Abstract

The village of Collebrincioni is located right at the center of the most damaged area of the April 6th 2009 L'Aquila earthquake. It is known from historical data that this site had been heavily affected by the 1461 and 1703 seismic events and for that reason remained abandoned for a long time; the area has been settled again at the end of the 18th century with the rebuilding of the present conurbation in a new safer location. After the April 6th 2009 earthquake many evidences of coseismic effects have been found around the Collebrincioni surroundings, even though the macroseismic intensity calculated in the new settlement was found to be moderate (6 MCS). The aim of this work is to verify whether the cause of the migration from the old location can be linked to site effects; for this purpose, several measurements of horizontal to vertical spectral ratio (HVSR) have been carried out at various locations to obtain information on amplification phenomena. To allow a quick evaluation of site seismic response, without requesting any information about stratigraphical and geotechnical properties of the foundation soils, the Nakamura's technique was adopted in this paper. Measurements of HV spectral ratio carried out in the area show relevant differences between the new and the ancient settlements confirming the hypothesis that the abandonment of the old Collebrincioni settlement can be related to the site effects which have amplified the destructive effects of the earthquakes. Abstract

Innovative microscopies for the measurements of biological parameters can be helpful to reduce environmental impact, to prevent food contamination and poisoning and to assess the biocompatibility of nano-materials. In this frame, we applied scanning nearfield optical microscopy (SNOM) and atomic force microscopy (AFM) to a model system constituted by Clostridium tyrobutyricum dormant spores and their vegetative cells. SNOM was used to acquire the information about optical properties of C. tyrobutyricum spores with sub-wavelength resolution. Comparing reflection and transmission SNOM images revealed the presence of structures inside the spores. A detailed topographic study of the spore's external layers was carried out with AFM. To obtain information about the morphology of spores and vegetative cells at the nanoscale level, we applied AFM in the lateral force mode, that allowed us to have a qualitative picture of the elastic and viscoelastic properties of the spore coat. Abstract

Objective: This study analyzes the effect of short- and long-term omeprazole (OM) on duodenal ulcer healing. recurrence, and H. pylori status. Methods: Patients affected by active duodenal ulcer were randomly allocated to treatment with OM 20 mg or 40 mg once daily for 4 wk. Subsequently, patients with healed duodenal ulcer were randomly assigned to one of the following three groups: OM 20 mg once daily for 12 months, OM 20 mg alternate days for 12 months, and no treatment. Endoscopy was performed at entry, at 4 wk, and after 4, 8, and 12 months. Two biopsy specimens from antrum, body, and fundus were taken for histology and search for H. pylori. One hundred and eighty patients with active duodenal ulcer were admitted. Ninety-one were treated with OM 20 mg once daily and 89 with OM 40 mg for 4 wk. Results: The results at 4 wk show 92.8% patients healed with OM 20 mg and 95.1% at 40 mg (NS). In the second part of the study, 96 of the patients who healed at 4 wk (45 with OM 20 mg, and 51 with OM 40 mg) entered the long-term study. Thirty-six patients received OM 20 mg daily for 12 months, 35 OM 20 mg on alternate days for 12 months, and 25 patients no treatment. The results show a healing rate of 100%, 100%, and 95% with OM 20 mg daily, of 97%, 95%, and 93% with OM 21) mg on alternate days, and of 81%, 50%, and 40% (p < 0.01) with no treatment at 4, 8, and 12 months, respectively. H. pylori that was found in 97% of patients at entry, at 4 wk was found in 92.8% of patients treated with OM 20 mg and in 97.5% of patients treated with OM 40 mg (NS). In one-third of the patients, H. pylori disappeared from the antrum but was found in the fundus. A 30% reduction in the presence of H. pylori was seen in the group treated with 20 mg daily for 12 months. Conclusions: We conclude that both continuous and alternate-day long-term OM treatment at 20 mg are similarly efficacious in the prevention of duodenal ulcer recurrence. Abstract

Aim. Irritable bowel syndrome (IBS) is frequently associated with an imbalance in intestinal bacteria. To date, few studies have evaluated the efficacy and safety of probiotic administration in patients with constipation-variant IBS. A new agent recently available in clinical practice is a symbiotic consisting of a probiotic, Bifidobacterium longum W11, and the short chain oligosaccharide prebiotic Fos Actilight. The aim of this study was to evaluate the efficacy and safety of this symbiotic in patients with constipation-variant IBS. Methods. A total of 636 patients (250 men, 386 women) diagnosed with constipation-type IBS according to the Roma II criteria were enrolled in 43 centers and received the symbiotic at a dose of 3 g/die for at least 36 days. A validated questionnaire investigating symptoms and stool frequency was administered before and after treatment. Results. Based on patient responses to visual scale items, frequency increased significantly after treatment in the "no symptom" class from 3% to 26.7% for bloating and from 8.4% to 44.1% for abdominal pain (P

Abstract

To evaluate whether deep infiltrating endometriosis (DIE) is associated with tubal alterations.|This was a retrospective study. Our study included 335 women with ovarian endometriosis (Group A), 66 women with DIE (Group B), and 72 women presenting with both conditions (Group C). We evaluated tubal morphology and patency during laparoscopic excision of endometriosis. Tubal patency was assessed by tubal dye test. Tubal morphology was determined using the tubal morphology score (TMS), obtained by a total grade of 4 parameters: tubal caliber, course, surface and fimbrial morphology.|There was no significant difference in the 3 groups regarding the presence of tubal occlusion (p = 0.23). Total TMS was not significantly different in the 3 groups (p = 0.13). A history of surgical treatment for endometriosis was associated with higher rate of tubal occlusion (p < 0.0005) and more severe morphological alterations (p < 0.0005). There was a positive correlation between number of previous surgical interventions and worse TMS (rho = 0.197, p < 0.0005).|Alterations in tubal patency and morphology were not significantly different in patients with DIE as compared to women with ovarian endometriosis. History of surgical interventions for endometriosis was related with the presence of tubal alterations.

Abstract

To report our experience on implementation and preliminary results of a decision-making model based on the recommendations of an Interdisciplinary Oncological Care Group developed for the management of colorectal cancer.The multidisciplinary team identified a reference guideline using appraisal of guidelines for research and evaluation (AGREE) tool based on a sequential assessment of the guideline quality. Thereafter, internal guidelines with diagnostic and therapeutic management for early, locally advanced and metastatic colonic and rectal cancer were drafted; organizational aspects, responsibility matrices, protocol actions for each area of specialty involved and indicators for performing audits were also defined.The National Institute for Health and Care Excellence (NICE) UK guideline was the reference for drafting the internal guideline document; from February to November 2013, 125 patients with colorectal cancer were discussed by and taken under the care of the Interdisciplinary Oncological Care Group. The first audit performed in December 2013 revealed optimal adherence to the internal guideline, mainly in terms of uniformity and accuracy of perioperative staging, coordination and timing of multi-modal therapies. To date, all patients under observation are within the diagnostic and therapeutic course, no patient came out from the multidisciplinary "path" and only in 14% of cases have the first recommendations proposed been changed. The selected indicators appear effective and reliable, while at the moment, it is not yet possible to assess the impact of the multidisciplinary team on clinical outcome.Although having a short observation period, our model seems capable of determining optimal uniformity of diagnostic and therapeutic management, to a high degree of patient satisfaction. A longer observation period is necessary in order to confirm these observations and for assessing the impact on clinical outcome.
