Abstract: Glycemic traits are used to diagnose and monitor type 2 diabetes and cardiometabolic health. To date, most genetic studies of glycemic traits have focused on individuals of European ancestry. Here we aggregated genome-wide association studies comprising up to 281,416 individuals without diabetes (30% non-European ancestry) for whom fasting glucose, 2-h glucose after an oral glucose challenge, glycated hemoglobin and fasting insulin data were available. Trans-ancestry and single-ancestry meta-analyses identified 242 loci (99 novel; P < 5 x 10-8), 80% of which had no significant evidence of between-ancestry heterogeneity. Analyses restricted to individuals of European ancestry with equivalent sample size would have led to 24 fewer new loci. Compared with single-ancestry analyses, equivalent-sized trans-ancestry fine-mapping reduced the number of estimated variants in 99% credible sets by a median of 37.5%. Genomic-feature, gene-expression and gene-set analyses revealed distinct biological signatures for each trait, highlighting different underlying biological pathways. Our results increase our understanding of diabetes pathophysiology by using trans-ancestry studies for improved power and resolution.

We calculated the sensitivity and PPV with 95% confidence intervals using the Clopper-Pearson Exact test.Results: We identified 153 MI cases in the gold standard. In the Norwegian Myocardial Infarction Register, we found a PPV of 97.1% (95% confidence interval (CI) 92.8â 99.2) and a sensitivity of 88.2% (95% CI 82.0â 92.9). In the Norwegian Patient Register, the PPV was 96.3% (95% CI 91.6â 98.8) and the sensitivity was 85.6% (95% CI 79.0â 90.8). Abstract: Glycemic traits are used to diagnose and monitor type 2 diabetes and cardiometabolic health. To date, most genetic studies of glycemic traits have focused on individuals of European ancestry. Here we aggregated genome-wide association studies comprising up to 281,416 individuals without diabetes (30% non-European ancestry) for whom fasting glucose, 2-h glucose after an oral glucose challenge, glycated hemoglobin and fasting insulin data were available. Trans-ancestry and single-ancestry meta-analyses identified 242 loci (99 novel; P < 5 × 10−8), 80% of which had no significant evidence of between-ancestry heterogeneity. Analyses restricted to individuals of European ancestry with equivalent sample size would have led to 24 fewer new loci. Compared with single-ancestry analyses, equivalent-sized trans-ancestry fine-mapping reduced the number of estimated variants in 99% credible sets by a median of 37.5%. Genomic-feature, gene-expression and gene-set analyses revealed distinct biological signatures for each trait, highlighting different underlying biological pathways. Our results increase our understanding of diabetes pathophysiology by using trans-ancestry studies for improved power and resolution.

