Antecedentes. La postura en sedente es un elemento esencial en la valoración global fisioterapéutica, el análisis en el campo de la ergonomía y la prescripción de silla de ruedas. De los métodos existentes, ninguno ofrece la combinación de ser comprensivo con el uso de técnicas de medición fáciles de implementar y económicas. Objetivo. Adaptar un método de medición tri-planar de la postura en bípedo a sedente. Materiales y métodos. Unidades de análisis incluyen el método de medición tri-planar de la postura en bípedo GOSS y documentos de referencia primaria y secundaria. Resultados. El método de medición GOSS-Petrocci identifica los parámetros (el sistema de referencia, los segmentos corporales, los puntos de referencia corporal) y los mecanismos (los instrumentos de medición y el protocolo de medición) de la postura en sedente. Conclusión. El método GOSS-Petrocci se utiliza para medir la postura en sedente en tres planos de manera comprensivo, utilizando fotometría lo cual permite entender la postura de acuerdo a la posición de mayor estabilidad/postura ideal y permite una implementación fácil en el ámbito clínico. Background. Seated posture is an essential element in global physical therapy evaluations, in analysis in the field of ergonomics and for the prescription of wheelchairs. Existing methods of seated posture do not offer the combination of measuring posture comprehensively in an easy and economic way. Objective. To adapt a tri-planar standing postural measurement method to sitting. Materials and methods. Units of analysis include the tri-planar measurement method in standing, GOSS method, and primary and secondary reference documents. Results. The GOSS-Petrocci method, which include the identification of the parameters (the reference system, the body segments, body reference points) and the mechanisms (the measurement instruments and the measurement protocol) of seated posture. Conclusion. The GOSS-Petrocci method is used to measure seated posture in three planes comprehensively, using photometry, which permits the understanding of posture according to the position of greatest stability/ideal posture and can be easily implemented in a clinical setting.

The aim was to analyze the prevalence of latent tuberculosis infection and associated risk factors in healthcare personnel at the Hospital de Tortosa Verge de la Cinta (Tarragona, Spain). This was a cross-sectional study of 398 workers at this hospital who underwent tuberculin skin testing for latent tuberculosis infection (LTBI) between 2001 and 2012.We also analyzed the relationship between LTBI and age, sex, job and work area according to their risk of exposure to tuberculosis(high, low, uncertain). The total prevalence of LTBI in our sample was 11.1% (95%CI 8.3%-14.5%). LBTI was associated with age and work area. Multivariate analysis showed that the risk of LTBI increased by 6.4% per 1 year increase in age. The prevalence of LTBI in this population approximates that of the general population in Spain.El objetivo del trabajo ha sido evaluar la presencia de infección tuberculosa latente en el personal sanitario del Hospital de Tortosa Verge de la Cinta (HTVC). Para ello se analizaron las historias clínicas laborales de 398 trabajadores de dicho hospital a los que se había realizado la prueba de tuberculina durante el periodo 2001-2012. La presencia de infección tuberculosa latente (ITL) se analizó en relación a las variables edad, sexo, categoría profesional, personal de áreas de trabajo de alto y bajo riesgo de exposición a la tuberculosis y personal de riesgo no definido. La prevalencia total de ITL en el grupo estudiado fue del 11,1% (IC 95%: 8,3%-14,5%), estando dicha prevalencia asociada a la edad y al área de trabajo. El análisis multivariable mostró que el riesgo de sufrir ITL se incrementa un 6,4% por cada año de edad. La prevalencia de ITL en el centro de estudio era similar a la de la población profesionalmente no expuesta de España.

