Fetal life and the first few months after birth represent a plastic age, defined as a “window of opportunity”, as the organism is particularly susceptible to environmental pressures and has to adapt to environmental conditions. Several perturbations in pregnancy, such as excessive weight gain, obesity, gestational diabetes mellitus and an inadequate or high-fat diet, have been associated with long-term metabolic consequences in offspring, even without affecting birth weight. Moreover, great interest has also been focused on the relationship between the gut microbiome of early infants and health status in later life. Consistently, in various epidemiological studies, a condition of dysbiosis has been associated with an increased inflammatory response and metabolic alterations in the host, with important consequences on the intestinal and systemic health of the unborn child. This review aims to summarize the current knowledge on the origins of NAFLD, with particular attention to the potential implications of intrauterine life and the early postnatal period. Due to the well-known association between gut microbiota and the risk of NAFLD, a specific focus will be devoted to factors affecting early microbiota formation/composition.

Fires are a growing problem even in temperate climate areas, such as those in Central Italy, due to climate change leading to longer and longer periods of drought. Thus, knowing the fire susceptibility of an area is crucial for good planning and taking appropriate countermeasures. In this context, it was decided to use only causal factors of a geomorphological and environmental nature in order to obtain a fire susceptibility analysis that can also be applied to climatically under-sampled areas. Vector data of fires in Central Italy from 2005 to 2023 were collected, and the correct areal extent was calculated for each. At the same time, six factors were selected that could have an influence on fire development, such as ecological units, topographic wetness index (TWI), geology, slope, exposure, and altitude. The model was obtained by means of the weight of evidence statistical method, which takes into account past data by reinterpreting them in a future-oriented way on the basis of the identified factors and classes. The model was validated with a test sample and shows an area under the curve (AUC) value of 0.72 with a reliability that can be described as good considering the total absence of climatic factors that are known to play a major role in fire development. Furthermore, the identified causal factors were divided into classes, and these were carefully weighted in order to define their relative influence in the study area. Particularly Ecological Units with characteristic and well-defined contrast (C) values, which could lead to a more complete definition of forests that tend to increase fire susceptibility and those that tend to decrease it, allowing the latter to be exploited as a hazard mitigation agent. The Setifian high-plains region, Northeast of Algeria, grapples with challenges in water resource management. As the water demand intensifies across a diverse range of sectors, assessing groundwater quality becomes indispensable. This article responds to the critical need for a thorough assessment of groundwater quality in the Wadi Boussellam sub-watershed. Employing a GIS-based method, we evaluate groundwater geochemistry by estimating the Water Quality Index (WQI), offering a comprehensive overview of water consumption. The analysis of groundwater samples reveals distinct facies, including calcic bicarbonate, calcic chloride, calcic sulfate, and magnesium sulfate, contributing to an enhanced understanding of the hydrochemical composition in the Setif region. Hydrochemical indices, specifically the WQI, Sodium Adsorption Ratio (SAR), and Na% are applied to assess groundwater suitability for various applications. The results indicate that most crops are generally suitable for irrigation, though they advise exercising caution with regard to human consumption. This study underscores the significance of regular monitoring to avert groundwater contamination and ensure sustainable use in the Setif region, providing insights that emphasize the ongoing necessity for efforts in water resource management and the preservation of this vital resource's quality. Aims: Given the increasing number of individuals developing metabolic dysfunction‐associated steatotic liver disease (MASLD) and the low rate of those with progressive liver disease, there is a pressing need to conceive affordable biomarkers to assess MASLD in general population settings. Herein, we aimed to investigate the performance of the ultrasound‐derived fat fraction (UDFF) for hepatic steatosis in high‐risk individuals. Methods: A total of 302 Europeans with obesity, type 2 diabetes, or a clinical history of hepatic steatosis were included in the analyses. Clinical, laboratory, and imaging data were collected using standardized procedures during a single screening visit in Rome, Italy. Hepatic steatosis was defined by controlled attenuation parameter (CAP) or ultrasound‐based Hamaguchi's score. UDFF performance for hepatic steatosis was estimated by the area under the receiver operating characteristic curve (AUC). Results: Overall, median (IQR) UDFF was 12% (7–20). UDFF was positively correlated with CAP (ρ = 0.73, p < 0.0001) and Hamaguchi's score (ρ = 0.79, p < 0.0001). Independent predictors of UDFF were circulating triglycerides, alanine aminotransferase (ALT), and ultrasound‐measured visceral adipose tissue (VAT). UDFF AUC was 0.89 (0.85–0.93) and 0.92 (0.88–0.95) for CAP‐ and ultrasound‐diagnosed hepatic steatosis, respectively. UDFF AUC for hepatic steatosis was higher than those of fatty liver index (FLI), hepatic steatosis index (HSI), CAP‐score (CAPS), and ALT (p < 0.0001). Lower age, ALT, and VAT were associated with discordance between UDFF and ultrasound. Conclusions: UDFF may be a simple and accurate imaging biomarker to assess hepatic steatosis and monitor changes in hepatic fat content over time or in response to therapeutic interventions beyond clinical trials. Arg-Gly-Asp (RGD)-binding integrins, e.g., αvβ3, αvβ1, αvβ5 integrins, are currently regarded as privileged targets for the delivery of diagnostic and theranostic agents, especially in cancer treatment. In contrast, scarce attention has been paid so far to the diagnostic opportunities promised by integrins that recognize other peptide motifs. In particular, α4β1 integrin is involved in inflammatory, allergic, and autoimmune diseases, therefore, it represents an interesting therapeutic target. Aiming at obtaining simple, highly stable ligands of α4β1 integrin, we designed hybrid α/β peptidomimetics carrying linkable side chains for the expedient functionalization of biomaterials, nano- and microparticles. We identified the prototypic ligands MPUPA-(R)-isoAsp(NHPr)-Gly-OH (12) and MPUPA-Dap(Ac)-Gly-OH (13) (MPUPA, methylphenylureaphenylacetic acid; Dap, 2,3-diamino propionic acid). Modification of 12 and 13 by introduction of flexible linkers at isoAsp or Dap gave 49 and 50, respectively, which allowed for coating with monolayers (ML) of flat zeolite crystals. The resulting peptide–zeolite MLs were able to capture selectively α4β1 integrin-expressing cells. In perspective, the α4β1 integrin ligands identified in this study can find applications for preparing biofunctionalized surfaces and diagnostic devices to control the progression of α4β1 integrin-correlated diseases.

