This paper presents an observer design for estimation of thermoacoustic instabilities in a Rijke tube. To study this problem, we consider that the acoustic dynamics is represented by the wave equation with a point source term representing the heat release. In turn, the heat release dynamics is given by a first-order ordinary differential equation (ODEs). The observer, whose design is based on the backstepping methodology, relies on measurements of acoustic pressure and velocity at an arbitrary point of the domain. The design employs a folding transformation (with two folds) around the measurements and the heat release point, allowing to write the system into a form with more states but boundary measurements and ODE couplings. Then, we formulate a well-posed and invertible integral transformation with both triangular and full terms that maps the observer error dynamics into an exponentially stable target system. The theoretical results were tested through numerical simulations in order to show the effectiveness of the design.

We present a critical reflection on the mechanical variable Player Load, which is based on acceleration data and commonly used in sports. Our motivation to write this paper came from the difficulties that we encountered in the calculation and interpretation of Player Load using our own data, since we did not use the Catapult Sports equipment, which is a merchandise of the company that proposed this variable. We reviewed existing literature in order to understand Player Load better; we found many inconsistencies in PL calculation methods and in the meanings attached to it. Accordingly, this paper presents a brief discussion on the meanings that have been assigned to Player Load, its limitations, and the lack of clear and complete information about Player Load calculation methods. Moreover, the use of arbitrary units and different practical meanings in the literature has associated Player Load with many physical quantities, thereby resulting in difficulties in determining what Player Load measures within the context of sports. It seems that Player Load is related to the magnitude of changes in acceleration, but not the magnitude of acceleration itself. Therefore, coaches and sports scientists should take this information into account when they use Player Load to prescribe and monitor external loads. We concluded that a deeper discussion of Player Load as a descriptor of external load is warranted in the sports sciences literature.

This study compared the physical, physiological, and spatiotemporal responses of soccer athletes in small-sided games (SSG) in two experimental conditions: progression to the target rule (PG), in which they should take the ball to the opponent's endline to score points, and SSG with regular rules (RG), in which they should score goals to win the game.Twenty U-20 athletes played both SSG formats. The SSG were played as four 4-minute bouts with four minutes of passive recovery in two consecutive days. Heart rate, physical (distances and accelerations), and positional data (length, width, and spatial exploration) were collected by a 10 hz GPS device and compared between the protocols using a MANOVA with Bonferroni's correction for multiple comparisons.Results showed that the RG condition demanded more spatial exploration eliciting greater occupation of the pitch width. There were higher mean and maximum heart rates and greater low-to-moderate distances and accelerations in the RG, while the PG rule increased the distances covered at the highest speed and acceleration zones.The progression to the target rule should be adopted to emphasize players' ability to use the width during the offensive phase. Additionally, the PG rule should also be used to emphasize the development of speed and acceleration skills.

Fabry disease (FD) is a genetic disease, with X-chromosome linked inheritance, due to variants in the GLA gene that encodes the α-galactosidase A (α-GAL) enzyme. The purpose of the present study was to create a consensus aiming to standardize the recommendations regarding the renal involvement of FD with guidelines on the diagnosis, screening, and treatment of pediatric patients. This consensus is an initiative of the Rare Diseases Committee (Comdora) of the Brazilian Society of Nephrology (SBN). Randomized controlled clinical studies and studies with real-life data added to the authors' experience were considered for this review. The result of this consensus was to help manage patient and physician expectations regarding treatment outcomes. Thus, this consensus document recommends the investigation of the pediatric family members of an index case, as well as cases with suggestive clinical signs. From the diagnosis, assess all possible FD impairments and grade through scales. From an extensive review of the literature including pediatric protocols and particularly evaluating pediatric cases from general studies, it can be concluded that the benefits of early treatment are great, especially in terms of neuropathic pain and renal impairment parameters and outweigh the possible adverse effects that were mainly manifested by infusion reactions. Resumo A doença de Fabry (DF) é uma doença genética, com herança ligada ao cromossomo X, devido a variantes no gene GLA que codifica a enzima α-galactosidase A (α-GAL). O propósito do presente estudo foi criar um consenso objetivando padronizar as recomendações em relação ao acometimento renal da DF com orientações sobre o diagnóstico, rastreamento, e tratamento de pacientes pediátricos. Esse consenso é uma iniciativa do Comitê de Doenças Raras (Comdora) da Sociedade Brasileira de Nefrologia (SBN). Foram considerados para esta revisão estudos clínicos controlados randomizados e estudos com dados de vida real somados à experiência dos autores. O resultado desse consenso foi auxiliar no gerenciamento das expectativas de pacientes e médicos quanto aos resultados do tratamento. Assim, este documento de consenso recomenda a investigação dos familiares pediátricos de um caso índice, assim como de casos com clínica sugestiva. A partir do diagnóstico, avaliar todos os possíveis acometimentos da DF e graduar através de escalas. A partir de uma revisão extensa da literatura incluindo protocolos pediátricos e avaliando particularmente os casos pediátricos de estudos gerais, pode-se concluir que os benefícios do tratamento precoce são grandes, principalmente quanto aos parâmetros de dor neuropática e do acometimento renal, e suplantam os possíveis adversos que foram sobretudo manifestados por reações infusionais.