Abstract: "This scholarly monograph is based on a doctoral dissertation, On Holding Part of a Child’s Life in One’s Hands. Conversations between Professionals and Children with Mentally Ill or Substance Addicted Parents: An Ethical and Narrative Perspective, the subject of which is children’s participation in group therapy discussions for children whose parents are mentally ill or substance users. The book builds on experiences from participant observation in seven different groups and follow-up conversations with the participating children and the professionals who lead the groups. The objective was to investigate the possibilities children have in such groups for being independent subjects in search of and making sense. The issue is: How are a child’s experiences handled in discussion groups for children of mentally ill or substance addicted parents? The book’s scientific anchor is in expository hermeneutics, which underpins the emphasis on the significance of balance and reciprocity in interactions between the children and the professionals who lead the groups. In this connection, exploring in concrete situations how the group leader created space for the individual child’s perspective by putting his/her own perspectives into play (or by not doing so) is central. Expository hermeneutics also makes possible identifying some of what is not put into words, barely mentioned or implied, in interactions between the professional and the child, but which nonetheless may have considerable bearing on the understanding that is formed. The experiences and observations from the discussion groups are conveyed via nine stories detailing specific conversations and activities in which the children conveyed their experiences, thoughts and feelings, without them becoming further topics in the group work. Through interpretation of these stories a perception emerges of group conversation for children with mentally ill or addicted parents as being a practice in which the child’s own experience of their parent’s illness or dependency is only to a limited degree made an issue. Through a theoretical discussion in which what has transpired in the group interactions is elevated to a more generally insightful level, a deeper understanding of this phenomenon is sought.", Dette er en vitenskapelig monografi basert på avhandlingen Om å holde noe av et barns liv i sin hånd. Samtaler mellom profesjonelle og barn som har psykisk syke eller rusavhengige foreldre: et etisk og narrativt perspektiv. Tema er barns deltakelse i samtalegrupper for barn som har psykisk syke eller rusavhengige foreldre. Boken bygger på erfaringer fra deltakende observasjon i sju forskjellige samtalegrupper samt oppfølgende samtaler med barna som deltok og de profesjonelle som ledet gruppene. Formålet har vært å undersøke hvilke muligheter barn i samtalegrupper får til å stå fram som selvstendige meningssøkende og meningsskapende subjekter. Problemstilling er: Hvordan blir barns erfaringer håndtert i samtalegrupper for barn som har psykisk syke eller rusavhengige foreldre? Boken er gitt en vitenskapsfilosofisk forankring i en avdekkende hermeneutikk. Dette har lagt føringer for å vektlegge betydningen av det likeverdige og gjensidige i samhandlingene mellom de deltakende barna og de profesjonelle som har ledet samtalegruppene. Erfaringene og observasjonene fra samtalegruppene er formidlet gjennom ni fortellinger. Disse beskriver konkrete samtaler og aktiviteter der barn formidler egne erfaringer, tanker og følelser, uten at disse blir gjort til videre tema i samhandlingen. Abstract: Obesity rates have nearly tripled in the past 50 years, and by 2030 more than 1 billion individuals worldwide are projected to be obese. This creates a significant economic strain due to the associated non-communicable diseases. The root cause is an energy expenditure imbalance, owing to an interplay of lifestyle, environmental, and genetic factors. Obesity has a polygenic genetic architecture; however, single genetic variants with large effect size are etiological in a minority of cases. These variants allowed the discovery of novel genes and biology relevant to weight regulation and ultimately led to the development of novel specific treatments.

Abstract: Glycaemic traits are used to diagnose and monitor type 2 diabetes, and cardiometabolic health. To date, most genetic studies of glycaemic traits have focused on individuals of European ancestry. Here, we aggregated genome-wide association studies in up to 281,416 individuals without diabetes (30% non-European ancestry) with fasting glucose, 2h-glucose post-challenge, glycated haemoglobin, and fasting insulin data. Trans-ancestry and single-ancestry meta-analyses identified 242 loci (99 novel; P-8), 80% with no significant evidence of between-ancestry heterogeneity. Analyses restricted to European ancestry individuals with equivalent sample size would have led to 24 fewer new loci. Compared to single-ancestry, equivalent sized trans-ancestry fine-mapping reduced the number of estimated variants in 99% credible sets by a median of 37.5%. Genomic feature, gene-expression and gene-set analyses revealed distinct biological signatures for each trait, highlighting different underlying biological pathways. Our results increase understanding of diabetes pathophysiology by use of trans-ancestry studies for improved power and resolution.

164. Complex lifestyle intervention among inactive older adults with elevated cardiovascular disease risk and obesity. A mixed-method, single-arm feasibility study for RESTART- a randomized controlled trial