In central Italy, particularly in the Umbria-Marche Apennines, there are some complete, high-altitude weather stations, which are very important for assessing the climate in these areas. The mountain weather stations considered in this study were Monte Bove Sud (1917 m.a.s.l.), Monte Prata (1816 m.a.s.l.) and Pintura di Bolognola (1360 m.a.s.l.). The aim of this research was to compare the differences between the precipitation measured by the rain gauges and the data obtained by satellite using the IMERG algorithm, at the same locations. The evaluation of possible errors in the estimation of precipitation using one method or the other is fundamental for obtaining a reliable estimate of precipitation in mountain environments. The results revealed a strong underestimation of precipitation for the rain gauges at higher altitudes (Monte Bove Sud and Monte Prata) compared to the same pixel sampled by satellite. On the other hand, at lower altitudes, there was a better correlation between the rain gauge value and the IMERG product value. This research, although localised in well-defined locations, could help to assess the problems in rain detection through mountain weather stations.

185. Voltammetric analysis for distinguishing portal hypertension-related from malignancy-related ascites: A proof of concept study.

BACKGROUND:Serum-ascites albumin gradient (SAAG) remains the most sensitive and specific marker for the differentiation of ascites due to portal hypertension from ascites due to other causes. SAAG has some limitations and may fail in selected conditions. Voltammetric analysis (VA) has been used for the detection of electroactive species of biological significance and has proven effective for detection infections in biological fluids. AIMS:In this study, we compared the accuracy of voltammetric analysis (VA) with that of SAAG to differentiate ascites due to portal hypertension from that having a different origin. METHODS:80 ascites samples were obtained from patients undergoing paracentesis at the Campus Bio-Medico Hospital of Rome. VA was performed using the BIONOTE device. The ability of VA to discriminate ascitic fluid etiology and biochemical parameters was evaluated using Partial Least Square Discriminant Analysis (PLS-DA), with ten-fold cross-validations. RESULTS:Mean age was 68.6 years (SD 12.5), 58% were male. Ascites was secondary to only portal hypertension in 72.5% of cases (58 subjects) and it was secondary to a baseline neoplastic disease in 27.5% of cases (22 subjects). Compared to SAAG≥1.1, e-tongue predicted ascites from portal hypertension with a better accuracy (92.5% Vs 87.5%); sensitivity (98.3% Vs 94.8%); specificity (77.3% Vs 68.2%); predictive values (PPV 91.9% Vs 88.7% and NPV 94.4% Vs 83.3%). VA correctly classified ascites etiology in 57/58 (98.2%) of cases with portal hypertension and in 17/22 (77.2%) of cases with malignancy. Instead, VA showed poor predictive capacities towards total white blood count and polymorphonuclear cell count. CONCLUSIONS:According to this proof of concept study, VA qualifies as a promising low-cost and easy method to discriminate between ascites secondary to portal hypertension and ascites due to malignancy.

