, who performed pre-treatment LVEF assessment. The primary endpoint was to evaluate the safety of ICIs among this population, assessed by CV events (composite of myocarditis, acute coronary syndrome, heart failure, and arrhythmias). The secondary endpoint was to evaluate the efficacy of ICIs, assessed by all-cause mortality and progression-free survival (PFS). Results The cohort included 307 patients, with 30 (10%) presenting with pre-treatment reduced LVEF, with a mean LVEF of 39 ± 7%. While a significantly higher incidence of CV events was observed in the reduced LVEF group (37% vs. 14%, p = 0.004), following a multivariate Cox regression analysis including baseline CV diseases and risk factors, pre-treatment reduced LVEF did not remain a significant independent predictor (p = 0.358). No significant differences were observed between the groups regarding all-cause mortality and PFS. Conclusions Pre-treatment reduced LVEF was not identified as an independent marker for clinical outcomes in patients treated with ICIs therapy.

195. Citizenship Pressure in Non-Formal Education Organizations: Leaders' Idealized Influence and Organizational Identification

Abstract

Educators in non-formal education organizations are often expected to display values of volunteering and giving to the community. These contributions, which are beyond the call of duty, are defined as organizational citizenship behavior (OCB). When such behavior is performed as a result of pressure rather than out of free will, that pressure is defined as citizenship pressure (CP). Building on the job demands-resources theory, the study examined a moderator-mediator model at the team level construct, to explore whether team CP mediates the relationship between both idealized influence behavior and idealized influence attributed (transformational leadership dimensions) and team OCB, and whether that mediation is moderated by organizational identification. The study sample consisted of 75 teams of educators and their direct superiors, who work in 11 youth movements. Results show that the negative relationship between both idealized influence behavior and idealized influence attributed and team CP is moderated by organizational identification. Furthermore, results show a negative relationship between team CP and team OCB. Theoretical and managerial implications are discussed.

196. Influence of Simulated Skin Color on the Accuracy of Face Scans

Abstract

Aims: this study aims to investigate the impact of simulated skin color and the use of fiducial markers on the accuracy of 3D facial scans, comparing two types of structured light scanners under constant ambient lighting conditions. Materials and Methods: Three mannequins with different skin colors—black, white, and pink—were scanned using two light based hand-held scanners (infrared light and blue-light). Each mannequin was scanned with and without fiducial markers placed on defined anatomical landmarks. A total of one hundred thirty-two scans were performed and converted into standard tessellation language (STL) files. STL files from each scanner were compared to their respective control scans using point cloud comparison software. Accuracy was evaluated based on root mean square (RMS) values. Descriptive statistics summarized the data, and a t-test was performed to assess differences in RMS values between scans with and without fiducial markers for each scanner type. Results: The infrared light scanner showed the highest accuracy for the white mannequin, as evidenced by lower RMS values compared to the pink and black mannequins. Adding fiducial markers significantly enhanced scan accuracy for the pink and black mannequins. The blue-light scanner achieved accuracy for the white and pink mannequins comparable to that of the infrared scanner. However, it was unable to scan the black mannequin, even with the use of markers. Conclusions: Within the limitations of this study, simulated skin color significantly affects the accuracy of facial 3D scans. Scans of lighter (white) tones demonstrate higher accuracy compared to darker tones. Fiducial markers enhance the accuracy for an infrared scanner; however, a blue-light scanner is unable to capture dark simulated skin, even with the addition of fiducial markers.

