Naloxone (NLX) is a pharmaceutical used as opioid antagonist. A molecular imprinted polymer electrochemical sensor for simple and rapid detection of NLX was prepared through the modification of commercial available screen printed carbon electrode (SPCE). The SPCE was modified with multi-walled carbon nanotubes (MWCNT) by drop coating to increase the signal response and improve the sensitivity. The MIP preparation was carried out via in situ electropolymerization using 4-aminobenzoic acid (4-ABA) as functional monomer. The morphology of the obtained sensor was characterized by scanning electron microscopy (SEM). Several parameters controlling the preparation and performance of the MIP sensor were studied and optimized. The electrochemical behavior of NLX at MIP and control non-imprinted (NIP) sensor was evaluated by differential pulse voltammetry (DPV), demonstrating a better MIP response and the success of the imprinting. The proposed MIP/MWCNT/SPCE sensor showed a linear relationship between peak current intensity and NLX concentration in the range between 0.25 and 10.0 μM, with limits of detection (LOD) and quantification (LOQ) of 0.20 μM and 0.67 μM respectively. The repeatability and reproducibility were also tested with relative standard deviations (RSD) of 4.6 and 9.6% respectively. Moreover, the applicability of the method was successfully confirmed with detection of NLX in biological samples (urine and human serum). The sensor is promising to be used for screening NLX in point-of-care people with opioid overdose.

Histoplasmosis is a fungal disease that is endemic in Brazil. It may present as chronic pulmonary infection or in disseminated form. Disseminated histoplasmosis frequently affects the adrenal gland; however, unilateral involvement in immunosuppressed patients is the usual presentation. We report a case of an elderly immunocompetent male with history of weight loss, fever and bilateral adrenal mass who was successfully treated with itraconazole.Histoplasmose é uma doença fúngica endêmica no Brasil que pode se apresentar como infecção pulmonar crônica ou na forma disseminada. A histoplasmose disseminada freqüentemente acomete a glândula adrenal; entretanto, ocorre mais em pacientes imunossuprimidos e de forma unilateral. Relatamos um caso de um homem idoso imunocompetente com história de perda de peso, febre e massa adrenal bilateral que foi tratada com itraconazol, com sucesso.

Hosting a world cup puts severe pressure on the urban infrastructure and services related to transportation and safety of any city. In order to address such pressure, the city of Natal (Brazil), a 2014 FIFA World Cup host city, in partnership with a local university have engaged in an initiative to create a smart tourism destination. This paper describes the implementation of this initiative and some initial results. First, this paper presents the Find Natal, a smart tourism application which aims to enhance tourists' travel experience through software programs designed to leverage the infrastructure mechanisms behind the city. We have detailed the architecture and its components. Finally, we analyzed the data gathered by Find Natal application to show how such application can be used during any big event.

The cloud lock-in problem is commonly addressed by three strategies: (i) use of intermediate layer between cloud services consumers and providers, (ii) use of standardized interfaces to access cloud services, or (iii) use of models with open specifications. This paper evaluated these strategies and concluded that despite the advances they introduced, none of them actually solves the cloud lock-in problem. To address this issue, this paper proposes the use of Semantic Web concepts to avoid cloud lock-in. The strategy consists of using RDF models to specify the features of a cloud service, which are managed by SPARQL queries. The contribution of this paper is fourfold: (i) to evaluate three multi-cloud solutions for the cloud lock-in problem in the context of three cloud platforms, (ii) to propose a strategy using RDF and SPARQL for managing cloud resources, (iii) to present the Cloud Query Manager (CQM), an SPARQL server that implements the proposal, and (vi) to compare three multi-cloud solutions with CQM in terms of response time and the effectiveness in the resolution of the cloud lock-in problem.

In a Cloud Computing scenario, cloud services are accessed via Internet and typically there is a communication latency between the service provider and its clients. As a consequence, QoS parameters noticed by the clients (such as service response time) have different values depending on their location. In this paper, we propose the relative QoS (RQoS) concept, which defines that the quality of a service is relative to its geographical location. We also introduce a monitoring mechanism that provides RQoS parameters of cloud services in the SaaS (Software as a Service) layer, so that clients can choose the services that better suit their needs. In order to validate this new concept and the monitoring mechanism, we present a proof of concept and some computational experiments to show that there are variations in the values of QoS depending on the geographic location where services are monitored. The performed experiments also aimed to evaluate the performance of the monitoring mechanism in terms of the assessment of RQoS parameters.