Aims The association between hyperglycemia at hospital admission and relevant short- and long-term outcomes in elderly population is known. We assessed the effects on mortality of hyperglycemia, disability, and multimorbidity at admission in internal medicine ward in patients aged ≥ 65 years. Methods Data were collected from an active register of 102 internal medicine and geriatric wards in Italy (RePoSi project). Patients were recruited during four index weeks of a year. Socio-demographic data, reason for hospitalization, diagnoses, treatment, severity and comorbidity indexes (Cumulative Illness rating Scale CIRS-SI and CIRS-CI), renal function, functional (Barthel Index), and cognitive status (Short Blessed Test) and mood disorders (Geriatric Depression Scale) were recorded. Mortality rates were assessed in hospital 3 and 12 months after discharge. Results Of the 4714 elderly patients hospitalized, 361 had a glycemia level ≥ 250 mg/dL at admission. Compared to subjects with lower glycemia level, patients with glycemia ≥ 250 mg/dL showed higher rates of male sex, smoke and class III obesity. These patients had a significantly lower Barthel Index (p = 0.0249), higher CIRS-SI and CIRS-CI scores (p = 0.0025 and p = 0.0013, respectively), and took more drugs. In-hospital mortality rate was 9.2% and 5.1% in subjects with glycemia ≥ 250 and p = 0.0010). Regression analysis showed a strong association between in-hospital death and glycemia ≥ 250 mg/dL (OR 2.07; [95% CI 1.34–3.19]), Barthel Index ≤ 40 (3.28[2.44–4.42]), CIRS-SI (1.87[1.27–2.77]), and male sex (1.54[1.16–2.03]). Conclusions The stronger predictors of in-hospital mortality for older patients admitted in general wards were glycemia level ≥ 250 mg/dL, Barthel Index ≤ 40, CIRS-SI, and male sex.

Among the other members of the adhesion molecules' family, α4β1 integrin, a heterodimeric receptor, plays a crucial role in inflammatory diseases, cancer development, metastasis and stem cell mobilization or retention. In many cases, its function in pathogenesis is not yet completely understood and investigations on ligand binding and related stabilization of active/inactive conformations still represent an important goal. For this reason, starting from the highlight of α4β1 functions in human pathologies, we report an overview of synthetic α4β1 integrin ligands under development as potential therapeutic agents. The small molecule library that we have selected represents a collection of lead compounds. These molecules are the object of future refinement in academic and industrial research, in order to achieve a fine tuning of α4β1 integrin regulation for the development of novel agents against pathologies still eluding an effective solution.

Potential evapotranspiration (ET0) is an indicator of great interest for water budget analysis and the agricultural sector. Therefore, the purpose of this study was to make the calculation reliable even if only the temperature data were present. In this research, the ET0 was initially calculated for a limited number of weather stations (12) using the Penman–Monteith method. In some cases, the simplified Penman–Monteith formula was adopted, while in others, as in the case of mountain weather stations, the complete formula was employed to consider the differences in vegetation, deduced from satellite surveys. Subsequently, the ET0 was calculated with the Hargreaves–Samani (HS) formula, calibrating the Hargreaves coefficient, through the spatialization of ET0, by the geostatistical method. The results showed a high reliability of the HS method in comparison with simplified PM (PM) method, and complete Penman–Monteith (cPM) method, with a minimum calibration of the empirical Hargreaves coefficient. In particular, a very good correlation between the results obtained in the mountain environment with the uncalibrated HS method and the cPM method was also observed in this area, while PM showed discordant and much higher results than ET0 compared with the other methods. It follows that this procedure allowed a more accurate estimate of potential evapotranspiration with a view to territory management, both in terms of water resources and the irrigation needs of the vegetation.