Abstract: Background: Physical inactivity and obesity are global public health challenges. Older adults are important to target for prevention and management of disease and chronic conditions. However, many individuals struggle with maintaining increased physical activity (PA) and improved diet. This feasibility study provides the foundation for the RESTART trial, a randomized controlled trial (RCT) to test a complex intervention to facilitate favourable lifestyle changes older adults can sustain. The primary objective of this study was to investigate study feasibility (recruitment, adherence, side-effects and logistics) using an interdisciplinary approach.Methods: This mixed-method one-year prospective single-arm feasibility study was conducted in Tromsø, Norway from September 2017. We invited by mail randomly selected participants from the 7 th survey of the Tromsø Study (2015-16) aged 55-75 years with sedentary lifestyle, obesity and elevated cardiovascular risk. Participants attended a six-month complex lifestyle intervention program, comprising instructor-led high-intensive exercise and nutritionist- and psychologist-led counselling, followed by a six-month follow-up. All participants used a Polar activity tracker for daily activity monitoring during the intervention. Participants were interviewed three times throughout the study. Primary outcome was study feasibility measures.Results: We invited potential participants (n=75) by mail of which 27 % (n=20) agreed to participate. Telephone screening excluded four participants, and altogether 16 participants completed baseline screening.The intervention and test procedures of primary and secondary outcomes were feasible and acceptable for the participants. There were no exercise-induced injuries, indicating that the intervention program is safe. Participants experienced that the dietary and psychological counselling were delivered too early in the intervention and in too close proximity to the start of the exercise program. Minor logistic improvements were implemented throughout the intervention period.Conclusion: This study indicates that it is feasible to conduct a full-scale RCT of a multi- component randomised intervention trial, based on the model of the present study. No drop-outs due to exercise-induced injury indicates that the exercises were safe. While minor improvements in logistics were implemented during the intervention, we will improve recruitment and adherence strategies, rearrange schedule of intervention contents (exercise, diet and psychology) as well as improving the content of the dietary and behavioural counselling to maximize outcome effects in the RESTART protocol.

165. Measuring Physical Activity Using Triaxial Wrist Worn Polar Activity Trackers: A Systematic Review

Abstract: Collecting objective physical activity data from research participants are increasingly done using consumer-based activity trackers. Several validation studies of Polar devices are conducted to date, but no systematic review of the current level of accuracy for these devices exist. The aim of this study is therefore to investigate the accuracy of current wrist-worn Polar devices that equips a triaxial accelerometer to measure physical activity. We conducted a systematic review by searching six databases for validation studies on modern Polar activity trackers. Studies were grouped and examined by tested outcome, i.e. energy expenditure, physical activity intensity, and steps. We summarized and reported relevant metrics from each study. The initial search resulted in 157 studies, out of which fourteen studies were included in the final review. Energy expenditure was reviewed in seven studies, physical activity intensity was reviewed in four studies, and steps was reviewed in 11 studies. There is a large difference in study protocols with conflicting results between the identified studies. However, for energy expenditure there is some indication that Polar devices perform better in free-living, compared to lab-based studies. In addition, step counting seems to have less average error compared to energy expenditure and physical activity intensity. There is large heterogeneity between the identified studies, both in terms of study protocols and results, and the accuracy of Polar devices remains unclear. More studies are needed for more recently developed devices, and future studies should take care to follow guidelines for assessment of wearable sensors designed for physical activity monitoring.

