155. Yenidoğanın Gastrointestinal Sistem Malformasyonları ile Birlikte Görülen Doğumsal Kalp Hastalıkları.

- Abstract
Aim: Our aim was to determine the incidence and distribution of congenital heart diseases (CHD) in newborns with gastrointestinal system malformations (GISM). Materials and Methods: The medical files of 32 successive newborns who were operated in the surgical intensive care unit of our hospital because of GISM, and had undergone cardiologic examinations were retrospectively reviewed, and the incidence of CHD in newborns with GISM was investigated. Results: A total of 32 patients with GISMs whose ages ranged between 1 and 35 days (median, 5.29 days) were evaluated. Our study population consisted of 15 (46%) male, and 17 (54%) female infants. Esophageal atresia/tracheoesophageal fistula was the most frequently seen GISM (n=13; 40.6%). One of these cases had anal atresia. Anal atresia was the second most frequently seen anomaly (n=7; 21.9%), followed by diaphragmatic hernia (n=6; 18.8%). These were followed by omphalocele (n=2), and pyloric stenosis, jejunal atresia, duodenal atresia, and Hirshsprung’s disease (for all, n=1). In 20 (20/32; 62.5%) patients CHD was observed. Eleven male (55%), and 9 (45 %) female patients had CHD. These patients were diagnosed with ASD (n=8), VSD (n=6) and Tetrology of Fallot (n=2), AVSD (n=1), PS (n=1), Bicuspid Aorta (n=1), Aortic insufficiency with MVP (n=1). Remaining patients had patent foramen ovale (PFO; n=9), patent ductus arteriosus (PDA; n=1), while 2 cases had normal echocardiographic findings. Conclusion: CHDs are more frequently and significantly encountered in newborns with GISM. Since these patients require early surgical intervention, even if no abnormality is found during physical examination, they should be evaluated by means of echocardiographic examinations for the presence of CHD. To determine the prevalence of CHD in Turkish population with GISM, larger scale patient series should be screened. [ABSTRACT FROM AUTHOR]

- Abstract
Purpose: Echocardiography is used in diagnosis and follow-up of congenital and acquired cardiovascular disease and in assessment of cardiac structure and function. There are many studies that used echocardiography to evaluate cardiovascular diseases but the number of the epidemiological studies is scarce. The aim of this study is to research the frequency of cardiovascular diseases detected by echocardiography. Methods: The study was conducted in May and June, 2010 in the Social health center located in Yigilca, the north-east of Duzce. 400 adult subjects (>17 years old) from each family physician representatively stratified for sex, age and for rural-urban distribution were randomly assigned and invited to participate the study. A total of 2231 subjects with a mean age of 50±15 (age range 18 to 92) were interviewed. Data were obtained by measurements and echocardiography. Results: There was significant difference between men and women in all measurements except deceleration time (DT), Ejection Time (ET) and pulmonary artery pressure PAP values (p<0.01). The prevalence of diastolic dysfunction (DD) was 67.6%. There was no significant difference between men and women (p>0.05). The prevalence of ascending aorta dilatation was 3.2%, heart failure was 1.7%, bicuspid aorta was 0.8%, aortic stenosis was 3.4%. Aortic regurgitation, tricuspid regurgitation and pulmonary regurgitation was detected in 38.9%, 52.8%, 12.4%. Conclusion: Because it is a portable and cheap method, the rate of using echocardiography in assessment of cardiovascular diseases increased. It should not ignore that by early detection and treatment, progression of diseases can be prevented and life quality of patients can be improved. Objective: Despite the fact that the vast majority of murmurs heard in children are innocent, the most common cause of pediatric cardiology consultation is cardiac murmur all around the world. The aim of this study was to compare the auscultation results with the findings of echocardiography. Material and Methods: We have assessed the auscultation and echocardiography results of 667 patients referred to the pediatric cardiology clinic with a diagnosis of cardiac murmur between December 2011 and June 2012. Results: The mean age of patients was 5.1 years and 54.6% of patients were male. There was no complaint accompanying cardiac murmur in 67.3% of the patients. The murmurs were considered pathological in 7.9% of patients. Echocardiographic examination was normal in 58.3% of patients, whereas various echocardiographic findings were detected in 41.7% of the patients. The most common findings were patent foramen ovale (PFO) (24.9%), atrial septal defect (ASD) (8.4%) and pulmonary stenosis (5.7%). There were various echocardiographic findings such as small ASD or trace valvular regurgitations in 38.4% of patients, while their murmurs were considered innocent. Accordingly, the results of physical examination and echocardiography showed statistically significant differences (p <0.001). After elimination of physiological mitral regurgitation and PFO, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy rate of physical examination to discriminate between normal and abnormal echocardiographic findings was 31%, 99.2%, 92.5%, 82.2% and 83% respectively. Conclusion: This study showed that some minor cardiac defects can be missed by auscultation in patients referred to pediatric cardiology clinics because of cardiac murmurs. This study aimed to demonstrate the link between the inflammatory process, cardio-metabolic risk factors and echocardiography data in type 2 diabetic (T2D) patients. A total number of fifty-two patients (12 male and 40 females) with a median age of 57 year were enrolled in this study. Anthropometric measurements that related to the cardio-metabolic risk factors and blood pressure were measured. Fasting serum glucose, lipid profiles and soluble interleukin -2 receptor (sIL-2R) using the technique of enzyme linked immunnosorbent assay (ELISA) were determined. The patients categorized according to the presence of metabolic syndrome components using the National Cholesterol Education program (NCEP) definition. Echocardiography data that related to systolic and diastolic left ventricular dysfunction were recorded using B-mode ultrasound with a frequency of 2-4MHz. There was non-significant difference in echocardiography data that related to systolic and/or diastolic dysfunction and the cardio-metabolic risk factors. The mean serum sIL-2R level was 3.469±3.062 pg/ml (range: 1.316-12.5); it inversely and non-significantly correlated with ejection fraction (r = - 0.12) and atherogenic index (r=-0.148). There is a link between the left ventricular systolic function and the circulating sIL-2R in asymptomatic T2D, which does not relate to the components of metabolic syndrome [ABSTRACT FROM AUTHOR]