In poultry industry should be given to water the same importance that is given to other factors such as facilities, feeding and management. Poultry production require large amounts of water for their development and welfare. Besides water for drinking, must also consider water as an input for the management of vaccination, cleaning, thermal control environment and disinfection of equipment and facilities. Several variables affect water consumption, including genetics, animal age, sex, ambient temperature, water temperature, the relative humidity and nutritional composition. Water is indispensable for life, because of the functions it plays in metabolism, so it is of fundamental importance to the rational use of water of good quality physical, chemical and microbiological. Because birds do not have sweat glands to aid in heat dissipation, the consumption of cold water is the alternative that these animals have to lower body temperature in situations of heat stress. It is undisputed that when the bird has water availability of adequate quality and quantity, this will present a performance improvement.

Objetivou-se avaliar o desempenho e qualidade de ovos de codornas europeias (Coturnix coturnix coturnix) alimentadas com dietas contendo 3% óleo de soja ou girassol e suplementadas com 0, 100, 200 e 400 ppm\kg de vitamina E. O experimento foi realizado no Laboratório de Ensaios Metabólicos (LabEM), na Fazenda Água Limpa (FAL) da Universidade de Brasília (UnB), com latitude de 150 56‟ S e longitude de 470 56‟W e altitude média de 1080 m, localizada no Núcleo Rural Vargem Bonita, Brasília – DF. Foram utilizadas 240 codornas europeias com 157 dias de idade, distribuídas em um delineamento inteiramente casualizado em esquema fatorial 2x4x5 (duas fontes de óleo x quatro níveis de vitamina E x cinco ciclos de produção), com 8 tratamentos e 6 repetições de 5 aves cada. O período experimental foi de 70 dias divididos em 5 ciclos de 14 dias. As codornas receberam alimento à vontade, fornecidos três vezes por dia, a água foi fornecida por bebedouros tipo “nipple” com taça e programa de luz de 17 horas de luz continua , sendo 12 horas de luz natural e 5 horas de luz artificial. A produção de ovos foi controlada diariamente em (%) e o consumo da dieta (g/ave/dia) ao final de cada ciclo de 14 dias. No último dia de cada ciclo os ovos coletados foram destinados para analise de qualidade física do ovo: peso do ovo, casca, albúmen e gema (g) porcentagem de casca, albúmen e gema (%) e espessura da casca (mm). Foram coletados amostras de ovos para realização de analises de oxidação de ovos processados por 12 minutos a 97,6 ºC e armazenados em câmara fria a 4ºC nos tempos de 0, 7, 14, 21 e 28 dias de estocagem e ainda de ovos frescos armazenados em geladeira a 10 ºC, simulando condições comerciais nos períodos de 0, 14, 28 e 42 dias. In this paper we present a new architecture for the Cloud Integrator platform that extends it in two directions. The first one aimed at improving the way of an application can be defined by a developer. Instead of dealing with the burden of defining applications only by specifying semantic workflows in terms of activities expressed as a tuple , a developer can rely on the definition of complete executions flows or partial execution flows. The second extension aimed at the automatic deployment of applications in a cloud platform, thus allowing the simultaneous use of it by several clients over the Internet. We also present a proof-of-concept that illustrates the use of the cloud-based platform in a flight booking application. Keywords—Cloud Computing; workflows; tasks; service

Ubiquitous computing (UC) encompasses sensor-instrumented environments, which are often endowed with wireless network interfaces, in which devices, software agents, and services are integrated in a seamless and transparent way and cooperate to meet high-level goals of human users. This chapter introduces some background concepts on UC. It presents the state-of-the-art for the field of middleware for UC. The requirement of the integration of UC middleware is addressed and an existent solution is described - the Open Context Platform Integration (OpenCOPI) platform. OpenCOPI provides an automatic service composition, a standardized context and communication model, and a fault tolerance mechanism that handles service failures. Another important feature of OpenCOPI is the use of semantic workflows. Finally, the chapter presents a new integration platform for UC. It talks about OpenCOPI as an example of a software system developed to integrate different context provision middleware, with the goal of facilitating the development of context-aware ubiquitous applications.

