Abstract

Backround: Use of indwelling urinary catheter (IUC) in older adults has negative consequences, including delirium. Aim: This analysis, from the "Delirium Day 2015", a nationwide multicenter prevalence study, aim to evaluate the association of IUC with delirium in hospitalized and Nursing Homes (NHs) patients. Methods: Patients underwent a comprehensive geriatric assessment, including the presence of IUC; inclusion criteria were age > 65 years, being Italian speaker and providing informed consent; exclusion criteria were coma, aphasia, end-of-life status. Delirium was assessed using the 4AT test (score >= 4: possible delirium; scores 1-3: possible cognitive impairment). Results: Among 1867 hospitalized patients (mean age 82.0 ± 7.5 years, 58% female), 539 (28.9%) had IUC, 429 (22.9%) delirium and 675 (36.1%) cognitive impairment. IUC was significantly associated with cognitive impairment (OR 1.60, 95% CI 1.19-2.16) and delirium (2.45, 95% CI 1.73-3.47), this latter being significant also in the subset of patients without dementia (OR 2.28, 95% CI 1.52-3.43). Inattention and impaired alertness were also independently associated with IUC. Among 1454 NHs residents (mean age 84.4 ± 7.4 years, 70.% female), 63 (4.3%) had IUC, 535 (36.8%) a 4AT score >= 4, and 653 (44.9%) a 4AT score 1-3. The multivariate logistic regression analysis did not show a significant association between 4AT test or its specific items with IUC, neither in the subset of patients without dementia. Discussion: We confirmed a significant association between IUC and delirium in hospitalized patients but not in NHs residents. Conclusion: Environmental and clinical factors of acute setting might contribute to IUC-associated delirium occurrence.

Abstract

HPC-TRES | The Italian National Institute of Oceanography and Applied Geophysics - OGS and CINECA drew up an agreement to implement a training program focussed on High Performance Computing (HPC) applications for Earth Sciences, the “HPC Training and Research for Earth Sciences” (HPC-TRES). The program is co-sponsored by the Minister of University and Research (MUR) under its extraordinary contribute for the Italian participation to activities related to the international infrastructure PRACE – The Partnership for Advanced Computing in Europe. The major objectives of the program are: capacity building, enhancement of human resources, and advanced training in the fields of Earth System modelling (atmosphere, hydrosphere, lithosphere and biosphere) and numerical models, the latter considered as a strategic cross cutting element for modelling activities. These objectives will be pursued through the use of national and European HPC infrastructures and services in the framework of PRACE, the optimization of algorithms and numerical codes, the optimal management of “Earth Sciences Big Data”, and the graphical visualization techniques for multidisciplinary applications in the Earth Sciences, also in the frame of the “Blue Growth” strategy. Therefore, HPC-TRES agreement establishes, sponsor and oversees training and research awards (i.e. contributes for training and research activities set up according to current regulations that regulate scholarships – to be also used for masters and specialized courses in HPC -, research grants and PhDs) that will support the research lines scientific plan of the HPC-TRES program. Some Italian research groups and institutions (INGV-Pisa, CNR/ISAC, CNR/IGG, CMCC, ICTP ESP), where the external members of the HPC-TRES Steering Committee lead their research activities and personally contributed to define the scientific plan of the HPC-TRES program, have already subscribed the HPC-TRES initiative, offering logistic support and hospitality to the winners of the training and research awards sponsored by OGS. In 2021, OGS, CINECA, CNR, INGV, CMCC, POLITO-DIATI, and ICTP signed the constitution of the Joint Research Unit HPC-TRES (JRU HPC-TRES), to further support the action of the HPC-TRES program. Abstract

Biochemical analysis of the soluble hydrogenase from the thermophilic bacterium Acetomicrobium flavidum showed that the enzyme is an alpha-2-beta-2 tetramer. The A. flavidum hydrogenase enzyme seems to be a typical (Ni-Fe) enzyme. Large amounts of hydrogenase were expressed in Escherichia coli in soluble form but were not active. Analysis of the recombinant large subunit indicated that it was not post-translationally processed at its C-terminus.

Background: Pain is a marker of bodily status, that despite being aversive under most conditions, may also be perceived as a positive experience. However, how bodily states represent, define, and interpret pain signals, and how these processes might be reflected in common language, remains unclear. Methods: Qualitative and quantitative methods were used to explore the relationship between bodily awareness, pain reactions, and descriptions. A list of pain-related terms was generated from open-ended interviews with persons with spinal cord injury (SCI), and 138 participants (persons with SCI, health professionals, and a healthy control group) rated each descriptor as representative of pain on a gradated scale. A lexical decision task was used to test the strength of the automatic association of the word "pain" with positive and negative concepts. The behavioral results were related to body awareness, experience of pain, and exposure to pain, by comparing the three groups. Results: Higher positive and lower negative pain descriptors, as well as slower response times when categorizing pain as an unpleasant experience were found in the SCI group. The effect was not modulated by either the time since the injury or the present pain intensity, but it was linked to the level of subjective bodily awareness. Compared with the SCI group, health experts and non-experts both associated more quickly the word "pain" and unpleasant in the lexical decision task. However, while health professionals attributed positive linguistic qualities to pain, pain was exclusively associated with negative descriptors in healthy controls group. Conclusions: These findings are discussed in terms of their theoretical and clinical implications. An awareness of bodily signals prominently affects both the sensory and linguistic responses in persons with SCI. Pain should be evaluated more broadly to understand and, by extension, to manage, experiences beyond its adverse side.