Los trastornos cutáneos son derivados, en la mayoría de las ocasiones, de agresiones continuas por elementos externos que tiene como respuesta reacciones como la pérdida de lípidos epidérmicos por efecto de la humedad, cambios en el pH cutáneo (alcalinización) y aumento del coeficiente de fricción (denudación) junto al desequilibrio de la flora saprofita de la piel, que se traducen en procesos irritativos y en un aumento de la infecciones fúngicas y bacterianas en la zona expuesta a humedad. Dada la importancia de evaluar la calidad de vida de los pacientes con dermatitis que son tratados con productos barrera no irritantes (PBNI) se ha desarrollado un estudio multicéntrico, con el objetivo de evaluar los aspectos relacionados con la mejora en la calidad de cuidados a pacientes tratados con Cavilon® que presentaban dermatitis por incontinencia mediante la escala ESCLI. Los pacientes incluidos en el estudio fueron 25. Los resultados muestran una mejora importante de los aspectos evaluados mediante la escala ESCLI sobre la evolución de la dermatitis. Esta escala valora aspectos como el área afectada por el enrojecimiento y la severidad, presentan una reversión del 47% de los casos. La denudación de la piel en la zona del pañal desaparece en el 56% de los casos y la severidad de la denudación que presenta una mejoría del 47%. La valoración del dolor en la zona de incontinencia se reduce en un 76% en la zona de incontinencia y en un 73% al realizar la limpieza de la zona. Las conclusiones del presente estudio ponen de manifiesto que la aplicación de productos formadores de una película barrera no irritante (PBNI) en los pacientes que presentan dermatitis por incontinencia representa una alternativa a los tratamientos convencionales como son las cremas o ungüentos de óxido de cinc o pastas al agua. La reversión y reducción de la dermatitis es un aspecto que presenta una mejoría importante. Los resultados obtenidos que podemos relacionar con la calidad de vida o más concretamente con la mejora del dolor son destacables. 161. Annexin A2 is critical for embryo adhesiveness to the human endometrium by RhoA activation through F-actin regulation

- Abstract
Annexin A2 (ANXA2) is present in vivo in the mid- and late-secretory endometria and is mainly localized in the luminal epithelium. Our aim was to evaluate its function in regulating the human implantation process. With an in vitro adhesion model, constructed to evaluate how the mouse embryo and JEG-3 spheroids attach to human endometrial epithelial cells, we demonstrated that ANXA2 inhibition significantly diminishes embryo adhesiveness. ANXA2 is also implicated in endometrial epithelial cell migration and trophoblast outgrowth. ANXA2 was seen to be linked to the RhoA/ROCK pathway and to regulate cell adhesion. We noted that ANXA2 inhibition significantly reduces active RhoA, although RhoA inactivation does not alter the ANXA2 levels. RhoA inactivation and ROCK inhibition also moderate embryo adhesiveness to endometrial epithelial cells. We corroborated that the induction of constitutively active RhoA partially reverses the effects of ANXA2 inhibition on endometrial adhesiveness. These molecules colocalize on the plasma membrane of endometrial epithelial cells, and a large proportion of ANXA2 and RhoA are colocalized in the F-actin networks. The functional effects of ANXA2 inhibition and RhoA/ROCK inactivation are associated with significant alterations in F-actin organization and its depolymerization. ANXA2 may act upstream of the RhoA/ROCK pathway by regulating F-actin remodeling and is a key factor in human endometrial adhesiveness.

Este artículo presenta la degradación observada en un generador de x-Si 7 kW de potencia tras 16 años de exposición en la terraza del Instituto de Energía Solar de la Universidad Politécnica de Madrid. La caída de la potencia pico ha sido del 9% durante dicho periodo o, lo que es equivalente, una degradación anual del 0,56%; mientras que la desviación típica se ha mantenido básicamente constante. Los principales defectos visuales observados han sido delaminaciones en la superficie PET/PVF de la cara posterior de los módulos y roturas en las cajas de conexión y en la unión entre marco y vidrio. Todos los módulos analizados han pasado con suficiencia las pruebas de aislamiento eléctrico dispuestas en la norma IEC 61215, tanto en seco como en mojado.

