164. Syncope as initial symptom for nephrotic syndrome: a case report

Abstract

Although syncope and nephrotic syndrome are frequently encountered independently in pediatric practice, syncope as the initial symptom for nephrotic syndrome is rarely observed in the pediatric age group. In this report, we present the case of 3-year-old boy with nephrotic syndrome who presented with a history of three episodes of syncope before admission. The syncope occurred after excessive fluid loss or inadequate intake of fluids and was relieved spontaneously. History taking revealed that the early morning palpebral edema, and laboratory tests showed decreased plasma protein levels and elevated serum lipid levels. Nephrotic syndrome was diagnosed, but could not be confirmed histopathologically because the patient’s parent refused consent for biopsy. The patient was managed with fluid expansion, correction of acidosis, and improvement of microcirculation to prevent recurrence of syncope, and glucocorticoids were administered to prevent disease progression.

165. In Vitro Seeding Activity of Glycoform-Deficient Prions from Variably Protease-Sensitive Prionopathy and Familial CJD Associated with PrPV180I Mutation.

Abstract

Both sporadic variably protease-sensitive prionopathy (VPSPr) and familial Creutzfeldt-Jakob disease linked to the prion protein (PrP) V180I mutation (fCJDV180I) have been found to share a unique pathological prion protein (PrPSc) that lacks the protease-resistant PrPSc glycosylated at residue 181 because two of four PrP glycoforms are apparently not converted into the PrPSc from their cellular PrP (PrPC). To investigate the seeding activity of these unique PrPSc molecules, we conducted in vitro prion conversion experiments using serial protein misfolding cyclic amplification (sPMCA) and real-time quaking-induced conversion (RT-QuIC) assays with different PrPC substrates. We observed that the seeding of PrPSc from VPSPr or fCJDV180I in the sPMCA reaction containing normal human or humanized transgenic (Tg) mouse brain homogenates generated PrPSc molecules that unexpectedly exhibited a dominant diglycosylated PrP isoform along with PrP monoglycosylated at residue 181. The efficiency of PrPSc amplification was significantly higher in non-CJDMM than in non-CJDVV human brain homogenate, whereas it was higher in normal TgVV than in TgMM mouse brain homogenate. PrPC from the mixture of normal TgMM and Tg mouse brain expressing PrPV180I mutation (Tg180) but not TgV180I alone was converted into PrPSc by seeding with the VPSPr or fCJDV180I. The RT-QuIC seeding activity of PrPSc from VPSPr and fCJDV180I was significantly lower than that of sCJD. Our results suggest that the formation of glycoform-selective prions may be associated with an unidentified factor in the affected brain and the glycoform-deficiency of PrPSc does not affect the glycoforms of in vitro newly amplified PrPSc. •Decreased resolvin D1 (RvD1) and increased leukotriene B4 (LTB4) levels indicate impaired resolution of inflammation in NMOSD patients.•CSF AQP4-IgG may contribute to increased inflammation and lead to unresolved inflammation in NMOSD.•The dysregulation of Annexin A1 and IL-6 may account for the destruction of blood brain barrier and the existence of CSF AQP4-IgG.

Abstract

Graphene based polymer composites have attracted considerable attention for both fundamental studies and practical applications. Fabricating polymer/reduced graphene oxide (rGO) composites via a readily mass-producible and environment-friendly technique is imperative for industry production. In this work, we tried to add various kinds of chemical reductants as “modifier” and study the possibility of in situ chemical reduction of graphene oxide (GO) during traditional thermoplastic processing of polymer and GO mixture. The thermal analysis and structural characterization revealed that among the 4 kinds reductants we investigated, hydroquinone (HQ) can effectively reduce GO during the commonly used melt-blending or twin-roller miller mixing processing. For an ethylene vinyl acetate copolymer (EVA) composite prepared by melt-blending of EVA and GO (95:5 in mass) at 100 °C, the heat of reduction corresponds to the remove of oxygen-containing groups on GO in the composite is 50.6 J/g. In contrast, the value for the composite with 5 wt% HQ is only 1.6 J/g. The significant decreased values of reduction heat suggested the effective reduction of GO due to the addition of HQ. Although the reducing ability of HQ is weak relatively, our work confirms that it is possible to chemically reduce GO during traditional processing of polymer materials, which is helpful to develop a large-scale producible, cost-effective and environment-friendly approach for the fabrication of polymer/rGO composites.