International audience; In this paper, we present OpenCOPI (Open COntext Platform Integration), a Service-Oriented Architecture-based middleware platform that supports the integration of services provided by distinct sources, ranging from services offered by simple systems to more complex services provided by context-provision middleware. OpenCOPI offers selection and composition mechanisms to, respectively, select and compose services provided by different sources, considering applications of both Quality of Service and Quality of Context requirements. It also offers an adaptation mechanism that enables to adapt the application execution due to service failures, service quality fluctuation and user mobility. OpenCOPI allows the definition of applications in a higher abstraction level by the specification of a semantic workflow that contains abstract activities. This paper illustrates the use of OpenCOPI in an application from the Gas & Oil Industry and it also shows the evaluation of the main mechanisms of OpenCOPI: the service selection, composition, adaptation and workflow execution.

Part 2: Work-in-Progress Papers; International audience; Cloud-based applications are built from services offered by distinct third-party cloud providers. Most cloud-related information (service properties such as price, availability, response time, etc.) can change at any time during application execution. Therefore, it is essential to support the adaptation of applications in such dynamic conditions in order to ensure that the cloud services currently provided to deployed applications adhere to the established requirements. In this paper we present an autonomous adaptation process for cloud-based applications by replacing a service by an alternative one that fulfills the application needs. We discuss the factors that trigger an adaptation in a Cloud Computing scenario and describe the adaptation process within Cloud Integrator, a service-oriented middleware platform for composing, executing, and managing services provided by different cloud platforms.

A atividade de lavagem e despolpa de frutos do cafeeiro, necessária para a redução do custo de secagem e a melhoria da qualidade de bebida, é geradora de grandes volumes de resíduos sólidos e líquidos, ricos em material orgânico e inorgânico. A água residuária da atividade de beneficiamento do café (ARC) é gerada anualmente em grande volume no Espírito Santo, e aliado aos nutrientes existentes nesse efluente indica a sua viabilidade de reaproveitamento na fertirrigação de culturas agrícolas. O objetivo do presente trabalho foi avaliar o efeito de diferentes doses de água residuária de café no crescimento, na absorção e interação entre nutrientes e no estado nutricional do milho. Para tanto, foi conduzido um experimento em casa de vegetação utilizando-se o delineamento experimental inteiramente casualizado, onde foram aplicadas 7 doses de ARC, com 3 repetições, em unidades experimentais constituídas por vasos com 2 dm³ de solo. As doses foram equivalentes a 0,00, 15,17, 30,35, 45,52, 60,70, 75,87 e 91,05 litros de ARC por m² de solo. Realizou-se a semeadura de cinco sementes de milho híbrido BR 206 por vaso e cinco dias após a germinação das plantas foi feito o desbaste, mantendo-se três plantas por vaso. Aos trinta dias após a germinação determinou-se o diâmetro do caule (DC), área foliar (AF), matéria seca da parte aérea (MSPA), matéria seca do sistema radicular (MSR), relação parte aérea/raiz (MSPA/MSR), razão massa radicular (matéria seca raiz/matéria seca total) e razão área foliar (área foliar/matéria seca total). Na parte aérea das plantas foi determinado os teores dos macronutrientes (N, P, K, Ca, Mg e S). Os dados foram submetidos a análise de variância e as variáveis em função das doses de ARC submetidas a análise de regressão. Para as variáveis dependentes foi calculado o coeficiente de correlação linear de Pearson. A ARC serviu como fonte de nutrientes para as plantas de milho, aumentou o rendimento da maioria das variáveis de crescimento e os teores de N, K e S. Porém diminuiu o teor de Ca, Mg e P da parte aérea das plantas além de indicar que altas doses promovem desbalanceamento na relação entre nutrientes. The coffee production always held a prominent place in the national economy, due to its importance in the economic and social community, looking for a distinct market with new technologies to improve the beverage quality. The activities of washing and pulping coffee, necessary to reduce drying costs and improve the quality of beverage, generates a large amount of solid and liquid waste which has a notable rates of organic and inorganic material. The residual water from processing coffee activity (RWC) has been generated every year in a large volume in Espírito Santo and the presence of nutrients indicates the viability to its reuse in fertigation of crops. The aim of this study was to evaluate the effect of different doses of residual water of coffee on growth, absorption, interaction of nutrients and the nutritional condition of corn (Zea mays L.). In this context, the experiment was conduct in a greenhouse applying the completely randomized design, where seven RWC doses were applied with three repetitions in experimental unities composed by vases with 2 dm³ of soil. The dosages had the following concentrations: 0.00, 15.17, 30.35, 45.52, 60.70, 75.87 and 91.05 liters of ARC per m² of soil. The seeding were performed using five seeds of BR 206 hybrid corn per vase and after five days were made the thinning, maintaining only three plants per vase. On the 30th day after germination, the following measurements were determined: stem diameter (SD), leaf area (LA), shoot dry matter (SDM), root dry matter (RDM), shoot/root ratio (SDM/RDM), root mass ratio (root dry weight / total dry matter) and leaf area ratio (leaf area / total dry matter). Aerial part of plants was used to established the levels of macronutrients (N, P, K, Ca, Mg and S). The data were subjected to analysis of variance and the doses of RWC were adjusted in regression models. For dependent variables was calculated the Pearson linear correlation coefficient. The application of ARC served as a fertilization input to corn crop, increasing most of the growth variables and N, K and S levels. However, it caused a decrease in shoot Ca, Mg and P levels, indicating that high dosages promote a disturb in nutrient relations.

