Epithelioid sarcoma is a rare soft tissue sarcoma and usually resistant to chemotherapy. It has high rates of local recurrence and distant metastasis, and the prognosis after metastasis is poor. We report a case of multiple lung metastases of an epithelioid sarcoma originating in the inguinal area that we treated with the multikinase inhibitor pazopanib. The patient was a 38-year-old male who began to experience discomfort in his left inguinal area. Magnetic resonance imaging showed a tumor extended from the medial aspect of the wing of the left ilium along the iliopsoas muscle to its site of insertion on the femur. The histopathological diagnosis with a biopsy was proximal-type epithelioid sarcoma. Although a positron emission tomography examination showed fluorodeoxyglucose accumulation in the left inguinal tumor, there was no distant metastasis. Wide resection by a combined iliac resection procedure was performed. Twelve months after surgery, computed tomography revealed multiple nodules and a diagnosis of bilateral multiple lung metastases was made. Treatment with pazopanib 800 mg was started. After 2.5 months of treatment, a clear reduction in the size of the pulmonary metastases was shown. Thirty months after the start of pazopanib treatment, most of the metastases have disappeared, and no development of new lesions has been seen. Therefore, it appeared that pazopanib was capable of serving as one of the choices of therapeutic agents that should be taken into consideration for the treatment of advanced epithelioid sarcoma.

Epithelioid sarcoma (ES) usually occurs in extremities. ES shows characteristic granulomatous morphologies, and its prognosis is not so poor. Proximal-type ES (PT-ES) is a variant of ES, and is characterized by central locations, severe atypical features, and poor prognosis. Both ES and PT-ES histologically show epithelial and sarcomatous patterns, and immunohistochemically express both vimentin and cytokeratins. A 77-year-old man presented with sore throat. The laryngoscope revealed a large polypoid tumor (4 × 3 × 4 cm) in upper pharynx, and a large biopsy was taken. The biopsy showed malignant epithelioid and spindle tumor cells. There were no apparent transitions between normal epithelial cells and tumor cells. The tumor consisted of malignant epithelioid and spindle cells with marked anaplasia. The tumor showed marked infiltrative features and lymphovascular permiations. Mitotic figures, atypical mitosis, and necrotic areas were present. No apparent features of sarcomas were seen. No rhabdoid cells were seen. Immunohistochemically, the malignant cells were positive for vimentin (3+, diffuse), CK AE1/3 (1+, focal), CK CAM5.2 (1+, diffuse), CK8 (1+, focal), CK18 (1+, focal), CD20 (1+, focal), p53 (3+, 84%), Ki-67 (labeling = 94%), CD68 (2+, only focal), and anti-trypsin (2+, only focal). They were negative for CK34BE12, CK5, CK6, CK7, CK14, CK19, CD99, CEA, CA19-9, CA125, EMA, S100 protein, NCAM, NSE, synaptophysin, chromogranin, α-smooth muscle actin, smooth muscle actin, desmin, CD34, CD31, CD45, D2-40, factor-VIII-related antigen, HMB-45, Melan-A, myoglobin, MDM2, CDK4, KIT, and PDGFRA. The author diagnosed this pharyngeal tumor as PT-ES, but the author cannot completely exclude the possibility of sarcomatoid carcinoma. The prognosis of patient was poor; the patient died of carcinomatosis 2 years after the manifestation.

BACKGROUND: Epithelioid sarcoma (ES) is a rare mesenchymal tumor that is divided into 2 types: classic and proximal. To the authors' knowledge, ES has been poorly studied in cytology, with fewer than 50 cases reported to date. The objective of the current study was to analyze the cytological and immunohistochemical information regarding 5 cases of ES. METHODS: Five cases of ES were analyzed: 4 of proximal type and 1 of classic type. The cases were taken from 4 female patients and 1 male patient. The patients ranged in age from 10 to 75 years. RESULTS: All smears obtained from the proximal type of ES were found to be hypercellular with a necrotic and inflammatory background. Smears from classic‐type ES were hypocellular with a hemorrhagic background. Large, dispersed epithelioid cells and loosely cohesive groups of cells were most frequently noted in all cases. All cases of proximal‐type ES demonstrated rhabdoid cells and marked nuclear atypia, but in only one case were rhabdoid cells found to be dominant. ES diagnoses were confirmed by immunohistochemistry in histological material. In 4 cases, epithelial marker expression was noted, whereas CD34 was found to be positive in only the classic type of ES. In 2 cases, total loss of SMARCB1/INI1 nuclear expression was observed. In the 2 SMARCB1/INI1–positive cases, loss of SMARCA2/BRM expression was observed in one case and partial loss was observed in the other case. CONCLUSIONS: The proximal type of ES differs from the classic type by the presence of rhabdoid cells and marked nuclear atypia. A specific immunohistochemical profile demonstrating loss of SMARCB1/INI1 or other proteins from the SWI/SNF complex also may be indicative of this diagnosis. The article presents a case study of a 55-year-old man who was admitted with painless lesion growing in the paratesticular location which on examination was determined to be paratesticular tumor or epithelioid sarcoma. It talks about the tumour being examined and determined with the help of computed tomography scan. It tells about the epithelioid sarcoma cells showing neoplastic cells similar to melanoma, carcinoma and rhabdoid tumor.

