154. The political pianist : protest, commissions, and programming for the twenty-first century

Abstract

Politics touches many aspects of modern life, from the food we eat to the products we buy and the sports we watch—as well as the music we listen to, study, and perform. Classical musicians are generally more cautious about entering political debates than popular musicians and celebrities. This caution is attributable, in part, to the historical suppression and censorship of classical musicians and composers. But classical musicians today are affected by—and are often implicated in—the systemic forces of racism, sexism, inequality, and other sociopolitical problems. In this dissertation I argue that classical pianists can and should engage in political speech and action. This study begins with a working definition of political music and surveys the history of political interference with classical music and musicians. I then outline three basic categories of political pianism: protest, commissioning, and programming. An exploration of protest highlights four notable pianists who engaged in political activism and speech. My discussion of commissions and programming critiques classical music’s structural and institutional biases towards white and male composers and contends that collaborations with composers and repertoire choices can function as political statements. The document also introduces two new piano works written by Joel Thompson and Peter S. Shin, commissioned with funding from the University of British Columbia’s Public Scholars Initiative. This dissertation is intended as a guide for pianists who wish to integrate their personal politics into their professional pursuits.

Abstract

Afghanistan has long been conventionally regarded as a remote space peripheral to the wider world. Yet scholarship produced in the 2nd decade of the 21st century suggests its multiple connections to a wide array of regions and settings. Such connections are especially visible when viewed through the lens of the trade networks originating from the territories of modern Afghanistan. Scholars have come to recognize that Afghan traders have long been active players in many contexts across Asia and beyond. Such traders and the networks they form play a critically important role in connecting different parts of Asia with one another, including South Asia and Eurasia, as well as East and West Asia. The connective role performed by Afghan caravanners and religious minorities in the trade between India and Central Asia are especially well documented by historians. Increasingly so too are the activities of Afghan merchants in Ottoman territories. The trading networks Afghan traders have participated in are historically dynamic. Their orientating values shift across time and space between various forms of religious, ethno-linguistic, and political identity. The capacity to adapt to changing circumstances is helpful in understanding the continuing relevance of Afghan traders to 21st-century forms of globalized capitalism, in contexts as varied as the former Soviet Union, China, and the Arabian Peninsula.

182. Lumboperitoneal shunts for the treatment of idiopathic normal pressure hydrocephalus.

Abstract

• 25 patients with iNPH who received an LP Shunt as first-line therapy. • LP shunts reduced gait, cognitive, and urinary dysfunction. • LP shunts may be an effective alternative to VP shunts for iNPH. The standard of care for idiopathic normal pressure hydrocephalus (iNPH) is placement of a ventriculoperitoneal (VP) shunt. However, VP shunts require intracranial intervention and are associated with notable postoperative complications, with some groups reporting complication rates for VP shunts ranging from 17 to 33%, along with failure rates up to 17.7%. Lumboperitoneal (LP) shunts are an alternative for cerebrospinal fluid diversion that do not require intracranial surgery, thus providing utility in patients where intracranial surgery is not possible or preferred. Here we retrospectively reviewed our 25 patients with LP horizontal-vertical (LP-HV) shunts placement for initial treatment for iNPH from 2014 to 2019. All patients had preoperative gait dysfunction, 16 (64%) had urinary incontinence, and 21 (84%) exhibited cognitive insufficiency. Two weeks post-shunt placement, 23/25 (92%) patients demonstrated improvement in gait, 11/16 (68%) had improvement in incontinence, and 14/21 (66%) had improvement cognitive insufficiency. At six months or greater follow up 13/20 (65%) had improvement in gait, 7/15 (47%) showed improvement in incontinence, and 11/15 (73%) demonstrated improvement in cognitive function. Six patients (24%) required at least one revision of the LP shunt. Shunt malfunctions resulted from CSF leak in one patient, shunt catheter migration in two patients, peritoneal catheter pain in one patient, and clinical symptoms for overdrainage in two patients. Thus, we demonstrate that LP-HV shunt placement is safe and efficacious alternative to VP shunting for iNPH, resulting in notable symptomatic improvement and low risk of overdrainage, and may be considered for patients where cranial approaches should be avoided.