Services selection is an important challenge for applications that use a composition of services provided by different cloud platforms. This paper presents an optimized cloud services selection approach that evaluates each alternative set of services that composed an execution plan. This approach considers cost and quality parameters for each cloud service in the execution plan, and excludes coincident services in the calculations. Coincident services are those services present in all available execution plans and therefore equally contribute in the calculations encompassing costs and quality parameters regarding these plans. By excluding coincident services, the service selection process by itself should be performed more quickly since it involves a smaller number of services, but with the trade-off of running an additional algorithm for identifying the coincident services among the available ones. In order to evaluate such trade-off and illustrate the proposed approach, we present a case study and an experimental evaluation that compares our new approach with a previous one that considers all services that compose all available execution plans.

With the advance of the Cloud Computing paradigm, a single service offered by a cloud platform may not be enough to meet all the application requirements. To fulfill such requirements, it may be necessary, instead of a single service, a composition of services that aggregates services provided by different cloud platforms. In this perspective, this work presents Cloud Integrator, a middleware platform for composing services provided by different Cloud Computing platforms. Composition is performed by considering metadata about the services such as QoS, prices etc. By integrating the complementary paradigms of Service-Oriented Computing and Cloud Computing, Cloud Integrator enables the use of services provided by providers in a transparent way for the user, being necessary solutions for publishing, discovering, and composing these services. In terms of cloud infrastructure, Cloud Integrator lies in the PaaS layer, allowing HaaS, SaaS and DaaS resources to be available for the users so that they can build applications consisting of the composition of services provided by these platforms. In this paper, the application of Cloud Integrator is illustrated using a simple cloud-based e-commerce application.