Introduction Myoepithelial carcinoma and proximal-type epithelioid sarcoma of the vulva are two rare malignancies with known aggressive behavior. In addition to a similar clinical course, these two disease entities also have significant histologic and pathologic overlap. Given the rarity of these malignancies, there is limited literature on the appropriate treatment regimen. Nevertheless, there is a consensus that early surgical resection is beneficial in both cases. Presentation We present a case of a patient who was initially diagnosed with myoepithelial carcinoma of the vulva with a differential later expanded to include possible epithelioid sarcoma. Discussion We demonstrate the importance in early identification of a SMARCB1 deficiency. Additionally, we suggest an appropriate treatment regimen for these patients going forward. Specifically, we encourage consideration of bilateral superficial and deep inguinal lymphadenectomies. Furthermore, sarcoma based chemotherapy regimens in the appropriate clinical setting may be beneficial in treating SMARCB1 deficient tumors. Additionally, there are now clinical trials involving EZH2 Inhibitors which may offer benefit for similar patients going forward. Conclusion This case highlights the difficulty in making a definitive diagnosis, and the importance in identifying a SMARCB1 deficiency as it will affect treatment options and may allow for enrollment in ongoing clinical trials. Epithelioid sarcoma is an uncommon soft tissue tumor of the hand characterized by high frequency misdiagnosis and high potential for local recurrence and regional lymph node metastasis. The recommended treatment is wide excision, which leaves complex tissue defects with tendon or neurovascular structure exposure requiring coverage. We present two patients who had tumor excision followed by wound coverage with dermal regeneration template and split-thickness skin graft. Aims Epithelioid sarcoma is a soft tissue sarcoma associated with a high rate of local recurrence after wide resection and high incidence of distant metastasis. Little is known about the clinical course and response to systemic treatments in epithelioid sarcoma patients. We carried out a retrospective analysis of clinical data from epithelioid sarcoma patients to provide a reference for the design of future epithelioid sarcoma-specific studies. Patients and methods Data from patients with epithelioid sarcoma entered in prospective multi-sarcoma phase II/III trials were pooled: EORTC trial 62012 (doxorubicin versus doxorubicin/ifosfamide), 62043 (pazopanib), 62072 (pazopanib versus placebo) and 62091 (doxorubicin versus trabectedin). Patients had either a local or a centrally confirmed diagnosis of epithelioid sarcoma, had inoperable/metastatic disease at study entry and were eligible for the according trial. Response was assessed according to RECIST 1.1. Progression-free survival (PFS) and overall survival were calculated from date of entry. Results Among 976 patients with advanced sarcomas, 27 epithelioid sarcoma patients (2.8%) were eligible for the analysis (17 men, median age at diagnosis 50 years, range 19–72). Eighteen (66.7%) received chemotherapy as first-line treatment (five doxorubicin, eight doxorubicin/ifosfamide, two pazopanib, three trabectedin) and nine (33.3%) received pazopanib as second line or later. The primary tumour was located in the lower extremity ( n = 8; 29.6%), upper extremity ( n = 5; 18.5%), retro/intra-abdominal ( n = 4; 14.8%) and in other locations ( n = 10; 37.0%). At entry, metastases were mainly found in lung ( n = 17; 63%), lymph nodes ( n = 9; 33.3%), bone ( n = 8; 29.6%) and soft tissue ( n = 7; 25.9%). The best response for first-line patients was four partial responses (22.2%), 10 stable disease (55.6%) and four progressive disease (22.2%). In subsequent lines, pazopanib achieved one partial response (11.1%), four stable disease (44.4%) and four progressive disease (44.4%). All patients but one progressed on treatment. The median PFS and overall survival were 3.8 (95% confidence interval 2.2–4.8) and 10.8 months (95% confidence interval 8.1–21.3), respectively. Five patients were still alive at the time of the according trial analysis. Conclusion With all limitations of such a rare disease and small data set, objective response and survival outcomes are similar in epithelioid sarcoma to non-selected sarcoma populations. Mesenchymal neoplasms rarely present in or adjacent to endocrine organs. In this context, the recognition of these rare tumor types can be challenging, with significant potential for misdiagnosis as sarcomatoid carcinomas (i.e., anaplastic thyroid carcinoma and sarcomatoid adrenal cortical carcinoma) or neuroendocrine carcinomas, depending