Abstract: Background Lack of physical activity (PA) is a risk factor for death and non-communicable disease. Despite this, more than one fourth of adults worldwide do not follow PA guidelines. As part of a feasibility study to test a complex intervention for increasing PA, we included a consumer-based activity tracker (AT) as a tool to measure PA outcomes and to track heart rate during exercise sessions. The aim of the present study was to identify factors that increase wear time when using a consumer-based AT for monitoring of participants in clinical research. Methods Sixteen participants aged 55–74 years, with obesity, sedentary lifestyle, and elevated cardiovascular risk were recruited to a 12-month feasibility study. Participants wore a Polar M430 AT to collect continuous PA data during a six-month intervention followed by 6 months of follow-up. We performed quantitative wear time analysis, tested the validity of the AT, and completed two rounds of qualitative interviews to investigate how individual wear-time was linked to participant responses. Results From 1 year of tracking, mean number of valid wear days were 292 (SD = 86), i.e. 80%. The Polar M430 provides acceptable measurements for total energy expenditure. Motivations for increased wear time were that participants were asked to wear it and the ability to track PA progress. Perceived usefulness included time keeping, heart rate- and sleep tracking, becoming more conscious about day-to-day activity, and improved understanding of which activity types were more effective for energy expenditure. Sources of AT annoyance were measurement inaccuracies and limited instruction for use. Suggestions for improvement were that the AT was big, unattractive, and complicated to use. Conclusions Adherence to wearing a consumer-based AT was high. Results indicate that it is feasible to use a consumer-based AT to measure PA over a longer period. Potential success factors for increased wear time includes adequate instruction for AT use, allowing participants to choose different AT designs, and using trackers with accurate measurements. To identify accurate trackers, AT validation studies in the target cohort may be needed. Abstract: Additional file 1. Participant Flow Chart. All inhabitants in Tromsø municipality aged 40 years and older (N = 32,591) were invited to the basic examination. A sub-sample (N = 13,028) was pre-marked for invitation to participate in the extended examination conducted approximately 2 weeks later. This sub-sample consisted of a) a randomized sample (N = 9925), and b) Tromsø 6 (2007–2008) participants who attended body composition, echocardiogram and eye examinations (n = 3103). A total of 21,083 women and men aged 40–99 years attended the basic examination (65%). A total of 8346 attended the extended examination (64% of the original pre-marked sub-sample and 90% of the sub-sample who also attended the basic examination). Abstract: I en grunnleggende engelsk setningsoppbygging, må adverbet always valigvis komme før adverbet completely, med mindre setningsstrukturen har blitt endret gjennom syntaktisk bevegelse. Spørsmålet om hvordan å redegjøre for denne type adverbsrekkefølge har ført til to ulike hold innen det lingvistiske fagmiljøet: en side favoriserer den tradisjonelle tilnærmingen med attributter; den andre foretrekker en mer «restriktiv» syntaks. Rundt århundreskiftet, vokste det frem to innflytelsesrike tilnærminger: med hans universalhierarki av funksjonelle projeksjoner argumenter Guglielmo Cinques (1999) for en tilnærming hvor adverbene spesifiserer fuksjonelle projeksjoner, mens i Thomas B. Ernst 2002 sin «Scope theory», gir han sin tilslutning til den tradisjonelle hypotese om attribusjon gjennom å argumentere for en mer sematikk-fokusert tilnærming til adverbenes rekkefølge. Bøkene hvor disse to synene blir etablert – Adverbs and Functional Heads (1999) og The Syntax of Adjuncts (2002) – er hovedfokuset i denne oppgaven. Cinque 1999 sin tilnærming viser seg å gjøre det mulig å forutse og forklare adverbenes rekkefølge gjennom en rigid organisering av adverb basert på en mangfoldig ordstillingsrekkefølge, men hindres av en mengde faktorer, hvorav spekulative frasebevegelser skiller spesielt ut. Enst 2002 sin redegjørelse klarer også å forutse og forklare rekkefølgen. Dette gjør han ved bruk av flere semantiske prinsipper og noen få syntaktiske, kun holdt tilbake av vanskeligheten i å bevise den abstrakte teorien gjennom empiri. Det at Ernst 2002 sin teori er mer fullkommen enn Cinque 1999 sin, gjør at denne sammenligningen foretrekker Ernst sin «Scope theory» over Cinques universalhierarki. Dog er begge teoriene troverdige. In basic English clauses, the adverb always must usually come before the adverb completely, unless the sentence structure has been altered through some kind of movement operation. The question of how to account for this kind of adverb ordering has led to two camps within the linguistics community: one favouring the traditional adjunction approach; the other preferring a more ‘restrictive’ way of representing the syntax of adverbs. Around the turn of the millennium, two influential approaches emerged: Cinque 1999’s universal hierarchy of functional projections, arguing for an approach in which adverbs are located in specifier positions of functional projections, and Ernst 2002’s Scope theory, giving its support to the traditional free adjunction hypothesis by arguing for a more semantic-oriented approach to adverb ordering. The two books in which these were established – Adverbs and Functional Heads (1999) and The Syntax of Adjuncts (2002) – are the main focus of this thesis. Cinque 1999’s approach is found to be able to predict and satisfactorily account for the order of adverbs through a proposed rigid order of adverbs on the basis of a rich functional sequence, but is held back by a number of factors, most notably unsubstantiated phrasal movement. Conversely, Ernst 2002’ approach manages to predict and account for adverb ordering through several semantic principles and some syntactic ones, only held back by the difficulty in explicating its cognitive nature for empirical evidence. The comparison thus favours Ernst’s free adjunction approach over Cinque’s adverbs-in-specifier approach but also considers both plausible.