pH and chlorinity content of estuarine water and behavior in the concentration of some heavy metals characteristics of acid mine drainage in the suspended matter and dissolved phase have allowed the identification of two mixing process in the Odiel-Tinto rivers estuary: a salt-induced mixing typical of marine estuaries and an acid neutralization process as a result of the mixture of acid fluvial water coming from the drainage basins, which are affected by acid mine drainage (AMD). Thus, the estuary is divided into three zones: (1) zone with acid water and with precipitation of sulphate phases during the beginning of the process of neutralization in the absence of salt-induced mixing, (2) zone with pH values between 4.5 and 7 where a removal of heavy metals in their dissolved phase is produced by coexistence of both mixture processes, and (3) zone where water is slightly basic and the salt-induced mixing produces a redissolution of some elements. Cardiotoxicity is a well recognized side effect of anthracyclines (doxorubicin and epirubicin) or antracenadiones (mitoxantrone) at cumulative or high doses. However the side effects have not been evaluated in adults with Hodgkin's disease who received therapeutic doses of these drugs. We analyzed the cardiac function studying the left ventricular ejection fraction (LVEF) at rest in 136 patients with Hodgkin's disease treated with doxorubicin, epirubicin or mitoxantrone used in combination with vinblastine, bleomycin and decarbazine. No other risk factors, such as radiation therapy to the mediastinum, were considered. The follow-up is 5 to 8 years for patients in complete remission. Forty-five patients received doxorubicin (from 325 to 685 mg/m2, median 475 mg/m2), 51 patients received epirubicin (from 310 to 610 mg/m2, median 510 mg/m2) and 40 patients were treated with mitoxantrone (from 70 to 165, median 125 mg/m2). The median time between the end of treatment and the evaluation was 6.7 years. Thirty seven percent of the patients (similar rates in the three groups) showed abnormalities in the LVEF with decreased rates independent of the drug dosage. These were compared with two control groups, 46 patients treated with the MOPP combination (mechlorethamine, vincristine, prednisone and procarbazine) or LOPP (chlorambucil, for mechlorethamine) and 35 healthy volunteers. We believe that the use of anthracyclines or antracenadione will produce late cardiac effects in a fraction of patients independently of the doses used and that the indications for these drugs be carefully monitoring so as to evaluate the development of late side effects.

Lipid rafts are membrane microdomains involved in cell signalling and characterized by their high contents of sphingolipids, cholesterol and saturated fatty acids. In this study, we have purified lipid rafts of human frontal brain cortex from normal and Alzheimer’s disease (AD) cell membranes and analyzed their lipid composition. Compared to non-AD brains, lipid rafts from AD brains displayed low levels of n-3 long chain highly unsaturated fatty acids (HUFA) specially 22:6n-3 (docosahexaenoic acid) and n-9 monoenes (specially 18:1n-9, oleic acid). Changes in lipid structure of AD lipid rafts also involved significant reductions in the unsaturation and peroxidability indexes as well as increased unsaturated/cholesterol unsaturated/sphingomyelin and saturates/n-3 HUFA ratios. These results indicate that the mechanisms preserving lipid raft order and viscosity in normal frontal cortex are disrupted in AD brains, which partially might explain the abnormal intracellular signalling mechanisms observed during AD. - Abstract
While TRAIL is a promising anticancer agent due to its ability to selectively induce apoptosis in neoplastic cells, many tumors, including pancreatic ductal adenocarcinoma (PDA), display intrinsic resistance, highlighting the need for TRAIL-sensitizing agents. Here we report that TRAIL-induced apoptosis in PDA cell lines is enhanced by pharmacological inhibition of glycogen synthase kinase-3 (GSK-3) or by shRNA-mediated depletion of either GSK-3 alpha or GSK-3 beta. In contrast, depletion of GSK-3 beta, but not GSK-3 alpha, sensitized PDA cell lines to TNF alpha-induced cell death. Further experiments demonstrated that TNF alpha-stimulated I kappa B alpha phosphorylation and degradation as well as p65 nuclear translocation were normal in GSK-3 beta-deficient MEFs. Nonetheless, inhibition of GSK-3 beta function in MEFs or PDA cell lines impaired the expression of the NF-kappa B target genes Bcl-xL and cIAP2, but not I kappa B alpha. Significantly, the expression of Bcl-xL and cIAP2 could be reestablished by expression of GSK-3 beta targeted to the nucleus but not GSK-3 beta targeted to the cytoplasm, suggesting that GSK-3 beta regulates NF-kappa B function within the nucleus. Consistent with this notion, chromatin immunoprecipitation demonstrated that GSK-3 inhibition resulted in either decreased p65 binding to the promoter of BIR3, which encodes cIAP2, or increased p50 binding as well as recruitment of SIRT1 and HDAC3 to the promoter of BCL2L1, which encodes Bcl-xL. Importantly, depletion of Bcl-xL but not cIAP2, mimicked the sensitizing effect of GSK-3 inhibition on TRAIL-induced apoptosis, whereas Bcl-xL overexpression ameliorated the sensitization by GSK-3 inhibition. These results not only suggest that GSK-3 beta overexpression and nuclear localization contribute to TNF alpha and TRAIL resistance via anti-apoptotic NF-kappa B genes such as Bcl-xL, but also provide a rationale for further exploration of GSK-3 inhibitors combined with TRAIL for the treatment of PD

