This study examined the extent to which concreteness influences the acquisition and subsequent processing of novel (low frequency) concepts. Participants were trained on 70 rare English words (35 concrete, 35 abstract) paired with definitions. ERPs were then recorded while participants performed a semantic categorisation (concrete vs. abstract) and a lexical decision task on single-meaning, multi-meaning and the newly acquired words. During training there was a significant effect of concreteness, in that participants were more successful at acquiring concrete concepts. In both the semantic categorisation and the lexical decision task, concreteness effects were evident in the behavioural and in the ERP data for all word types, with concrete words eliciting more negative waveforms than abstract words in the N400 time window. Behaviourally, participants experienced greater difficulty in judging the concreteness of multi-meaning words, yet concreteness effects in the N400 were equally strong for all three word types across both tasks. These findings indicate that concreteness represents a fundamental distinction in the way that items are represented in memory, which is independent of the participant's perceived judgement. They further demonstrate that novel concepts can be acquired rapidly after minimal training, and that the neurophysiological correlates associated with processing novel words are modulated by the specific nature of the conceptual characteristics assigned to the word., (© 2013 Published by Elsevier B.V.)

Although it seems intuitive to assume that recognition memory fades over time when information is not reinforced, some aspects of word learning may benefit from a period of consolidation. In the present study, event-related potentials (ERP) were used to examine changes in recognition memory responses to familiar and newly learned (novel) words over time. Native English speakers were taught novel words associated with English translations, and subsequently performed a Recognition Memory task in which they made old/new decisions in response to both words (trained word vs. untrained word), and novel words (trained novel word vs. untrained novel word). The Recognition task was performed 45 minutes after training (Day 1) and then repeated the following day (Day 2) with no additional training session in between. For familiar words, the late parietal old/new effect distinguished old from new items on both Day 1 and Day 2, although response to trained items was significantly weaker on Day 2. For novel words, the LPC again distinguished old from new items on both days, but the effect became significantly larger on Day 2. These data suggest that while recognition memory for familiar items may fade over time, recognition of novel items, conscious recollection in particular may benefit from a period of consolidation.

Forty three European population samples of mealy aphids from various winter and summer host plants were attributed to respective species of Hyalopterus by means of their partial sequences of mitochondrial COI gene. Used Hyalopterus samples emerged as monophyletic relative to outgroup and formed three major clades representing three host specific mealy aphid species in the Neighbor joining, Maximum parsimony, Maximum likelihood and Bayesian inference trees. Hyalopterus pruni and Hyalopterus persikonus emerged as a sister species, whilst Hyalopterus amygdali was located basally. Samples representing different clades in the molecular trees were used for canonical discrimination analysis based on twenty two morphological characters. Length of the median dorsal head hair enabled a 97.3 % separation of Hyalopterus amygdali from the remaining two species. No single character enabled satisfactory discrimination between apterous viviparous females of Hyalopterus pruni and Hyalopterus persikonus. A modified key for the morphological identification of Hyalopterus species is suggested and their taxonomic status discussed.

It has recently been shown that presenting additional visuospatial information alongside to-be-remembered numbers in a digit span task enhances participants' memory for those items. However, the mechanisms behind this visuospatial bootstrapping effect have remained unspecified. In this article, we report evidence that this effect involves an integration of information from verbal and visuospatial temporary memory with long-term-memory (LTM) representations and that the existence of a relevant LTM representation is necessary for bootstrapping to occur.

Aulacorthum sp. collected on Artemisia princeps and Artemisia stolonifera (Asteraceae) in the Korean Peninsula was compared with the eight Aulacorthum species using molecular and morphological characters. In the sequence analysis, the genetic divergences of two mitochondrial genes, COI and CytB, between Aulacorthum sp. and the congeneric species were higher than those among the congeneric species. In the morphometric analysis, principal components analysis using 13 morphological characters reveals that Aulacorthum sp. is distinguished from the congeneric species, especially Aulacorthum albimagnoliae, which is morphologically most similar to Aulacorthum sp.. Based on these results, we propose a new species, Aulacorthum (Aulacorthum) artemisiphaga Lee, Havelka, and Lee sp. nov., with a description of apterous and alate viviparous females and an identification key to species in the genus Aulacorthum in the Korean Peninsula based on apterous viviparous females.

