Introduction: Ionizing radiation (IR) is effectively used in the treatment of oral malignancies; however, it might also significantly harm the surrounding tissues. Whey protein isolate (WP) is a protein derived from milk that exhibits a wide range of bioactivities. Therefore, the present research aimed to delineate the mitigating impact of WP against gamma irradiation-induced lingual damage.Methods: Rats were randomized into 5 groups: Control (saline, orally, 14 days), WP (WP; 0.5 g/kg b. w., orally, 14 days), IR (saline, orally, 14 days, exposed to 6 and 3 Gy on days 4 and 6, respectively), WP+IR (WP was given orally for 14 days before and after IR exposure; exposed to 6 and 3 Gy on days 4 and 6, respectively), and IR+WP (WP, orally, started 24 h after 1st IR exposure till the end of the experiment) groups. Samples were collected at two-time intervals (on the 7th and 14th days).Results and Discussion: Oxidative stress was stimulated upon IR exposure in tongue, indicated by boosted malondialdehyde (MDA) level, along with a decrease in the total antioxidant capacity (TAC) level, superoxide dismutase (SOD), and catalase (CAT) activities. Additionally, IR exposure depicted an increase of serum IgE, inflammatory cytokines, including tumor necrosis factor-α (TNF-α), interleukin (IL)-6, along with overexpression mRNA levels of nuclear factor kappa-B transcription factor/p65 (NF-κB/p65), and down-regulation of nuclear factor erythroid 2–related factor 2 (NRF2) and heme oxygenase (HO-1) mRNA levels in tongue tissue. Moreover, IR triggered alterations in lingual histological architecture. The antioxidant and anti-inflammatory properties of WP mitigated oxidative damage, inflammation, and desquamation that were brought on following IR exposure. The protective administration of WP markedly decreases IR-induced lingual harm compared to the mitigation protocol. Our findings recommend WP supplements to the diets of cancer patients undergoing IR that might aid radioprotective effects.

Multiple inhibitions of CA, COX-2 and 5-LOX enzymes has been recognised as a useful strategy for the development of anti-inflammatory drugs that can avoid the disadvantages of using NSAIDs alone. Here, we report new pyridazine-based sulphonamides (5a-c and 7a-f) as potential multi-target anti-inflammatory candidates. First, the furanone heterocycle in the dual CA/COX-2 inhibitor Polmacoxib was replaced with the pyridazinone one. Then, a hydrophobic tail was appended through benzylation of the 3-hydroxyl group of the pyridazinone scaffold to afford benzyloxy pyridazines 5a-c. Furthermore, the structures were adorned with the polar sulphonate functionality, in pyridazine sulphonates 7a-f, that are expected to be engaged in interactions with the hydrophilic half of the CA binding sites. All of the disclosed pyridazinones were tested for inhibitory activities against 4 hCA isoforms (I, II, IX, and XII), as well as against COX-1/2, and 5-LOX. Furthermore, in vivo anti-inflammatory and analgesic effects of pyridazinones 7a and 7b were examined.

AbstractAtorvastatin calcium (ATV) is a well-known anti-hyperlipidemic drug currently being recognized for possessing an anti-inflammatory effect. Introducing it as a novel remedy for periodontitis treatment necessitates developing a syringeable modified delivery system capable of targeting inflammation within the periodontal pockets. Thus, a 33 Box-Behnken design was used to generate eugenol enriched PEGylated cubosomes. Based on the desirability function, the optimized formulation (OEEPC) was selected exhibiting a solubilization efficiency (SE%) of 97.71 ± 0.49%, particle size (PS) of 135.20 ± 1.11 nm, polydispersity index (PDI) of 0.09 ± 0.006, zeta potential (ZP) of −28.30 ± 1.84 mV and showing a sustained drug release over 12 h. It displayed a cubic structure under the transmission electron microscope, furthermore, it was stable upon storage for up to 30 days. Hence, it was loaded into an optimum syringeable in-situ gel (ISG) which displayed the desired periodontal gelation temperature (34 ± 0.70 °C) and an adequate gelation time (46 ± 2.82 sec), it also released approximately 75% of the drug within 72 h. Clinical evaluation of the ISG showed a promising percentage reduction of about 58.33% in probing depth, 90% in the bleeding index, 81.81% in the plaque index, and 70.21% in gingival levels of transforming growth factor–β1. This proved that the formulated syringeable intra-pocket delivery system of ATV is an efficient candidate for diminishing inflammation in periodontitis.