Este trabalho analisa o comportamento tribológico de aços com microestruturas e composições químicas inspiradas nos aços Hadfield. Foram estudadas doze composições com as quais se analisou o efeito dos teores de C, Mn, Cr e N. Foram realizados ensaios de desgaste por deslizamento (pino contra disco), com os pinos fabricados com os aços em estudo e os discos de aço H-13 (600 HV). A força normal foi variada entre 50 e 500 N e velocidade de deslizamento de 0,18 m/s. A força de atrito foi avaliada durante os ensaios e a perda de massa obtida após os ensaios. Foram realizados perfis de microdureza na seção transversal, caracterização das superfícies de desgaste utilizando microscopia eletrônica de varredura e de transmissão, Astar, espectroscopia Raman e difração de Raio X. Ensaios de resistência ao impacto Charpy foram realizados. Os resultados de desgaste mostraram que as ligas com maiores teores de N (família G3) apresentaram expressiva resistência ao desgaste na faixa de força normal entre 50 e 500 N. A liga com 0,26% N não mostrou aumento de perda de massa expressiva ao longo de toda a faixa de força. Para as demais ligas (famílias G1 e G2), com menores teores de N, as maiores perdas de massa ocorreram nos ensaios com 500 N, entretanto foi observado um máximo relativo de perda de massa para forças intermediárias. O efeito do C, Mn e Cr no desgaste não teve um comportamento generalizável. Esse trabalho sugere que ligas com composições e microestruturas semelhantes à de aços Hadfield podem ter seu desempenho melhorado por meio da adequação do teor de N para algumas aplicações. O coeficiente de atrito foi similar para todas as ligas, diminuindo com aumento da força normal, devido ao aumento da fração de óxidos nas superfícies. As medidas de microdureza mostraram que as famílias G1 e G2 tem maior dureza que a família G3. As análises com DRX, MET e Astar, mostraram a formação de uma camada recristalizada nanocristalina e formação de martensita (´e ) para as forças entre 300 e 500 N para todas as ligas. Para a liga com maior teor de N o mecanismo de desgaste oxidativo foi predominante em todas as forças com ocorrência limitada de evidências de formação e ruptura de junção. Para as famílias G1 e G2 o desgaste oxidativo foi predominante para forças menores e para forças maiores as evidências de abrasão, formação e ruptura de junção foram mais intensas e crescem com a força. Para explicar os resultados de desgaste e atrito em função da força e composição química foi proposto que as ligas com maiores teores de N têm uma capacidade de deformação superior às demais, evidenciadas pela maior espessura da camada nanocristalina e pelos menores teores de martensita. As ligas com maior teor de N apresentaram valores de resistência ao impacto ainda compatíveis com aplicações industriais, embora menores do que das demais famílias. This work presents an analysis of the tribological behaviour of steels with microstructure inspired by Hadfield steels, with twelve different chemical compositions varying C, Mn, Cr and N contents. Sliding wear tests (pin-to-disk) were performed, the pins were fabricated with Hadfield steels and the discs with H-13 steel (600 HV). The normal load was varied between 50 and 500 N and the sliding speed was 0.18 m/s. The friction force was monitored during the tests, as well as the mass loss obtained after the tests. Microhardness profiles were performed in the cross section, in addition to evaluation of the wear surfaces using scanning and transmission electron microscopy, Astar, Raman spectroscopy and X-ray diffraction. Charpy impact resistance tests were performed. The wear results showed that the alloys with higher N content (family G3) showed significant wear resistance with an increase in normal load between 50 and 500 N. The 0.26% N alloy did not show significant increase in mass loss over the entire load range. For the other alloys (G1 and G2 families), with lower contents of N, the higher mass losses occurred in the tests with 500 N, however, a relative maximum of mass loss was observed for an intermediate load. The effect of C, Mn and Cr on wear did not have a generalizable behavior. This work suggests that alloys with compositions and microstructures similar to Hadfield steel may have their performance improved by adapting the N content for some applications. The friction coefficient was similar for all alloys, decreasing with an increase in normal load, due to the increase in the fraction of oxides on the surfaces. The microhardness measurements showed that the G1 and G2 families have higher hardness than the G3 family. The use of XRD, MET and Astar techniques showed the formation of a recrystallized nanocrystalline layer and the formation of martensites (´e ) for loads between 300 and 500 N for all alloys. For the alloy with a higher N content, the oxidative wear mechanism was predominant in all loads with limited evidence of formation and rupture of the junction. For the G1 and G2 families, oxidative wear was predominant for lower loads and for higher loads the evidence of abrasion, formation and rupture of the adhesion joint was more intense and increased with the load. To explain the results of wear and friction as a function of load and chemical composition, it was proposed that the alloys with the highest N content have a higher deformation capacity than the others, evidenced by the higher thickness of the nanocrystalline layer and the lower contents of martensite. Alloys with a higher N content showed impact resistance values still compatible with industrial applications, although lower than those of other families.

Previous studies on soccer small-sided games (SSGs) chose between with and without offside rule conditions without evidence-based criteria. The current study aimed to compare the positional dynamics of 3v3 SSGs with and without the offside rule.Twenty-four Brazilian U-17 national-level soccer players were recruited. They were split into eight teams and played SSGs with and without the offside rule . Positional data were gathered by a Global Positioning System, and teams' length, width, the length per width ratio (LPWratio), stretch index and players' spatial exploration index were calculated. Data were compared between the experimental conditions by a paired t-test.Results showed that SSGs with the offside rule presented lower values of length (~16.63%, large effect), the LPWratio (~19.74%, large effect), and players' spatial exploration (~4.13%, small-to-moderate effect).We conclude that the offside rule impacts players' positional behavior in SSGs, inducing a less exploratory behavior mainly in the width axis. For this reason, we recommend caution when interpreting previous results on SSG when the offside rule was not applied since adopting this rule might elicit different responses to the players . Coaches can manipulate this rule aiming to achieve specific tactical goals for the training session.

Background Atypical hemolytic uremic syndrome (aHUS) is an ultra-rare disease. Therefore, studies involving large samples are scarce, making registries powerful tools to evaluate cases. We present herein the first analysis of the Brazilian aHUS Registry (BRaHUS). Methods Analysis of clinical, laboratory, genetic and treatment data from patients inserted in the BRaHUS, from 2017 to 2020, as an initiative of the Rare Diseases Committee of the Brazilian Society of Nephrology. Results The cohort consisted of 75 patients (40 adults and 35 pediatric). There was a predominance of women (56%), median age at diagnosis of 20.7 years and a positive family history in 8% of cases. Renal involvement was observed in all cases and 37% had low C3 levels. In the Conclusions The cohort of BRaHUS was predominantly composed of female young adults, with renal involvement in all cases. Pediatric patients had lower hemoglobin and platelet levels and higher LDH levels than adults, and the most common genetic variants were identified in CFH and the CFHR1-3 deletion with no preference of age, a peculiar pattern of Brazilian patients.