Recently, Spain witnessed the emergence and widespread of the concept of ‘obstetric violence’. Some legal texts appeared in Latin-America (Venezuela 2007, Mexico 2007-2014, Argentina 2009), activists and scholars define the concept of 'obstetric violence' as a kind of gender-based violence exercised by healthcare personnel on women and fetuses/children during pregnancy and childbirth. This takes place in form of dehumanized treatments, medicalization and pathologizing of women’s reproductive processes through the appropriation of their bodies, reproductive capacities and sexuality. Harmful practices that have been reported in case files include: the denial of information about the procedures employed during the labor process, humiliations and miscarrying attitudes, excessive rates of cesarean birth, routine medical practices that do not have proven advantages for women and fetuses/children's welfare ( enemas, episiotomies, sedatives, supine position as mandatory, or practices that have been proven risky as the Kristeller‘s method).. The reports made under the label of obstetric violence generally express severe misgiving about the medicalizing approach, professional authoritarianism and sexist attitudes towards pregnant women within the current healthcare system. Through the case of Spain this research aims to understand what are the possibilities of the concept of 'obstetric violence' to raise awareness about the patriarchal, medicalizing and authoritarian practices that seem to take place within the field of obstetrics, since the concept has begun to be used within different movements related to respected childbirth claims in the country. The research departs from the work done by the biopolitical and feminist approaches to healthcare, the issues addressed by women's health movements, the sexual and reproductive rights advocacy and respected childbirth's statements in the Western context, with special emphasis in the context of Spain.

Los tumores pancreáticos de Frantz son una patología infrecuente, aún de patogénesis incierta, con una población blanco predominantemente de mujeres jóvenes. Su grado de malignidad es bajo y con la resección completa se tiene muy buena sobrevida. El propósito del presente artículo es analizar de forma retrospectiva dos casos que por sus dificultades diagnósticas aportan mucho acerca de la aproximación a esta patología, y de su importancia en el diagnóstico diferencial de las masas abdominales en niños, que de manejarse con los procedimientos quirúrgicos adecuados, permite a los pacientes una evolución y un pronóstico favorables. Frantz pancreatic tumors are an infrequent pathology, histogenesis remain controversial, they occur predominantly in young women and because of their low-grade malignant potential, complete resection means good survival. The aim of this review is to present a retrospective analysis of two cases, which, because of their diagnostic difficulties can teach a lot in the evaluation process of this pathology and of its importance in the abdominal tumors differential diagnosis in children, which, when well managed with the adequate surgical method, allows the patients an excellent recovery and prognosis. Os tumors dos pancreáticos de Frantz é um pathology infrequënte, destilador do patogénesis incerto, com uma população branca predominantly de mulheres novas. Seu grau de malignidad é baixo e com resección completo o sobrelife muito bom é tido. A intenção do artigo atual é analisar dos casos retrospective do formulário dois que por suas dificuldades que você diagnostica contribua muito sobre a aproximação a este pathology, aquele ao punho próprio com os procedimentos cirúrgicos adaptados, permita aos pacientes uma evolução favorável e um prognosis.