INTRODUCAO: As doencas glomerulares sao uma causa frequente de doenca renal cronica, sobretudo nos paises em desenvolvimento. OBJETIVO: O objetivo deste estudo foi determinar o perfil destas glomerulopatias em um hospital publico da cidade de Brasilia, Distrito Federal. METODOS: Foram realizadas 121 biopsias renais pela equipe de nefrologia do Hospital Regional da Asa Norte (HRAN) entre agosto de 2005 e maio de 2009. Foram excluidas oito biopsias realizadas em pacientes transplantados renais e analisados os prontuarios dos 113 pacientes restantes. Dados analisados: sexo, idade, exames laboratoriais, sindrome glomerular, diagnostico clinico, grau de fibrose intersticial, uso de imunossupressores, necessidade de dialise e desfecho clinico. RESULTADOS: A media de idade foi 34,9 ± 16,2 anos, com predominio masculino (51,3%). As principais sindromes glomerulares foram: sindrome nefrotica (41,6%) e glomerulonefrite rapidamente progressiva (35,4%). Entre as glomerulopatias primarias, houve predominio da glomeruloesclerose segmentar e focal (26,9%) e da nefropatia por IgA (25%) e entre as secundarias a nefrite lupica (50%) e a glomerulonefrite proliferativa exsudativa difusa (34,2%). A maioria dos pacientes fez uso de imunossupressores (68,1%) e quase um terco deles (29,2%) necessitou de dialise durante a internacao. Evoluiram para terapia dialitica cronica 13,3% dos pacientes e 10,6% evoluiram a obito. CONCLUSAO: Este estudo podera contribuir para melhor entendimento epidemiologico das doencas glomerulares no Distrito Federal, orientando na adocao de politicas publicas visando permitir rapido diagnostico e manejo clinico das mesmas.

INTRODUCTION: Glomerular diseases are a frequent etiology of chronic kidney disease, especially in the developing countries. OBJECTIVE: To determine the profile of such glomerulopathies in a public hospital located in the city of Brasilia, Federal District. METHODS: 121 renal biopsies in different patients were performed by the Renal Division of Hospital Regional da Asa Norte (HRAN) between August 2005 and May 2009. Eight renal biopsies in renal-transplant patients were excluded and the medical records of 113 remaining patients were analyzed. Analyzed data: sex, age, laboratory exams, glomerular syndrome, clinical diagnosis, degree of interstitial fibrosis, immunosuppressants use, need for dialysis and clinical outcome. RESULTS: The age average was 34.9 ± 16.2 years-old, a predominance of male patients (51.3%). Major glomerular syndromes were: nephrotic syndrome (41.6%) and the rapidly- progressive glomerulonephritis (35.4%). Among primary glomerulopathies focal glomerulosclerosis (26.8%) followed by IgA nephropathy (25%) were predominant; and among the most prevalent secondary glomerulopathies we had lupus nephritis (50%) and diffuse exudative proliferative glomerulonephritis (34.2%).The majority of the patients used immunosuppressants (68.1%) and almost one third of them (29.2%) needed dialysis during their hospitalization. Progressed to chronic dialysis therapy 13.3% of the patients and 10.6% died. CONCLUSION: This study may contribute to better epidemiological understanding of glomerular diseases in the Federal District, guiding the adoption of public policies aiming the quick clinical treatment of such diseases.

Although the incidence of breast cancer has been declining in recent years, the disease is still one of the leading causes of cancer deaths in women. Recently, breast cancer has been treated with innovative approaches that use hormone-sensitive therapies. This is because in at least one-third of breast cancers, estrogens mediated via the estrogen receptor pathway act as endocrine growth factors. Fulvestrant has been studied as both first- and second-line therapy for locally advanced and metastatic breast cancer, but few studies have shown its effect as third-line therapy alone. To observe the disease time to progression (TTP) obtained with fulvestrant when used on metastatic breast cancer as first-, second-, and also third-line therapy. We also aimed to correlate the TTP obtained with fulvestrant with hormone receptor, HER2 expression, and metastatic site. This was a cohort study that retrospectively examined medical records of 73 postmenopausal women with advanced breast cancer who were treated with fulvestrant (250 mg/month i.m. injection) and followed at the Department of Medical Oncology at Hospital do Cancer A. C. Camargo in Sao Paulo, Brazil from August 2003 to December 2006. The median TTP with fulvestrant was about 11 months. When used as the first-line therapy, TTP was about 13 months; when used as second-line, TTP was about 6 months; and when used as third-line, it was about 12 months. No statistically significant difference was observed regarding the therapy line. In patients with positive ER tumors, TTP was 11 months. No significant difference in TTP was observed in negative ER tumors (TTP = 10 months). In patients with positive PgR tumors, TTP was 13 months and for negative PgR, TTP was 6 months (P = 0.008). According to the HER2 status, the TTP was 5 months for HER2+ and 10 months for HER2−. Our findings indicate that fulvestrant is an effective alternative for treatment of metastatic breast cancer.