Collecting objective physical activity data from research participants are increasingly done using consumer-based activity trackers. Several validation studies of Polar devices are conducted to date, but no systematic review of the current level of accuracy for these devices exist. The aim of this study is therefore to investigate the accuracy of current wrist-worn Polar devices that equips a triaxial accelerometer to measure physical activity. We conducted a systematic review by searching six databases for validation studies on modern Polar activity trackers. Studies were grouped and examined by tested outcome, i.e. energy expenditure, physical activity intensity, and steps. We summarized and reported relevant metrics from each study. The initial search resulted in 157 studies, out of which fourteen studies were included in the final review. Energy expenditure was reviewed in seven studies, physical activity intensity was reviewed in four studies, and steps was reviewed in 11 studies. There is a large difference in study protocols with conflicting results between the identified studies. However, for energy expenditure there is some indication that Polar devices perform better in free-living, compared to lab-based studies. In addition, step counting seems to have less average error compared to energy expenditure and physical activity intensity. There is large heterogeneity between the identified studies, both in terms of study protocols and results, and the accuracy of Polar devices remains unclear. More studies are needed for more recently developed devices, and future studies should take care to follow guidelines for assessment of wearable sensors designed for physical activity monitoring.

Abstract: Master i sosialfag Major societal changes are needed to overcome an increasing amount of plastic waste in the environment. We must focus on sustainability and the long-term perspective. In this master's thesis, the focus lies on the plastic and marine pollution in the municipality of Oslo and the Oslo Fjord. It is a research report that aims to elucidate the background and motives for entering a public-private partnership on plastics in Oslo. In addition, there is an examination on what sort of partnerships exist and what is needed for them to succeed. The City of Oslo has created an action plan which includes a plastic manifesto. The manifesto serves as a tool to help private businesses achieve the goals of the municipal action plan. Furthermore, the findings show that the City of Oslo's background for participation lies in the fact that they have both a responsibility and an obligation to prepare measures to address the increasing problem with plastics. Their motive for participation is the need to collaborate with the private sector in order to achieve the goals they have set in their action plan. Therefore, the City of Oslo has invited local businesses to participate in a partnership. Several parties are involved with resolving the problem and have committed to the plastic manifesto. The background for participation, as stated by the businesses which have signed the manifesto, is characterized by a similar desire to solve the problem with plastics, however, their individual views and level of involvement differ. The results showed that the partnership studied here uses networks as its form of governance and is mainly an institutional partnership. Plastic pollution and surrounding themes are complicated, and the solutions are not always obvious. It is the responsibility of the municipality to lead this partnership, and it is important that the correct governance is put in place. For the partnership to succeed, it is important that the project coordinator manages as facilitator in a positive and insightful way. Since the partnership is at an early stage, further research, at a later stage, should study the process and its effects.

Abstract: Introduction - Surveillance of physical activity at the population level increases the knowledge on levels and trends of physical activity, which may support public health initiatives to promote physical activity. Physical activity assessed by accelerometry is challenged by varying data processing procedures, which influences the outcome. We aimed to describe the levels and prevalence estimates of physical activity, and to examine how triaxial and uniaxial accelerometry data influences these estimates, in a large population-based cohort of Norwegian adults. Methods - This cross-sectional study included 5918 women and men aged 40–84 years who participated in the seventh wave of the Tromsø Study (2015–16). The participants wore an ActiGraph wGT3X-BT accelerometer attached to the hip for 24 hours per day over seven consecutive days. Accelerometry variables were expressed as volume (counts·minute-1 and steps·day-1) and as minutes per day in sedentary, light physical activity and moderate and vigorous physical activity (MVPA). Results - From triaxial accelerometry data, 22% (95% confidence interval (CI): 21–23%) of the participants fulfilled the current global recommendations for physical activity (≥150 minutes of MVPA per week in ≥10-minute bouts), while 70% (95% CI: 69–71%) accumulated ≥150 minutes of non-bouted MVPA per week. When analysing uniaxial data, 18% fulfilled the current recommendations (i.e. 20% difference compared with triaxial data), and 55% (95% CI: 53–56%) accumulated ≥150 minutes of non-bouted MVPA per week. We observed approximately 100 less minutes of sedentary time and 90 minutes more of light physical activity from triaxial data compared with uniaxial data (p Conclusion - The prevalence estimates of sufficiently active adults and elderly are more than three times higher (22% vs. 70%) when comparing triaxial bouted and non-bouted MVPA. Physical activity estimates are highly dependent on accelerometry data processing criteria and on different definitions of physical activity recommendations, which may influence prevalence estimates and tracking of physical activity patterns over time.

