Objective: The purpose of this study was to determine the efficacy of a 3D-printed aneurysm simulation model (3DPM) in educating patients and improving physicians’ comprehension and performance.Methods: This prospective study involved 40 patients who were diagnosed with unruptured intracranial aneurysms (UIAs) and scheduled for surgical clipping or endovascular coiling and randomly divided into two groups (the 3DPM group and the non-3DPM group). The 3DPM was used in preoperative consultation with patients and intraoperatively referenced by surgeons. Background: High-fidelity meteorological data is a prerequisite for the realistic simulation of atmospheric dispersion of radioactive materials near nuclear power plants (NPPs). However, many meteorological models frequently overestimate near-surface wind speeds, failing to represent local meteorological conditions near NPPs. This study presents a new high-resolution (approximately 1 km) meteorological downscaling method for modeling short-range (< 100 km) atmospheric dispersion of accidental NPP plumes.Materials and Methods: Six considerations from literature reviews have been suggested for a new dynamic downscaling method. The dynamic downscaling method is developed based on the Weather Research and Forecasting (WRF) model version 3.6.1, applying high-resolution land-use and topography data. In addition, a new subgrid-scale topographic drag parameterization has been implemented for a realistic representation of the atmospheric surface-layer momentum transfer. Finally, a year-long simulation for the Kori and Wolsong NPPs, located in southeastern coastal areas, has been made for 2016 and evaluated against operational surface meteorological measurements and the NPPs’ on-site weather stations.Results and Discussion: The new dynamic downscaling method can represent multiscale atmospheric motions from the synoptic to the boundary-layer scales and produce three-dimensional local meteorological fields near the NPPs with a 1.2 km grid resolution. Comparing the year-long simulation against the measurements showed a salient improvement in simulating near-surface wind fields by reducing the root mean square error of approximately 1 m/s. Furthermore, the improved wind field simulation led to a better agreement in the Eulerian estimate of the local atmospheric dispersion. The new subgrid-scale topographic drag parameterization was essential for improved performance, suggesting the importance of the subgrid-scale momentum interactions in the atmospheric surface layer.Conclusion: A new dynamic downscaling method has been developed to produce high-resolution local meteorological fields around the Kori and Wolsong NPPs, which can be used in short-range atmospheric dispersion modeling near the NPPs.

Interleukin-7 (IL-7) is an essential cytokine for T-cell homeostatic proliferation and maintenance. Clinical studies have shown the potential benefits of IL-7 therapy in various diseases associated with lymphopenia. However, the kinetics of the T-cell response to a single administration of IL-7 in humans have not been fully elucidated. Here, we investigated the effects of Fc-fused long-acting recombinant human IL-7 (hIL-7-hyFc, efineptakin alfa) on lymphocytes in healthy adults after a single subcutaneous or intramuscular administration. Administration of hIL-7-hyFc increased the CD8+ and CD4+ T-cell numbers up to 2.5-fold, with corresponding upregulation of Ki-67 and Bcl-2 expression, peaking at day 3 or 7. Regulatory T cells (Tregs) did not expand. Among CD8+ and CD4+ T cells, all T-cell subsets (TN, TEM, TCM, TEMRA, and TSCM) increased for 56 days. The T-cell receptor repertoire diversity of naive CD8+ and CD4+ T cells was increased by hIL-7-hyFc, whereas the memory T-cell subsets did not differ between day 56 and day 0. Transcriptomic analysis revealed that hIL-7-hyFc induced robust T-cell expansion without changes in gene expression profiles associated with T-cell functions or genes related to T-cell exhaustion, senescence, and anergy. The effector functions of antigen-specific CD8+ T cells were preserved after hIL-7-hyFc administration. Our results suggest that hIL-7-hyFc administration induced a sustained increase in the numbers of CD8+ and CD4+ T cells, but not Tregs, without qualitative changes. These results support the potential of hIL-7-hyFc as a treatment for patients with compromised T-cell immunity or as a vaccine adjuvant.