Glomerular diseases are a frequent etiology of chronic kidney disease, especially in the developing countries.To determine the profile of such glomerulopathies in a public hospital located in the city of Brasilia, Federal District.121 renal biopsies in different patients were performed by the Renal Division of Hospital Regional da Asa Norte (HRAN) between August 2005 and May 2009. Eight renal biopsies in renal-transplant patients were excluded and the medical records of 113 remaining patients were analyzed. Analyzed data: sex, age, laboratory exams, glomerular syndrome, clinical diagnosis, degree of interstitial fibrosis, immunosuppressants use, need for dialysis and clinical outcome.The age average was 34.9 ± 16.2 years-old, a predominance of male patients (51.3%). Major glomerular syndromes were: nephrotic syndrome (41.6%) and the rapidly- progressive glomerulonephritis (35.4%). Among primary glomerulopathies focal glomerulosclerosis (26.8%) followed by IgA nephropathy (25%) were predominant; and among the most prevalent secondary glomerulopathies we had lupus nephritis (50%) and diffuse exudative proliferative glomerulonephritis (34.2%).The majority of the patients used immunosuppressants (68.1%) and almost one third of them (29.2%) needed dialysis during their hospitalization. Progressed to chronic dialysis therapy 13.3% of the patients and 10.6% died.This study may contribute to better epidemiological understanding of glomerular diseases in the Federal District, guiding the adoption of public policies aiming the quick clinical treatment of such diseases.

Dentre os debates a respeito da filosofia política de Hobbes, as relações entre lei de natureza e lei civil são objeto de grande controvérsia. Principalmente dois pontos são discutidos em relação à lei de natureza. O primeiro corresponde à sua imperatividade: a lei de natureza tem força de comando no estado de natureza ou é mera recomendação? O segundo corresponde à sua relação com a lei civil: são ambas a mesma coisa, são contrárias, são complementares? Com vistas a responder a esses questionamentos, a presente dissertação investiga três utilizações do conceito de lei de natureza por Hobbes como preceito da razão, como comando de Deus e como lei moral , duas utilizações do conceito de lei civil como comando do soberano e como medida das ações dos súditos , bem como as relações que Hobbes estabelece entre os dois conceitos. Among the discussions concerning Hobbes political philosophy, the relations between the law of nature and the civil law raise great controversy. Two points related to the law of nature remain unsolved. The first one deals with its imperative character: does the law of nature have the force of a true command in the state of nature or is it a mere recommendation? The second point deals with its relation with the civil law: are they the same, are they contradictory, are they complementary? Aiming to answer these questions, this work analyses three different uses of the law of nature concept as a rational precept, as a command of God and as moral law , two uses of the civil law concept as a command of the sovereign and as a rule for the subjects actions , as well as the relations that Hobbes states between those two concepts.

Resumo: O objetivo desse estudo experimental foi de comparar cinco diferentes técnicas diretas de moldagem (n=5): grupo 1-transferentes separados, grupo 2-transferentes unidos com resina acrílica, grupo 3-transferentes unidos com resina acrílica, segmentados e unidos novamente com resina acrílica, grupo 4-transferentes revestidos com o adesivo da material de moldagem e grupo 5-transferentes unidos com resina acrílica, separados e unidos novamente com cianoacrilato e com um tubo de látex sobre análogo de forma a proporcionar um duplo vazamento de gesso. Para tanto, foi fabricada uma matriz em resina acrílica onde foram fixados quatro análogos de implantes com plataforma 4.1 mm em hexágono externo. Os implantes foram moldados, utilizando-se de moldeira individualizada, em acrílico, e poliéter. No total 25 modelos com 100 medições. Todas as medições foram feitas utilizando-se um Microscópio Comparador. As desadaptações médias medidas foram: 6.42 m para o grupo 1, 11.53 m no grupo 2, no grupo 3 foi de 7.03 m e nos grupos 4 e 5, 5.12 e 6.10 m, respectivamente. Não houve diferença estatisticamente relevante entre os grupos com transferentes unidos e não unidos. Os piores resultados foram mostrados pelo grupo 2. Abstract: The purpose of this study was to compare 5 different direct impression techiniques (n=5): group 1-separeted impression copings, group 2-impression copings splinted with acrylic resin, group 3- impression copings were splinted with acrilic resin, sectioned, and joined together with acrylic resin , group 4-impression copings coated with the manufacturer-recommended impression adhesive and group 5- impression copings were splinted with acrylic resin, sectioned, and joined together with cyanoacrilate and with a latex tube fitted onto each analog so that a double pouring technique could be possible. Thus, a acrylic resin matrix was fabricated, where four 4.1 plataform implant analogs were fixed. The implants impressions were performed with the use of a custom acrylic resin impression tray and polyether. 5 groups were formed with 5 casts each. A total of 25 models with 100 gaps (1 per implant). All the measurements were made with the use of a travelling microscope. The mean gaps measured were: 6.42 m for group 1, 11.53 m in group 2, in group 3 it was 7.03 m and in groups 4 and 5, 5.12 e 6.10 m, respectively. There was no statistic relevant difference among the splinted and unsplinted groups. The worst results were shown by group 2.

