Research has been conducted on the acyl-homoserine lactones, cell-to-cell signaling molecules in Proteobacteria species. The ability of these compounds to be degraded biologically has been investigated and the results are presented.

Abstract

IMPORTANCE Early exposure to complex dietary proteins may increase the risk of type 1 diabetes in children with genetic disease susceptibility. There are no intact proteins in extensively hydrolyzed formulas. OBJECTIVE To test the hypothesis that weaning to an extensively hydrolyzed formula decreases the cumulative incidence of type 1 diabetes in young children. DESIGN, SETTING, AND PARTICIPANTS An international double-blind randomized clinical trial of 2159 infants with human leukocyte antigen-conferred disease susceptibility and a first-degree relative with type 1 diabetes recruited from May 2002 to January 2007 in 78 study centers in 15 countries; 1081 were randomized to be weaned to the extensively hydrolyzed casein formula and 1078 to a conventional formula. The follow-up of the participants ended on February 28, 2017. INTERVENTIONS The participants received either a casein hydrolysate or a conventional adapted cows milk formula supplemented with 20% of the casein hydrolysate. The minimum duration of study formula exposure was 60 days by 6 to 8 months of age. MAIN OUTCOMES AND MEASURES Primary outcome was type 1 diabetes diagnosed according to World Health Organization criteria. Secondary outcomes included age at diabetes diagnosis and safety (adverse events). RESULTS Among 2159 newborn infants (1021 female [47.3%]) who were randomized, 1744 (80.8%) completed the trial. The participants were observed for a median of 11.5 years (quartile [Q] 1-Q3, 10.2-12.8). The absolute risk of type 1 diabetes was 8.4% among those randomized to the casein hydrolysate (n = 91) vs 7.6% among those randomized to the conventional formula (n = 82) (difference, 0.8%[95% CI, -1.6% to 3.2%]). The hazard ratio for type 1 diabetes adjusted for human leukocyte antigen risk group, duration of breastfeeding, duration of study formula consumption, sex, and region while treating study center as a random effect was 1.1 (95% CI, 0.8 to 1.5; P = .46). The median age at diagnosis of type 1 diabetes was similar in the 2 groups (6.0 years [Q1-Q3, 3.1-8.9] vs 5.8 years [Q1-Q3, 2.6-9.1]; difference, 0.2 years [95% CI, -0.9 to 1.2]). Upper respiratory infections were the most common adverse event reported (frequency, 0.48 events/year in the hydrolysate group and 0.50 events/year in the control group). CONCLUSIONS AND RELEVANCE Among infants at risk for type 1 diabetes, weaning to a hydrolyzed formula compared with a conventional formula did not reduce the cumulative incidence of type 1 diabetes after median follow-up for 11.5 years. These findings do not support a need to revise the dietary recommendations for infants at risk for type 1 diabetes. Abstract

The discovery of high-temperature conventional superconductivity in H3S with a critical temperature of Tc=203 K was followed by the recent record of Tc ~250 K in the face-centered cubic (fcc) lanthanum hydride LaH10 compound. It was realized in a new class of hydrogen-dominated compounds having a clathrate-like crystal structure in which hydrogen atoms form a 3D framework and surround a host atom of rare earth elements. Yttrium hydrides are predicted to have even higher Tc exceeding room temperature. In this paper, we synthesized and refined the crystal structure of new hydrides: YH4, YH6, and YH9 at pressures up to 237 GPa finding that YH4 crystalizes in the I4/mmm lattice, YH6 in Im-3m lattice and YH9 in P63/mmc lattice in excellent agreement with the calculations. The observed very high-temperature superconductivity is comparable to that found in fcc-LaH10: the pressure dependence of Tc for YH9 also displays a "dome like shape" with the highest Tc of 243 K at 201 GPa. We also observed a Tc of 227 K at 237 GPa for the YH6 phase. However, the measured Tcs are notably lower by ~30 K than predicted. Evidence for superconductivity includes the observation of zero electrical resistance, a decrease of Tc under an external magnetic field and an isotope effect. The theoretically predicted fcc YH10 with the promising highest Tc>300 K was not stabilized in our experiments under pressures up to 237 GPa.