- Abstract
The golden ratio is described, as 'the small is to large as the large is to whole. It is an irrational, continuous and non-repeating number and goes on forever. It has been found all throughout life and the physical cosmos. We hypothesized that there may be a golden ratio in the structure of the healthy heart. Healthy male individuals without any cardiovascular risk factors enrolled to this study. We obtained vertical and transvers cardiac dimensions from the 4-chambered echo view at the end of the diastole. Afterwards, the cardiac ratio (golden ratio of the heart) was accepted as the divine of the vertical dimension (large one) to the transvers dimension (small one). One hundred and five healthy male individuals (median (IQR) age 23 (3) years) were evaluated. The mean cardiac vertical dimension was 11.55 ± 0.84 cm and the mean cardiac transvers dimension was 7.82 ± 7.11 cm. The cardiac ratio was continued at approximately 1.6183 (0.02). We determined that there is a constant ratio between the overall cardiac vertical and transvers dimension similar to the golden ratio of of 1.618 in the healthy heart of the young Turkish male adults. 171. Romatoid artrit hastalarında sol ventrikül diyastolik disfonksiyonunu belirleyen faktörler.

- Abstract
Amaç: Romatoid artrit (RA) hastalarında, kardiyovasküler hastalık sıklığının ve bununla ilişkili ölümlerin genel popülasyona göre arttığı bilinmektedir. Sol ventrikül diyastolik disfonksiyonu (LVDD) ise kardiyak tutulumun erken bir bulgusu olarak görülebilmektedir. Çalışmamızın amacı RA hastalarında LVDD sıklığını ve buna etki eden faktörleri ortaya koymaktır. Yöntem: Kesitsel olarak dizayn edilen çalışmaya, 2010 ACR kriterlerine göre RA tanısı almış 83 hasta, kontrol grubu olarak ise 2009 ASAS kriterlerine göre spondiloartrit (SpA) tanısı alan 41 hasta ve 39 sağlıklı gönüllü dahil edilmiştir. İleri yaş (≥70), şiddetli obezite (BMI ≥35), diabetes mellitus, kardiyovasküler hastalık, kontrolsüz hipertansiyon ve kalp kapak hastalığı varlığı dışlama kriteri olarak belirlenmiştir. Sol ventrikül diastolik disfonksiyonu, transmitral akımlardan PW Doppler ile alınan erken diyastolik E dalga hızı, atriyum kontraksiyonunda A dalga hızı, E/A oranı, deselerasyon zamanı ve doku Doppleri ile mitral annulustan ölçülen E' hızı ve E/E' oranı ile değerlendirilmiştir. RA hastalarında LVDD'ye etki eden faktörler; multivariate lojistik regresyon analizleri ile incelenmiştir. Bulgular: Çalışmaya dahil edilen 83 RA hastasının (K/E: 68/15) yaş ortalaması 47,7±12,3 yıl, hastalık süresi 69 (IQR 42-144) ay idi. Gruplar arasında yaş, cinsiyet, hipertansiyon, hiperlipidemi ve obezite varlığı açısından fark saptanmadı (p>0,05). RA hastalarının 21 (%25,3) tanesi biyolojik DMARD kullanıyordu ve hastalık aktivite ortalaması (DAS 28) 3,36±1,2 idi. Ekokardiyografik değerlendirmede LVDD; RA hastalarında %32,5 (n=27), AS ve sağlıklı kontrol gruplarında ise %21,9 (n=9) ve %23,1 (n=9) oranında saptandı (p>0,05) (Tablo 1). RA hastalarında diyastolik disfonksiyon varlığını etkileyen değişkenlerin analizinde; yaş (p<0,01) ve sigara kullanımının (p=0,04) bağımsız risk faktörleri olduğu tespit edildi. Sonuç: Çalışmamızda RA hastalarında, kardiyak tutulumun erken göstergelerinden biri olan diyastolik disfonksiyonun bir miktar artmış olduğu gözlenmiş, ancak fark istatistiksel anlamlılığa ulaşmamıştır. Amaç: Çocuk kardiyoloji polikliniğimize kardiyak üfürüm nedeni ile gönderilen hastaların kardiyak üfürüm ve ekokardiyografi bulgularının değerlendirilmesi amaçlanmıştır. Materyal ve Metod: Hastaların üfürümleri masum ve patolojik olmak üzere ikiye ayrıldı. Hastalara elektrokardiyografik ve ekokardiyografik değerlendirme