Introduction: Hyperparathyroidism (SHPT) secondary to chronic kidney disease (CKD) is characterized by high levels of parathyroid hormone (PTH), hyperplasia of the parathyroid glands and cardiovascular disease. Selective and non-selective and selective vitamin D-receptor activators, calcimimetics, are available in the Brazilian market to reduce PTH levels. Objectives: To develop a cost-effectiveness (C/E) and budgetary impact (BI) analysis of intravenous paricalcitol vs. oral calcitriol for patients on dialysis with SHPT, from the perspective of the Brazilian Public Health Care System (SUS). Methodology: We built a decision-tree model to analyze C/E, which considered the outcome of avoided death and a time horizon of 1 year. As for the BI analysis, two scenarios were considered, one of demand and one of epidemiological approach, based on data from the Brazilian Society of Nephrology. Results: The analysis showed that the C/E ratio was R$ 1,213.68 per year, and an incremental effectiveness of 0.032, referring to avoided death. The incremental C/E ratio was R$37,927.50 per death averted by paricalcitol. It was estimated that the incremental BI with the expansion of paricalcitol use will be between R$1,600,202.28 and R$4,128,565.65 in the first year, considering the main and epidemiological scenarios. At the end of 5 years after the expansion of its use, an incremental BI was estimated between R$ 48,596,855.50 and R$ 62,90,555.73. Conclusion: Intravenous paricalcitol has superior efficacy and similar safety to oral calcitriol, reducing the overall mortality of dialysis patients, although it implies a higher cost. Resumo Introdução: O hiperparatireoidismo secundário (HPTS) à doença crônica renal (DRC) é caracterizado por elevados níveis de paratormônio (PTH), hiperplasia das glândulas paratireoides e doença cardiovascular. Para a redução dos níveis do PTH, estão disponíveis no mercado brasileiro os ativadores não seletivos e seletivos do receptor da vitamina D e os calcimiméticos. Objetivos: Desenvolver análise de custo-efetividade (C/E) e de impacto orçamentário (IO) do paricalcitol intravenoso vs. calcitriol oral para pacientes em diálise com HPTS, na perspectiva do Sistema Único de Saúde. Metodologia: Foi construído um modelo de árvore de decisão para a análise de C/E, que considerou o desfecho morte evitada e um horizonte temporal de 1 ano. Quanto à análise de IO, foram considerados dois cenários, sendo um de demanda aferida e um de abordagem epidemiológica, baseado nos dados da Sociedade Brasileira de Nefrologia. Resultados: A análise mostrou que a relação de C/E foi de R$ 1.213,68 ao ano, e uma efetividade incremental de 0,032, referente à morte evitada. A razão de C/E incremental foi de R$ 37.927,50 por morte evitada para o paricalcitol. Estimou-se que o IO incremental com a ampliação do uso do paricalcitol estará entre R$ 1.600.202,28 e R$ 4.128.565,65 no primeiro ano, considerando os cenários principal e o epidemiológico. Já no fim de 5 anos após a ampliação do uso, estimou-se IO incremental entre R$ 48.596.855,50 e R$ 62.90.555,73. Conclusão: O paricalcitol intravenoso tem eficácia superior e segurança semelhante ao comparador calcitriol oral, diminuindo a mortalidade geral dos pacientes em diálise, embora implique maior custo.

Solar absorption plants are renewable energy systems with a special advantage: the cooling demand follows the solar energy source. The problem is that this plant presents solar intermittency, phenomenological complexity, and nonlinearities. That results in a challenge for control and energy management. In this context, this paper develops a Digital Twin of an absorption chiller High Temperature Generator (HTG) seeking accuracy and low computational efort for control and management purposes. A neuro-fuzzy technique is applied to describe HTG, internal Lithium-Bromide temperature, and water outlet temperature. Two Adaptative Neuro-Fuzzy Inference Systems (ANFIS) are trained considering real data of eight days of operation. Then, the obtained model is validated considering two days of real data. The validation shows a RMSE of 1.65e−2 for the internal normalized temperature, and 2.05e−2 for the outlet normalized temperature. Therefore, the obtained Digital Twin presents a good performance capturing the dynamics of the HTG with adaptive capabilities considering that each day can update the learning step.

Objective: to describe the changes implemented in the work environment of nurses in university hospitals considering the COVID-19 pandemic. Methods: this qualitative and descriptive research was developed from an online survey with 75 nurses from three Brazilian university hospitals. Data processing occurred through textual analysis with the aid of software IRAMUTEQ. Results: five semantic classes were obtained: Organization of units for exclusive care of patients with COVID-19; Adaptations in the use of personal protective equipment; Physical structure adaptation; Care flow institution; Increased number of beds and training courses. Final considerations: the results show the effort of healthcare and nursing professionals/managers in the development of structural adaptations and reorganizations of care processes, in the hospital context, to respond with quality and efficiency to the demands arising from the COVID-19 pandemic.

Equalization of training protocols by time under tension determines the magnitude of changes in strength and muscular hypertrophy. J Strength Cond Res 36(7): 1770-1780, 2022-The aim of this study was to investigate the effects of 2 training protocols equalized by tension (TUT) on maximal strength (1 repetition maximum [RM]), regional cross-sectional areas (proximal, middle, and distal), and total cross-sectional areas (sum of the regional cross-sectional areas) of the pectoralis major and triceps brachii muscles. Thirty-eight men untrained in resistance training participated in the study and were allocated under 3 conditions: Protocol 3s (n = 11; 12 repetitions; 3s repetition duration), Protocol 6s (n = 11; 6 repetitions; 6s repetition duration), and Control (n = 11; no training). Training protocols (10 weeks; bench press exercise) were equated for TUT (36 seconds per set), number of sets (3-4), intensity (50-55% of 1RM), and rest between sets (3 minutes). Analysis of variance was used to examine a percentage change in variables of interest across the 3 groups with an alpha level of 0.05 used to establish statistical significance. Protocols 3s and 6s showed no differences in the increase of total and regional muscle cross-sectional areas. There were no differences in regional hypertrophy of the pectoralis major muscle. In the triceps brachii muscle, the increase in distal cross-sectional area was greater when compared with the middle and proximal regions. Both experimental groups had similar increases in the 1RM test. In conclusion, training protocols with the same TUT promote similar strength gains and muscle hypertrophy. Moreover, considering that the protocols used different numbers of repetitions, the results indicate that training volumes cannot be considered separately from TUT when evaluating neuromuscular adaptations.