A correct landslide hazard assessment (LHA) is fundamental for any purpose of territorial planning. In Italy, the methods currently in use to achieve this objective alternate between those based on mainly qualitative (geomorphological) and quantitative (statistical–numerical) approaches. The present study contributes to the evaluation of the best procedure to be implemented for LHA, comparing the results obtained using two different approaches (geomorphological and numerical) in a territorial context characterized by conditioning and triggering factors, favorable to the instability of the slopes. The results obtained, although preliminary, evidence the respective limitations of the methods and demonstrate how a combined approach can certainly provide mutual advantages, by addressing the choice of the best numerical model through direct observations and surveys.

- Abstract
Background & Aims: Obeticholic acid (OCA) has recently been restricted in patients with primary biliary cholangitis (PBC) with “advanced cirrhosis” because of its narrow therapeutic index. We aimed to better define the predicting factors of hepatic serious adverse events (SAEs) and non-response in cirrhotic patients undergoing OCA therapy. Methods: Safety and efficacy of treatment were evaluated in a cohort of consecutive PBC cirrhotic patients started with OCA. OCA response was evaluated according to the Poise criteria. Risk factors for hepatic SAEs and non-response were reported as risk ratios (RR) with 95% confidence intervals (CIs). Results: One hundred PBC cirrhotics were included, 97 Child-Pugh class A and 3 class B. Thirty-one had oesophageal varices and 5 had a history of ascites. Thirty-three per cent and 32% of patients achieved a biochemical response at 6 and 12 months respectively. Male sex (adjusted-RR 1.75, 95%CI 1.42–2.12), INR (1.37, 1.00–1.87), Child-Pugh score (1.79, 1.28–2.50), MELD (1.17, 1.04–1.30) and bilirubin (1.83, 1.11–3.01) were independently associated with non-response to OCA. Twenty-two patients discontinued OCA within 12 months: 10 for pruritus, 9 for hepatic SAEs (5 for jaundice and/or ascitic decompensation; 4 for upper digestive bleeding). INR (adjusted-RR 1.91, 95%CI 1.10–3.36), lower albumin levels (0.18, 0.06–0.51), Child-Pugh score (2.43, 1.50–4.04), history of ascites (3.5, 1.85–6.5) and bilirubin (1.30, 1.05–1.56), were associated with hepatic SAEs. A total bilirubin≥1.4 mg/dl at baseline was the most accurate biochemical predictor of hepatic SAEs under OCA. Conclusions: An accurate baseline assessment is crucial to select cirrhotic patients who can benefit from OCA. Although OCA is effective in one third of cirrhotics, bilirubin level ≥1.4 mg/dl should discourage from its use.

- Abstract
Background & Aims: Machine learning (ML) provides new approaches for prognostication through the identification of novel subgroups of patients. We explored whether ML could support disease sub-phenotyping and risk stratification in primary biliary cholangitis (PBC). Methods: ML was applied to an international dataset of PBC patients. The dataset was split into a derivation cohort (training set) and a validation cohort (validation set), and key clinical features were analysed. The outcome was a composite of liver-related death or liver transplantation. ML and standard survival analysis were performed. Results: The training set was composed of 11,819 subjects, while the validation set was composed of 1,069 subjects. ML identified four clusters of patients characterized by different phenotypes and long-term prognosis. Cluster 1 (n = 3566) included patients with excellent prognosis, whereas Cluster 2 (n = 3966) consisted of individuals at worse prognosis differing from Cluster 1 only for albumin levels around the limit of normal. Cluster 3 (n = 2379) included young patients with florid cholestasis and Cluster 4 (n = 1908) comprised advanced cases. Further sub-analyses on the dynamics of albumin within the normal range revealed that ursodeoxycholic acid-induced increase of albumin >1.2 x lower limit of normal (LLN) is associated with improved transplant-free survival. Conclusions: Unsupervised ML identified four novel groups of PBC patients with different phenotypes and prognosis and highlighted subtle variations of albumin within the normal range. Therapy-induced increase of albumin >1.2 x LLN should be considered a treatment goal.