The Russsian wheat aphid (RWA), Diuraphis noxia (Kurdjumov), is a worldwide pest of cereals. Despite its economic importance, little is known about its genome. Here we investigated physical genomic features in RWA by karyotype analysis using differential staining with AgNO(3), CMA(3), and DAPI, by chromosomal localization of ribosomal DNA (rDNA), H3 and H4 histone genes, and the "arthropod" telomeric sequence (TTAGG)(n) using fluorescence in situ hybridization (FISH), and by measuring the RWA genome size using flow cytometry. The female karyotype, 2n = 10, is composed of four autosome pairs and a pair of X chromosomes, whereas the male karyotype, 2n = 9, has a single X. The X chromosome is the largest element in the karyotype. All three molecular markers used, i.e., 18S rRNA and both H3 and H4 probes are co-localized at one end of the X chromosome. The FISH probes revealed that the AgNO(3)-positive bridge between two prometaphase X chromosomes of females, which is believed to be responsible for the elimination of one X chromosome in aphid oocytes determined to undergo male development, contains clusters of both histone genes, in addition to an rDNA cluster. Interestingly, RWA lacks the (TTAGG)(n) telomeric sequence in its genome, in contrast to several previously investigated aphid species. Additionally, we compared female and male genome sizes. The female genome size is 2C = 0.86 pg, whereas the male genome size is 2C = 0.70 pg. The difference between the DNA content in the two genders suggests that the RWA X chromosome occupies about 35% of the female haploid genome (1C = 0.43 pg), which makes it one of the largest sex chromosomes in the animal kingdom.

Two experiments examined whether the age of acquisition (AoA) of a concept influences the speed at which native English speakers are able to name pictures using a newly acquired second language (L2) vocabulary. In Experiment 1, participants were taught L2 words associated with pictures. In Experiment 2 a second group of participants were taught the same words associated with L1 translations. Following training both groups performed a picture naming task in which they were asked to name pictures using the newly acquired words. Significant AoA effects were observed only in Experiment 1, in that participants were faster at naming pictures representing early acquired relative to late acquired concepts. The results suggest that the AoA of a concept can exert influence over processing which is independent of the AoA of the word form. The results also indicate that different training methods may lead to qualitative differences in the nature of the links formed between words and concepts during the earliest stages of second language learning., (Copyright © 2010 Elsevier B.V. The purpose of this investigation was to test the assumption of asymmetric mapping between words and concepts in bilingual memory as proposed by the Revised Hierarchical Model (RHM, Kroll & Stewart, 1994). Twenty four Spanish-English bilinguals (experiment 1) and twenty English-Spanish bilinguals (experiment 2) were presented with pairs of words, one in English and one in Spanish, and asked to indicate whether or not the words had the same meaning. In half the trials the Spanish word preceded the English, and in the other half the English word preceded the Spanish. In each condition half of the words had the same meaning, and the experiment included both concrete and abstract word trials. Event-related potentials (ERPs) were used to examine lexical-semantic activation during word translation. As predicted, a direction-dependent translation asymmetry was observed in the magnitude of the N400 repetition effect. Specifically, the N400 effect was larger during backward translation (L2-L1) than during forward translation (L1-L2) in both groups of bilinguals. Traditionally, working memory is held to comprise separate subcomponents dedicated to the temporary storage of visuospatial and verbal information. More recently, the addition of an episodic buffer has been proposed where information from multiple memory systems is integrated. We report an experiment designed to investigate the effects of providing additional visuospatial information in a verbal working memory task. When to-be-remembered digits were arranged in a horizontal line, performance was no better than when digits were presented in a single location. However, when digits were presented in a keyboard array, performance was significantly better. It is argued that this pattern is hard to reconcile with the traditional model of working memory, and that the "spatial bootstrapping" effect provides evidence towards models of working memory that incorporate an episodic buffer.

This article presents affective ratings for 210 British English and Finnish nouns, including taboo words. The norms were collected with 135 native British English and 304 native Finnish speakers, who rated the words according to their emotional valence, emotional charge, offensiveness, concreteness, and familiarity. The ratings between the two languages were found to be strongly correlated. The present ratings were also strongly correlated with the American English emotional valence and arousal ratings available in the Affective Norms for English Words database (Bradley & Lang, 1999) and the Janschewitz (2008) database for taboo words. These ratings will help researchers to select stimulus materials for a wide range of experiments involving both monolingual and bilingual processing of British English and Finnish emotional words. Eye Movement Desensitization and Reprocessing (EMDR) is a method of psychotherapy that has been extensively researched for the treatment of trauma. The current treatment guidelines of the American Psychiatric Association and the International Society for Traumatic Stress Studies designate EMDR as an effective treatment for PTSD.In this article the author writes about the history of this "breakthrough therapy" and describes EMDR as a standardized protocol. In the second part describes trauma recovery where she uses EMDR in treating post-traumatic stress disorder in a case study about a 25 year old woman, who has been a rubbery survivor.

