Background Conduction system pacing (CSP), encompassing left bundle branch area pacing (LBBAP) and His-bundle pacing (HBP), is a novel pacing strategy, which overcomes limitations of conventional right ventricular pacing. Feasibility and early safety of both strategies have been established previously. While HBP is mentioned as an alternative pacing strategy in the latest European and American pacing guidelines, implantation recommendations regarding LBBAP are scarce. Notably, HBP was described to be associated with a significantly higher procedural failure rate compared to LBBAP, making it potentially more contestable. Purpose To compare procedural outcomes after HBP and LBBAP lead implantation. Methods We prospectively assessed 303 consecutive CSP lead implantation attempts at our center from 08/2018 to 11/2022. 81% of all implantations were performed by two high-volume device implanters. Successful conduction system pacing was established according to standard criteria (HBP: QRS morphology, R-wave peak time in V6, programmed stimulation, visibility of a His potential; LBBAP: R-wave peak time in V6, V1-V6-interpeak interval, programmed stimulation or visibility of a left bundle potential) and pacing thresholds (HBP: Results Patients with a HBP attempt were younger and had a narrower QRS complex but did not differ with respect to left or right ventricular function and dimension (table showing percentages and median values/interquartile ranges). LBBAP systems were more often implanted in lieu of CRT, whereas HBP systems were more commonly used in case of AV block without heart failure (table). Conventional stylet-driven and non-stylet-driven leads have been used as a conduction system pacing lead. Procedural outcome data comparing both lead types are, however, scarce. Purpose To compare procedural outcomes during CSP device implantation with respect to the use of stylet-driven vs. non-stylet-driven leads. Methods We prospectively assessed 170 consecutive LBBAP lead implantation attempts at our center from 09/2021 to 11/2022. All implanters had previous experience with His-bundle pacing lead implantation using the non-stylet-driven lead and 91% of all systems were implanted by two high-volume device implanters. Successful conduction system pacing was established according to standard criteria (LBBAP: R-wave peak time in V6, V1-V6-interpeak interval, programmed stimulation or visibility of a left bundle potential) and pacing thresholds ( Results Clinical baseline characteristics, echocardiography/ECG data and indications did not differ between patients, who received a stylet-driven or non-stylet-driven lead (table showing percentages and median values/interquartile ranges). Patients underwent implantation of a non-stylet driven 3830 SelectSure lead in 20% of cases and a stylet-driven lead in 80% of cases (Solia 76%, Ingevity+ 3%, Tendril STS 1%). Electrical outcome data did not differ between lead types. However, procedural success rate was significantly higher using stylet-driven leads (p=0.025) and a trend towards shorter overall intervention duration was observed as well (p=0.1). Conclusion Stylet-driven leads offer higher LBBAP lead implantation success rates while shortening implant duration.

Background Leadless pacemakers (LP) capable of VDD function are a cornerstone of modern pacemaker therapy reducing complications in elderly patients while enabling atrioventricular synchrony through mechanical sensing of atrial contraction. Because mechanical sensing is less reliable than electrical sensing, the reliability of VDD function in LP varies. We evaluated the p-wave amplitude from patient ECGs as a predictor for successful VDD function in LP. Materials and methods Consecutive patients undergoing LP implantation capable of VDD function were included in this multi-center retrospective cohort study. As VDD function is mainly dependent on the A4 signal amplitude in the current generation of the device, ECG parameters pre-implantation were evaluated separately and in combination for association with A4 signal amplitude post-implantation. Results 122 Patients (mean age 81, female 44%, mean BMI 25.9 kg/m2) were included. Main indication for LP implantation was AV-Block (70%). BMI-adjusted P-wave amplitude demonstrated a statistically significant positive correlation with A4 signal amplitude in leads I, II, aVL, aVF, aVR, V1 and V2 (all p Conclusions P-wave amplitudes in selected ECG leads can predict A4 signal amplitude and therefore the reliability of VDD function in LP.