No. of bitstreams: 0 Previous issue date: 2021-11-01 Background Atypical Hemolytic Uremic Syndrome (aHUS) is an ultra-rare disease that potentially leads to kidney graft failure due to ongoing Thrombotic Microangiopathy (TMA). The aim was evaluating the frequency of TMA after kidney transplantation in patients with aHUS in a Brazilian cohort stratified by the use of the specific complement-inhibitor eculizumab. Methods This was a multicenter retrospective cohort study including kidney transplant patients diagnosed with aHUS. We collected data from 118 transplant centers in Brazil concerning aHUS transplanted patients between 01/01/2007 and 12/31/2019. Patients were stratified into three groups: no use of eculizumab (No Eculizumab Group), use of eculizumab for treatment of after transplantation TMA (Therapeutic Group), and use of eculizumab for prophylaxis of aHUS recurrence (Prophylactic Group). Results Thirty-eight patients with aHUS who received kidney transplantation were enrolled in the study. Patients' mean age was 30 years (24-40), and the majority of participants was women (63% of cases). In the No Eculizumab Group (n = 11), there was a 91% graft loss due to the TMA. The hazard ratio of TMA graft loss was 0.07 [0.01-0.55], p = 0.012 in the eculizumab Prophylactic Group and 0.04 [0.00-0.28], p = 0.002 in the eculizumab Therapeutic Group. Conclusion The TMA graft loss in the absence of a specific complement-inhibitor was higher among the Brazilian cohort of kidney transplant patients. This finding reinforces the need of eculizumab use for treatment of aHUS kidney transplant patients. Cost optimization analysis and the early access to C5 inhibitors are suggested, especially in low-medium income countries. This study aimed to analyze the variations of fitness status, as well as test the relationships between accumulated training load and fitness changes in women soccer players. This study followed an observational analytic cohort design. Observations were conducted over 23 consecutive weeks (from the preseason to the midseason). Twenty-two women soccer players from the same first Portuguese league team (22.7 ± 5.21 years old) took part in the study. The fitness assessment included anthropometry, hip adductor and abductor strength, vertical jump, change of direction, linear speed, repeated sprint ability, and the Yo-Yo intermittent recovery test. The training load was monitored daily using session rating of perceived exertion (s-RPE). A one-way repeated ANOVA revealed no significant differences for any of the variables analyzed across the three moments of fitness assessments (p &gt, 0.05). The t-test also revealed no differences in the training load across the moments of the season (t = 1.216, p = 0.235). No significant correlations were found between fitness levels and accumulated training load (range: r = 0.023 to −0.447, p &gt, 0.05). This study revealed no differences in the fitness status during the analyzed season, and the fitness status had no significant relationship with accumulated training load.

We examined the association between 50 m freestyle performance (50FS) and average speed (AS), peak speed (PS), minimum speed (MS) and intracyclic speed variation (ISV) and compared the speed curves from swimmers with different performance levels using functional analysis of variance (FANOVA). Fourteen male swimmers (50FS: 22.50 ± 0.58 s) performed a maximal sprint with a speedometer and AS, PS, MS, and IVV were assessed for correlational analysis. 50FS were obtained in official competitions. Swimmers were assigned to three groups based on actual 50FS: G21 (n = 2, 21.99 ± 0.04 s), G22 (n = 6, 22.82 ± 0.10 s) or G23 (n = 6, 23.55 ± 0.18 s). FANOVA compared the average curves. 50FS correlated to AS (r = −0.781, p = 0.001) and PS (ρ = −0.766, p = 0.001), but not to MS (r = −0.185, p = 0.527) or IVV (r = −0.323, p = 0.259). FANOVA showed that faster swimmers achieved higher PS and stayed longer at the upper part of the curve. 50FS performance is related to average and peak speed assessed with the speedometer. Swimmers should seek techniques to maintain the speed at the upper part of the curve as long as possible.

Objetivos: Este artigo pretende avaliar a introdução da audiência de custódia nos juízos e tribunais brasileiros aos presos em situação de flagrante delito. Pretende-se discorrer, inicialmente, a respeito da evolução das convenções internacionais de promoção dos direitos humanos e de sua recente penetração no ordenamento jurídico interno, reavaliando a noção de soberania nacional. Logo mais, conveniente mencionar as modificações impostas pela Lei 12.403/11 na disciplina da prisão em flagrante, notadamente com a consolidação de sua natureza pré-cautelar, com sua consequente necessidade de convolação em prisão preventiva, aplicação de medidas cautelares pessoais diversas ou a concessão da liberdade provisória. Buscar-se-á, então, aferir o fundamento normativo para a implantação da audiência de custódia em solo brasileiro, com a menção dos atos normativos internos que a pretenderam disciplinar. Por fim, analisará o primeiro panorâma acerca dos novos aperfeiçoamentos trazidos pelo Lei 13.964/2019Metodologia: Empregar-se-á o método dedutivo, optando-se pela pesquisa bibliográfica científica como procedimento metodológico para a realização deste trabalho.Resultados: O trabalho demonstrará que a implementação das audiências de custódia em solo pátrio consagra a aplicação de um direito humano de natureza processual que visa à salvaguarda da dignidade da pessoa humana dos custodiados, levando-os à presença da autoridade judiciária para a obtenção de uma decisão sobre seu status libertatis em prazo razoável.Contribuições: O artigo apresenta panorâma atual da audiência de custódia, contemplando aspectos essenciais desde sua criação, até as alterações evoluitivas do método, pouco estudados hodiernamente.Palavras-chave: audiência de custódia; prisão em flagrante; direitos humanos processuais; dignidade da pessoa humana; Pacto de San José da Costa Rica. ABSTRACTObjective: This article intends to evaluate the introduction of custody hearing in Brazilian courts and tribunals to prisoners in a situation of flagrante delicto. It is intended to discuss, initially, about the evolution of international conventions for the promotion of human rights and their recent penetration into the domestic legal system, reevaluating the notion of national sovereignty. Therefore, it is appropriate to mention the changes imposed by Law 12,403 / 11 in the discipline of imprisonment in flagrante delicto, notably with the consolidation of its pre-precautionary nature, with its consequent need for convolutional detention, application of various personal precautionary measures or the concession of provisional release. Nevertheless, it will analyze the first panorama about the new improvements brought by Law 13.964 / 2019. It will be sought, then, to assess the normative basis for the implantation of the custody hearing on Brazilian soil, with the mention of the internal normative acts that intended to discipline it.Methodology: The method will be used deductive, opting for scientific bibliographic research as a methodological procedure for carrying out this work.Results: The work will demonstrate that the implementation of custody hearings on national soil enshrines the application of a procedural human right aimed at safeguarding the human dignity of the custodians, leading them to the presence of the judicial authority to obtain a decision on their libertatis status within a reasonable time.Contributions: The article presents a current overview of the custody hearing, contemplating essential aspects from its creation, until the evolutionary changes of the method, little studied today.Keywords: Custody hearing; arrest in flagrante; procedural human rights; dignity of human person; Pact of San José of Costa Rica.

