- Abstract
Objectives: We aimed to examine the association of muscle evaluation, including muscle ultrasound, with hospital-associated disability (HAD), focusing on ADL categories. Design: A prospective observational cohort study. Setting and Participants: We recruited patients aged 65 years or older who were admitted to the geriatric ward of an acute hospital between October 2019 and September 2021. Measurements: Handgrip strength, bioimpedance analyzer-determined skeletal muscle mass, bilateral thigh muscle thickness (BATT), and the echo intensity of the rectus femoris on muscle ultrasound were performed as muscle assessments. HAD was evaluated separately for mobility impairments and self-care impairments. Results: In total, 256 individuals (mean age, 85.2 years; male sex, 41.8%) were analyzed. HAD in mobility was more common than HAD in self-care (37.5% vs. 30.0%). Only BATT was independently associated with HAD in mobility in multiple logistic regression analysis. There was no significant association between muscle indicators and HAD in self-care. Conclusion: A lower BATT was associated with a higher prevalence of HAD in mobility, suggesting the need to reconsider muscle assessment methods in hospitalized older adults. In addition, approaches other than physical may be required, such as psychosocial and environmental interventions to improve HAD in self-care.

- Abstract
Despite their importance for regional water resource planning and as indicators of climate change, records of in situ glacier mass balance remain short and spatially sparse in the Himalaya. Here, we present an updated series of in situ mass-balance measurements from Rikha Samba Glacier, Nepal, between 2011 and 2021. The updated in situ mass balance is -0.39 +/- 0.32 m w.e. for this period. We use an energy-mass balance model to extend the annual mass-balance series back to 1974. The model is forced using daily meteorological variables from ERA5-Land reanalysis data that is linearly bias-corrected using observations from an automatic weather station situated near the glacier terminus. The modeled mass balance is consistent with the in situ mass-balance series measured 2011-2021 and with previous glaciological and geodetic estimates. The model results indicate a mass balance of -0.56 +/- 0.27 m w.e. a(-1) over the reconstruction period of 1974-2021, which is comparable to the mass losses experienced by other Himalayan glaciers during this time. An assessment of the sensitivity of the glacier mass balance to meteorological forcing suggests that a change in temperature of +/- 1 K has a stronger effect on the calculated mass balance compared to a +/- 20% change in either precipitation, or relative humidity, or solar radiation.

We report on the results from a beam test of a binary silicon strip system proposed for ATLAS. The data were collected during the H8 beam test at CERN in 1995 and at a beam test at KEK in 1996. The binary modules tested had been assembled from silicon micro-strip detectors of different layout and from front-end electronics chips of different architecture. The efficiency, noise occupancy, and position resolution were determined as a function of the threshold setting for various bias voltages and angles of incidence. Interstrip effects were also evaluated using a high resolution telescope. The performance of a prototype detector module irradiated to a fluence of 1.2×1014p/cm2 is also characterized.

We conducted a 24-week, rater-masked, randomized trial of aripiprazole (6-30 mg/d) vs blonanserin (4-24 mg/d) in schizophrenia patients who were not taking any antipsychotic medication for more than 2 weeks before enrollment (UMIN000011194).Methods: The primary outcome measure for efficacy was improvement of Positive and Negative Syndrome Scale (PANSS) total score at week 24. Secondary outcomes were PANSS subscale scores, 21-item Hamilton Rating Scale for Depression (HAMD-21) score, response rate, discontinuation rate, and individual adverse events.Results: Forty-four patients were recruited. The discontinuation rate was 86.4% in the aripiprazole group and 68.2% in the blonanserin treatment group. There was no significant difference in mean time to discontinuation between the groups. Although both treatment groups showed significant reductions in the PANSS total score, PANSS subscale scores, and HAMD-21 scores at week 24, the magnitudes of the changes did not differ between the groups. There were no significant differences in the incidences of adverse events including somnolence, extrapyramidal symptoms, prolactin-related adverse events, and weight change between the groups.Conclusion: Our results suggest similar efficacy and safety profiles of aripiprazole and blonanserin in the patients with schizophrenia. Double-blind controlled studies are needed to further explore the efficacy and safety of aripiprazole and blonanserin in schizophrenia. 170. Automatic detection of snowline altitude using high-resolution satellite imageries over the Himalayas

