BACKGROUND: Patients with systolic heart failure and anemia have worse symptoms, functional capacity, and outcomes than those without anemia. We evaluated the effects of darbepoetin alfa on clinical outcomes in patients with systolic heart failure and anemia. METHODS: In this randomized, double-blind trial, we assigned 2278 patients with systolic heart failure and mild-to-moderate anemia (hemoglobin level, 9.0 to 12.0 g per deciliter) to receive either darbepoetin alfa (to achieve a hemoglobin target of 13 g per deciliter) or placebo. The primary outcome was a composite of death from any cause or hospitalization for worsening heart failure. RESULTS: The primary outcome occurred in 576 of 1136 patients (50.7%) in the darbepoetin alfa group and 565 of 1142 patients (49.5%) in the placebo group (hazard ratio in the darbepoetin alfa group, 1.01; 95% confidence interval, 0.90 to 1.13; P=0.87). There was no significant between-group difference in any of the secondary outcomes. The neutral effect of darbepoetin alfa was consistent across all prespecified subgroups. Fatal or nonfatal stroke occurred in 42 patients (3.7%) in the darbepoetin alfa group and 31 patients (2.7%) in the placebo group (P=0.23). Thromboembolic adverse events were reported in 153 patients (13.5%) in the darbepoetin alfa group and 114 patients (10.0%) in the placebo group (P=0.01). Cancer-related adverse events were similar in the two study groups. CONCLUSIONS: Treatment with darbepoetin alfa did not improve clinical outcomes in patients with systolic heart failure and mild-to-moderate anemia. 161. How do sonographers improve access to cardiology care? An outpatient study.

- Abstract
We report our experience working with cardiac sonographers (SONO) in outpatient settings between April 2019 and December 2022. Our observation focused on 7 outpatient cardiac centers in the Auvergne Rhône Alpes region, 35,743 patients had at least one consultation between April 2019 and December 2022. In total, 23,657 patients an echocardiogram (ECHO) performed (66%). Seven SONOs were involved, supervised by 15 cardiologists. All ECHOs were technically performed based on a standard acquisition protocol. Data was saved on a dedicated cloud-based Picture Archiving and Communication System (PACS) allowing the cardiologist to check the image quality, results and complete the report, automatically included in the electronic health record. Out of a total of 25,686 ECHO (i.e., an average of 1.1 ECHO/patient), ∼19,800 (77%) were performed by SONOs (range from 72% to 92% depending on the medical center). In total, ∼2000 ECHO (∼10%) required technical expertise and additional acquisition of images by the referring cardiologist (range from 5% to 25% depending on the experience level of the SONO). Based on a study sample, the average ECHO acquisition time was 12 ± 7 minutes. Thus, over the entire period of 33-month period, the total medical time saved was estimated up to 4,700 hours. Teamwork is mandatory to improve access to outpatient care in cardiology. Working with cardiac sonographers is efficient and saves cardiologists up to 700 hours/SONO/year of medical time. Patients with COVID-19 who develop severe acute respiratory distress syndrome (ARDS) can have symptoms that rapidly evolve to profound hypoxaemia and death. The efficacy of extracorporeal membrane oxygenation (ECMO) for patients with severe ARDS in the context of COVID-19 is unclear. We aimed to establish the clinical characteristics and outcomes of patients with respiratory failure and COVID-19 treated with ECMO.