BackgroundDeep brain stimulation (DBS) has shown promise in effectively treating chronic pain. This study aimed to assess the efficacy of DBS in this context.MethodsWe conducted a systematic literature search using PubMed, Scopus, and Web of Science, following the PRISMA guidelines. A well-constructed search strategy was utilized. Our literature search identified two groups of subjects: one group underwent DBS specifically for chronic pain treatment (DBS-P), while the second group received DBS for other indications (DBS-O), such as Parkinson’s disease or dystonia, with pain perception investigated as a secondary outcome in this population. Meta-analysis was performed using R version 4.2.3 software. Heterogeneity was assessed using the tau^2 and I^2 indices, and Cochran’s Q-test was conducted.ResultsThe analysis included 966 patients in 43 original research studies with chronic pain who underwent DBS (340 for DBS-P and 625 for DBS-O). Subgroup analysis revealed that DBS-P exhibited a significant effect on chronic pain relief, with a standardized mean difference (SMD) of 1.65 and a 95% confidence interval (CI) of [1.31; 2.00]. Significant heterogeneity was observed among the studies, with an I^2 value of 85.8%. However, no significant difference was found between DBS-P and DBS-O subgroups. Subgroup analyses based on study design, age, pain diseases, and brain targets demonstrated varying levels of evidence for the effectiveness of DBS across different subgroups. Additionally, meta-regression analyses showed no significant relationship between age or pain duration and DBS effectiveness for chronic pain.ConclusionThese findings significantly contribute to the expanding body of knowledge regarding the utility of DBS in the management of chronic pain. BackgroundAlthough observational studies have shown percutaneous patent foramen ovale (PFO) closure to be a safe means of reducing the frequency and duration of migraine, randomized clinical trials have not met their primary efficacy endpoints.ObjectivesThe authors report the results of a pooled analysis of individual participant data from the 2 randomized trials using the Amplatzer PFO Occluder to assess the efficacy and safety of percutaneous device closure as a therapy for episodic migraine with or without aura.MethodsThe authors analyzed individual patient-level data from 2 randomized migraine trials (the PRIMA [Percutaneous Closure of Patent Foramen Ovale in Migraine With Aura] and PREMIUM [Prospective Randomized Investigation to Evaluate Incidence of Headache Reduction in Subjects with Migraine and PFO Using the Amplatzer PFO Occluder Compared to Medical Management] studies). Efficacy endpoints were mean reduction in monthly migraine days, responder rate (defined as ≥50% reduction in monthly migraine attacks), mean reduction in monthly migraine attacks, and percentage of patients who experienced complete cessation of migraine. The safety endpoint was major procedure- and device-related adverse events.ResultsAmong 337 subjects, 176 were randomized by blocks to device closure and 161 to medical treatment only. At 12-month follow-up, the analysis met 3 of the 4 efficacy endpoints: mean reduction of monthly migraine days (-3.1 days vs. -1.9 days; p = 0.02), mean reduction of monthly migraine attacks (-2.0 vs. -1.4; p = 0.01), and number of subjects who experienced complete cessation of migraine (14 [9%] vs. 1 [0.7%]; p

Plasma processes enhance the high-quality, semiconductor fabrication employed in large-scale industries. A crucial element in this process are the many gases that generate different reactive species at low temperatures. Capacitive plasma discharge radio frequency CCPs play a major role in semiconductor scale fabrication. The analysis of nonlinear dynamics in CCPs are complicated even under simple approaches. Therefore, we study self consistent collision fluid model, particle in cell simulation PIC, step approximation and finally the global model to find the accurate solution of merging plasma sheath -bulk region. For this purpose we study the self consistent collisional fluid model to find the accurate charge–voltage distribution V(Q) which controls the nonlinear dynamics of CCPs. The results against the PIC simulations and the more elaborate model step approximation is accomplished with higher accuracy. Consequently, the cubic polynomial formula V(Q) are applied in the global model. Accurate analytical global sheath-bulk calculations of the higher harmonic effect-exhibiting average power distributions, bulk potentials, and currents are obtained. This study provides an effective solution of collision regime for any arbitrary plasma reactor designs, including single-frequency and double frequency CCPs. Moreover, we used the argon gas as a good example in our study for a good ion etching. Finally, we can assist in achieving effective results that are consistent with experimental data and in large-scale applications in the semiconductor sector.