Abstract

Objectives: The aim of this study was to compare the durabilities of efavirenz (EFV) and rilpivirine (RPV) in combination with tenofovir/emtricitabine (TDF/FTC) in first-line regimens. Methods: A multicentre prospective and observational study was carried out. We included all patients participating in the Italian Cohort Naive Antiretrovirals (ICONA) Foundation Study who started first-line combination antiretroviral therapy (cART) with TDF/FTC in combination with RPV or EFV, with a baseline viral load < 100 000 HIV-1 RNA copies/mL. Survival analyses using Kaplan–Meier (KM) curves and Cox regression with time-fixed covariates at baseline were employed. Results: Overall, 1490 ART-naïve patients were included in the study, of whom 704 were initiating their first cART with EFV and 786 with RPV. Patients treated with EFV, compared with those on RPV, were older [median 36 (interquartile range (IQR) 30–43) years vs. 33 (IQR 27–39) years, respectively; P < 0.001], were more frequently at Centers for Disease Control and Prevention (CDC) stage C (3.1% vs. 1.4%, respectively; P = 0.024), and had a lower median baseline CD4 count [340 (IQR 257–421) cells/μL vs. 447 (IQR 347–580) cells/μL, respectively; P < 0.001] and a higher median viral load [4.38 (IQR 3.92–4.74) log10 copies/mL vs. 4.23 (IQR 3.81–4.59) log10 copies/mL, respectively], (P = 0.004). A total of 343 patients discontinued at least one drug of those included in the first cART regimen, more often EFV (26%) than RPV (13%), by 2 years (P < 0.0001). After adjustment, patients treated with EFV were more likely to discontinue at least one drug for any cause [relative hazard (RH) 4.09; 95% confidence interval (CI) 2.89–5.80], for toxicity (RH 2.23; 95% CI 1.05–4.73) for intolerance (RH 5.17; 95% CI 2.66–10.07) and for proactive switch (RH 10.96; 95% CI 3.17–37.87) than those starting RPV. Conclusions: In our nonrandomized comparison, RPV was better tolerated, less toxic and showed longer durability than EFV, without a significant difference in rates of discontinuation because of failures.

Abstract

Background Patlak’s graphical analysis can provide tracer net influx constant (Ki) with limitation of assuming irreversible tracer trapping, that is, release rate constant (kb) set to zero. We compared linear Patlak’s analysis to non-linear three-compartment three-parameter kinetic model analysis (3P-KMA) providing Ki, kb, and fraction of free 18F-FDG in blood and interstitial volume (Vb). Methods Dynamic PET data of 21 lung cancer patients were retrospectively analyzed, yielding for each patient an 18F-FDG input function (IF) and a tissue time-activity curve. The former was fitted with a three-exponentially decreasing function, and the latter was fitted with an analytical formula involving the fitted IF data (11 data points, ranging 7.5–57.5 min post-injection). Bland-Altman analysis was used for Ki comparison between Patlak’s analysis and 3P-KMA. Additionally, a three-compartment five-parameter KMA (5P-KMA) was implemented for comparison with Patlak’s analysis and 3P-KMA. Results We found that 3P-KMA Ki was significantly greater than Patlak’s Ki over the whole patient series, + 6.0% on average, with limits of agreement of ± 17.1% (95% confidence). Abstract

During the 2014-2015 mutual events season, the Institut de Mécanique Céleste et de Calcul des Éphémérides (IMCCE), Paris, France, and the Sternberg Astronomical Institute (SAI), Moscow, Russia, led an international observation campaign to record ground-based photometric observations of Galilean moon mutual occultations and eclipses.We focused on processing the complete photometric observations data base to compute new accurate astrometric positions. We used our method to derive astrometric positions from the light curves of the events. We developed an accurate photometric model of mutual occultations and eclipses, while correcting for the satellite albedos, Hapke's light scattering law, the phase effect, and the limb darkening. We processed 609 light curves, and we compared the observed positions of the satellites with the theoretical positions from IMCCE NOE-5-2010-GAL satellite ephemerides and INPOP13c planetary ephemeris. The standard deviation after fitting the light curve in equatorial positions is ±24 mas, or 75 km at Jupiter. The rms (O-C) in equatorial positions is ±50 mas, or 150 km at Jupiter. © 2017 The Author(s).