Objetivos: a) Conocer los hábitos dietéticos y de ejercicio físico de los alumnos de la EU de Fisioterapia de Soria. b) Concienciar a nuestros alumnos, como futuros profesionales de la salud y agentes de Educación Sanitaria, para que potencien y promuevan una alimentación adecuada entre la población general y entre sus pacientes. Metodología: Encuesta a los alumnos de la Escuela Universitaria de Fisioterapia de Soria sobre el consumo de alimentos y ejercicio físico realizado a lo largo de siete días (n = 131; 93 mujeres y 38 varones). Resultados: Tomando como referencia la nueva Pirámide Nutricional de la Sociedad Española de Nutrición (SEN), en cuanto a consumo de grupos de alimentos como carne, pescado, leche y derivados y práctica de ejercicio físico, hemos detectado que un porcentaje alto de nuestros alumnos consumen estos alimentos y realizan ejercicio físico de acuerdo a las recomendaciones hechas por la SEN. Para el resto de los grupos de alimentos, pasta, pan y cereales, verduras y hortalizas, frutas, legumbres, y aceite de oliva, su consumo dista mucho de cumplir las recomendaciones. El consumo de comida rápida se sitúa en una media de una vez por semana superando la recomendación de hacerlo de forma esporádica o alguna vez al mes. Conclusiones: En función de los resultados obtenidos, parece conveniente realizar algún tipo de intervención educativa entre nuestros alumnos para que ellos sean capaces de promover una alimentación saludable. De esta forma podrán prevenir enfermedades relacionadas con malos hábitos dietéticos y transmitir estos conocimientos a la población. No se puede hacer promoción de algo que no se conoce o no se practica. Objective: a) To know the eating and exercising habits of undergraduate Physical Therapy students; b) To promote awareness among these future health professionals and educators of the need to foster adequate eating habits among patients and the general population. Methods: A dietary and physical activity questionnaire, regarding a seven-day period, distributed among students of the School of Physical Therapy (University of Valladolid) in the city of Soria, Spain (n = 131; sex (m/f): 38/93). Results: Results show that a high percentage of the subjects follow the recommendations of the new Nutritional Pyramid of the Spanish Nutrition Society (SEN) regarding intake of meat, fish, milk, dairy products and exercise habits. This is not the case, however, for products such as pasta, bread and cereals, vegetables, fruits, legumes and olive oil. Furthermore, fast food is taken, on average, once a week despite the recommended sporadic monthly intakes. Conclusions: According to results, formative programmes should be carried out to enable these students to encourage healthy diets. Thus, diseases related to nonhealthy eating habits could be prevented, and information on the issue could be spread among the population.

Grifola frondosa es un hongo Basidiomycete comestible y medicinal que produce polisacáridos tipo glucanos (β-1,6 y β-1,3) con actividad antitumoral e inmunomoduladora. Para el presente trabajo, el objetivo fue evaluar la incidencia de diferentes fuentes de carbono (FC) en la producción de biomasa y exopolisacáridos (EPS), bajo condiciones de cultivo sumergido, utilizando un diseño estadístico factorial con dos factores: fuente de carbono (FC(1-7)) y medio (MB y MS). La máxima producción de biomasa micelial (21,10 ± 0,82) g/l y la máxima producción de EPS (6,53 ± 0,14) g/l se logró utilizando el medio MB suplementado con FC4. Además de lo anterior, se logró desarrollar un medio de cultivo que cumple con el objetivo de disminuir costos, de Col $18.455 a $656 por litro de medio, produciendo (14,14 ± 0,82) g/l de biomasa y (5,16 ± 0,14) g/l de EPS, utilizando el medio MS suplementando con FC4. Estos resultados abren nuevas alternativas y posibilidades en nuestro país para el desarrollo de productos a partir de esta especie con costos que estén al alcance de la población. Grifola frondosa is a Basidiomycete fungus edible and medicinal, it produces polysaccharides such a glucans (β-1,6 and β-1,3) with anti-tumor and inmuno-stimulating activity. For this work, the aim was evaluate the incidence of different carbon sources (FC) in biomass and exo-polymer (EPS) production, under submerged culture conditions, by using a factorial statistical design with two factors: carbon source (FC(1-7)) and medium (MB and MS). The maximal yield of mycelial biomass (21,10 ± 0,82) g/l and the maximal yield in EPS (6,53 ± 0,14) g/l was achieved by using the MB medium supplemented with FC4. Besides, it was possible to develop a medium that fulfills the objective of reducing costs from $18.455 to $656 Colombian pesos for each liter of medium used, which produces (14,14 ± 0,82) g/l of biomass and (5,16 ± 0,14) g/l of EPS, using the MS medium supplemented with FC4. This result opens new alternatives and possibilities in our country for developing products of this species with cost that can be reached by population.