- Abstract
La cardiomyopathie du péripartum est une insuffisance cardiaque systolique survenant à la fin de la grossesse ou dans les mois qui suivent l’accouchement. Il s’agit d’un diagnostic d’exclusion. L’incidence de cette pathologie est élevée chez les femmes noires, âgées de plus de 40 ans, ayant une hypertension artérielle pendant la grossesse, ou ayant eu de nombreuses grossesses. Les symptômes d’insuffisance cardiaque sont voisins de ceux de la fin de grossesse normale, retardant souvent le diagnostic et favorisant ainsi les complications. L’échocardiographie est l’examen fondamental permettant d’affirmer l’altération de la fonction systolique du VG (<45 %). Le traitement est celui de l’insuffisance cardiaque mais adapté pendant la grossesse avec nécessité de respecter les contre-indications. L’évolution peut être la récupération complète, la persistance d’une insuffisance cardiaque et/ou d’une atteinte de la fraction d’éjection, des complications parfois sévères (arythmies, thromboembolies) et décès. Des grossesses ultérieures peuvent être envisagées avec surveillance chez les patientes ayant totalement récupérées, mais sont fortement déconseillées s’il n’y a pas eu de récupération avec un risque important d’aggravation et de décès. Le traitement par la bromocriptine reste en cours d’évaluation.

HistoryA 58-year-old man who was an active smoker was admitted twice to the intensive care unit (ICU) of a tertiary referral thoracic center for severe hypercapnic acute respiratory failure and persistent bilateral chest radiograph opacities that were unchanged over the course of the two ICU admissions within a 3-month period (Fig 1). He had obesity (body mass index, 36 kg/m2), stage 3 vascular chronic renal insufficiency, and hebephrenic schizophrenia treated with haloperidol, carbamazepine, and cyamemazine. He reported chronic dyspnea on exertion, which worsened for 6 months.At the second ICU admission, he was afebrile, with a blood pressure of 160/72 mm Hg and pulse oximetry of 93% on 6 L/min oxygen therapy through a nonrebreathing mask.Physical examination showed signs of respiratory failure with wheezing and active abdominal expiration, bilateral pulmonary crackles without chest pain, hemoptysis, clubbing, or signs of cardiac failure. He had no peripheral lymphadenopathy and no enlarged spleen. Blood gases (on 6 L/min oxygen) showed respiratory acidosis (pH, 7.15 [normal range, 7.38–7.42]; Pao2level, 67 mm Hg [normal range, 80–100 mm Hg]; Paco2level, 102 mm Hg [normal range, 38–42 mm Hg]; Hco3−level, 29 mmol/L [normal range, 22–27 mmol/L]). Noninvasive ventilation was initiated.Imaging performed during the second ICU hospitalization included noncontrast chest CT (Fig 2), MRI of the chest without contrast enhancement (Fig 3), and fluorine 18 fluorodeoxyglucose PET/CT (Fig 4).

Highlights • Ten screening/isolation approaches for multidrug-resistant organisms (MDROs) were described in French intensive care units. • In most cases, screening was performed on admission for at least 1 MDRO. • Preemptive isolation was initiated on admission, mostly in a targeted way. • Discrepancies compared to the 2009 French national guidelines were observed. Background The prevalence of multidrug-resistant organisms (MDROs) has dramatically increased. The aim of this survey was to describe and analyze the different screening and isolation policies regarding MDROs in French adult intensive care units (ICUs). Materials and Methods A multicenter online survey was performed among French ICUs, including 63 questions distributed into 4 parts: characteristics of the unit, MDRO screening policy, policy regarding contact precautions, and ecology of the unit. Results From April 2015 to June 2016, 73 of 301 ICUs (24%) participated in the survey. MDRO screening was performed on admission in 96% of ICUs, for at least 1 MDRO (78%). MDRO screening was performed weekly during ICU stay in 83% of ICUs. Preemptive isolation was initiated on admission in 82% of ICUs, mostly in a targeted way (71%). Imported and acquired MDRO rates >10% were reported in 44% and 27% of ICUs, respectively. An MDRO outbreak had occurred within the past 3 years in 48% of cases. Conclusion French ICUs have variable screening and isolation approaches for MDROs, as up to 10 combinations were met. Discrepancies with the 2009 national guidelines were observed. - Abstract
Almost 10% of patients with acute respiratory distress syndrome (ARDS) have no identified ARDS risk factor at ARDS diagnosis. A prompt identification of the cause of ARDS is required to initiate a targeted treatment. The purpose of this review is to delineate the main rare causes of ARDS and to provide clinicians with a pragmatic diagnostic workup. Recent epidemiological data have proposed the identification of a subgroup of ARDS patients lacking exposure to common risk factors. These can be categorized as having immune-mediated, drug-induced, malignant, and idiopathic diseases. A standardized diagnostic workup including chest imaging, the objective assessment of left-heart–filling pressures, bronchoalveolar lavage fluid microbiological investigations, and cytologic analysis, immunologic tests, and search for pneumotoxic drugs may allow for identifying the cause of ARDS in most cases. Open-lung biopsy should be considered in other cases.

