A number of illegal drug tablets with unknown constituents are supplied to countries around the world, most of which are habit forming. Amphetamine constitutes the majority of illegal tablets supplied to Saudi Arabia. In this study, we investigated illicit amphetamine tablets seized from Jazan region located in the southwest of Saudi Arabia to identify the insidious additives present in them and their health-related risks. Tablets were analyzed for the presence of amphetamine and other additives using gas chromatography-mass spectroscopy (GC-MS) technique. Amphetamine was detected in good to high area % values in all analyzed tablets in the range of 16.29–41.23%. Interestingly, a number of other additives were also detected with amphetamine in most of the tested samples including caffeine, lidocaine, diphenhydramine, and 8-chlorotheophylline in considerable area %. Caffeine may have been added to enhance the psychotic effect of amphetamine, whereas lidocaine was added to prevent the cardiovascular side effects of amphetamine. Diphenhydramine was probably added to prevent other undesirable side effects of amphetamine such as insomnia and tremors. Chemometric hierarchical cluster analysis was carried out to make samples clusters which have similar characteristics. It resulted into a dendrogram tree showing eight clusters signifying different sources of tablet samples. Owing to the toxic effects of amphetamine and other psychoactive constituents in the tested tablets, the illegal trafficking of these tablets should be prevented by all means and public awareness should be increased.

Severe acute respiratory syndrome coronavirus (CoV)-2 (SARS-CoV-2), previously called 2019 novel CoV, emerged from China in late December 2019. This virus causes CoV disease-19 (COVID-19), which has been proven a global pandemic leading to a major outbreak. As of June 19, 2020, the data from the World Health Organization (WHO) showed more than 8.7 million confirmed cases in over 200 countries/regions. The WHO has declared COVID-19 as the sixth public health emergency of international concern on January 30, 2020. CoVs cause illnesses that range in severity from the common cold to severe respiratory illnesses and death. Nevertheless, with technological advances and imperative lessons gained from prior outbreaks, humankind is better outfitted to deal with the latest emerging group of CoVs. Studies on the development of in vitro diagnostic tests, vaccines, and drug re-purposing are being carried out in this field. Currently, no approved treatment is available for SARS-CoV-2 given the lack of evidence. The results from preliminary clinical trials have been mixed as far as improvement in the clinical condition and reduction in the duration of treatment are concerned. A number of new clinical trials are currently in progress to test the efficacy and safety of various approved drugs. This review focuses on recent advancements in the field of development of diagnostic tests, vaccines, and treatment approaches for COVID-19.

Cardiovascular and related diseases (CVRDs) are among the most prevalent chronic diseases in the 21st century, with a high mortality rate. This review summarizes the various nanomedicines for diagnostic and therapeutic applications in CVRDs, including nanomedicine for angina pectoris, myocarditis, myocardial infarction, pericardial disorder, thrombosis, atherosclerosis, hyperlipidemia, hypertension, pulmonary arterial hypertension and stroke. Theranostic nanomedicines can prolong systemic circulation, escape from the host defense system, and deliver theranostic agents to the targeted site for imaging and therapy at a cellular and molecular level. Presently, discrete non-invasive and non-surgical theranostic methodologies are such an advancement modality capable of targeted diagnosis and therapy and have better efficacy with fewer side effects than conventional medicine. Additionally, we have presented the recent updates on nanomedicine in clinical trials, targeted nanomedicine and its translational challenges for CVRDs. Theranostic nanomedicine acts as a bridge towards CVRDs amelioration and its management.