Background: The range of motion (RoM) of dorsiflexion (DF) plays an important role in human mobility, such as absorption of body weight during gait deceleration, jump landings, balance and eccentric movements. This limitation can generate potentially damaging movements. This way, evaluating techniques for DF RoM increase could help improve immediate performance in such functional activities. This being the case, the objective of this study will be to verify the sum effect of different joint mobilization techniques for DF gain in persons practicing physical activities and its relationship with functional performance and balance. Methodology: This is a randomized, controlled and double-blind clinical trial. Sixty-four (64) volunteers will be recruited, aged between 18 and 40 years, who have DF limitation. After checking eligibility criteria, the participants will be submitted to a physiotherapeutic evaluation. Volunteers will be evaluated as to a DF for open kinetic chain (OKC) and closed kinetic chain (CKC), and also submitted to functional Y Balance Test under a force platform and Triple Hop Test, by a blind examiner. Another researcher, also blind, will perform the randomization of patients in groups: (A) Joint Mobilization - Mulligan Concept and (B) Joint Mobilization - Maitland Method. After the randomization, volunteers will be submitted to initial evaluation (A0) and, after 2-3 days, they will be submitted to application of the techniques, which will be randomized, and reassessed immediately (A1). After 3-4 days of the application of the techniques, the re-evaluation will be performed (A2). Discussion: It is already known that DF RoM limitation can lead to compensatory and potentially damaging lower limb movements and that joint mobilizations are effective to treatment. However, there is no consensus whether the application of these techniques would also improve aspects of dynamic postural balance and performance in individuals practicing physical activity, and whether the sum of two joint mobilization techniques could enhance this effect. International audience; Rapid technological innovations are constantly influencing the complexification and automatization of the work lines pushing human operators to use diverse cognitive processes for supervising complex industrial machines. This urges factories to offer wearable cognitive assistants to human operators to analyze, integrate and maintain a considerable amount of information. The aim of this review is twofold. First, we borrow theoretical elements from the working memory literature to question the way these wearable cognitive assistants could optimize human operators’ cognitive load. Second, we argue that Technology Acceptance Model (TAM) and Job Characteristics Model (JCM) may theoretically predict the effectiveness of cognitive wearable assistants in enhancing the person–job fit, namely their cognitive performance and well-being. A critical review method was used to collect and summarize the most studied models associated with application of wearable devices in the workplace. Our review suggests that the current literature on working memory give useful insights concerning the way in which information should be displayed to operators to optimize the efficiency of wearable cognitive assistants. Moreover, JCM suggests original explanations on the way they can facilitate access to information and in turn increase job satisfaction. Finally, a small number of studies that used TAM with wearable devices in an industrial setting provide some interesting theoretical and empirical evidence on the acceptance of wearable cognitive assistants. As a conclusion, we argue that using wearable cognitive assistants properly would enhance both cognitive performance and well-being of human operators through promoting the person–job fit.

Resonant sensors are widely employed in several applications, and resonant cavity sensors have advantages when noninvasive and non-destructive measurements are required. In this technology, the resonant frequency of the sensor is used to estimate different measurement parameters, since it varies according to the permittivity of the material under test. In this paper, a resonant frequency tracking system, combining a resonant cavity sensor and a phase-locked loop circuit, is investigated. The system has a quadrature phase detector being able to estimate both, phase and magnitude information, and is well suited for applications involving water as a material under test. A complete description of the measurement system combining experimental results with theoretical models, previously reported in the literature, is accomplished. Such theoretical models are based on the amplitude-phase dynamics and assume that the sensor is modeled as a linear resonator. Hence, a semi-empirical modeling is elaborated and the validation results precisely demonstrate the reliability of the modeling, proving to be very useful for further analysis.

Longer concentric action increases muscle activation and neuromuscular fatigue responses in protocols equalized by repetition duration. J Strength Cond Res 33(6): 1629-1639, 2019-The aim of this study was to investigate the impact of protocols equalized by the repetition duration but composed of different concentric (CON) and eccentric (ECC) durations on muscle activation and neuromuscular fatigue. Seventeen males with previous experience in resistance training performed 3 training protocols (A - 2 second CON: 4 second ECC; B - 3 second CON: 3 second ECC; and C - 4 second CON: 2 second ECC) with the Smith machine bench press exercise, all with 3 sets, 6 repetitions, 3 minutes' rest, and 60% of 1RM. The normalized root mean square of the electromyographic signal (EMGRMS) and mean frequency electromyography (EMGMF) for pectoralis major and triceps brachii muscles were calculated for second and fifth repetitions in each set. The results showed an EMGMF decrease across the repetitions accompanied by a progressive increase of the EMGRMS across the repetitions for all protocols and muscles. The EMGRMS was higher in protocol C when compared with protocol A and B for pectoralis major. The EMGMF was lower in protocols B and C than in protocol A for pectoralis major throughout the sets and repetitions. A higher EMGRMS and a lower EMGMF were observed in protocols B and C compared with protocol A for triceps brachii, solely in the fifth repetition. In conclusion, training protocols conducted with the same repetition duration, but with different concentric and eccentric durations, produce distinct muscle activation and neuromuscular fatigue responses, in which performing longer concentric durations could be the more appropriate strategy to increase muscle activation and neuromuscular fatigue.