- Abstract
Snow and glacier meltwater are crucial regulators of freshwater resources in High Mountain Asia (HMA). In spite of its importance, the detailed distribution and variation of snow cover are not well known. With anticipated global warming, it is increasingly imperative to estimate the temporal and spatial distribution of snow cover. Despite various studies presenting detailed changes in snow cover in individual catchments, most large-scale assessments have been focused on annual values without seasonal variations, or have utilized moderate-resolution sensors (>250 m, such as MODIS). Moreover, global snow cover distribution products suffer from cloud cover and topographic shadows, both of which are common in high mountain areas and cause biases. To overcome this limitation, we have developed an automatic detection algorithm for snowline altitude (SLA) using high-resolution multispectral satellite imageries. SLA is a valuable variable to examine snow cover variation in HMA since it is less biased by cloud cover than snow cover extent. The proposed tool was applied to five glacierized watersheds across the Himalayas to quantify variations in seasonal and annual snow cover distribution over the past two decades, and to analyze the meteorological drivers of SLA. Our findings reveal remarkable differences in SLA among the sites in terms of decadal trends, seasonal patterns, and meteorological drivers. The application of the algorithm to a wider geographical area can offer essential insights into the spatio-temporal variability of snow cover and its drivers, which could lead to a better understanding of the future state of snow cover and high mountain hydrology., The 28th IUGG General Assembly (IUGG2023) (Berlin 2023)

Methods: Using electronic medication records from 142 community pharmacies, outpatients older than 40 years of age taking DPZ, with or without other prescription medicines, were assessed over 6 years, beginning in 2007. We estimated the number of medicines administered along with DPZ, the number of anticholinergics administered along with DPZ, and the medicines' anticholinergic cognitive burden (ACB) scale cumulative score based on data from the top four pharmacies that filled the highest number of prescriptions for DPZ for outpatients with dementia in 2010. Data were gathered from records of 431 patients; only three patients were younger than 60 years. Results: There was a 1.94-fold increase in the number of prescriptions including DPZ over 6 years. The proportion of patients to whom other medicines were administered along with DPZ was 65.6% (n=283) and the proportion of those taking at least one anticholinergic agent was 24.1% (n=104). The mean number of medicines among subjects taking at least one anticholinergic was 5.7, and the mean cumulative ACB score for anticholinergics contained in these medicines was 2.6. Among 104 patients to whom the anticholinergics were administered along with DPZ, two outpatients taking urologic medicines such as oxybutynin hydrochloride or tolterodine tartrate were found. Conclusion: Our findings suggest that it is necessary to pay attention to a decline in cognitive function when prescribing multiple medicines, especially to elderly patients who have already been prescribed DPZ. For several decades, the origin of ultra-high-energy cosmic rays (UHECRs) has been an unsolved question of highenergy astrophysics. One approach for solving this puzzle is to correlate UHECRs with high-energy neutrinos, since neutrinos are a direct probe of hadronic interactions of cosmic rays and are not deflected by magnetic fields. In this paper, we present three different approaches for correlating the arrival directions of neutrinos with the arrival directions of UHECRs. The neutrino data are provided by the IceCube Neutrino Observatory and ANTARES, while the UHECR data with energies above ∼50 EeV are provided by the Pierre Auger Observatory and the Telescope Array. All experiments provide increased statistics and improved reconstructions with respect to our previous results reported in 2015. The first analysis uses a high-statistics neutrino sample optimized for pointsource searches to search for excesses of neutrino clustering in the vicinity of UHECR directions. The second analysis searches for an excess of UHECRs in the direction of the highest-energy neutrinos. The third analysis searches for an excess of pairs of UHECRs and highest-energy neutrinos on different angular scales. None of the analyses have found a significant excess, and previously reported overfluctuations are reduced in significance. In this epidemiologic review regarding neonatal sepsis, the recent epidemiology and causative pathogens in Japan are described and compared with data from international organizations. Recent Japanese multicenter studies report the incidence of neonatal sepsis at 0.74%. Although the incidence of early-onset sepsis has been low (0.13%), approximately 5% of very or extremely low birth weight infants develop late-onset sepsis with a high mortality rate. In Japan for such cases, the most predominant causative pathogen is methicillin-resistant Staphylococcus aureus; internationally, the most commonly reported pathogen is coagulase-negative staphylococci. The present study aimed to examine early prediction of blonanserin in patients with schizophrenia. Methods: An 8-week, prospective, single-arm, flexible-dose clinical trial of blonanserin in patients with schizophrenia was conducted under real-world conditions. The inclusion criteria were antipsychotic naïve, and first-episode schizophrenia patients or schizophrenia patients with no consumption of any antipsychotic medication for more than 4 weeks before enrollment in this study. The positive predictive value, negative predictive value, sensitivity, specificity, and predictive power were calculated for the response status at week 4 to predict the subsequent response at week 8.Results: Thirty-seven patients were recruited (56.8% of them had first-episode schizophrenia), and 28 (75.7%) completed the trial. BackgroundPrior studies indicated a decrease in the incidences of aneurysmal subarachnoid haemorrhage (aSAH) during the early stages of the COVID-19 pandemic. We evaluated differences in the incidence, severity of aSAH presentation, and ruptured aneurysm treatment modality during the first year of the COVID-19 pandemic compared with the preceding year.MethodsWe conducted a cross-sectional study including 49 countries and 187 centres. We recorded volumes for COVID-19 hospitalisations, aSAH hospitalisations, Hunt-Hess grade, coiling, clipping and aSAH in-hospital mortality. Diagnoses were identified by International Classification of Diseases, 10th Revision, codes or stroke databases from January 2019 to May 2021.ResultsOver the study period, there were 16 247 aSAH admissions, 344 491 COVID-19 admissions, 8300 ruptured aneurysm coiling and 4240 ruptured aneurysm clipping procedures. Declines were observed in aSAH admissions (−6.4% (95% CI −7.0% to −5.8%), p=0.0001) during the first year of the pandemic compared with the prior year, most pronounced in high-volume SAH and high-volume COVID-19 hospitals. There was a trend towards a decline in mild and moderate presentations of subarachnoid haemorrhage (SAH) (mild: −5% (95% CI −5.9% to –4.3%), p=0.06; moderate: −8.3% (95% CI −10.2% to –6.7%), p=0.06) but no difference in higher SAH severity. The ruptured aneurysm clipping rate remained unchanged (30.7% vs 31.2%, p=0.58), whereas ruptured aneurysm coiling increased (53.97% vs 56.5%, p=0.009). There was no difference in aSAH in-hospital mortality rate (19.1% vs 20.1%, p=0.12).ConclusionDuring the first year of the pandemic, there was a decrease in aSAH admissions volume, driven by a decrease in mild to moderate presentation of aSAH. There was an increase in the ruptured aneurysm coiling rate but neither change in the ruptured aneurysm clipping rate nor change in aSAH in-hospital mortality.Trial registration numberNCT04934020.