The oxidation kinetics of porous Ni-12 wt%Cr alloy with 23.7% porosity was investigated for 100 h at 1000 °C in air. The phases forming the oxide scale were determined by using X-ray diffractometry. The topography of the surface of the oxide scale and the microstructure of the cross-section of the oxide scale were investigated by utilizing optical and scanning electron microscopy and X-ray energy dispersive analysis. The oxidation rate was initially rapid and then it gradually decreased to lower values over a period of about 24 h until realizing the steady-state condition with linear rate constant of about 3.7 × 10−8 g/m2 sec. Double multilayered porous scale of NiO-Cr2O3 sublayers with internal precipitates of nickel spinel within the NiO layers was formed. The growth of the double multilayer scale was explained on the basis of inward migration of molecular oxygen. Genome analysis of individuals affected by retinitis pigmentosa (RP) identified two rare nucleotide substitutions at the same genomic location on chromosome 11 (g.61392563 [GRCh38]), 69 base pairs upstream of the start codon of the ciliopathy gene TMEM216 (c.-69G>A, c.-69G>T [GenBank: NM_001173991.3]), in individuals of South Asian and African ancestry, respectively. Genotypes included 71 homozygotes and 3 mixed heterozygotes in trans with a predicted loss-of-function allele. Haplotype analysis showed single-nucleotide variants (SNVs) common across families, suggesting ancestral alleles within the two distinct ethnic populations. Clinical phenotype analysis of 62 available individuals from 49 families indicated a similar clinical presentation with night blindness in the first decade and progressive peripheral field loss thereafter. No evident systemic ciliopathy features were noted. Functional characterization of these variants by luciferase reporter gene assay showed reduced promotor activity. Nanopore sequencing confirmed the lower transcription of the TMEM216 c.-69G>T allele in blood-derived RNA from a heterozygous carrier, and reduced expression was further recapitulated by qPCR, using both leukocytes-derived RNA of c.-69G>T homozygotes and total RNA from genome-edited hTERT-RPE1 cells carrying homozygous TMEM216 c.-69G>A. In conclusion, these variants explain a significant proportion of unsolved cases, specifically in individuals of African ancestry, suggesting that reduced TMEM216 expression might lead to abnormal ciliogenesis and photoreceptor degeneration.

Abstract Background Rural areas in the state of New Mexico have been the “ground-zero” for the epidemic of diabetic Chronic Kidney Disease (CKD) in the United States. However, there is limited research about risk factors of diabetic CKD in this area and scarce data regarding the performance of emerging markers of renal filtration and epigenetic biomarkers of renal function and diabetes in this area with its unique ethnic/racial population. We designed the COMPASS study as a community-based program in rural New Mexico aiming to screen for CKD and to discover CKD-related translational biomarkers. Methods/design The study involves a prospective, longitudinal cohort design involving individuals living in rural New Mexico. Participants undergo a screening for kidney disease using markers of abnormal renal filtration (impaired glomerular filtration rate) or damage (albuminuria). Those found to have CKD on the basis of these tests or those at risk for CKD are enrolled in a prospective longitudinal cohort. We measure markers of renal function, insulin resistance and epigenetics (microRNAs) on patients. Individuals are invited to participate in interviews and focus groups in order to characterize their attitudes towards research and barriers or facilitators to participation in future research studies about kidney disease. Discussion This study will provide important data about the local epidemiology of kidney disease in a high-risk rural setting and the utility of emerging renal filtration markers (Beta 2 Microglobulin and Cystatin C), while generating data and methods for the analyses of microRNA biomarkers. The qualitative research subproject will identify factors associated with increased willingness to participate in future translational research projects. With its geographical focus, this study will address a critical disparity in kidney disease research, while generating novel epigenetic data that are relevant for future studies in the general population.

Culinary spices and herbs have been used to impart a characteristic flavour and aroma in food due to their appealing fragrance. Recently, bioactive compounds from herbs, especially phenolics, have gained much attention due to their potential health outcomes. The aim of this study was to characterize and quantify the phenolic compounds from 10 widely used Australian-grown herbs (oregano, rosemary, bay, basil, sage, fenugreek, dill, parsley, mint and thyme). For this purpose, liquid chromatography mass spectrometry (LC-MS) was used for the complete profiling of polyphenolic compounds and quantification of abundant phenolic compounds was completed with high-performance liquid chromatography—photodiode array detection (HPLC-PDA). Polyphenols from Australian-grown herbs were estimated through total phenolic content (TP), total flavonoids (TF) and total tannins (TT) along with their in-vitro antioxidant activities. Oregano and mint were estimated with the highest value of TP (140.59 ± 9.52 and 103.28 ± 8.08 mg GAE/g, milligram gallic acid equivalent/gram) while rosemary and mint had the highest TF (8.19 ± 0.74 and 7.05 ± 0.43 mg QE (quercetin equivalent)/g). In this study, eighty-four (84) phenolic compounds were screened and confirmed through LC-MS/MS by comparing their masses and fragmentation pattern with published libraries. The results of this study validate the use of these herbs as bioactives and their positive impact on human health.