The paper presents description of original biofeedback computer program called PhysioSoft. It has been designed on the basis of the experience in development of biofeedback techniques of interdisciplinary team of experts of the Department of Health Psychology of the University of Applied Health Studies, Faculty of Electrical Engineering and Computing, University of Zagreb, and "Mens Sana", Private Biofeedback Practice in Zagreb. The interest in the possibility of producing direct and voluntary effects on autonomic body functions has gradually proportionately increased with the dynamics of abandoning the Cartesian model of body-mind relationship. The psychosomatic approach and studies carried out in the 50-ies of the 20th century, together with the research about conditioned and operant learning, have proved close inter-dependence between the physical and mental, and also the possibility of training the individual to consciously act on his autonomic physiological functions. The new knowledge has resulted in the development of biofeedback techniques around the 70-ies of the previous century and has been the base of many studies indicating the significance of biofeedback techniques in clinical practice concerned with many symptoms of health disorders. The digitalization of biofeedback instruments and development of user friendly computer software enable the use of biofeedback at individual level as an efficient procedure of a patient's active approach to self care of his own health. As the new user friendly computer software enables extensive accessibility of biofeedback instruments, the authors have designed the PhysioSoft computer program as a contribution to the development and broad use of biofeedback.

Memories of war, terrorism, and natural disaster play a critical role in the construction of group identity and the persistence of group conflict. Here, we argue that personal memory and knowledge of the collective past become entwined only when public events have a direct, forceful, and prolonged impact on a population. Support for this position comes from a cross-national study in which participants thought aloud as they dated mundane autobiographical events. We found that Bosnians often mentioned their civil war and that Izmit Turks made frequent reference to the 1999 earthquake in their country. In contrast, public events were rarely mentioned by Serbs, Montenegrins, Ankara Turks, Canadians, Danes, or Israelis. Surprisingly, historical references were absent from (post-September 11) protocols collected in New York City and elsewhere in the United States. Taken together, these findings indicate that it is personal significance, not historical importance, that determines whether public events play a role in organizing autobiographical memory.

The interaction between length and lexical status is one of the key findings used in support of models of reading aloud that postulate a serial process in the orthography-to-phonology translation (B. S. Weekes, 1997). However, proponents of parallel models argue that this effect arises in peripheral visual or articulatory processes. The authors addressed this possibility using the special characteristics of the Serbian and Japanese writing systems. Experiment 1 examined length effects in Serbian when participants were biased to interpret phonologically bivalent stimuli in the alphabet in which they are words or in the alphabet in which they are nonwords (i.e., the visual characteristics of stimuli were held constant across lexical status). Experiment 2 examined length effects in Japanese kana when words were presented in the kana script in which they usually appear or in the script in which they do not normally appear (i.e., the phonological characteristics of stimuli were held constant across lexical status). Results in both cases showed a larger length effect when stimuli were treated as nonwords and thus offered strong support to models of reading aloud that postulate a serial component.

In Experiments 1 and 2, we investigated long-term repetition priming effects in Serbian under cross-alphabet and cross-modal conditions. In both experiments, results followed the same pattern: significant priming in all conditions and no significant reduction in priming in the cross-modal as opposed to the cross-alphabet condition. These results are different from those obtained in English (Experiment 3), in which a modality shift led to a reduction in priming. The findings are discussed within a theoretical framework, in which long-term priming is a by-product of learning within the language system. The Serbian writing system was used to investigate whether a serial procedure is implicated in print-to-sound translation and whether components of the reading aloud system can be strategically controlled. In mixed- and pure-alphabet lists, participants read aloud phonologically bivalent words comprising bivalent letters in initial or final positions. Words with bivalent letters in initial positions were disadvantaged relative to nonbivalent controls to a greater degree than were words with bivalent letters in final positions, and the size of the effect was greater in the mixed-alphabet situations than it was in the pure-alphabet situations. A dual-route theory of bialphabetic reading aloud is proposed in which the nonlexical procedure operates serially and nonlexical spelling-sound correspondences for each script can be strategically emphasized or deemphasized., (2005 APA)

Diapause larvae of Aphidoletes aphidimyza were stored at a temperature of 3 degrees C under continuous darkness for up to 7 months with survival rates above 50%; after storage for 1 year the survival rate dropped to 12%. Diapause was terminated in the majority of individuals within 120 days of chilling under storage conditions. Brief exposure (10-60 s) to the vapor of n-hexane appeared to be a useful alternative to chilling for the termination of diapause. The larvae with terminated diapause required, on average, an additional 31 days at 22 degrees C and long-day conditions in order to reach the adult stage. The 10-90% adult emergence spanned a period of 21.1 days. When the larvae with terminated diapause were exposed to 30 degrees C for 1 week after the end of low temperature storage, the survival rate was not affected, the average "time-to-adult" shortened moderately to 28 days, and the synchrony of adult emergence improved considerably to 10 days. Low temperature storage of nondiapause larvae resulted in a decrease in survival from 98 to 31% during the first 60 days of storage. Nondiapause larvae did not enter diapause during low temperature storage and, as a consequence, the adults emerged relatively rapidly (after 14-15 days) and synchronously (within 2-3 days) after the end of storage. Directions for future research, which might bring further improvement in low temperature storability and synchrony of adult emergence in A. aphidimyza, are proposed.