Background Left bundle branch area pacing (LBBAP) represents a novel cardiac pacing approach that overcomes limitations of conventional right ventricular (septo-) apical pacing. Feasibility and early safety have been shown previously. However, technical limitations (non-dedicated leads, limited availability of suitable implantation sheats) still may preclude successful conduction system pacing (CSP) lead implantation. So-called drilling and entanglement effects can prevent lead implantation at a suitable site and multiple screw-in attempts may be required. However, lead implantation may ultimately still be unsuccessful. Purpose To evaluate the number of total screw-in attempts until abandonment of LBBAP lead implantation during the procedure. Methods We prospectively assessed 170 consecutive LBBAP lead implantation attempts at our center from 09/2021 to 11/2022. All implanters had previous experience with His-bundle pacing lead implantation. Successful conduction system pacing was established according to standard criteria (R-wave peak time in V6, V1-V6-interpeak interval, programmed stimulation or visibility of a left bundle potential) and pacing thresholds Results Patient baseline characteristics and procedural outcomes are shown in the table (percentages and median values with interquartile range in brackets). Overall CSP lead implantation success rate was 91%. Two patients (1%) experienced complications (one atrial lead dislodgement, one pneumothorax). Median number of screw-in attempts in all interventions was 2 (interquartile range [IQR] 1-4, maximum 12). If implantation was successful, a maximum of four attempts provided already 90% of the overall chance of success (figure). Additional lead implantation attempts rarely provided an additional benefit but prolonged overall implantation duration. Conclusion To balance implantation success rates with procedure duration, the number of screw-in attempts during LBBAP lead implantation may be limited to 4-5 attempts. Bail-out strategies (implantation of a cardiac resynchronization pacemaker or a conventional pacemaker) may be considered if CSP lead implantation is unsuccessful after 4-5 attempts.

Funding Acknowledgements Type of funding sources: None. Background Pulsed-field ablation (PFA) has recently been introduced for pulmonary vein isolation (PVI). It might also be of potential value for redo procedures after failed radiofrequency or cryo ablation for atrial fibrillation (AF). Purpose To describe our initial experience using a multipolar PFA catheter for redo procedures after failed AF ablation. Methods Consecutive patients undergoing a redo procedure for recurrent atrial arrhythmias at our institution using a multipolar PFA catheter from Mai 2021 until October 2022 were included. A 3D electro-anatomical bipolar voltage map was generated to identify pulmonary vein (PV) reconnection, quantify the level of isolation (antral vs. ostial isolation), and characterize the extent of extra-PV low-voltage tissue. In case of PV reconnection, re-isolation was performed using PFA, with at least four applications in both basket and flower configuration. In case of only ostial PV isolation, additional PFA applications in flower configuration were delivered to widen the isolation to a more antral level. If all veins were already isolated on an antral level and/or if low-voltage areas were found on the posterior wall, a posterior wall ablation was added with the same PFA device at the discretion of the operator. Results We identified 170 patients (mean age: 64 ±11years, 45 (27%) women), undergoing a redo procedure using PFA. The median number of previous ablations was 1 (range 1-6), the most recent ablation was performed using radiofrequency in 122 (72%), cryo in 42 (25%), and surgery in 5 (3%) patients. In 104 (61%) patients, at least one reconnected PV was found. In total, 244 of 680 PV (36%) showed reconnection. In addition to PV reisolation, additional antral applications were delivered in 205 of 436 still isolated veins (47%). Posterior wall ablation was added in 112 (66%) patients (43% in paroxysmal AF, 78% in persistent AF, Figure). Median procedure duration was 101 minutes (IQR 80-137). There was no cardiac tamponade, no stroke, and no esophageal fistula observed in this cohort. Conclusion For patients with recurrence after catheter ablation of atrial fibrillation, re-ablation using PFA is feasible and safe. Most common ablation targets were the pulmonary veins and the posterior wall. Whether the complementary effect of a different technology improves outcome needs further investigation.