Objetivo Conhecer e analisar a produção científica no período de 2005 à 2016 em relação cuidados paliativos e a importância da comunicação na estratégia dos cuidados paliativos. Método: Esta pesquisa possui características descritivas, exploratórias com abordagem quantitativa, realizado pelo método da revisão integrativa. Resultados: A partir da leitura dos 14 artigos, os artigos foram agrupados observando-se a temática em questão e, assim obteve-se a seguinte categorização: Cuidados Paliativos e a Relação interpessoal do enfermeiro e do paciente; Comunicação como estratégia para fortalecimento do vinculo entre enfermeiro e o usuário dos Cuidados Paliativos; A importância da comunicação do enfermeiro e o familiar/cuidador. Conclusão: O enfermeiro tem um papel fundamental para a promoção do CP, como na aceitação do diagnostico e auxilio para conviver com a doença, prestando assistência integral ao usuário e a todos envolvidos com o doente.

This work proposes a hybrid control strategy for the optimal operation of a solar membrane distillation facility This kind of plants presents a hybrid nature allowing their operation in several modes, which must be properly selected according to the operating conditions. Thus, the control algorithm is based on a mixed logical dynamical characterization of the solar membrane distillation facility, combined with the application of a practical nonlinear model predictive control strategy for calculating the optimal control actions. The main objectives of the control system are to increase the operating temperature and the distillate production of the membrane distillation module, as well as the number of operational hours. For these goals, the control algorithm has to select the most appropriate operating mode and the optimal operating points in terms of water flow rate at each sampling time. Simulation results are presented for evidencing the benefits of the proposed control approach.

This paper deals with the optimal control of a H2 production system supplied by a photovoltaic solar energy and assisted by the grid. For that, we propose a simple dynamic model accounting for the main features of the system, where the electrolizer current is used as the control variable, and we obtain the control law that optimizes the grid energy consumption despites the solar energy variability An important constraint on the obtained solution is the physical limits of the hydrogen storage tank, which results in a nonsmooth and noncontinuous problem. The Pontryagin’s maximum principle is used to solve the boundary value problem and an explicit piecewise continuous control law is found. Most literature has neglected the possible influence of game phase – offensive and defensive – on players’ and teams’ behaviors measured by positional data. Also, the combined effect of contextual variables – match period, match venue, and match outcome – and game phases has not been investigated yet. Therefore, this study aimed to integrate notational and positional analysis to investigate individual and collective tactical behavior in different game phases (offensive and defensive) in football matches under different contextual conditions. The sample comprised 18 matches played by 24 U-20 elite Brazilian athletes during the 2021 national championship. The authors designed a notational system to classify the start and the end of the offensive phase (and consequently, the defensive phase of the opposing team). The positional data was gathered through global positioning system devices and manually integrated with the notational analysis. Contextual variables comprised match period (first vs second halves), match venue (home vs away matches), and match outcome (win vs draw versus lose matches). Results showed significant differences between game phases, with a more spread tactical positioning observed in the offensive phase. Furthermore, interactions between contextual variables and the game phase were observed, although the game phase presented the highest impact on the dependent variables. We conclude that a more spread tactical positioning characterizes offensive behavior and that the impact of the game phase was higher than contextual variables on tactical behavior, which emphasizes the need for future studies to split positional data into attacking and defensive ones. Also, integrating notational and positional analysis are a viable alternative to enrich the data interpretation of football matches concerning the influence of game phases on players’ and teams’ behaviors.

This study followed a cross-sectional design and aimed to compare the offensive and defensive tactical-technical actions and the physical and physiological responses between four 3vs.3 basketball small-sided games (SSGs) played on the half-court: with regular rules (3vs.3REGULAR), with defensive pressure (3vs.3DEFPRESS), with the close-shot rule (3vs.3CLOSESHOT), and with offensive numerical superiority (4vs.3). Fifty-one U-14 and U-15 male athletes participated in the study. They were divided into 3-player teams and played one 4-min bout of each SSG type. Heart rate and the time spent in four acceleration zones (0.0–0.5, 0.5–1.0, 1.0–1.5, and 1.5–2.0 g) were recorded using heart rate monitors and triaxial accelerometers. SSGs were filmed for the analysis of Space Creation Dynamics and defensive tactical-technical actions. Results showed a higher frequency of space creation without the ball and a mean number of passes per offense in the 4vs.3, with this SSG showing higher offensive performance than the 3vs.3CLOSESHOT (large effect sizes). Defensive behavior was significantly impacted by all rules: in general, 4vs.3 presented the highest frequency of closeouts, 3vs.3DEFPRESS increased the frequency of close off-ball marking and the 3vs.3CLOSESHOT increased the frequency of double-teaming, all differences presenting large effect sizes. Heart rate and the time spent in moderate and high accelerations (zones 2 and 3) were the highest with defensive pressure and the lowest with numerical superiority; heart rate and accelerations in the close-shot rule are higher than in numerical superiority but similar to the 3vs.3REGULAR. We concluded that the SSGs investigated in this study can stimulate different offensive and defensive actions and be used to develop the performance of basketball athletes.