- Abstract
On 2020 December 5 at 17:28 UTC, Japan Aerospace Exploration Agency's Hayabusa2 sample return capsule (SRC) re-entered Earth's atmosphere. The capsule passed through the atmosphere at supersonic speeds, emitting sound and light. The inaudible sound was recorded by infrasound sensors installed by Kochi University of Technology and Curtin University. Based on analysis of the recorded infrasound, the trajectory of the SRC in two cases, one with constant-velocity linear motion and the other with silent flight, could be estimated with an accuracy of 0° 5 in elevation and 1° in direction. A comparison with optical observations suggests a state of flight in which no light is emitted but sound is emitted. In this paper, we describe the method and results of the trajectory estimation.

The prevalence of age-related macular degeneration (AMD) is increasing in today’s aging population. The purpose of this paper is: (1) to discuss, by reviewing the current literature, the impact of AMD on patients’ QoL and the utility of QoL assessments in evaluating the impact of AMD and its treatment; and (2) to make a recommendation for incorporating QoL into clinical practice.Methods: We conducted a PubMed and an open Internet search to identify publications on the measurement of QoL in AMD, as well as the impact of AMD and the effect of treatment on QoL. A total of 28 articles were selected.Results: AMD has been found to cause a severity-dependent decrement in QoL that is comparable to systemic diseases such as cancer, ischemic heart disease, and stroke. QoL impairment manifests as greater social dependence, difficulty with daily living, higher rates of clinical depression, increased risk of falls, premature admission to nursing homes, and suicide. The National Eye Institute Visual Functioning Questionnaire (NEI VFQ-25) is the most widely used eye disease-specific QoL instrument in AMD. It has been shown to correlate significantly with visual acuity (VA). QoL reflects aspects of AMD including psychological well-being, functional capacity, and the ability to perform patients’ valued activities, which are not captured by a single, numerical VA score.Conclusion: The literature shows that the adverse impact of AMD on QoL is comparable to serious systemic disease. Eye disease-specific instruments for measuring QoL, such as the NEI VFQ-25, have shown a significant correlation of QoL decrement with measures of disease severity, as well as significant QoL improvement with treatment. The NEI VFQ-25 and other validated instruments provide a wide-ranging assessment of vision-related functioning that is important to patients and complementary to VA measurement. The primary efficacy measurements were the Hamilton Rating Scale for Depression (HRSD) and the Montgomery–Åsberg Depression 6-point Rating Scale (MADRS) scores. The second efficacy measurements were the response and remission rates of treatment. Tolerability assessments were also performed. Fifty-six patients (43 male; age, 43.6 years) were recruited. There was no significant difference in the discontinuation rate between the mirtazapine and duloxetine treatment groups (P = 0.867). Both mirtazapine and duloxetine significantly improved the HRSD and MADRS scores from baseline (P < 0.0001–0.0004). While mirtazapine was superior to duloxetine in the reduction of HRSD scores (P = 0.0421), there was no significant change in MADRS scores in terms of between-group differences (P = 0.171). While more somnolence was observed with mirtazapine (P = 0.0399), more nausea was associated with duloxetine (P = 0.0089). No serious adverse events were observed for either antidepressant. Mirtazapine and duloxetine were safe and well-tolerated treatments for Japanese patients with MDD. Double-blind controlled studies are needed to further explore the efficacy and safety of mirtazapine and duloxetine in Japanese patients with MDD.