Background While the sexuality of patients with endometriosis is an established topic in research, the possible effect of endometriosis on partnership sexuality has come to the fore only recently. To improve counseling, more information is needed on how both partners experience sexuality in the context of endometriosis. Aim Previous research regarding endometriosis and sexuality normally focused on one partner to explore couples’ intimate relations, whereas this study provides a comparison on both partners’ perspectives on their common sexuality. Methods An overall 302 couples received a questionnaire based on the Brief Index of Sexual Functioning and Sexual History Form, which was modified by endometriosis specialists to better focus on endometriosis-specific aspects. To detect different perspectives on common sexuality within the couple, the Wilcoxon test and the Pearson chi-square test were performed. Outcomes Various aspects of couple sexuality were assessed by both partners to investigate divergent perspectives between the man and the woman within a couple. Results On one hand, male and female partners seem to have divergent perspectives on sexual satisfaction in general, desired frequency of sexual contacts, and the question of the female partner engaging in sexual activity despite discomfort. On the other, they have similar perspectives on who takes initiative in sexual contacts, satisfaction with variety in the sexual relationship, and the impact of sexual limitations on their satisfaction within the partnership. Clinical implications Endometriosis research addressing issues related to sexuality should include male partners; the same applies to consulting women with endometriosis in the context of their relationships rather than as individuals. Strengths and Limitations This is the first analysis conducted on a larger scale of data from both partners in couples dealing with endometriosis. As it provides quantitative information only, some qualitative information remains unexplored. Conclusion As both partners showed tendencies to overestimate their partners’ sexual satisfaction and had different perspectives on sensitive topics in sexuality, such as the female partner engaging in sexual activity despite discomfort, addressing sexual communication could be a starting point in counseling couples dealing with endometriosis.

AimsSubcutaneous implantable cardioverter defibrillators (S-ICDs) are well established. However, inappropriate shocks (IAS) remain a source of concern since S-ICDs offer very limited troubleshooting options. In our multicentre case series, we describe several patients who experienced IAS due to a previously unknown S-ICD system issue.Methods and resultsWe observed six patients suffering from this novel IAS entity. The IAS occurred exclusively in primary or alternate S-ICD sensing vector configuration (therefore called ‘Sense-B-noise’). IAS were caused by non-physiologic oversensing episodes characterized by intermittent signal saturation, diminished QRS amplitudes, and disappearance of the artefacts after the IAS. Noise/oversensing could not be provoked by manipulation, X-ray did not show evidence for lead/header issues and impedance measurements were within normal limits. The pooled experience of our centres implies that up to ∼5% of S-ICDs may be affected. The underlying root cause was discussed extensively with the manufacturer but remains unknown and is under further investigation.ConclusionSense-B-noise is a novel cause for IAS due to non-physiologic signal oversensing, arising from a previously unknown S-ICD system issue. Sense-B-noise may be suspected if episodes of signal saturation in primary or alternate vector configuration are present, oversensing cannot be provoked, and X-ray and electrical measurements appear normal. The issue can be resolved by reprogramming the device to secondary sensing vector.