Bananas are an essential source of staple food and fruit worldwide and are widely regarded as the world’s largest fruit crop, with more than 100 million tons total annual production. Banana peel, a by-product that represents about 40% of the entire banana’s weight, and pulp are rich in bioactive compounds and have a high antioxidant capacity. As the production of polyphenols in fruit and vegetables is highly dependent on environmental conditions, genetic factors, and the level of maturity, this study aims to characterize six Australian banana cultivars in various stages of ripening for their phenolic compounds using the liquid chromatography-electrospray ionization quadrupole time of flight mass spectrometry (LC-ESI-QTOF-MS/MS), polyphenols quantification with the high-performance liquid chromatography coupled with photodiode array detector (HPLC-PDA), and their antioxidant capacity. All bananas were analysed for total polyphenols content (TPC), total flavonoids content (TFC), and total tannin content (TTC) and their antioxidant activities. Ripe Ducasse peel and pulp contained the highest amounts of total polyphenols content (1.32 and 1.28 mg gallic acid equivalent (GAE) per gram of sample), total tannin contents (3.34 mg catechin equivalent (CE) per gram of sample), and free radical scavenging capacity (106.67 mg ascorbic acid equivalent (AAE) per g of sample). In contrast, ripe Plantain peel had the greatest total flavonoids (0.03 mg quercetin equivalent (QE) per g of sample). On the other hand, unripe Ladyfinger pulp possessed the highest total antioxidant activity (1.03 mg AAE/g of sample). There was a positive correlation between flavonoids and antioxidant activities. By using LC-ESI-QTOF-MS/MS, a total of 24 phenolic compounds were tentatively characterized in this research, including six phenolic acids, 13 flavonoids, and five other polyphenols. Quantification of phenolic compounds by the high-performance liquid chromatography coupled with photodiode array detector (HPLC-PDA) revealed a higher content of phenolic acids. These findings confirmed that banana peel and pulp have considerable antioxidant activity and can be employed in human food and animal feed for variant health enhancement uses.

The current food options, especially in the Western countries, are acidogenic, which reduce extracellular pH and perturb ionic homeostasis. The acidity-activated enzymes mediate a large panel of chronic and acute diseases. To quell the morbidities and mortalities associated with the metabolic syndromes, nonpharmacologic approaches are preferred. Dietary interventions are critical in this regard. Among the array of dietary patterns, paleo diet has garnered much attention in the last decade. This diet rich in phytochemicals and unsaturated fatty acids is deemed wholesome for the body. Several studies have proven this claim, yet their nutritional adequacy for all age groups is contentious. The popularity of ethnic foods is rising; but their nutrition and safety position can vary. Although some of them are close to paleo food, some are unhealthy. This review explores the mechanism by which paleo diet restores homeostasis of the body and how its inherent nutritional deficiency can be amended with conventional diet, by the formulation of personalized diet. This compilation is expected to be insightful for dietary optimization.

The oxidation behavior of two types of inhomogeneous nickel was investigated in air at 1273 K for a total oxidation time of 100 h. The two types were porous sintered-nickel and microstructurally inhomogeneous cast-nickel. The porous-nickel samples were fabricated by compacting Ni powder followed by sintering in vacuum at 1473 K for 2 h. The oxidation kinetics of the samples was determined gravimetrically. The topography and the cross-section microstructure of each oxidized sample were observed using optical and scanning electron microscopy. X-ray diffractometry and X-ray energy dispersive analysis were used to determine the nature of the formed oxide phases. The kinetic results revealed that the porous-nickel samples had higher trend for irreproducibility. The average oxidation rate for porous- and cast-nickel samples was initially rapid, and then decreased gradually to become linear. Linear rate constants were 5.5 × 10−8 g/cm2 s and 3.4 × 10−8 g/cm2 s for the porous- and cast-nickel samples, respectively. Initially a single-porous non-adherent NiO layer was noticed on the porous- and cast-nickel samples. After a longer time of oxidation, a non-adherent duplex NiO scale was formed. The two layers of the duplex scales were different in color. NiO particles were observed in most of the pores of the porous-nickel samples. Finally, the linear oxidation kinetics and the formation of porous non-adherent duplex oxide scales on the inhomogeneous nickel substrates demonstrated that the addition of new layers of NiO occurred at the scale/metal interface due to the thermodynamically possible reaction between Ni and the molecular oxygen migrating inwardly.

