Abstract

To overcome hypoxia in tissue engineering, it has been suggested that photosynthesis could function as an alternative source of oxygen supply. In this work, C. reinhardtii algae were seeded in collagen constructs to produce photosynthetic scaffolds for dermal regeneration. Further, C. reinhardtii were genetically modified to produce the angiogenic growth factors hVEGF-165 and hPDGF-B, in addition to oxygen. This work gives evidence for the use of gene modified algae as an alternative gene therapy and proposes their implementation to treat a broad spectrum of hypoxic conditions., Das regenerative Potenzial künstlicher Gewebe ist durch Hypoxie begrenzt. Um diesem Nachteil entgegen zu wirken wurde die lokale Sauerstoffzufuhr durch Photosynthese vorgeschlagen. Im Rahmen dieser Arbeit wurden C. reinhardtii Algen in Kollagenmatrices eingesäht um photosynthetisch aktive Scaffolds für die dermale Regeneration herzustellen. Durch Erzeugung transgener Algen wurde zusätzlich die Freisetzung der Wachstumsfaktoren hVEGF-165 und hPDGF-B ermöglicht. Die Ergebnisse setzen die Grundlage für die Verwendung von genetisch veränderten Algen als alternative Gentherapie zur Behandlung eines breiten Spektrums hypoxischer Konditionen.

Abstract

The prevention of ischaemia and the adequate restitution of blood flow to ischaemic tissue are pivotal to halt the progression of cellular injury associated with decreased oxygen and nutrient supply. Accordingly, the search for novel strategies which aim at preventing ischaemia-reperfusion-induced tissue damage is still of major interest in flap surgery. Preconditioning represents an elegant approach to render the tissue more resistant against deleterious ischaemic insults. For many decades, 'surgical delay' has been the standard method of tissue preconditioning. During the last 10 years, ischaemic preconditioning was added to the repertoire of plastic surgeons to protect flaps from ischaemic necrosis. The invasiveness and expenditure of time of these procedures, however, have always been major drawbacks, hindering a wide distribution in clinical practice. Consequently, the motivation has all along been to further refine and simplify protective strategies. Recent experimental studies have now shown that efficient protection from ischaemic necrosis can also be achieved by remote preconditioning or pretreatment with chemical agents and growth factors, which mimic the action of surgical delay and ischaemic preconditioning. In addition, the local application of unspecific stressors, including both heating and cooling, have been shown to effectively improve flap microcirculation and, thus, tissue survival. In view of successful translational research, it is now time that the efficacy of these novel preconditioning procedures is proven in prospective randomised clinical trials.

Abstract

Skin-sparing mastectomy is a new surgical approach that allows a mastectomy while preserving the natural envelope of the breast. Skin-sparing mastectomy followed by immediate reconstruction can be used for prophylaxis for high- risk patients or BRCA carriers. It represents an effective treatment option for patients with extensive DCIS or early invasive breast cancer, but is contraindicated for inflammatory breast cancer and extensive skin involvement by the tumor. Skin-sparing mastectomy had similar surgical outcomes compared to non- skin- sparing mastectomy, but skin flap ischemia and necrosis is more common and is associated with a range of risk factors, including smoking. Skin-sparing mastectomy seems to be an oncologically safe technique and does not increase in particular the risk of local, regional or systemic recurrences. It facilitates immediate breast reconstruction using implants or myocutaneous flaps, resulting in excellent cosmesis and high level of patient satisfaction. This article reviews the published data on skin-sparing mastectomy and immediate reconstruction and aim to establish its current role in clinical practice, as there is a lack of prospective data.

Abstract

Early pedicle occlusion following microvascular flap transfer is the major cause for flap loss, which is most often surgery-associated and occurs usually within the first 48 h postoperatively. In unusual postoperative courses, patient-associated factors (e.g. thrombophilic disposition) have to be investigated. We present a case of posttraumatic reconstruction of a full-thickness skin defect of the foot in a healthy young man with a microvascular flap. At day 4 after surgery, a complete pedicle occlusion of the flap occurred resulting in a removal of the flap. Six days after transfer of another flap, a complete pedicle occlusion occurred as well in the second flap. The second flap survived due to engraftment through neovascularization. Retrospectively, antithrombin deficiency was detected. This case presents flap survival resulting from early engraftment after total pedicle occlusion, indicating the importance of a thorough and focused medical history and specific preoperative investigation if patient history is suspect for coagulopathy.