BACKGROUND: Transvenous lead extraction (TLE) is the recommended management strategy for a variety of cardiac implantable electronic device (CIED) infections, malfunctions and other conditions. Large registries have established the safety and efficacy of TLE per se but temporal outcome data after the introduction of an institutional lead management programme remain scarce. OBJECTIVE: To investigate the impact of a structured institutional lead management programme on TLE outcomes. METHODS: All patients who underwent TLE at our institution between January 2013 and December 2020 were included. We assessed procedural outcomes after TLE for two separate time periods: from January 2013 to December 2018 and January 2019 to December 2020 (after introduction of a structured institutional lead management programme). RESULTS: In 2013–2018, the median number of TLE procedures per year at our centre was 14 (range 10–19, total 84). Background Every year, more than 2.5 million critically ill patients in the ICU are dependent on mechanical ventilation. The positive pressure in the lungs generated by the ventilator keeps the diaphragm passive, which can lead to a loss of myofibers within a short time. To prevent ventilator-induced diaphragmatic dysfunction (VIDD), phrenic nerve stimulation may be used. Objective The goal of this study is to show the feasibility of transesophageal phrenic nerve stimulation (TEPNS). We hypothesize that selective phrenic nerve stimulation can efficiently activate the diaphragm with reduced co-stimulations. Methods An in vitro study in saline solution combined with anatomical findings was performed to investigate relevant stimulation parameters such as inter-electrode spacing, range to target site, or omnidirectional vs. sectioned electrodes. Subsequently, dedicated esophageal electrodes were inserted into a pig and single stimulation pulses were delivered simultaneously with mechanical ventilation. Various stimulation sites and response parameters such as transdiaphragmatic pressure or airway flow were analyzed to establish an appropriate stimulation setting. Results Phrenic nerve stimulation with esophageal electrodes has been demonstrated. With a current amplitude of 40 mA, similar response figures of the diaphragm activation as compared to conventional stimulation with needle electrodes at 10mA were observed. Directed electrodes best aligned with the phrenic nerve resulted in up to 16.9 % higher amplitude at the target site in vitro and up to 6 cmH20 higher transdiaphragmatic pressure in vivo as compared to omnidirectional electrodes. The activation efficiency was more sensitive to the stimulation level inside the esophagus than to the inter-electrode spacing. Most effective and selective stimulation was achieved at the level of rib 1 using sectioned electrodes 40 mm apart. Conclusion Directed transesophageal phrenic nerve stimulation with single stimuli enabled diaphragm activation. In the future, this method might keep the diaphragm active during, and even support, artificial ventilation. Meanwhile, dedicated sectioned electrodes could be integrated into gastric feeding tubes.

Introduction: Contact force-sensing catheters are widely used for ablation of cardiac arrhythmias. They allow quantification of catheter-to-tissue contact, which is an important determinant for lesion formation and may reduce the risk of complications. The accuracy of these sensors may vary across the measurement range, catheter-to-tissue angle, and amongst manufacturers and we aim to compare the accuracy and reproducibility of four different force sensing ablation catheters. Methods: A measurement setup containing a heated saline water bath with an integrated force measurement unit was constructed and validated. Subsequently, we investigated four different catheter models, each equipped with a unique measurement technology: Tacticath Quartz (Abbott), AcQBlate Force (Biotronik/Acutus), Stablepoint (Boston Scientific), and Smarttouch SF (Biosense Webster). For each model, the accuracy of three different catheters was measured within the range of 0-60 grams and at contact angles of 0°, 30°, 45°, 60°, and 90°. Results: In total, 6685 measurements were performed using 4x3 catheters (median of 568, IQR 511-606 measurements per catheter). Over the entire measurement-range, the force measured by the catheters deviated from the real force by the following absolute mean values: Tacticath 1.29g ±0.99g, AcQBlate Force 2.87g ±2.37g, Stablepoint 1.38g ±1.29g, and Smarttouch 2.26g ±2.70g. For some models, significant under- and overestimation of >10g were observed at higher forces. Mean absolute errors of all models across the range of 10-40g were Conclusion: Contact measured by force-sensing catheters is accurate with 1-3g deviation within the range of 10g to 40g. Significant errors can occur at higher forces with potential clinical consequences.

Conduction system pacing (CSP) encompassing His bundle (HBP) and left bundle branch area pacing (LBBAP) is gaining increasing attention in the electrophysiology community. These relatively novel physiological pacing modalities have the potential to outperform conventional pacing approaches with respect to clinical endpoints, although data are currently still limited. While HBP represents the most physiological form of cardiac stimulation, success rates, bundle branch correction, and electrical lead performance over time remain a concern. LBBAP systems may overcome these limitations. In this review article, we provide a comprehensive overview of the current evidence, implantation technique, device programming, and follow-up considerations concerning CSP systems. Moreover, we discuss ongoing technical developments and future perspectives of CSP.