Between 1984 and 1997, six cases of urothelial cancer and 14 cases of renal cell cancer occurred in a group of 500 underground mining workers in the copper-mining industry of the former German Democratic Republic, with high exposures to explosives containing technical dinitrotoluene. Exposure durations ranged from 7 to 37 years, and latency periods ranged from 21 to 46 years. The incidences of both urothelial and renal cell tumors in this group were much higher than anticipated on the basis of the cancer registers of the German Democratic Republic by factors of 4.5 and 14.3, respectively. The cancer cases and a representative group of 183 formerly dinitrotoluene-exposed miners of this local industry were interviewed for their working history and grouped into four exposure categories. This categorization of the 14 renal cell tumor cases revealed no dose-dependency concerning explosives in any of the four exposure categories and was similar to that of the representative group of employees, whereas the urothelial tumor cases were predominantly confined to the high-exposure categories. Furthermore, all identified tumor patients were genotyped by polymerase chain reaction, using lymphocyte DNA, regarding their genetic status of the polymorphic xenobiotic metabolizing enzymes, including the N-acetyltransferase 2 and the glutathione-S-transferases M1 and T1. This genotyping revealed remarkable distributions only for the urothelial tumor cases, who were exclusively identified as "slow acetylators." This points to the possibility of human carcinogenicity of dinitrotoluene, with regard to the urothelium as the target tissue.

We determine the correlation between viremia in serum specimens, transaminase activity (ALT and AST) and histological grading in 37 patients with chronic hepatitis C. In addition we compared two PCR methods for hepatitis C virus (HCV)-RNA in serum specimens. For the histological grading we used a modified Knodell score. For detection and quantification we measured the viremia (HCV-RNA titer) with a standardized "nested primer" PCR (end-point dilution method) and the commercially available Amplicor HCV Monitor. The mean HCV-RNA and AST level was significantly higher in patients with a histologically active inflammation. In the individual patient we could not conclude from the titer of HCV-RNA on the histologic grading because of the wide range of the results. We did not find a significant difference in ALT in patients having varying histological gradings. HCV-RNA titer and transaminases (ALT and AST) did not correlate significantly. The HCV-RNA titer was significantly marked in older patients (above 40 years) and patients having sporadic hepatitis than in younger patients and patients with chronic hepatitis after drug abuse. The "nested primer" PCR (end-point dilution method) was more sensitive for detection of HCV-RNA in serum specimens than Amplicor HCV Monitor. The lack of HCV-RNA with Amplicor HCV Monitor in 12 of 37 patients (32%) did not rule out viremia. We conclude that in patients with a chronic hepatitis C marked viremia points to a histologically active inflammation. In the individual patient we could not conclude from the titer of HCV-RNA on the histological grading. Because of the lower sensitivity of Amplicor HCV Monitor it is necessary to confirm negative results with a "nested primer" PCR.

Biliary leakages are more frequent in laparoscopic than in open cholecystectomy. The aim of our study was to evaluate the diagnostic and therapeutic value of endoscopic retrograde cholangiography (ERC) in the management of bile leakages after laparoscopic cholecystectomy. The primary management was endoscopic sphincterotomy and extraction of remaining gallstones in the common bile duct to provide a decrease of biliary pressure, allowing closure of the fistulas. We report on 20 patients with bile leakage at ERCP, presenting between January 1991 and October 1995 with persistent bile discharge out of drainages, increasing fluid collections subhepatic (termed bilomas) or in the free peritoneal cavity (cholascos), abdominal pain, fever, cholestasis, intraoperatively diagnosed choledocholithiasis, and subhepatic abscess in one case. In 19 cases, after fluoroscopic visualization of the biliary tree and the leak, endoscopic sphincterotomy was performed. The biliary leaks were located at the cystic duct remnant (n = 12), at the gallbladder fossa (n = 5), or at lesions at the hepatic or common bile duct (n = 2). In 7 patients residual common bile duct stones were endoscopically removed. Most patients had localized small subhepatic fluid collections (n = 13) and the others had cholascos (n = 6). The leaks closed with endoscopic sphincterotomy alone in 11 patients, 4 patients had endoscopic sphincterotomy plus percutaneous or laparoscopic drainage of the bile collections, and 4 patients underwent laparotomy. All 6 cases with cholascos but only 2 of the 13 patients with localized collections underwent a second therapeutic procedure (drainage, laparotomy). Five of these procedures were performed within 7 days of ERCP, in most cases due to persistent bile leak. We conclude that biliary leakages after laparoscopic cholecystectomy require laparotomy only exceptionally. Endoscopic sphincterotomy, combined with percutaneous drainage in the case of large collections, represents a safe and successful strategy ensuring closure of the leaks in most cases.