Objetivos: Analizar mediante ultrasonografía cuantitativa de calcáneo (QUS) la prevalencia de osteoporosis en ancianos institucionalizados, para contribuir al establecimiento de un punto de corte en unidades de T-score en el diagnóstico de osteoporosis, lo que permitiría avanzar en la prevención de fracturas. Material y métodos: Se analizó mediante QUS a la población (n = 171) por sexos y por grupos de edad (tres décadas entre los 70 y los 90 años). Resultados: Para todas las variables densitométricas, tanto de forma global como en los grupos de edad más jóvenes, las mujeres mostraron valores significativamente más bajos que los varones (p < 0,001). En el grupo de mayor edad sólo se aprecian diferencias significativas en la T-score y en el BUA (p = 0,039 y p = 0,025, respectivamente). Bajo cualquiera de los criterios aplicados y en todos los grupos de edad, la prevalencia en las mujeres es superior a la del varón. Aplicando los criterios de la OMS con QUS, la prevalencia de osteoporosis en la población total se acerca a la establecida mediante DEXA con esos mismos criterios. Conclusión: La técnica QUS puede ser muy útil para valorar la evolución de la masa ósea con la edad y para el diagnóstico y seguimiento de la osteoporosis. En nuestra población anciana, los criterios de la OMS para la técnica DXA, son también los más adecuados para el empleo de la técnica QUS. Objectives: To analyze using quantitative ultrasound of calcaneus (QUS) the prevalence of osteoporosis in institutionalized elderly people, in order to contribute to state reliable criteria (T-score units) for the diagnosis of osteoporosis which allow advances in bone fracture prevention. Material and methods: Elderly people (n=171) were divided into separate groups according to sex and age criteria (three decades, from 70 to 90 years) and analyzed using QUS. Results: Both globally and in the younger groups, women showed significantly lower values than men in all densitometry variables (p < 0.001). In the oldest group only T-score and BUA showed statistical differences (p = 0.039 y p = 0.025, respectively). The prevalence of osteoporosis in women was higher than in men whichever criteria were applied in all age groups. Applying the WHO criteria with QUS, the prevalence of osteoporosis in global population is close to that stated by DEXA using the same criteria. Conclusions: QUS could be useful to assess the bone mass evolution with age and for the diagnosis and monitoring of osteoporosis. In our elderly population, the WHO criteria for DEXA, are also the most suitable ones for QUS utilization.

Background. Seated posture is an essential element in global physical therapy evaluations, in analysis in the field of ergonomics and for the prescription of wheelchairs. Existing methods of seated posture do not offer the combination of measuring posture comprehensively in an easy and economic way.Objective. To adapt a tri-planar standing postural measurement method to sitting.Materials and methods. Units of analysis include the tri-planar measurement method in standing, GOSS method, and primary and secondary reference documents.Results. The GOSS-Petrocci method, which include the identification of the parameters (the reference system, the body segments, body reference points) and the mechanisms (the measurement instruments and the measurement protocol) of seated posture.Conclusion. The GOSS-Petrocci method is used to measure seated posture in three planes comprehensively, using photometry, which permits the understanding of posture according to the position of greatest stability/ideal posture and can be easily implemented in a clinical setting., Antecedentes. La postura en sedente es un elemento esencial en la valoración global fisioterapéutica, el análisis en el campo de la ergonomía y la prescripción de silla de ruedas. De los métodos existentes, ninguno ofrece la combinación de ser comprensivo con el uso de técnicas de medición fáciles de implementar y económicas.Objetivo. Adaptar un método de medición tri-planar de la postura en bípedo a sedente.Materiales y métodos. Unidades de análisis incluyen el método de medición tri-planar de la postura en bípedo GOSS y documentos de referencia primaria y secundaria.Resultados. El método de medición GOSS-Petrocci identifica los parámetros (el sistema de referencia, los segmentos corporales, los puntos de referencia corporal) y los mecanismos (los instrumentos de medición y el protocolo de medición) de la postura en sedente.Conclusión. El método GOSS-Petrocci se utiliza para medir la postura en sedente en tres planos de manera comprensivo, utilizando fotometría lo cual permite entender la postura de acuerdo a la posición de mayor estabilidad/postura ideal y permite una implementación fácil en el ámbito clínico.

- Abstract
Hydropower is a potential large source of electricity supply in Wales. The Upper River Severn in Mid Wales is a typical stream where a high head hydropower scheme could be developed and the river system at Plynlimon has some of the longest records for weather and flow in Wales. A micro-hydropower potential of 99 kW is demonstrated at Plynlimon and the potential impacts of climate change are simulated to assess the effects on flows and power outputs of such schemes under climate uncertainty. Based on UK Climate Projections 2009 (UKCP09) projections, the impacts of climate change are to significantly decrease both the stream flows and energy production during summer months but to increase flows and power production in the winter, with a net tendency to cancel out over the course of a full year. A methodology for assessing impacts of climate change on hydropower is established, which could be applied more widely to other potential hydropower sites such as lowland rivers or high base flow rivers in other parts of the UK. This will be useful for developers, water companies and environmental agencies to assess hydropower potential, economic viability and environmental impacts of micro-hydropower, under future climate change.