Disseminated histoplasmosis frequently affects the adrenal gland; however, unilateral involvement in immunosuppressed patients is the usual presentation. We report a case of an elderly immunocompetent male with history of weight loss, fever and bilateral adrenal mass who was successfully treated with itraconazole. Histoplasmose é uma doença fúngica endêmica no Brasil que pode se apresentar como infecção pulmonar crônica ou na forma disseminada. A histoplasmose disseminada freqüentemente acomete a glândula adrenal; entretanto, ocorre mais em pacientes imunossuprimidos e de forma unilateral. Relatamos um caso de um homem idoso imunocompetente com história de perda de peso, febre e massa adrenal bilateral que foi tratada com itraconazol, com sucesso.

As redes de abastecimento de água e saneamento têm vindo a aumentar nos últimos tempos, com este aumento cresce também a necessidade de apostar na melhoria da qualidade de serviço. Neste projecto foi desenvolvido um Sistema de Informação Geográfica contendo toda a informação referente à rede de abastecimento de água e saneamento da região Transmontana e Alto Duriense. Ao longo deste artigo serão descritas as funcionalidades e serviços a disponibilizar, bem como a arquitectura a utilizar. Por fim, serão apresentadas algumas conclusões e trabalho futuro com o projecto implementado. In Portugal, water supply networks and sewerage systems have been improved over the last few years. This improvement of the infrastructure leads to an increase in the need to improve the quality of service. We have deployed a project to develop a geographic information system to store all information regarding the water supply network and sewerage system of Trás-o-Montes e Alto Douro. In this paper we present the functionalities and services of this system, as well as its architecture. Finally, some conclusions and future work will be presented.

643 Background: Factors predictive of response to anthracyclines can help selecting patients who really benefit from its use. TOP2A (a target of anthracyclines) and HER2 genes are both amplified in 40% of breast cancers. Data from literature are conflicting regarding the potential of TOP2A as a predictor of response to anthracyclines. Methods: We retrospectively analyzed data on the outcome of patients undergoing neoadjuvant anthracycline-based chemotherapy and evaluated nuclear TOP2A protein expression in breast cancer biopsies by immunohistochemistry (IHC) and reviewed the pathological responses after completion of neoadjuvant treatment. Results: From 2002 to 2011, data from 82 patients was reviewed. Forty nine patients had the paraffin blocks corresponding to the pre-treatment biopsy. All biopsies had some degree of TOP2A IHC expression. Patients were considered positive for TOP2A if the level of expression in IHC was higher than 10%. This was found in 77.6% of cases. Forty percent of the patients achieved pathologic complete response in pos-treatment surgical specimens. Correlation was observed between TOP2A IHC expression and tumor pathological response (p0.02). It was observed a difference in pathological complete response in favour of the group positive for TOP2A (47.4% vs. 22.7%), although not significant (p=0.09). No difference in PFS and OS was observed between patients with complete response after median follow-up of 34 months. Conclusions: The expression of TOP2A seems to be a promising marker to predict pathological response to antracycline-based neoadjuvant chemotherapy in HER2 breast cancer; however these findings would be better evaluated in prospective trials.