The purpose of our study was to verify the correlation between biochemical and histological inflammatory activity in chronic hepatitis C. The histological activity index (HAI) of 42 patients with chronic hepatitis C was correlated with their transaminase activity. There was indeed a significant correlation between histological activity and the AST (r = 0.54, p = 0.0002) and ALT level (r = 0.36, p = 0.018) respectively. Due to considerable scatter of the results, transaminase activity did not allow reliable assessment of inflammatory activity in individual cases. In the majority, markedly elevated transaminase activity was associated with marked histological inflammatory activity, whereas normal or slightly elevated transaminase activity was found in all stages of inflammatory activity. To assess inflammatory activity in chronic hepatitis C, a liberal indication for liver biopsy should therefore be adopted.

The aim of the study was to determine the frequency and course of hepatitis C viremia in clinically healthy, anti-HCV positive test subjects, and to ascertain whether the HCV antibodies of the IgM type differed between viremia and immunity. In 21 anti-HCV positive blood donors (test subjects) with normal transaminase activity, two serum samples, taken at an interval of 25 +/- 10 months, have been investigated for HCV-RNA and HCV-IgM antibodies. In a total of 16 test subjects (76%) HCV-RNA was found during the first test and/or the follow-up: 14 of them were positive on both occasions, and one test subject each was HCV-RNA positive exclusively at the first test and the follow-up respectively. At the time of the follow-up the serum transaminase level was elevated in 4 test subjects. 3 of these 4 were HCV-RNA positive also. On the other hand, the results of the HCV-PCR were nonuniform in HCV-IgM antibody negative test subjects. The above results demonstrate that in the majority of clinically healthy, anti-HCV positive test subjects with normal transaminase activity, a viremia exists which persists and the course of which may include inflammatory phases. The proof of HCV-IgM antibodies correlates with a viremia. On the other hand, the lack of HCV-IgM antibodies does not exclude viremia.

The purpose of this study was to evaluate the frequency, nature and significance of the complications of endoscopic sphincterotomy. Between 1989 and 1993 endoscopic sphincterotomy was performed in 973 patients with benign affections of the bileducts. 398 patients were our own and 575 were admitted from other hospitals. The complications were recorded retrospectively on the basis of the discharge reports, which were available for each of our own patients (group A) and for 85% of the admitted patients (group B). Complications in all group A patients mentioned in the discharge reports were recorded and their clinical relevance was evaluated based on the patient records. Complications were mentioned in the reports of 82 (9.2%) of 887 cases and were more frequent in patients of group B (52 of 489: 10.6%) than in patients of group A (30 of 398: 7.6%). Pancreatitis was mentioned significantly more frequent in group B (2.5% vs 5.7%, p < 0.02). Reviewing the patient records of group A showed that clinically significant complications occurred in only 8 (2%) of 398 cases. The other complications mentioned in the discharge reports could not be confirmed or were without clinical significance. Hospitalization was short for these patients (5.3 +/- 2.2 days), which points to the fact that the findings were insignificant. 2 (0.2%) of 887 patients underwent surgery and 2 (0.2%) of 887 patients died due to a complication. These results clearly show that the complication rate determined on the basis of the discharge reports alone overestimates the true morbidity of the procedure. Often, an elevated amylase after ERCP/endoscopic sphincterotomy was erroneously interpreted as a pancreatitis.(ABSTRACT TRUNCATED AT 250 WORDS)