The pharmacist’s role is expanding from medication dispensing to patient care. There are several newer horizons in which clinical pharmacists have a vital role to play such as diabetic care, handling asthma patients, pain management, anti-coagulation therapy and in hyperlipidemia. Among these, the disorder that is perceived to reach epidemic proportions globally is diabetes mellitus. In the Arab world, particularly in countries like Saudi Arabia, the number affected and the cost incurred in the management of diabetes are relatively high. Apart from adding to the economic burden, the complications arising due to a lag in identification and management are manifold. In developed countries, several practicing hospitals and clinics have inducted clinical pharmacists as vital members of the healthcare team. With profound knowledge about medications, clinical pharmacists are inclined to stay abreast with recent developments in research and latest guidelines, thus supporting physicians in evidence based practice. Clinical pharmacists through Medication Therapy Management can render services by guiding, identifying and monitoring drug related issues faced by the patients. This can contribute in reducing the prevalence of diabetes mellitus and improving the quality of life. To keep pace with the growing prevalence of diabetes in the kingdom, clinical pharmacists need to undergo specialized training in high risk areas. Health regulating bodies should be keen in adopting provisions of inducting them in discrete specialties which can also help in trimming heathcare expenditure.

The popularity of energy beverages among young adult population is high. These drinks are claimed to boost energy and performance and contain high concentration of caffeine as one of the several ingredients. Discrepancies have been encountered by some of the previous studies between the actual quantity of caffeine present in the product and the amount mentioned on the label, making the determination of caffeine content in these drinks very important. Thus, in this study, we analyzed the caffeine concentration in most popular energy drinks available in Saudi Arabia. The energy drink samples (n = 9) were procured from retail outlets. Sample solutions were prepared in methanol and analyzed for caffeine content by GC-MS. Chromatographic parameters were optimized to achieve optimum resolution and various validation parameters were evaluated. The method was successfully applied for the quantification of caffeine in energy drinks by directly injecting the multifold diluted samples in methanol. The method was linear (r2 = 0.999) over a concentration range of 5–25 µg/mL, specific, precise (%RSD of peak area = 0.56–0.78), and accurate (%recovery = 99.3–101.2%). The amounts of caffeine determined were found in the range of 20.82–33.72 mg/100 mL (52.05–84.3 mg/pack). Results revealed that the amount of caffeine actually present in the tested drinks varied within ±10% range from the amount specified on the product labels. The amounts of caffeine detected in tested beverages were within the USFDA safe upper limit of per-day caffeine consumption, which has indicated that the consumption of one serving of energy drink is unlikely to produce any adverse health effect.

Objectives. National estimates of healthcare expenditures by types of services for adults with comorbid diabetes and eye complications (ECs) are scarce. Therefore, the first objective of this study is to estimate total healthcare expenditures and expenditures by types of services (inpatient, outpatient, prescription, and emergency) for adults with ECs. The second objective is to estimate the out-of-pocket spending burden among adults with ECs. Study Design. A cross-sectional study design using data from multiple panels (2009-2015) of the Medical Expenditure Panel Survey was employed. The sample included adults aged 21 years or older with diabetes (n=8,420). Principal Findings. Of adults with diabetes, 18.9% had ECs. Adults ECs had significantly higher incremental total medical expenditures of $3,125. The highest incremental expenditures were associated with outpatient and prescription drugs. After controlling for sex, age, race, poverty level, insurance coverage, prescription coverage, perceived physical and mental health, the number of chronic physical and mental conditions, marital status, education, the region of residence, smoking status, exercise, and chronic kidney disease (CKD), there was no difference in the out-of-pocket spending burden between adults with and those without ECs. However, adults with comorbid diabetes and CKD were more likely to have the out-of-pocket spending burden than those without CKD. Conclusions. The study showed that ECs in individuals with diabetes are associated with high incremental direct medical and out-of-pocket expenditures. Therefore, it requires more health initiatives, interventions, strategies, and programs to address and minimize the risk involved in such affected individuals.