Objective: To compare autogenous dentin nanoparticles with allograft bone grafts in the treatment of stage III periodontitis. Method: The randomised study was conducted at the Department of Oral Medicine and Periodontology, Faculty of Oral and Dental Medicine, Kafrelsheikh University Hospital, Egypt, from January 2021 to January 2022, and comprised adult patients of either gender with stage III periodontitis. Each patient’s bilateral intrabony defect was randomly treated with an allograft on one side and a graft made of dentin nanoparticles on the test side. Each patient’s removed tooth was ground into these nanoparticles. Both groups had their probing pocket depth and clinical attachment loss evaluated at baseline and six months after surgery. Additionally, digital periapical films were collected in both groups at baseline and six months after therapy to assess vertical bone loss. Data was analysed using SPSS 20. Results: Of the 20 patients, 8(40%) were males and 12(60%) were females with overall mean age 31.00±4.06 years (range: 18-50 years). Of the 40 sites, 20(50%) each were in test and control groups. Compared to baseline values, both groups showed significant improvement in probing pocket depth, clinical attachment loss and vertical bone loss postintervention (p0.05). Conclusion: Autogenous dentin nanoparticles were found to be an effective and promising biomaterial for bone regeneration in intrabony defects. Objective: To explore if the oral lesions in coronavirus disease-2019 patients are caused by the drugs used in the treatment or by the virus itself. Method: The cross-sectional study was conducted from September 2020 to September 2021 at the Kafrelsheikh University Hospital, Egypt, and comprised coronavirus disease-2019 patients of either gender aged 20-60 years having severe pneumonia and breathing difficulties who had no comorbidities. Based on the level of interleukin-6 and procalcitonin, the patients were classified into high group I receiving tocilizumab and methylprednisolone, medium group II receiving methylprednisolone alone, and low group III receiving antiviral drugs. The oral manifestations were recorded at the time of admission before treatment and at 2 weeks after the respective treatment. Data was analysed using SPSS 20. Results: Of the 90 patients, 30(33.3%) were in group I; 16(%) males and 14(%) females with mean age 44.82±6.10 years. Group II had 27(%) patients; 14(%) males and 13(%) females with mean age 43.74±4.87 years. Group III had 33(%) patients; 9(%) males and 14(%) females with mean age 42.66±2.51 years (p>0.05). There was no significant difference among the groups at baseline and after 2 weeks of treatment regarding oral manifestations. Objective: To evaluate the effect of interleukin-6 and procalcitonin levels in plasma on the development of oral manifestation in patients of coronavirus disease-2019. Methods: The case-control study was conducted from January to September 2021 at Kafrelsheikh University Hospital, Egypt, and comprised severe coronavirus disease. One hundred patients of either gender aged 30-60 years were included. The patients were divided into two equal groups, with group I having patients with oral manifestations, and group II had those without any oral symptoms. Plasma samples from both the groups were used to determine serum interleukin-6 and procalcitonin levels using electrochemiluminescence immunoassay. Data was analysed using SPSS 20. Result: Of the 100 patients, 50(50%) were in each of the two groups. Group I had 29(58%) males and 21(42%) females with overall mean age 44.83±6.12 years. Group II had 26(52%) males and 24(48%) females with overall mean age of 43.68±4.62 years. Interleukin 6 was significantly high in group I than in group II (p0.05). Conclusion: Interleukin-6 level could play an important role in the development of oral manifestation in coronavirus disease-2019 patients. Background: Depression is highly prevalent and a significant cause of mortality and morbidity across the globe. Although antidepressants and/or psychotherapy are often used to treat depression, some recent studies indicate that exercise may play an important role in lowering depression symptoms among patients meeting the clinical criteria of a depressive episode. Objectives: This study aimed to evaluate the mental health and wellbeing of adult participants diagnosed with major depressive disorder (MDD) after fourteen weeks of receiving a supervised physical exercise program. Methods: In a pre-post design, the assessments were performed at baseline, seven weeks and fourteen weeks following the exercise intervention program using facilitated self-reported psychometric scales. The Beck Depression Inventory (BDI-2) and Clinical Outcomes in Routine Evaluation—Outcome Measure (CORE-OM) were used to assess depression. The short form of the International Physical Activity Questionnaire (IPAQ) was used for the self-reporting of participants’ physical activity. Results: At the beginning of the study, the baseline total mean scores and standard deviations for the BDI-2, CORE-OM, and IPAQ in both genders did not differ significantly (p-value > 0.05). Patients showed significant improvement in all assessment scales after completing fourteen weeks of the physical exercise program. Based on the BDI-2, the baseline score dropped from 31.25 (indicating moderate to severe depression) to 14.25 (indicating mild to minimal depression), with a p-value of p-value < 0.005 indicating effective clinical improvement in depression symptoms. The IPAQ total MET-minutes/week mean score increased from 1713.22 to 4367.62, indicating an improvement in the participants’ weekly P.E. intake; however, the change was not statistically significant (p = 0.07). Conclusions: Exercise treatment is linked with considerable therapeutic improvement in patients with MDD, particularly when exercise is sustained over time. The BDI-2 and CORE-OM total scores significantly decreased after the fourteen-week P.E. programme, indicating a change from moderate and severe depression to minimal and mild depression. Our findings offer insightful information to primary care doctors and psychiatrists, indicating that prescribing exercise to depressed patients may be a helpful adjunctive therapy.