Biliary complications are more frequent in laparoscopic than in open cholecystectomy. The aim of the study was to evaluate the diagnostic and therapeutic value of endoscopic retrograde cholangiopancreatography (ERCP) in the management of complications of laparoscopic cholecystectomy. We therefore report on the result of 49 ERCP after laparoscopic cholecystectomy done at our department between January 1991 and March 1993. Patients were referred from 16 different surgical institutions. In 29 cases endoscopic sphincterotomy was performed without complications. Indications for ERCP were "persistent biliary pain" (n = 27), bile leakage (n = 7), pancreatitis (n = 5), abscess (n = 5), painless jaundice (n = 3) and asymptomatic bile duct stone in routine cholangiography (n = 2). In the group of patients with "persistent biliary pain" we found bile duct stones in 12 (80%) of 15 cases with cholestasis and in 3 (30%) of 10 without cholestasis. The stones were endoscopically removed after sphincterotomy. In 2 patients without cholestasis, cannulation of the bile duct failed. 7 patients showed biliary leakage, 4 from inadequate clipping of the cystic stump (2 in combination with a common bile duct stone), 2 from the hepatic duct and 1 from insufficient anastomosis after reconstruction of a common bile duct. After endoscopic sphincterotomy and, if necessary, stone extraction by Dormia basket, leakage from the cystic stump and hepatic duct healed. The insufficient common bile duct anastomosis required reconstruction by hepaticojejunostomy. Three of 5 patients with postoperative pancreatitis had common bile duct stones, while one with chronic pancreatitis had a concrement in the pancreatic duct which was endoscopically removed.(ABSTRACT TRUNCATED AT 250 WORDS)

The aim of our study was to compare the sensitivity of hepatitis C virus polymerase chain reaction (HCV-PCR) by use of two different primer sets which amplify PCR products of different length. Serum samples of 70 patients with chronic hepatitis C were tested by "nested primer" PCR, using either "NCR primers" that amplify cDNA-fragments of 340 basepairs (bp), or by "PT primers" which amplify fragments of 59 bp only. HCV-RNA was detected in 40 patients (57%) by "NCR primers" and in 69 patients (90%) by "PT primers" (p < 0.001). 23 of 70 patients (33%), which were HCV-RNA negative by "NCR primers", were positive by "PT primers", but no patient negative by "PT primers" was found to be positive by "NCR primers". 20 healthy controls tested by both primer sets were all HCV-RNA negative. We conclude that the sensitivity of HCV-PCR is significantly improved by use of primers that amplify "short" PCR products and recommend the use of "PT primers" for HCV-PCR.

Parenteral transmission of hepatitis C virus (HCV) through blood transfusions and intravenous drug misuse is well established. Since 50% of patients with hepatitis C have no history of parenteral exposure, other ways of transmission must exist. The purpose of this study was to examine the epidemiological importance of heterosexual intercourse for transmission of HCV infection. 29 heterosexual contacts (13 men, 16 women, mean age 47 years) of 29 index patients (16 men, 13 women, mean age 49 years) with histologically und serologically documented chronic hepatitis C were questioned about parenteral exposure and sexual behaviour. Their serum samples were examined for ALT activity and anti-HCV antibodies (first-generation Ortho-HCV-ELISA). Five contacts were using condoms to prevent infection. Five of 24 contacts (21%) practising unprotected sexual intercourse, but none of 5 contacts using condoms, showed evidence of heterosexual HCV transmission: 4 were anti-HCV positive (one with elevated ALT activity) and another showed elevated ALT activity with a negative anti-HCV test. None of these contacts had a history of parenteral exposure. Our results indicate that heterosexual transmission of HCV is epidemiologically important. The true rate of infection may be even higher, for two reasons: (1) not every HCV infection is detected by the anti-HCV test, and (2) the anti-HCV test may turn negative again in uncomplicated infection.

The aim of our study was to evaluate whether a negative HCV test of the first generation (HCV-ELISA 1) using the antigen C100-3 excludes chronic HCV infection, or whether patients exist who are negative for antibodies to C100-3 in spite of chronic hepatitis C. 27 patients with histologically proven chronic non-A, non-B hepatitis, all of whom were HCV-ELISA 1 negative, were tested by the HCV test systems of the second generation (Ortho-HCV-ELISA 2 and Chiron-HCV-RIBA 2) based on the distinct HCV antigens 5-1-1, C100-3, C33c and C22-3. To determine the presence of viremia, serum samples were also tested for HCV-RNA with "nested" PCR. 10 of 27 patients proved to be persistently negative when tested with the second generation assays. One patient showed low grade reactivity by HCV-ELISA 2, but non-reactivity by HCV-RIBA 2. In none of these 11 patients was HCV-RNA detected. 16 (60%) of 27 patients negative with HCV-ELISA 1 were positive with HCV-ELISA 2. HCV-RIBA 2 detected antibodies to the structural core antigen C22-3 in all of these 16 patients and antibodies to the non-structural antigen C33c in 14 of them, while antibodies to 5-1-1 or C100-3 were not found in any of these cases. 10 (63%) of the 16 HCV-ELISA 1 negative, but HCV-ELISA 2 and HCV-RIBA 2 positive patients were positive for HCV-RNA by "nested" PCR.(ABSTRACT TRUNCATED AT 250 WORDS)