OBJECTIVE--To evaluate the prevalence of cardiac toxoplasmosis in a series of 182 necropsies performed between 1987 and 1991 on patients infected with the human immunodeficiency virus (HIV), to correlate this prevalence with the ante mortem diagnosis of cardiac involvement, and to assess the role of such cardiac lesions in the immediate cause of death. PATIENTS AND METHODS--Complete necropsies of 182 HIV-infected patients (48 women, 134 men) were performed consecutively between 1987 and 1991. Risk factors, identified in 174 cases, included drug abuse (111/182), homosexuality (51/182), and blood transfusions (12/182). 16 samples were systematically obtained from each heart for histological study. If trophozoites or lymphocytic myocarditis were seen, immunohistochemical investigations were carried out with polyclonal antibodies for Toxoplasma gondii. An ultrastructural study was performed in four patients with toxoplasma myocarditis. Myocardial lesions were defined by the Dallas classification. Clinical data (and information on electrocardiograms and echocardiograms) were obtained from medical records. RESULTS--Cardiac toxoplasmosis was diagnosed at necropsy in 21 (12%) patients. Cardiac lesions were associated with toxoplasmic encephalitis in 18 patients and were solitary in three patients. Acute diffuse myocarditis was present in 6/21, rare foci of myocarditis were seen in 8/21, and intramyocytic toxoplasmic cysts without any inflammatory reaction or necrosis were seen in 4/21. Anti-toxoplasma immunolabelling showed cardiac toxoplasmosis in three patients with lymphocytic myocarditis. Particles with the ultrastructural characteristics of Toxoplasma gondii trophozoites were seen in four cases. Six patients had presented with cardiac symptoms, confirmed by electrocardiographic and echocardiographic abnormalities during their disease course, and their cardiac lesions were directly responsible for the death. CONCLUSION--Cardiac toxoplasmosis was common in this necropsy series of HIV-infected patients. Cardiac toxoplasmosis had been suspected clinically in four patients. Myocardial lesions were generally asymptomatic and were not discovered until necropsy. Solitary cardiac involvement was not uncommon reflecting parasite reactivation at a myocardial site. The incidence of cardiac toxoplasmosis in this group of immunodepressed subjects from an area with a high prevalence of this parasitic disease justifies regular follow up of such patients by electrocardiography and echocardiography as well as immediate administration of anti-toxoplasma treatment should sudden heart failure occur.