Abstract: Background It is not clear how physical activity affects body composition in adolescents. Physical activity levels are often reduced during this period, and the relative proportion of body fat mass and lean mass undergo natural changes in growing adolescents. We aimed to examine whether self-reported physical activity in leisure time at baseline or change in activity during follow-up affect changes in four measures of body composition; body mass index (kg/m2), waist circumference, fat mass index (fat mass in kg/m2) and lean mass index (lean mass in kg/m2). Methods We used data from the Tromsø Study Fit Futures, which invited all first year students in upper secondary high school in two municipalities in northern Norway in 2010–2011. They were reexamined in 2012–2013. Longitudinal data was available for 292 boys and 354 girls. We used multiple linear regression analyses to assess whether self-reported level of physical activity in leisure time at baseline predicted changes in body composition, and analysis of covariance to assess the effects of change in level of activity during follow-up on change in body composition. All analyses were performed sex-specific, and a p-value of Results There were no associations between self-reported leisure time physical activity in the first year of upper secondary high school and changes in any of the considered measure of body composition after 2 years of follow up, with the exception of waist circumference in boys (p = 0.05). In boys, change in fat mass index differed significantly between groups of activity change (p p = 0.04), with girls adopting physical activity having the highest increase. Conclusions Self-reported leisure time physical activity does not predict changes in body composition in adolescents after 2 years of follow up. Change in the level of physical activity is associated with change in fat mass index in boys and lean mass index in girls.

In this short comment/debate article, we outline some of the experiences from the event. Importantly, there are many common challenges. More collaboration across the borders could ensure a broader range of experience in the field and provide better ways of developing and evaluating PPI in health research.

Abstract: Aim: The prevalence of overweight and obesity has risen rapidly worldwide, and the ongoing obesity pandemic is one of the most severe public health concerns in modern society. The average body mass index (BMI) of people living in Northern Norway has also steadily increased since the late 1970s. This study aimed to understand how individuals’ health behavior is associated with the general health behavior of the people in their neighborhood. Methods: Using the population-based Tromsø Study, we examined the life course association between average leisure time physical activity at the neighborhood level and the BMI of individuals living in the same neighborhood. We used a longitudinal dataset following 25,604 individuals living in 33 neighborhoods and performed a linear mixed-effects analysis. Results: The results showed that participants living in neighborhoods whose residents were more physically active during their leisure time, were likely to have a significantly lower BMI (−0.9 kg/m², 95% CI −1.5 to −0.4). Also, individuals living in neighborhoods whose residents were doing mainly manual work, had significantly higher BMIs (0.7 kg/m², 95% CI 0.4−1.0). Conclusions: Our results showed a strong association between the average leisure time physical activity level of neighborhood residents and the higher BMI levels of residents of the same neighborhood.

Abstract: Vegetables, fruits and berries are associated with reduced disease risk, and recommended intake is “five-a-day”. We studied the intake of vegetables, fruits and berries and compliance with “five-a-day” in 11,425 adults (40–96 years) who completed a food frequency questionnaire in the seventh wave of the Tromsø Study (2015–2016). Intake and proportion above/below recommended intake (250 g/day of vegetables and 250 g/day of fruits/berries, combined and separately) were analyzed. Logistic regression was used to examine compliance with recommendations in 10-year age-groups, and level of education, body mass index (BMI) and physical activity, in strata of sex and adjusted for total energy. Median intake of vegetables was 228 and 168 g/day, and fruits/berries 292 and 268 g/day, in women and men, respectively. In total, 31% of women and 17% of men met the five-a-day recommendation, and 44% and 60% of women, and 25% and 54% of men, met the recommendation for vegetables and fruits/berries, respectively. Odds of compliance with recommendation for both vegetables and fruits/berries were positively associated with age, education and physical activity (p trend &lt, 0.001). The intake of vegetables, fruits and berries was suboptimal, in particular for vegetables. More women than men met the recommendation, and intake varied by education, physical activity level, age and BMI.