To clarify the relationship between hepatitis C virus (HCV) infection and hepatocellular carcinoma (HCC), frozen serum samples from 213 patients with histologically proven liver cirrhosis alone (96 alcoholics, 59 HBsAg positive, 29 non-A, non-B hepatitis, 29 cryptogenic) and 40 patients with liver cirrhosis and HCC (12 alcoholics, 7 HBsAg positive, 7 non-A, non-B hepatitis, 14 cryptogenic) were analyzed for antibodies to hepatitis C virus (anti-HCV) with the ortho-HCV-ELISA. The results were as follows. 50 of 253 (20%) patients were anti-HCV positive. The prevalence of anti-HCV was significantly higher in patients with HCC than in patients without HCC (14 of 40 [35%] vs 36 of 213 [17%]; p less than 0.001). In anti-HCV-positive patients HCC were significantly more frequent than in anti-HCV-negative patients (14 of 50 [28%] vs 26 of 203 [13%]; p less than 0.001). The significantly higher occurrence of HCC in anti-HCV-positive patients was not related to other known risk factors such as alcoholism or chronic hepatitis B virus (HBV) infection. Patients with HCV infection as the only risk factor also had a significantly higher occurrence of HCC (12 of 38 [32%] vs 26 of 203 [13%]; p less than 0.001). Our data suggest that chronic HCV infection plays an important role in the pathogenesis of HCC, in particular in patients with cirrhosis unrelated to alcohol or HBV infection.

Our study was designed to evaluate the clinical significance of the ELISA-anti-HCV test. 90 patients with histologically proven chronic non-A, non-B hepatitis (NANBH) and a control group consisting of 36 patients with primary biliary cirrhosis (PBC) were analyzed for antibodies to hepatitis C virus (anti-HCV). Frozen serum samples were tested with the ortho-HCV-ELISA. 67 of 90 (74%) patients with chronic NANBH showed antibodies to the hepatitis C virus. The prevalence of anti-HCV in patients with parenterally transmitted and sporadic chronic NANBH was 70% (23 of 33) and 77% (44 of 57) and in patients with chronic active and chronic persistent NANBH 73% (27 of 37) and 76% (40 of 53) respectively. No significant difference in the prevalence of anti-HCV in the 4 subgroups of patients with chronic NANBH was found. 2 of 36 patients (6%) with PBC were anti-HCV positive without evidence of previous HCV infection. Our results bear out the experience of other authors, that HCV is the commonest pathogen of chronic NANBH. The low prevalence of anti-HCV in patients with PBC suggests good specificity. There have been reports in the literature of "false positive results", particularly in autoimmune hepatitis, and it has been shown that borderline positive test results are often unspecific and therefore to be interpreted with care.

A radioimmunoanalytical determination of the immunoreactive calcitonine in the humen serum was worked out using the authors' own specific antiserum, the preparation and properties of which are reported in the present paper. The precision and reliability of the analytical procedure is within the usual limits (the intraserial variation coefficient was 8.6%, the calculated sensitivity was 6 pg/ml). The reported procedure was employed to determine calcitonine in a larger number of patients who were examined because of suspected medullar carcinoma of the thyroid gland and in patients suffering from other thyropathies. The results are discussed from the viewpoint of heterogeneity of calcitonine in the circulation.

Stored serum samples of 20 patients with clinically and bioptically proven non-A, non-B hepatitis (NANBH) in the acute stage were tested for the presence of antibodies to hepatitis C virus (anti-HCV) by means of the Ortho ELISA system. After a mean period of 8 weeks from onset of the disease, 8 of 20 patients (40%) had anti-HCV. Our follow-up study included 14 patients. Of 9 primarily anti-HCV-negative patients, 2 became positive after 2 and 7 months respectively, whereas 7 patients remained anti-HCV-negative up to 52 months (range 1-128) after the onset of hepatitis. The prevalence of anti-HCV was 71% in 7 patients with parenteral hepatitis related to transfusions (n = 2) or drug abuse (n = 5), and 38% in 13 patients with sporadic NANBH. Of the 8 anti-HCV-negative patients with sporadic NANBH, 5 had stayed in one of the countries where enterically transmitted NANBH is endemic 3 to 6 weeks before the onset of their disease. Our results show that at present the anti-HCV-test supplies an etiologic basis for approximately half of all cases of NANBH in acute stage. Nevertheless, in most cases the acute NANBH remains a diagnosis of exclusion.