Purpose: We investigated the clinical features of Korean patients with retinal capillary hemangioblastoma (RCH) and genetic variants of the von Hippel-Lindau (VHL) gene.Methods: A retrospective analysis was performed on patients with RCH from 2003 to 2021 at Seoul National University Bundang Hospital. Sporadic and hereditary RCH associated with VHL disease were classified based on the specific tumors and family history. Clinical features, including the location and number of RCH and bilateral involvement, were investigated. Multiplex ligation-dependent probe amplification and direct sequencing targeting the VHL gene were performed for six RCH cases associated with VHL disease.Results: A total of 18 patients (23 eyes) were enrolled in this study. The mean age at diagnosis was 37 ± 15 years. Twelve patients had hereditary RCH associated with VHL disease, and six patients had sporadic RCH. All five patients with bilateral RCH were clinically diagnosed with VHL disease, and 13 patients had unilateral RCH. Juxtapapillary RCH was only observed in patients with VHL. The most common complication of RCH was the epiretinal membrane, followed by the subretinal fluid. Pathogenic variants were identified in four patients. All three patients with type 1 VHL had the well-known missense mutation p.Glu70Lys, and one patient with type 2 VHL had the nonsense mutation p.Trp88Ter.Conclusions: In Korean patients with RCH, bilateral involvement and juxtapapillary RCH are highly likely to be associated with VHL disease. Because RCH may be the first clinical manifestation in patients with VHL, active genetic testing of the VHL gene and systemic evaluation are required.

Background: This research aimed to provide young people in a juvenile rehabilitation centre (JRC) with access and exposure to virtual reality (VR) as a growing media technology industry, to offer media education that was fun and engaging, and to introduce the digital arts to participants as a potential career path. The project evolved through a partnership with the Washington State Librarian who, as a part of her role overseeing public library activities, wanted to ensure that newly acquired VR equipment was made available to as many people as possible. This effort included libraries within sites of incarceration. Analysis: This article presents a reflective and analytical discussion on the success and challenges of creating, implementing, and researching a VR art design program in a JRC. Conclusions and Implications: Carceral logics are entangled in research and education, in constant tension with anti-oppressive methods in place. Improvisational action as a design method in media education programs, including VR art design with incarcerated youth, may support greater participation and stronger research outcomes.

Background and Objectives: Little is known about studies evaluating positive airway pressure (PAP) compliance after re-prescription. Therefore, the aim of this study was to investigate PAP compliance after re-prescription in obstructive sleep apnea (OSA) patients who failed initial PAP therapy.Methods: We retrospectively reviewed OSA patients who had received a re-prescription for PAP from March 2020 to June 2021. We compared the compliance rate between initial prescription and re-prescription for PAP and investigated the reasons for PAP failure after the first prescription.Results: A total of 10 consecutive OSA patients (mean age=45.6±13.7 years and male:female=8:2) who received a re-prescription for PAP were included. Of them, 8 patients (80%) met the compliance criteria (i.e., Korean National Health Insurance criteria) for PAP after re-prescription. The compliance rate increased from 36.3±18.2% (initial prescription) to 61.3±28.8% (re-prescription); this was not of statistical significance (p=0.074). PAP/mask-related discomfort was the most common reason for PAP failure, followed by nasal obstruction, unintentional mask removal, and pressure-related discomfort.Conclusion: Even if initial PAP therapy fails, the proportion of patients who meet the compliance criteria may be improved through various forms of clinical aid and support after re-prescription of PAP.

The field of Library and Information Science (LIS) has historically struggled to connect theory and practice in its graduate education, specifically when it comes to building partnerships with diverse stakeholders in public library communities. One possible technique for bridging the gap of stakeholder experiences is academically based community service (ABCS) courses. ABCS courses attempt to directly focus on research-practitioner partnerships to solve real-world problems by involving the community to democratically co-design the curriculum. In this paper, we present a reflective case study of an ABCS course on integrating participatory design methods to co-design new digital learning activities for youth in an urban public library. By conducting 10 co-design sessions and 26 semi-structured interviews, we examined a partnership between a university and a library, and how this ABCS course impacted different stakeholder participation. Our findings highlight how ABCS courses require systematic structural endeavors from higher education and care between all stakeholders within a library setting. This work contributes to a growing body of work that calls for more courses in LIS education with greater civic and democratic engagement between graduate students, university partners, and library stakeholders.

In this study, we aim to propose a methodology for determining priorities and sites for additional installation of emergency firefighting gear, so as to address areas vulnerable to fire response. This study is divided into three parts. The first part involves using GIS spatial analysis for analyzing areas vulnerable to fire response, the second part involves analyzing the current status of buildings in these vulnerable areas, and the third part involves selecting locations for installing emergency firefighting devices and establishing priorities. Masan, Changwon-si, Gyeongsangnam-do, Korea, which is considered a difficult location for dispatching fire trucks due to the presence of several narrow alleys, was selected as the study area. A total of 1,563 (2.80%) buildings from the area were determined as vulnerable to fire response inside the critical time window. Based on the density and function of buildings, the potential locations and priorities for the installation of emergency firefighting devices were determined. It was ascertained that 569 buildings (36.40%) in areas vulnerable to fire response could be aided during a fire if additional emergency firefighting devices are installed in the 10 selected locations. Going forward, the findings of this study are expected to be applied to basic research aimed at establishing standards for placement of emergency firefighting equipment.