Since description of the expansion of the number of CTG trinucleotides on the long arm of chromosome 19 in the 19 q 13.2-13.3 interval as being responsible for myotonic dystrophy (DM), many studies have established a direct relationship between the size of the expansion and the severity of the manifestations.To evaluate the clinico-genetic correlation in a population with DM in Eastern Andalucia and to establish the predictive value of the increase in number of CTG repetitions with regard to clinical gravity.A transverse study of persons from families known to have members with DM, classified with regard to the clinical gravity of their illness. Diagnosis was by means of clinical neurological, ophthalmological and neurophysiological examination and subsequently by genetico-molecular study, Southern blot, PCR, oligonucleotide CTC hybridization and Northern blot.Genetic studies confirmed the previous clinical diagnosis of DM in 78 persons, of the 145 studied, who came from 32 families. The average size of the mutation was 1-5 kb. There was close correlation between the size of the expansion and the clinical condition. Logistic repression studies permitted adequate classification in function of the size of the expansion in 87.23% of the cases of clinically relevant illness and in 90% of the cases in which the condition was not clinically relevant.There is a strong association between the clinical features of DM and the magnitude of the mutation. The size of the expansion has considerable predictive value in prognosis of the disorder. This may be useful when making decisions during prenatal diagnosis.

The Spanish PETHEMA group designed a protocol for newly diagnosed PML/RARalpha-positive acute promyelocytic leukemia (APL) in which induction and consolidation followed the original AIDA regimen, except for the omission of cytarabine and etoposide from consolidation. Induction consisted of 45 mg/m(2) all-trans retinoic acid (ATRA) daily until complete remission (CR) and 12 mg/m(2) idarubicin on days 2, 4, 6, and 8. Patients in CR received 3 monthly chemotherapy courses: idarubicin 5 mg/m(2)/d x 4 (course no. 1), mitoxantrone 10 mg/m(2)/d x 5 (course no. 2), and idarubicin 12 mg/m(2)/d x 1 (course no. 3). Maintenance therapy consisted of 90 mg/m(2)/d mercaptopurine orally, 15 mg/m(2)/wk methotrexate intramuscularly, and, intermittently, 45 mg/m(2)/d ATRA for 15 days every 3 months. Between November 1996 and December 1998, 123 patients with newly diagnosed PML/RARalpha-positive APL from 39 centers were enrolled. A total of 109 patients achieved CR (89%; 95% confidence interval [CI], 83 to 95), 12 died of early complications, and the remaining 2 were resistant. Consolidation treatment was associated with very low toxicity and no deaths in remission were recorded. Molecular assessment of response by reverse transcriptase-polymerase chain reaction (RT-PCR) showed conversion to PCR-negative in 48 of 99 (51%) and 82 of 88 patients (93%) after induction and consolidation, respectively. The 2-year Kaplan-Meier estimates of overall survival and event-free survival were 82% +/- 4% and 79% +/- 4%, respectively. For patients who achieved CR, the 2-year disease-free survival (DFS) was 92% +/- 3%. These data indicate that a significant reduction in toxicity might be obtained in APL using a less intensive consolidation without apparently compromising the antileukemic effect. These results also suggest a minor role for cytarabine and etoposide in the treatment of newly diagnosed PML/RARalpha-positive APL patients.

El Sistema Nacional de Salud español (SNS) comparte con el resto de los sistema sanitarios europeos la necesidad de hacer compatibles los principios éticos de universalidad e igualdad en el acceso a los servicios sanitarios con los requerimientos inherentes de la Comunidad Europea. A tal fin, las reformas realizadas se encaminan hacia la separación de las funciones de financiación y producción, convirtiendo así el contrato programa en un elemento central del proceso de reforma de todos los sistemas regionales de salud (SRS). Dado que los decisores de los SRS no desean optimizar un único objetivo, sino satisfacer diversos objetivos simultáneamente, la Programación por Metas se presenta como el método multiobjetivo más apropiado para la modelización y resolución del problema. El propósito de esta comunicación es desarrollar un modelo para la asignación de recursos a los hospitales públicos, cuando el centro decisor tiene diversos objetivos, evaluando los conflictos potenciales que existen entre ellos. El modelo diseñado se aplica al caso especial del Servicio Andaluz de Salud (SAS). Concretamente, el centro decisor del SAS debe resolver el problema de la asignación presupuestaria a determinados grupos de hospitales, como un elemento central de su estrategia de elaboración del contrato programa.