Abstract

Spin-based quantum information processing makes extensive use of spin-state manipulation. This ranges from dynamical decoupling of nuclear spins in quantum sensing experiments to applying logical gates on qubits in a quantum processor. Here we present an antenna for strong driving in quantum sensing experiments and theoretically address challenges of the strong driving regime. First, we designed and implemented a micron-scale planar spiral RF antenna capable of delivering intense fields to a sample. The planar antenna is tailored for quantum sensing experiments using the diamond's nitrogen-vacancy (NV) center and should be applicable to other solid-state defects. The antenna has a broad bandwidth of 22 MHz, is compatible with scanning probes, and is suitable for cryogenic and ultrahigh vacuum conditions. We measure the magnetic field induced by the antenna and estimate a field-to-current ratio of $113\pm 16$ G/A, representing a x6 increase in efficiency compared to the state-of-the-art. We demonstrate the antenna by driving Rabi oscillations in $^1$H spins of an organic sample on the diamond surface and measure $^1$H Rabi frequencies of over 500 kHz, i.e., $\mathrm{\pi}$-pulses shorter than 1 $\mu s$ - faster than previously reported in NV-based nuclear magnetic resonance (NMR). Finally, we discuss the implications of driving spins with a field tilted from the transverse plane in a regime where the driving amplitude is comparable to the spin-state splitting, such that the rotating wave approximation does not describe the dynamics well. We present a recipe to optimize pulse fidelity in this regime based on a phase and offset-shifted sine drive, that may be optimized without numerical optimization procedures or precise modeling of the experiment. We consider this approach in a range of driving amplitudes and show that it is particularly efficient in the case of a tilted driving field.

198. The Early Ultraviolet Light-Curves of Type II Supernovae and the Radii of Their Progenitor Stars

Abstract

We present a sample of 34 normal SNe II detected with the Zwicky Transient Facility, with multi-band UV light-curves starting at $t \leq 4$ days after explosion, as well as X-ray detections and upper limits. We characterize the early UV-optical colors and provide prescriptions for empirical host-extinction corrections. We show that the $t > 2\,$days UV-optical colors and the blackbody evolution of the sample are consistent with the predictions of spherical phase shock-cooling (SC), independently of the presence of `flash ionization" features. We present a framework for fitting SC models which can reproduce the parameters of a set of multi-group simulations without a significant bias up to 20% in radius and velocity. Observations of about half of the SNe II in the sample are well-fit by models with breakout radii $<10^{14}\,$cm. The other half are typically more luminous, with observations from day 1 onward that are better fit by a model with a large $>10^{14}\,$cm breakout radius. However, these fits predict an early rise during the first day that is too slow. We suggest these large-breakout events are explosions of stars with an inflated envelope or a confined CSM with a steep density profile, at which breakout occurs. Using the X-ray data, we derive constraints on the extended ($\sim10^{15}$ cm) CSM density independent of spectral modeling, and find most SNe II progenitors lose $<10^{-4} M_{\odot}\, \rm yr^{-1}$ a few years before explosion. This provides independent evidence the CSM around many SNe II progenitors is confined. We show that the overall observed breakout radius distribution is skewed to higher radii due to a luminosity bias. We argue that the $66^{+11}_{-22}\%$ of red supergiants (RSG) explode as SNe II with breakout radii consistent with the observed distribution of field RSG, with a tail extending to large radii, likely due to the presence of CSM., Comment: Submitted to ApJ. Abstract

By rearranging its terms, the Quantum Focusing Conjecture (QFC) can be viewed as a quantum energy condition, and we can consider various limits. A recent restricted version is a limiting form where the quantum focusing vanishes $\Theta \rightarrow 0$, and has been proven for Braneworld scenario. As a result, we derive an improved quantum null energy condition (INEC) $T_{kk}\geq \frac{\hbar}{2\pi \mathcal{A}}\left(S''_{out}-\frac{1}{2}\theta S'_{out}\right)$, that can be proven with field theory techniques. Abstract

Fusion-in-Decoder (FiD) is an effective retrieval-augmented language model applied across a variety of open-domain tasks, such as question answering, fact checking, etc. In FiD, supporting passages are first retrieved and then processed using a generative model (Reader), which can cause a significant bottleneck in decoding time, particularly with long outputs. In this work, we analyze the contribution and necessity of all the retrieved passages to the performance of reader models, and propose eliminating some of the retrieved information, at the token level, that might not contribute essential information to the answer generation process. We demonstrate that our method can reduce run-time by up to 62.2%, with only a 2% reduction in performance, and in some cases, even improve the performance results., Comment: EMNLP 2023 Main Conference