upon the dominant histologic patterns. In this review, we address potential pitfalls in diagnosing selected mesenchymal neoplasms arising within or near endocrine organs, including dedifferentiated liposarcoma, synovial sarcoma, angiosarcoma, PEComa, proximal-type epithelioid sarcoma, Ewing sarcoma, and neuroblastoma. For each of these tumor types, we review clinical and pathologic features, histologic clues to distinguish them from endocrine neoplasms, and recently developed immunohistochemical markers that can be particularly useful for establishing the correct diagnosis. Circumscribed solid nodules are common in pulmonary metastases from sarcoma, but cystic pulmonary metastases from sarcomas are extremely rare. An unusual case of a 33-year-old man presenting with left spontaneous pneumothorax in association with cystic pulmonary metastases from an epithelioid sarcoma in the left forearm was discussed in the present study. The patient underwent wide resection of an epithelioid sarcoma in the left forearm 6 years prior. Periodical computed tomography revealed a gradual increase in the number and size of bilateral multiple cystic pulmonary metastases despite repeated chemotherapy treatment. At 33 years of age, the patient visited the outpatient department for sudden left chest pain and dyspnea. Plain radiography demonstrated spontaneous pneumothorax in the left lung; chest tube insertion and pleurodesis were successfully performed thereafter. At the last follow-up, 24 months after spontaneous pneumothorax, he is still alive and receiving chemotherapy treatment. Spontaneous pneumothorax is a common complication associated with cystic pulmonary metastases from epithelioid sarcoma. Orthopedic oncologists need to be aware of this unusual sarcomatous metastasis during the follow-up of advanced epithelioid sarcoma. [ABSTRACT FROM AUTHOR]

Purpose An epithelioid sarcoma is a rare histological subtype of a soft tissue sarcoma with a high local recurrence rate, which frequently shows lymph node metastasis. However, because of the rarity of this tumor, the impact of nodal metastasis and its appropriate management remain unclear. The present study investigated the clinical outcomes of patients with epithelioid sarcomas, with a focus on lymph node metastasis. Methods We retrospectively evaluated the clinical outcomes of 27 patients with epithelioid sarcomas treated between 1985 and 2015. The log-rank test was used to assess the prognostic variables. Results The overall local recurrence rate was 33%, and the estimated overall 5-year survival rate was 62%. Hand and foot locations were associated with favorable overall survival. During the follow-up period, new nodal metastasis was noted in 14 patients (52%). The incidence of local recurrence was higher in patients with new nodal metastasis than in patients who did not develop nodal metastasis. The development of new nodal metastasis had a tendency to worsen survival; however, this association was not statistically significant. Lymphadenectomy did not affect overall survival. Conclusions Peripheral tumor location is associated with a better prognosis. The development of new nodal metastasis tends to be associated with poor prognosis; however, among patients with nodal metastasis, resection of the metastatic lesions has a low impact on survival. The pseudomyogenic hemangioendothelioma (PMH) is a low-grade malignant vascular neoplasm of different tissue planes including skin and soft tissue. Primary tumors in the skeletal muscle and bone have also been diagnosed. The PMH was introduced into the WHO classification of tumors of soft tissue and bone in 2013. This is the first description of oral involvement. A 21-year-old female presented with a 2-month old swelling of her gingiva. The swelling appeared red in color and was soft in consistency. A clinical diagnosis of a pyogenic granuloma was made and an incisional biopsy was submitted for histopathological evaluation. The lesion consisted of a proliferation of spindle and epithelioid looking cells. Cells were arranged in loose fascicles and sheets. Rhabdomyoblast-like cells were also seen. No mitotic figures were present. Lesional cells were reactive to cytokeratin AE1/AE3 and CD31. Lesional cell reactivity to S100 protein, HMB 45, SMA, Desmin and CD34 was negative. Following the diagnosis, a wide excision for clear margins was performed. No recurrence has been reported 2 years since the removal. The PMH is a cutaneous tumor that behaves in an indolent fashion. This is the first report of oral involvement by this neoplasm. Recognition of its histopathological features and immunohistochemical reactivity will prevent misadventures in the diagnosis of oral lesions. [ABSTRACT FROM AUTHOR]