The authors compared the results of two radioimmunological methods for the estimation of immunoreactive calcitonin in human serum. Parallel with the commercial RIA kit the estimation was made by the authors' own modification of this process with their own specific antiserum and radioligand prepared in the laboratory. The results in a large group of patients with medullary carcinoma of the thyroid gland (MCT) in different stages of the disease revealed that although the values obtained by the two methods differ, there is a statistically significant correlation between the values and the clinical evaluation is also comparable.

The authors compared the results of two methods for the estimation of antibodies against thyroglobulin: the principle of the Thymune-T test (Wellcome) is a haemagglutination method; parallel examinations were made by the author's modification of the radiochemical coprecipitation test (RCT). The laboratory results were evaluated in a large clinical group of patients with different types of thyroid disease. It was confirmed that the haemagglutination method gives more frequently positive results than the radiochemical test, in particular in the range of low titres. The chi square test provided, however, evidence that between methods a significant correlation exists. Although the results of the two tests are not necessarily identical in individual patients, it may be stated that RCT is an adequate method of choice, as it is simpler, more rapid and cheaper, and that it can be done in large series with objective automated evaluation of data.

In 42 patients (17 hypothyroid -Hypo, 14 euthyroid -Eu and 11 thyrotoxic -TX) the intravenous glucose tolerance test was performed before and after triamcinolone sensitization. The blood sugar (BS), immunoreactive insulin (IRI), growth hormone (GH), inorganic phosphorus (P), calcium (Ca), and total proteins (TP) were assessed and glucose disappearance rate was calculated from absolute values (KGt) and from increments (KGi) of glucose. Triamcinolone caused a rise of BS in Eu an Tx subgroups and a decrease of KGt and KGi in Hypo and decrease of KGt in Tx. No differences were found in patterns of IRI, GH, TP and Ca, respectively, but a significant rise of P after triamcinolone was found in Hypo and Eu subgroup. When comparing the subgroups according to thyroid function, only mild differences could be demonstrated in BS pattern both during IVGTT and TIVGTT, but significant differences of KGt and KGi were found between Eu and Hypo subgroups. No differences were observed in patterns of IRI, GH and Ca (except fasting values in Eu and Hypo subgroups during IVGTT, but significant differences were found in the pattern of P and particularly of TP. Except of differences of TP and P, changes during TIVGTT were not more pronounced than those during IVGTT. The comparison of effects of triamcinolone pretreatment on IVGTT and on OGTT (as were described previously) demonstrated similar effects of triamcinolone in Eu subgroups (significant changes of BS, IRI and P), but different action in Hypo- and Tx subgroups (where the significant changes of BS, IRI and P during OGTT and IVGTT were not parallel). These findings may suggest a possible role of some gastrointestinal and/or hepatic factors in action of triamcinolone in impaired thyroid function, but not in normal persons.

In a group of 35 subjects (12 hypothyroid, 11 euthyroid and 12 hyperthyroid) the changes of FFA, inorganic phosphorus, calcium and total proteins were investigated during glucose tolerance test with (TGTT) or without (OGTT) triamcinolone pretreatment. In hypothyroidism the decrease of FFA during TGTT was blunted, whereas the pattern of phosphatemia was unaltered and total proteins were increased. In euthyroidism triamcinolone exerts the opposite effect on the behaviour of FFA (decline was smaller and shorter) and phosphatemia (decline was pronounced and persists for longer period). In hyperthyroidism the fasting level of FFA decreased in TGTT but during the test no significant differences in FFA and phosphatemia patterns were observed. The calcemia was not influenced by triamcinolone in any subgroup. Our results suggest the importance of thyroid function as modifying factor in some metabolic effects of triamcinolone in man. Changes in hypothyroidism are in good agreement with changes of blood glucose and insulinemia, as described previously. Opposite effect of triamcinolone on FFA and phosphatemia pattern in euthyroidism may suggest the changes of sensitivity of some peripheral tissues to insulin action. The decline of fasting level of FFA after triamcinolone in hyperthyroidism is surprising and might be of importance in maintaining unaltered glucose tolerance after triamcinolone in this subgroup.

There is discussed the surgical treatment of massive otherwise uncontrolable haemorrhage from oesophageal varices during advanced pregnancy. The authors have performed in the above situation a splenorenal shunt in a women suffering from the prehepatic portal hypertension during her sixth month of pregnancy. The pregnancy was maintained but an unviable child was born. The authors were able to find only 5 quotations from the references available to them dealing with 5 similar cases in women who delivered healthy babies thanks to a surgical intervention (5 times a portal systemic shunt, once a direct operation). The most important prevention of those exceptional surgical indications is the careful evaluation of all circumstances for a pregnancy in women with portal hypertension.