Background: This study assessed the potential cost-effectiveness of high (80–100%) vs low (21–35%) fraction of inspired oxygen (FiO2) at preventing surgical site infections (SSIs) after abdominal surgery in Nigeria, India, and South Africa. Methods: Decision-analytic models were constructed using best available evidence sourced from unbundled data of an ongoing pilot trial assessing the effectiveness of high FiO2, published literature, and a cost survey in Nigeria, India, and South Africa. Effectiveness was measured as percentage of SSIs at 30 days after surgery, a healthcare perspective was adopted, and costs were reported in US dollars ($). Results: High FiO2 may be cost-effective (cheaper and effective). In Nigeria, the average cost for high FiO2 was $216 compared with $222 for low FiO2 leading to a −$6 (95% confidence interval [CI]: −$13 to −$1) difference in costs. In India, the average cost for high FiO2 was $184 compared with $195 for low FiO2 leading to a −$11 (95% CI: −$15 to −$6) difference in costs. In South Africa, the average cost for high FiO2 was $1164 compared with $1257 for low FiO2 leading to a −$93 (95% CI: −$132 to −$65) difference in costs. The high FiO2 arm had few SSIs, 7.33% compared with 8.38% for low FiO2, leading to a −1.05 (95% CI: −1.14 to −0.90) percentage point reduction in SSIs. Conclusion: High FiO2 could be cost-effective at preventing SSIs in the three countries but further data from large clinical trials are required to confirm this.

Patent foramen ovale (PFO), an embryonic remnant of the fetal circulation, is present in 20-25% of adults. Although recent observational studies and clinical trials have established the link between PFO-mediated right-to-left shunting with cryptogenic stroke and migraine with aura, the role of a PFO in exacerbating hypoxemic medical conditions (ie, sleep apnea, chronic obstructive pulmonary disease, pulmonary hypertension, platypnea-orthodeoxia, pulmonary arteriovenous malformation, high-altitude pulmonary edema, and exercise desaturation) remains less understood. PFO-mediated hypoxemia occurs when deoxygenated venous blood from the right atrium enters and mixes with oxygenated arterial blood in the left atrium. Patients with an intracardiac right-to-left shunt may have profound hypoxemia out of proportion to underlying primary lung disease, even in the presence of normal right-sided pressures. The presence of right-to-left cardiac shunting can exacerbate the degree of hypoxemia in patients with underlying pulmonary disorders. In a subset of these patients, percutaneous PFO closure may result in marked improvement in dyspnea and hypoxemia. This review discusses the association between PFO-mediated right-to-left shunting with medical conditions associated with hypoxemia and explores the role of percutaneous PFO closure in alleviating the hypoxemia.

Presence of symptoms at baseline (adjusted odds ratio [aOR], 2.88; 95% CI 2.09– 3.97), contact with a confirmed case (aOR 2.34; 95% CI 1.37– 3.98), and unrestricted work type (aOR 4.71; 95% CI 3.24– 6.86) were associated with a higher risk of infection, while increasing age was associated with a lower risk of infection.Conclusion: Prevalence of SARS-CoV-2 infection as determined by seropositivity was higher among women who were not subject to workplace restrictions.Keywords: SARS-CoV-2, COVID-19, work-place restrictions, women, infections, Qatar

Quanta image sensor (QIS) is envisioned to be the next generation image sensor after CCD and CMOS. In this paper, we discuss how to design color filter arrays for QIS and other small pixels. Designing color filter arrays for small pixels is challenging because maximizing the light efficiency while suppressing aliasing and crosstalk are conflicting tasks. We present an optimization-based framework which unifies several mainstream color filter array design methodologies. Our method offers greater generality and flexibility. Compared to existing methods, the new framework can simultaneously handle luminance sensitivity, chrominance sensitivity, cross-talk, anti-aliasing, manufacturability and orthogonality. Extensive experimental comparisons demonstrate the effectiveness of the framework.

Quanta Image Sensor (QIS) is a single-photon detector designed for extremely low light imaging conditions. Majority of the existing QIS prototypes are monochrome based on single-photon avalanche diodes (SPAD). Passive color imaging has not been demonstrated with single-photon detectors due to the intrinsic difficulty of shrinking the pixel size and increasing the spatial resolution while maintaining acceptable intra-pixel cross-talk. In this paper, we present image reconstruction of the first color QIS with a resolution of $1024 \times 1024$ pixels, supporting both single-bit and multi-bit photon counting capability. Our color image reconstruction is enabled by a customized joint demosaicing-denoising algorithm, leveraging truncated Poisson statistics and variance stabilizing transforms. Experimental results of the new sensor and algorithm demonstrate superior color imaging performance for very low-light conditions with a mean exposure of as low as a few photons per pixel in both real and simulated images.