No. of bitstreams: 0 Previous issue date: 2021-01-01 Background: A low supply of donated organs led to the expansion of criteria for kidney transplantation (KT), and the impact on late glomerular function rates (eGFR) is still uncertain. This study aimed to correlate the histologic findings at time-zero biopsy (TzB) with the final eGFR, to identify criteria that could help achieve a more thorough preimplantation evaluation of the organ. Methods: Records from 395 adult deceased KTs were reviewed. TzBs were analyzed considering histologic criteria by compartment (vascular, interstitial, tubular, and inflammatory) and correlated with the eGFR after 1 year. Results: Among donors, 56.9% were men (mean age 39 years), with the main causes of death being brain trauma (44.2%) and stroke (46.0%). Histologic analysis of TzB revealed 6.0% of glomerulosclerosis; 18.8% presenting vascular alterations; interstitial fibrosis in 54.6%; tubular changes in 76.9%, and nonspecific inflammatory infiltrate in 2.3%. Linear regression analysis showed that the main histologic findings that had impact in the eGFR were interstitial fibrosis (P = .000), followed by tubular alterations (P = .036) and glomerulosclerosis (P = .008). Conclusions: Histologic variables like interstitial fibrosis and tubular alterations show the most significant negative correlation with final eGFR. The effect of glomerulosclerosis may not be as important as formerly suggested in the literature. However, nutritional changes can occur, with clinical repercussions. The aim of the study was to compare the nutritional status in the initial and late post-KT period. Methods: A cross-sectional study was conducted involving 169 outpatients post-KT. Clinical, demographic, biochemical, food intake, handgrip strength (HGS), and anthropometric data were collected from medical records for the first nutritional care after KT. Statistical tests were performed to compare the groups according to the time of KT: early (≤1 year) and late (>1 year). The level of significance adopted was 5%. Results: The median age of the patients was 46 years (range, 38-57), 50.3% were men, and it was observed that 66.9% underwent KT with a deceased donor. There was a higher prevalence of diabetes mellitus (42.6% vs 23.5%; P =.011), and higher body mass index (28.80 ± 7.26 vs 26.51 ± 6.62 kg/m2; P =.046), arm muscle circumference (25.84 ± 4.63 vs 24.09 ± 3.36 cm; P =.019), and HGS (26.97 ± 10.70 vs 20.21 ± 10.83 kg; P =.010) in patients with late KT. Linear regression analysis showed that at each log of time, there was an increase of 1.90 kg in HGS (P =.045) and 0.48 cm (P =.036) in mid-arm muscle circumference. Conclusion: The present study demonstrated that late kidney transplantation was associated with higher values of body mass index, mid-arm muscle circumference, and HGS. No. of bitstreams: 0 Previous issue date: 2021-05-01 Background Predicting waiting time for a deceased donor kidney transplant can help patients and clinicians to discuss management and contribute to a more efficient use of resources. This study aimed at developing a predictor model to estimate time on a kidney transplant waiting list using a machine learning approach. Methods A retrospective cohort study including data of patients registered, between January 1, 2000 and December 31, 2017, in the waiting list of São Paulo State Organ Allocation System (SP-OAS) /Brazil. Data were randomly divided into two groups: 75% for training and 25% for testing. A Cox regression model was fitted with deceased donor transplant as the outcome. Sensitivity analyses were performed using different Cox models. Cox hazard ratios were used to develop the risk-prediction equations. Results Of 54,055 records retrieved, 48,153 registries were included in the final analysis. During the study period, approximately 1/3 of the patients were transplanted with a deceased donor. The major characteristics associated with changes in the likelihood of transplantation were age, subregion, cPRA, and frequency of HLA-DR, -B and -A. The model developed was able to predict waiting time with good agreement in internal validation (c-index = 0.70). Conclusion The kidney transplant waiting time calculator developed shows good predictive performance and provides information that may be valuable in assisting candidates and their providers. Moreover, it can significantly improve the use of economic resources and the management of patient care before transplant. Background and Aims Sodium-glucose co-transporter 2 (SGLT2) inhibitors, the most recent glucose lowering oral agents, prevent glucose and sodium reabsorption at proximal tubules. These drugs have the potential of nephroprotection by their glucose lowering effects, but also through direct renal effects, such as reducing intraglomerular pressure and suppressing the production of pro-inflammatory and pro-fibrotic factors. Several randomized control trials have demonstrated these effects with slowing chronic kidney disease progression. The aim of our study was to evaluate the effect of SGLT2 inhibitors on estimated glomerular filtration rate throughout five years of treatment. Method This was a retrospective observational and longitudinal study that included patients diagnosed with DM2 and treated with an iSGLT2 in a Diabetic Clinic. Demographic and clinical variables were collected, including HbA1c, disease duration and cardiovascular risk factors (CVRF). Patient glucose-control therapy as well as RAAS inhibitor drugs were gathered. The estimated Glomerular Filtration Rate (eGFR) by the formula CKD-EPI and albuminuria, using the urinary albumin-to-creatinine ratio, were evaluated at the iSGLT2 introduction date and at 6, 12, 24, 36, 48 and 60 months of treatment. Categorical variables are presented as frequencies and percentages, and continuous variables as means or medians for variables with skewed distributions. All reported p values are two-tailed, with a p value of 0.05 indicating statistical significance. A paired Student’s t-test or paired-sample test were performed for continuous variables and a chi-squared test was performed for categorical variables. Results A total of 205 patients, 54,6 % male, mean age 65,4 years, median baseline HbA1c 8,4% and median duration of DM2 14 years were studied. Of all patients, 94,1% had at least 1 CVRF, 71,2% hypertensive, 83,4% dyslipidemia, 48,8% obese, and 25,4% had past/current smoking habits. Regarding glucose-control therapy 17,1% were treated with one drug class and the remaining patients were treated with 2 or more classes. Among the patients 82,0% of them were taking metformin and 51,2% were on insulin therapy. Also, 56,6 % were on a RAAS inhibitor. Overall, 129 (62,9 %) patients received dapaglifozin therapy (5 or 10mg) and 76 (37,1%) empaglifozin (10 or 25mg). Regarding albuminuria at baseline, 37 patients had normal albuminuria (300mg/g). As for eGFR, 15,6 % patients had an eGFR between 30 and 60 and 84,4% patients had an eGFR greater than 60 ml/min/1,73m2. These clinical features were similar between the patients that received either SGLT2 inhibitor, with the exception of obesity being more prevalent in the dapaglifozin group (55,8 % vs 36,8%, p=0,009). Assessing the eGFR during the 5 years of treatment, a significant decrease was noticed in the first 6 months from an 82,3 to 78,6 ml/min/1,73m2 (p=0,002), followed by a significant increase in the second semester to 82,9 ml/min/1,73m2 (p Conclusion This observational study shows that after an initial reduction in eGFR during the first 6 months of treatment, the use of iSGLT2 after five years slowed the decrease in eGFR, similar to what randomized control trials have demonstrated, promoting a renoprotective effect on type 2 diabetic patients.