Abstract

OBJECTIVE There are variable reports of risk of concordance for progression to islet autoantibodies and type 1 diabetes in identical twins after one twin is diagnosed. We examined development of positive autoantibodies and type 1 diabetes and the effects of genetic factors and common environment on autoantibody positivity in identical twins, nonidentical twins, and full siblings. RESEARCH DESIGN AND METHODS Subjects from the TrialNet Pathway to Prevention Study (N = 48,026) were screened from 2004 to 2015 for islet autoantibodies (GAD antibody [GADA], insulinoma-associated antigen 2 [IA-2A], and autoantibodies against insulin [IAA]). Of these subjects, 17,226 (157 identical twins, 283 nonidentical twins, and 16,786 full siblings) were followed for autoantibody positivity or type 1 diabetes for a median of 2.1 years. RESULTS At screening, identical twins were more likely to have positive GADA, IA-2A, and IAA than nonidentical twins or full siblings (all P < 0.0001). Younger age, male sex, and genetic factors were significant factors for expression of IA-2A, IAA, one or more positive autoantibodies, and two or more positive autoantibodies (all P ≤ 0.03). Initially autoantibody-positive identical twins had a 69% risk of diabetes by 3 years compared with 1.5% for initially autoantibody-negative identical twins. In nonidentical twins, type 1 diabetes risk by 3 years was 72% for initially multiple autoantibody–positive, 13% for single autoantibody–positive, and 0% for initially autoantibody-negative nonidentical twins. Full siblings had a 3-year type 1 diabetes risk of 47% for multiple autoantibody–positive, 12% for single autoantibody–positive, and 0.5% for initially autoantibody-negative subjects. CONCLUSIONS Risk of type 1 diabetes at 3 years is high for initially multiple and single autoantibody–positive identical twins and multiple autoantibody–positive nonidentical twins. Genetic predisposition, age, and male sex are significant risk factors for development of positive autoantibodies in twins.

Abstract

Beta carotene supplementation may not reduce mortality from cardiovascular disease or other causes, though high beta carotene levels before supplementation may be associated with reduced risk of death. Some research has suggested that beta carotene may reduce the risk of coronary heart disease and cancer, though these findings are controversial. For a median of 4.3 years, 1,188 men and 532 women took either 50 milligrams a day of beta carotene or a placebo. During a median follow-up of 8.2 years, 285 of the participants died. In comparison to the people with the lowest beta carotene blood levels before supplementation, the people with the highest levels had approximately half the risk of death from cardiovascular disease or all causes. However, mortality rates among people who received placebos and among those who received beta carotene supplements were similar.

Abstract

The risk of breast cancer in women who have had a spontaneous or induced abortion appears to be small and could be confounded by biases in reporting abortions. Researchers identified 6,650 cases of breast cancer from cancer registries in Wisconsin, Maine, New Hampshire and western Massachusetts. They compared the history of induced or spontaneous abortion (miscarriage) between these women and 9,227 women without breast cancer. Twenty-seven percent of the women with breast cancer had had an abortion compared to 25% of those without breast cancer. In over 90% of the cases and controls, the fetus was lost through a miscarriage. Women with a history of spontaneous or induced abortion were 12% more likely to develop breast cancer than those who never had an abortion. Induced abortions gave a woman a slightly higher risk than miscarriages, especially those performed before abortion was legalized. This could reflect biases in reporting abortion statistics., Objective.--To evaluate the association between pregnancy terminations and risk of breast cancer. Design and Setting.--Population-based case-control study in Wisconsin, Massachusetts, Maine, and New Hampshire. Study Participants.--Cases were women younger than 75 years with a new diagnosis of breast cancer (n=6888), identified from statewide tumor registries. Controls younger than 65 years (n=9529) were randomly selected from lists of licensed drivers, or for older subjects, from lists of Medicare beneficiaries. Exposures and Outcomes.--Breast cancer risk in relation to spontaneous or induced abortions. Results.--After adjustment for parity, age at first birth, and other risk factors, pregnancy termination (induced or spontaneous) was associated with a relative risk (RR) of breast cancer of 1.12 (95% confidence interval [CI], 1.04to 1.21), compared with the risk among women who had never had a termination. Induced terminations were associated with a RR of 1.23 (95% CI, 1.00 to 1.51), which was somewhat greater than the risk associated with spontaneous terminations (RR, 1.11; 95% CI, 1.02 to 1.20). The association with induced abortions was stronger for those performed before legalization of abortion in 1973 (RR, 1.35; 95% CI, 1.01 to 1.80) than after this time (RR, 1. 1 2; 95% CI, 0.84 to 1.49), suggesting a bias in reporting this sensitive procedure. Conclusions.--A weak positive association was observed between abortion whether induced or spontaneous--and risk of breast cancer. The increase in risk of breast cancer was somewhat greater among women with a history of induced terminations. However, this association may be due to reporting bias and was not significantly different than the slight risk for spontaneous terminations. (JAMA. 1996;275:483-287)

Rhizobium leguminosarum produces an N-acylated homoserine lactone autoinducer signal necessary for the activation of the rhiABC operon by the transcriptional activator RhiR, which is involved in root nodulation. This autoinducer also activates other growth-inhibiting functions encoded by the Sym plasmid pRL1J1, a function never before observed in autoinducers. Generally, these results suggest the existence of a novel method of intercellular signaling during the formation of root nodules.

Abstract

It appears that taking supplemental beta carotene and vitamins A and D does not help prevent colorectal adenoma. Colorectal adenoma is a precursor of cancer of the bowels, which is less common in people with diets that include antioxidant vitamin-rich foods such as vegetables and fish. Different combinations of a placebo, beta carotene, and vitamins A and D were given to 751 people who had previously had an adenoma removed. After four years, plasma levels of beta carotene had risen in the group receiving beta carotene supplements, and vitamin C and D levels rose in the groups receiving those vitamins. The incidences of colorectal adenoma were similar in all groups, suggesting that beta carotene and vitamins A and D do not provide added protection against cancer.