Lipid rafts are membrane microdomains involved in cell signalling and characterized by their high contents of sphingolipids, cholesterol and saturated fatty acids. In this study, we have purified lipid rafts of human frontal brain cortex from normal and Alzheimer’s disease (AD) cell membranes and analyzed their lipid composition. Compared to non-AD brains, lipid rafts from AD brains displayed low levels of n-3 long chain highly unsaturated fatty acids (HUFA) specially 22:6n-3 (docosahexaenoic acid) and n-9 monoenes (specially 18:1n-9, oleic acid). Changes in lipid structure of AD lipid rafts also involved significant reductions in the unsaturation and peroxidability indexes as well as increased unsaturated/cholesterol unsaturated/sphingomyelin and saturates/n-3 HUFA ratios. These results indicate that the mechanisms preserving lipid raft order and viscosity in normal frontal cortex are disrupted in AD brains, which partially might explain the abnormal intracellular signalling mechanisms observed during AD.

- Abstract
The Cuban forests have suffered an indiscriminate exploitation in the past. In spite of the efforts carried out with the reforestation plans and the establishment of plantations, the structure for ages of these plantations are unbalanced. It is difficult to make a stable flow of goods and services. In this paper the evolution of a model of forest planning based on multiple approaches is presented. It was applied to a plantation of Pinus caribaea of the Integral Forest Enterprice of “Pinar del Río” in Cuba, with the main goal of achieving a balanced structure of ages at the end of the planning horizon in order to guarantee a stability in the flow of goods and services taking into account the area restrictions and the general amount and management. The model goes from a Linear Lexicographical Goal Programming Model to a Linear Fractional Goal Programming Model. Their objectives have been in progress in the attainment of a structure of ages in balance until the stabilization of the net carbon fixed by the plantation. It is one of the functions of the forests like possible regulators of the climate. The models I and II detailed can be consulted in the references. In these work we will refer to the main proposed objectives and the results obtained in each of them and we will formalize the third modelización that includes the captured carbon, Los bosques cubanos sufrieron una explotación indiscriminada en el pasado. A pesar de los esfuerzos realizados con los planes de reforestación, y el establecimiento de plantaciones, la estructura por edades de estos bosques está muy desequilibrada, lo que dificulta un flujo estable de bienes y servicios. En este trabajo se presenta la evolución de un modelo de planificación forestal con criterios múltiples, aplicado a una plantación de Pinus caribaea de la Empresa Forestal Integral Pinar del Río, Cuba, con el objetivo de lograr una estructura equilibrada de edades al finalizar el horizonte de planificación y con ello garantizar una estabilidad en el flujo de bienes y servicios sujeto a restricciones de área, volumen y manejo en general. El modelo se apoya en la Programación por Metas y evoluciona de un modelo de Programación por Metas lexicográficas lineal a un Modelo lexicográfico con metas Fraccionales. Sus objetivos también han evolucionado, de la consecución de una estructura de edades en equilibrio hasta la estabilización del carbono neto fijado por la plantación, una de las funciones de los bosques como posibles reguladores del clima. Los modelaciones I y II detalladas se pueden consultar en las referencias. En el presente trabajo nos referiremos a los principales objetivos propuestos y los resultados obtenidos en cada uno de ellos y formalizaremos la tercera modelación que incluye el carbono secuestrado.

Un falso aneurisma es una complicación rara de una fractura de la diáfisis femoral. Se presenta un caso que fue diagnosticado tardíamente debido a la ausencia de sintomatología típica. Se discuten la etiología, profilaxis, diagnóstico y posibilidades terapéuticas; destacándose la novedad del diagnóstico y tratamiento mediante arteriografía y embolización selectiva. A false aneurysm following a closed femoral-shaft fracture is a rare complication. The author report a case that was lately diagnosed because of the absence of typical symptomatology. Aetiology, prophylaxis, diagnosis and treatment are discussed, emphasizing the new method of diagnosis and treatment by arteriography and selective embolism.