O perfil e alinhamento horizontal da rodovia são usados pelo Highway Safety Manual para estimar o efeito de greides e curvas no número previsto de acidentes. No entanto, nem sempre o projeto da via se encontra disponível aos planejadores de transporte. Além disso, a extração de parâmetros de geometria de forma convencional pode ser inviável em projetos em larga escala. Neste contexto, este artigo propõe um algoritmo capaz de extrair dados de geometria de bancos de dados geográficos ou dispositivos de GPS para uso em análises de segurança e operação. Em especial, o componente de análise de curvas horizontais foi desenvolvido de forma a identificar transições em espiral. Um algoritmo genético foi usado para calibrar o algoritmo de identificação e caracterização de curvas, de forma que os raios, desenvolvimentos e transição espiral reflitam as características reais da via. Uma rodovia de pista simples real foi usada para testar e calibrar o algoritmo proposto. Foram comparadas as características estimadas com o projeto as-built para 26 curvas horizontais. Todas as curvas foram identificadas pelo algoritmo, e todas as transições em espiral foram corretamente classificadas. Os raios estimados foram similares aos reais, indicando que o método proposto tem o potencial de ser validado e aplicado em outros locais em análises de planejamento.

Objetivo: compreender a importância da sensibilidade moral para lidar com o paciente oncologico sob cuidados paliativos. Materiais e metodo : revisao de literatura do tipo exploratoria descritiva atraves da base de dados LILACS. Resultados : os resultados se deram por meio de duas categorias: “cuidado paliativo ao paciente oncologico e a relacao enfermeiro, paciente e familiar” e “sensibilidade moral como premissa para a tomada de decisao etica em prol do paciente oncologico sob cuidado paliativo”. Conclusao: a sensibilidade moral proporciona ao enfermeiro identificar o componente moral de cada situacao e realizar a tomada de decisao em prol dos pacientes que se encontram sem prognostico com maior discernimento, empatia, benevolencia, amor e compaixao.

PurposeTo determine whether left ventricular (LV) global longitudinal strain (GLS) measured by feature-tracking (FT) cardiac magnetic resonance (CMR) improves after kidney transplantation (KT) and to analyze associations between LV GLS, reverse remodeling, and myocardial tissue characteristics.MethodsThis is a prospective single-center cohort study of kidney transplant recipients who underwent two CMR examinations in a 3T scanner, including cines, tagging, T1 and T2 mapping. The baseline exam was done until 10 days after transplantation and the follow-up after 6 months. Age and sex-matched healthy controls were also studied for comparison. ResultsA total of 44 patients (mean age 50 ± 11 years-old, 27 (61.4%) male) completed the two CMR exams. LV GLS improved from -13.4% +/- 3.0 at baseline to -15.2% +/- 2.7 at follow-up (p < 0.001), but remained impaired when compared with controls (-17.7% +/- 1.5, p = 0.007). We observed significant correlation between improvement in LV GLS with reductions of left ventricular mass index (r = 0.356, p= 0.018). Improvement in LV GLS paralleled improvements in LV stroke volume index (r = -0.429, p = 0.004), ejection fraction (r = -0.408, p = 0.006), global circumferential strain (r = 0.420, p = 0.004) and global radial strain (r = -0.530, p = 0.002). There were no significant correlations between LV GLS, native T1 or T2 measurements (p > 0.05). Conclusion In this study, we demonstrated that LV GLS measured by FT-CMR improves 6 months after KT in association with reverse remodeling, but not native T1 or T2 measurements.

Of 3992 deceased donor KT performed in 2014-2015, the incidence of DGF was 54%, ranging from 29.9% to 87.7% among centers. Risk factors ((lower-bound-95%CI) OR (upper-bound-95%CI)) were male gender ((1.066)1.249(1.463)), diabetic kidney disease ((1.053)1.296(1.595)), time on dialysis ((1.005)1.007(1.009)), retransplantation ((1.035)1.397(1.885)), preformed anti-HLA antibodies ((1.011)1.383(1.892)), HLA mismatches ((1.006)1.066(1.130)), donor age ((1.011)1.017(1.023)), donor final serum creatinine (sCr) ((1.239)1.317(1.399)), cold ischemia time (CIT) ((1.031)1.043(1.056)), machine perfusion ((0.401)0.542(0.733)), and induction therapy with rabbit antithymocyte globulin (rATG) ((0.658)0.800(0.973)). Duration of DGF > 4 days was associated with inferior renal function and DGF > 14 days with the higher incidences of acute rejection, graft loss, and death. In conclusion, the incidence and duration of DGF were high and associated with inferior graft outcomes. While late referral and poor donor maintenance account for the high overall incidence of DGF, variability in donor and recipient selection, organ preservation method, and type of induction agent may account for the wide variation observed among transplant centers. No. of bitstreams: 0 Previous issue date: 2021-01-01 To determine whether left ventricular (LV) global longitudinal strain (GLS) measured by feature-tracking (FT) cardiac magnetic resonance (CMR) improves after kidney transplantation (KT) and to analyze associations between LV GLS, reverse remodeling and myocardial tissue characteristics. This is a prospective single-center cohort study of kidney transplant recipients who underwent two CMR examinations in a 3T scanner, including cines, tagging, T1 and T2 mapping. The baseline exam was done up to 10 days after transplantation and the follow-up after 6 months. Age and sex-matched healthy controls were also studied for comparison. A total of 44 patients [mean age 50 ± 11 years-old, 27 (61.4%) male] completed the two CMR exams. LV GLS improved from − 13.4% ± 3.0 at baseline to − 15.2% ± 2.7 at follow-up (p < 0.001), but remained impaired when compared with controls (− 17.7% ± 1.5, p = 0.007). We observed significant correlation between improvement in LV GLS with reductions of left ventricular mass index (r = 0.356, p = 0.018). Improvement in LV GLS paralleled improvements in LV stroke volume index (r = − 0.429, p = 0.004), ejection fraction (r = − 0.408, p = 0.006), global circumferential strain (r = 0.420, p = 0.004) and global radial strain (r = − 0.530, p = 0.002). There were no significant correlations between LV GLS, native T1 or T2 measurements (p > 0.05). In this study, we demonstrated that LV GLS measured by FT-CMR improves 6 months after KT in association with reverse remodeling, but not native T1 or T2 measurements.