Finite-element (FE) simulations that go beyond the linear elastic limit of materials can aid the development of polymeric products such as stretch blow molded angioplasty balloons. The FE model requires the input of an appropriate elastoplastic material model. Up to the onset of necking, the identification of the hardening curve is well established. Subsequently, additional information such as the cross-section and the triaxial stress state inside the specimen is required. The present study aims to inversely identify the post-necking hardening behavior of the semi-crystalline polymer polyamide 12 (PA12) at different temperatures. Our approach uses structural FE simulations of a dog-bone tensile specimen in LS-DYNA with mesh sizes of 1 mm and 2 mm, respectively. The FE simulations are coupled with an optimization routine defined in LS-OPT to identify material properties matching the experimental behavior. A Von Mises yield criterion coupled with a user-defined hardening curve (HC) were considered. Up to the beginning of necking, the Hockett-Sherby hardening law achieved the best fit to the experimental HC. To fit the entire HC until fracture, an extension of the Hockett-Sherby law with power-law functions achieved an excellent fit. Comparing the simulation and the experiment, the following coefficient of determination R2 could be achieved: Group I: R2 > 0.9743; Group II: R2 > 0.9653; Group III: R2 > 0.9927. Using an inverse approach, we were able to determine the deformation behavior of PA12 under uniaxial tension for different temperatures and mathematically describe the HC.

Background: A pace and ablate strategy may be performed in refractory atrial fibrillation with rapid ventricular response. Objective: We aimed to assess sex-related differences in patient selection and clinical outcomes after pace and ablate. Methods: In a retrospective multicentre study, patients undergoing AV junction ablation were studied. Sex-related differences in baseline characteristics, all-cause mortality, heart failure (HF) hospitalizations, and device-related complications were assessed. Results: Overall, 513 patients underwent AV junction ablation (median age 75 years, 50% men). At baseline, men were younger (72 vs. 78 years, p < 0.001), more frequently had non-paroxysmal AF (82% vs. 72%, p = 0.006), had a lower LVEF (35% vs. 55%, p < 0.001) and more frequently had cardiac resynchronization therapy (75% vs. 25%, p < 0.001). Interventional complications were rare in both groups (1.2% vs. 1.6%, p = 0.72). Patients were followed for a median of 42 months in survivors (IQR 22–62). After 4 years of follow-up, the combined endpoint of all-cause death or HF hospitalization occurred more often in men (38% vs. 27%, p = 0.008). The same was observed for HF hospitalizations (22% vs. 11%, p = 0.021) and all-cause death (28% vs. 21%, p = 0.017). Sex category remained an independent predictor of death or HF hospitalization after adjustment for age, LVEF and type of stimulation. Lead-related complications, infections, and upgrade to ICD or CRT occurred in 2.1%, 0.2% and 3.5% of patients, respectively. Conclusions: Pace and ablate is safe with a need for subsequent device-related re-interventions in 5.8% over 4 years. We found significant sex-related differences in patient selection, and women had a more favourable clinical course after AV junction ablation.

Objective : Autonomic dysregulation in preterm infants requires continuous monitoring of vital signs such as heart rate over days to months. Unfortunately, common surface electrodes are prone to electrocardiography (ECG) signal artifacts and cause serious skin irritations during long-term use. In contrast, esophageal ECG is known to be very sensitive due to the proximity of electrodes and heart and insensitive to external influences. This study addresses if multichannel esophageal ECG qualifies for heart rate monitoring in preterm infants. Methods : We recorded esophageal leads with a multi-electrode gastric feeding tube in a clinical study with 13 neonates and compared the heartbeat detection performance with standard surface leads. A computationally simple and versatile ECG wave detection algorithm was used. Results : Multichannel esophageal ECG manifested heartbeat sensitivity and positive predictive value greater than 98.5% and significant less false negative ( FN ) ECG waves as compared to surface ECG due to site-typical electrode motion artifacts. False positive bradycardia as indicated with more than 13 consecutive FN ECG waves was equally expectable in esophageal and surface channels. No adverse events were reported for the multi-electrode gastric feeding tube. Conclusion : Heart rate monitoring of preterm infants with multiple esophageal electrodes is considered as feasible and reliable. Less signal artifacts will improve the detection of bradycardia, which is crucial for immediate interventions, and reduce alarm fatigue. Significance : Due to the possibility to integrate the multichannel ECG into a gastric feeding tube and meanwhile omit harmful skin electrodes, the presented system has great potential to facilitate future intensive care of preterm infants.