Salvadora persica L. (Salvadoraceae) is an evergreen shrub growing in the Middle East, Africa, and Southern Asia. It is traditionally known as “miswak” and used as toothbrushes and for the treatment of toothache, gum diseases, boils, chest infection, gonorrhea, headache, spleen troubles, stomachache, and ulcers. To the best of our knowledge, this is the first study aimed at conducting phytochemical, cytotoxic, and antimicrobial investigations of the fruits (berries) of S. persica collected from the Jazan region of Saudi Arabia. Analysis of the ethanol extract of S. persica fruits using GC-MS showed the presence of six esters (20.71%), seven alkanes (15.47%), tetracosamethyl-cyclododecasiloxane (9.91%), eicosamethyl-cyclodecasiloxane (7.27%), and 1-monolinoleoylglycerol (5.17%). The predominant constituents were acetyl dasycarpidan-1-methanol (10.47%), tetracosamethyl-cyclododecasiloxane (9.91%), eicosamethyl-cyclodecasiloxane (7.27%), and 1-monolinoleoylglycerol (5.17%). The petroleum ether extract of the fruits contained mainly eicosamethyl-cyclodecasiloxane (23.81%), 1-monolinoleoylglycerol (11.78%), (Z,Z,Z)-9,12,15-octadecatrienoic acid ethyl ester derivative (10.56%), and tetracosamethyl-cyclododecasiloxane (9.91%). The cytotoxic properties of the ethanol extract were investigated by MTT assay against the breast MCF7, ovary A2780, and colon HT29 cells. The fruit extract of S. persica was selective against the ovarian and colon cancer cells compared to normal fibroblast cells (MRC5) as it showed IC50 values 17.50, 8.35, and 5.12, against MCF7, A2780, and HT29 cells, respectively. Interestingly, the fruit extract was also found to possess selective antimicrobial activity for Streptococcus mutans isolates with minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) values of 3.12 and 6.25 mg/mL, respectively. Interestingly, it was found to be ineffective against other Gram-positive as well as Gram-negative microorganisms. This study provides insight into the bioactive components present in the fruits of the plant that can be utilized for its cytotoxic and antimicrobial properties.

Previous studies indicated that a high proportion of adults with diabetes do not receive recommended preventive care in the United States. Nevertheless, a comprehensive evaluation of the factors associated with the receipt of most recommended preventive care measures collectively is lacking. Therefore, this study describes the utilization of multiple preventive care measures collectively. Moreover, this paper aims to identify factors associated with receiving the recommended preventive care. A cross-sectional study design was implemented using data from multiple panels (2009–2015) of the Medical Expenditure Panel Survey. The sample included adults aged 21 years or older with diabetes (n = 8415). The outcome for this study was either receiving five selected preventive care measures (HbA1c tests, cholesterol tests, foot examinations, dilated eye examinations, and influenza vaccines) collectively or not. Multivariable logistic regressions were performed among all adults with diabetes, those with multimorbidity, chronic kidney disease (CKD) or eye complications. Adults with diabetes were poorly adherent to receiving the five preventive care measures collectively (15.6%). Among all adults with diabetes, factors associated with receiving all the selected preventive practices included age, education, health insurance, prescription drug coverage, duration of diabetes, number of chronic conditions and smoking status. Similar results were observed among adults with multimorbidity. Among adults with CKD, those with private insurance and drug prescription coverage were more likely to receive the recommended practices. The findings suggest low adherence to receiving all five recommended practices. It is crucial to increase the awareness about the need for all the recommended practices among adults with diabetes. The increased bacterial resistance to synthetic antibiotics and consumer awareness about the health and food safety concerns have triggered the ban on the use of antibiotic growth promotors (AGPs) in the poultry industry. This situation encouraged the poultry sector and industry to explore safe alternatives to AGPs and focus on developing more sustainable feed management strategies to improve the intestinal health and growth performance of poultry. Consequently, phytogenic feed additives (PFAs) have emerged as natural alternatives to AGPs and have great potential in the poultry industry. In recent years, cinnamon (one of the most widely used spices) has attracted attention from researchers as a natural product with numerous health benefits for poultry. The essential oils in cinnamon, in particular, are of interest because of their antioxidant, anti-microbial, anti-inflammatory, antifungal, and hypocholesterolaemic effects, in addition to their ability to stimulate digestive enzymes in the gut. This review mainly emphasizes the potential impact of cinnamon as a natural feed additive on overall gut health, nutrient digestibility, blood biochemical profile, gene expression, gut microbiota and immune response.