This open study compared the efficacy and side effects of two dose regimens of intravenous rt-PA initiated before the fifth hour after the onset of myocardial infarction. The first 40 patients (group A) received 1 mg/kg of rt-PA infused over 90 minutes, including a 10-mg initial bolus. The following 41 patients (group B) received 1.5 mg/kg (20-mg initial bolus, 60% over 60 minutes and 40% over 120 minutes). Noninvasive clinical, electrocardiographic, and biochemical parameters of reperfusion were recorded systematically, along with serial measurements of fibrinogen and hemoglobin levels. Coronary arteriography and left ventriculography were performed at the 48th hour or earlier, on an emergency basis, in the absence of signs of reperfusion, or if there were clinical and ECG signs of rethrombosis. En France, l’hémoptysie sévère sur cancer est devenue la première cause d’hémoptysie alors elle est exceptionnelle en cas d’hémopathie. En l’absence d’un traitement adéquate, la mortalité hospitalière dépasse 60 % et atteint 100 % à 6 mois. La prise en charge, discutée avec l’oncologue, comprend l’angio-TDM volumique incontournable, en dehors de la situation d’hémoptysie menaçante où la fibroscopie bronchique peut être réalisée de première intention. L’angio-TDM volumique permet de localiser le saignement, d’en déterminer la cause et le mécanisme vasculaire. Dans plus de 90 % des cas, l’hémoptysie est liée à l’hypervascularisation systémique bronchique ou non bronchique, dans moins de 5 % des cas elle est en rapport avec une atteinte de la circulation pulmonaire et exceptionnellement avec une lésion de la barrière alvéolocapillaire. Les patients les plus graves doivent être pris en charge en réanimation dans des centres disposant d’une radiologie interventionnelle, d’une chirurgie thoracique et idéalement d’une bronchoscopie interventionnelle. La radiologie interventionnelle est le traitement symptomatique de première intention. L’artériographie bronchique avec embolisation permet un contrôle immédiat dans plus de 80 % des cas. La chirurgie en urgence doit être évitée, car elle est associée à une mortalité significative. Cette prise en charge adéquate réduit la mortalité hospitalière à 30 % et permet aux patients de bénéficier des traitements les plus récents qui améliorent leur survie.

- Abstract
Web services technology standards enable description, publication, discovery of and binding to services distributed towards the Internet. However, current standards do not address the service selection issue : how did a consumer select the service that matches its functional (e.g. operations' semantics) and non-functional (e.g. price, reputation, response time) properties ? Most projects advocate automatic selection mechanism, advising adaptation or modification of the web-services model and its entities (UDDI, WSDL, Client, Provider). These proposals also do not take advantage of distributed-systems' state of the art, mainly with respect to the collection and the dissemination of services' QoS. This paper presents an extension of the initial model that permits automatic service selection, late binding and collection of metrics that characterize the quality of service. The extension consists on a web-service access infrastructure, made of web service proxies and a peer to peer network of QoS metrics repository (the proposal does not impose modification on UDDI registries or services). The proxies interact with common UDDI registries to find suitable services for selection and to publish descriptions. They collect QoS metrics and store them on a p2p network. [ABSTRACT FROM AUTHOR]

We measured B(E2; 0g.s.+ → 21+) values for the proton-rich 46Cr, 50Fe, and 54Ni nuclides by intermediate-energy Coulomb excitation in order to study the systematic behavior of collectivity in the Z=20-28 region. The present study completes the B(E2) values for the Tz=±1 even-even pair nuclei up to Z=28. The double ratios of proton and neutron matrix elements, (

Background: Only a minority of patients with primary hyperparathyroidism (pHPT) present with the “classic” symptoms. Most patients have numerous nonspecific symptoms. The aim of this study was to evaluate patients’ quality of life and nonspecific symptoms before and after cure for pHPT. Methods: This prospective, multicentric study, which took place from May 2003 to September 2004, included 100 patients. Six academic departments of Endocrine Surgery in France participated in the study: the University of Angers, Limoges, Nancy, Nantes, Marseille, and Poitiers. Only cured patients were included. All patients were given preoperative and postoperative questionnaires (the SF-36v2 Health Survey) at 3, 6, and 12 months to evaluate quality of life and nonspecific symptoms. Results: Preoperatively, the main nonspecific symptoms included the following: anxiety (89%); muscular, bone, or join pain (87%); abdominal distention (82%); forgetfulness (81%); headaches (81%); and mood swings (79%). Quality of life was significantly improved at 3 and 6 months (P < .05). At 1 year postoperatively, statistically significant improvement (P < .05) persisted in all 8 domains of the SF-36v2. At 1 year after parathyroidectomy, 5 symptoms remained significantly improved: appetite loss, weight loss, thirst, headache, and nausea. Conclusions: Operative cure of primary hyperparathyroidism significantly improves quality of life and nonspecific symptoms for at least 1 year. [Copyright &y& Elsevier]