Chronic liver disease is characterized by several hematological derangements resulting in a complex and barely rebalanced haemostatic environment. Thrombocytopenia is the most common abnormality observed in these patients and recent advances have led to researchers focus the attention on the multifactorial origin of thrombocytopenia and on the key role of thrombopoietin (TPO) in its physiopathology. Severe thrombocytopenia (platelet count50000/μL) complicates the management of patients with chronic liver disease by increasing the potential risk of bleeding for invasive procedures, which may be therefore delayed or canceled even if lifesaving. In the very last years, the development of new drugs which exceed the limits of the current standard of care (platelet transfusions, either immediately before or during the procedure) paves the way to a new scenario in the management of this population of patients. Novel agents, such as the TPO-receptor agonists avatrombopag and lusutrombopag, have been developed in order to increase platelet production as an alternative to platelet transfusions. These agents have demonstrated a good profile in terms of efficacy and safety and will hopefully allow reducing limitations and risks associated with platelet transfusion, without any delay in scheduled interventions. Altogether, it is expected that patients with chronic liver disease will be able to face invasive procedures with one more string in their bow.

In many areas of the Umbria-Marche Apennines, evident traces of huge landslides have been recognized; these probably occurred in the Upper Pleistocene and are conditioned by the tectonic-structural setting of the involved Meso-Cenozoic formations, in a sector of the Sibillini Mountains (central Italy). The present work aimed to focus on a geomorphological hazard in the tectonic-structural setting of a complex area that is the basis of several gravitational occurrences in different types and mechanisms, but nonetheless with very considerable extension and total destabilized volume. An aerophoto-geological analysis and geomorphological survey allowed verification of how the main predisposing factor of these phenomena is connected with the presence in depth of an important tectonic-structural element: the plane of the Sibillini Mountains thrust, which brings the pre-evaporitic member of the Laga Formation in contact with the Cretaceous-Eocene limestone lithotypes (from the Maiolica to the Scaglia Rosata Formations) of the Umbria-Marche sedimentary sequence. Another important element for the mass movements activation is the presence of an important and vast water table and related aquifer, confined prevalently by the different structural elements and in particular by the thrust plane, which has acted and has continued to act, weakening the rocky masses and the overlaying terrains.

Background and aims: Programmed cell death 1/programmed cell death-ligand 1 (PD-1/PDL-1) axis has been reported to modulate liver inflammation and progression to hepatocellular carcinoma (HCC) in patients with nonalcoholic fatty liver disease (NAFLD). Here, we examined whether the PDCD1 variation is associated with NAFLD severity in individuals with liver biopsy. Methods: We examined the impact of PDCD1 gene variants on HCC, as robust severe liver disease phenotype in UK Biobank participants. The strongest genetic association with the rs13023138 G>C variation was subsequently tested for association with liver damage in 2889 individuals who underwent liver biopsy for suspected nonalcoholic steatohepatitis (NASH). Hepatic transcriptome was examined by RNA-Seq in a subset of NAFLD individuals (n = 121). Transcriptomic and deconvolution analyses were performed to identify biological pathways modulated by the risk allele. Results: The rs13023138 C>G showed the most robust association with HCC in UK Biobank (p = 5.28E-4, OR = 1.32, 95% CI [1.1, 1.5]). In the liver biopsy cohort, rs13023138 G allele was independently associated with severe steatosis (OR 1.17, 95% CI 1.02-1.34; p = .01), NASH (OR 1.22, 95% CI 1.09-1.37; p C allele was linked to higher hepatic representation of M1 macrophages, paralleled by upregulation of pathways related to inflammation and higher expression of CXCR6. Conclusions: The PDCD1 rs13023138 G allele was associated with HCC development in the general population and with liver disease severity in patients at high risk of NASH.