Breast cancer is the most widespread malignancy in women worldwide. Nanostructured lipid carriers (NLCs) have proven effective in the treatment of cancer. NLCs loaded with imatinib (IMA) (NANIMA) were prepared and evaluated for their in vitro efficacy in MCF-7 breast cancer cells. The hot homogenization method was used for the preparation of NANIMAs. An aqueous solution of surfactants (hot) was mixed with a molten mixture of stearic acid and sesame oil (hot) under homogenization. The prepared NANIMAs were characterized and evaluated for size, polydispersity index, zeta potential, encapsulation efficiency, release studies, stability studies, and MTT assay (cytotoxicity studies). The optimized NANIMAs revealed a particle size of 104.63 ± 9.55 d.nm, PdI of 0.227 ± 0.06, and EE of 99.79 ± 0.03. All of the NANIMAs revealed slow and sustained release behavior. The surfactants used in the preparation of the NANIMAs exhibited their effects on particle size, zeta potential, encapsulation efficiency, stability studies, and release studies. The cytotoxicity studies unveiled an 8.75 times increase in cytotoxicity for the optimized NANIMAs (IC50 = 6 µM) when compared to IMA alone (IC50 = 52.5 µM) on MCF-7 breast cancer cells. In the future, NLCs containing IMA will possibly be employed to cure breast cancer. A small amount of IMA loaded into the NLCs will be better than IMA alone for the treatment of breast cancer. Moreover, patients will likely exhibit less adverse effects than in the case of IMA alone. Consequently, NANIMAs could prove to be useful for effective breast cancer treatment.

Chicory and lucerne are used as specialised forages in sheep or dairy production systems in some parts of the world. Recently, these plants are gaining attention as raw materials in the search for natural antioxidants for use in animal feeds, human foods and nutraceutical formulations. The antioxidant potential of these plants is credited to polyphenols, a subgroup of phytochemicals. Therefore, phenolic characterisation is an essential step before their use as ingredients in animal feeds, human food or nutraceutical preparations. In this study, we performed qualitative and quantitative analysis of polyphenols in chicory and lucerne. Profiling of polyphenols from chicory and lucerne was performed by LC-ESI/QTOF-MS with a total of 80 phenolic compounds identified in chicory and lucerne. The quantification of polyphenols was achieved by high performance liquid chromatography, coupled with a photo diode array (HPLC-PDA). Chicoric acid was the major phenolic acid found in chicory, with the highest concentration (1692.33 ± 0.04 µg/g DW) among all the polyphenols quantified in this study. 2-hydroxybenzoic acid was the major phenolic acid found in lucerne, with the highest concentration of 1440.64 ± 0.04 µg/g DW. Total phenolic, flavonoids and total tannin contents were measured, and the antioxidant potential was determined by 2,2-Diphenyl-1-picrylhydrazyl, Ferric Reducing Antioxidant Power, 2,2-Azino-bis-3-ethylbenzothiazoline-6-sulfonic Acid, Hydroxyl (OH−) Radical Scavenging Activity, Chelating Ability of Ferrous Ion (Fe2+) and Reducing Power (RPA) assays. Both chicory (8.04 ± 0.33 mg AAE/g DW) and lucerne (11.29 ± 0.25 mg AAE/g DW) showed high values for Hydroxyl (OH−) Radical Scavenging Activity. The current study allowed us to draw a profile of polyphenols from chicory and lucerne. They provided a molecular fingerprint useful for the application of these plant materials in human foods, animal feeds and pharmaceutical formulations.