Breastfeeding may slightly reduce the risk of developing breast cancer among premenopausal women. Among 14,094 women under 75 years old, 5,878 had breast cancer and 8,216 did not (control group). Premenopausal women who had breast-fed had a slightly lower risk of developing breast cancer than those who had not. The risk of developing breast cancer decreased with the length of time a woman breastfed. Among postmenopausal women, breastfeeding was not associated with a significant decrease in the risk of developing breast cancer. The reduction in risk among premenopausal women may be caused by hormonal changes associated with milk production.

An analysis of the mechanisms by which marine luminescent bacterium Vibrio fischeri employs the LuxR and LuxI proteins for intercellular communication confirms the presence of multiple autoinduction circuits in a single bacterial strain. An adequately large population of bacteria is crucial for some bacterial behaviors. A quorum of bacteria represents this minimum behavioral unit. LuxR-LuxI type systems offer a facile pathway for bacteria enumeration.

Background: In two large, randomized prevention trials, supplementation with [beta]-carotene increased the risk of lung cancer. Subjects in these studies were predominantly cigarette smokers, and the adverse effects were concentrated among those who also drank alcohol. Although [beta]-carotene supplementation appeared not to increase the risk of cancer generally, it is not clear if smoking and/or alcohol use alters the effect of [beta]-carotene on carcinogenesis at sites outside the lung. Methods: We studied the effect of [beta]-carotene supplementation on colorectal adenoma recurrence among subjects in a multicenter double-blind, placebo-controlled clinical trial of antioxidants for the prevention of colorectal adenomas. A total of 864 subjects who had had an adenoma removed and were polyp-free were randomly assigned (in a factorial design) to receive [beta]-carotene (25 mg or placebo) and/or vitamins C and E in combination (1000 mg and 400 mg, respectively, or placebo), and were followed with colonoscopy for adenoma recurrence 1 year and 4 years after the qualifying endoscopy. A total of 707 subjects had two follow-up examinations and provided smoking and alcohol use data. Adjusted multivariate risk ratios (RRs) and 95% confidence intervals (CIs) were used to assess the effects of [beta]-carotene on adenoma recurrence. Results: Among subjects who neither smoked cigarettes nor drank alcohol, [beta]-carotene was associated with a marked decrease in the risk of one or more recurrent adenomas (RR = 0.56, 95 % CI = 0.35 to 0.89), but [beta]-carotene supplementation conferred a modest increase in the risk of recurrence among those who smoked (RR = 1.36, 95% CI = 0.70 to 2.62) or drank (RR = 1.13, 95% CI = 0.89 to 1.43). For participants who smoked cigarettes and also drank more than one alcoholic drink per day, [beta]-carotene doubled the risk of adenoma recurrence (RR = 2.07, 95 % CI = 1.39 to 3.08; P for difference from nonsmoker/nondrinker RR < .001). Conclusion: Alcohol intake and cigarette smoking appear to modify the effect of [beta]-carotene supplementation on the risk of colorectal adenoma recurrence.

Abstract

Use of diethylstilbestrol (DES) during pregnancy appears to increase the risk of breast cancer, but the risk does not appear to increase dramatically over time. In a long term study, there was almost no difference in the rate of breast cancer in the first 20 years after exposure between women who were exposed to DES during pregnancy and women who were not. At 20 years of follow-up, the incidence of breast cancer was 1.53% among women exposed to DES and 1.29% among unexposed women. At 30 years of follow-up, the incidence of breast cancer was 3.77% among women exposed to DES and 2.87% among unexposed women. A 1984 study, which was based on only 25 cases, had suggested the risk might double over time, but the more recent findings indicate otherwise., Objective.--Further assessment of the long-term risk of breast cancer associated with diethylstilbestrol (DES) during pregnancy. Design.--Follow-up continuation through June 1, 1989, of a historical cohort of DES-exposed and unexposed mothers ascertained by review of obstetric records. Participants.--Totals of 3029 each of DES-exposed and unexposed mothers who had delivered live babies at four centers in the United States during 1940 through 1960. Questionnaires were returned for 92.6% of the DES-exposed and 88.8% of the unexposed women. Main Outcome Measures.--Breast cancer incidence and mortality assessed from returned questionnaires and review of medical records and death certificates. Main Results.--The relative rate of breast cancer associated with DES exposure, after adjustment for demographic and reproductive variables was 1.35 (95% confidence interval, 1.05 to 1.74). For 30 years or more following exposure, the relative rate was not appreciably higher (relative rate, 1.33; 95% confidence interval 0.95 to 1.87) than that earlier periods. Surveillance and increased detection seemed unlikely explanations for the increased risk, since DES-exposed women had excesses of both large and small breast cancers and the two cohorts reported similar breast cancer detection practices. A history of miscarriage before first term delivery was not associated with breast cancer occurrence. Conclusion.--Exposure to DES during pregnancy is associated with a modest but statistically significant increased risk of breast cancer. Contrary to prior indications, the risk does not appear to increase greatly over time. The findings are sufficient to exclude the possibility of a doubling of risk for the period of 30 or more years following exposure. (JAMA. 1993;269:2096-2100)