yapıldı. Bulgular: Çalışmamıza üfürüm ile başvuran 219 hasta alındı. Hastaların yaş ortalaması 21,61 ± 34,3 ay. Masum üfürümü olan 138 hastada şu an için tedavi gerektirmeyen patent foramen ovale (40 hasta), küçük sekundum atriyal septal defekt (1 hasta) saptandı. Objective: We sought to analyze short-term clinical, echocardiographic, and hemodynamic consequences of percutaneous mitral balloon valvuloplasty (PMBV) in the treatment of rheumatic mitral stenosis (MS) in our clinic. Methods: We retrospectively reviewed 53 patients (49 females, 5 males; mean age 38±11 years) who underwent PMBV for moderate or severe MS between January 2010 and October 2014. Procedural success was defined as the reaching a mitral valve area (MVA) >1.5 cm2 and absence of grade 3 mitral regurgitation. Results: We were able to perform PMBV in 48 of 53 patients (91%) and all the procedures that were completed were successful. PMBV couldn't be performed in 3 patients due to inability to pass the mitral valve, 1 patient due to membranous obstructive vena cava and 1 patient due inability to perform septostomi. Two patients who underwent successful PMBV became symptomatic after an average of 14 months follow-up and mitral valve replacement was performed due to moderate-to-severe MS. Patients were followed for an average of 13 ± 8 months and atrial fibrillation development or embolic complications were not observed. Conclusion: Percutaneous mitral balloon valvuloplasty procedure is a safe and effective treatment approach with high success and low complication rates in experienced centers. [ABSTRACT FROM AUTHOR]

Objective: To show etiology, diagnostic methods, and treatment options of patients with severe pericardial effusion determined after echocardiography. Methods: In this study, we retrospectively analyzed etiology, diagnosis and treatment options of 43 patients with severe pericardial effusions (i.e. effusions more than 20 mm either in front of the right ventricle or posterior to left ventricle as assessed by transthoracic echocardiography). The pericardiocentesis procedures were performed via subxiphoid approach. Glucose, protein, lactate dehydrogenase levels, polymerase chain reaction for tuberculosis, cytological, microbiological examinations and cultures were obtained from pericardial fluid. Results: Cardiac tamponade was diagnosed in 23 patients (54%) and pericardiocentesis was immediately performed in these cases. Twenty patients who were unresponsive to empirical treatment, underwent pericardiocentesis to evaluate etiology and treatment.. Pericardial fluid was found to be exudate in 36 patients (83.7%) and transudate in 7 patients (16.2%). The most common causes were malignancy (26%), and uremia (16%) while idiopathic cases constituted 23% of the patient group. While malignant pericardial effusion was more common in males, idiopathic etiology and uremia were more common in female patients. Conclusion: Pericardiocentesis is the gold standard for clarifying the etiology and is also a lifesaving measure for cardiac tamponade. Delineating the specific etiology is particularly important for cases that do not respond to empirical treatment. A thorough history and physical examination, together with pericardiocentesis in selected cases will enable the accurate diagnosis of specific etiology and starting the treatment for this etiology. Aortic aneurysm is potentially a lethal clinical condition unless it was diagnosed and managed earlier. It is associated with increased risk for sudden death among young subjects in competitive sports. So a complete medical evaluation is recommended especially for official athletes. However certain military personnel in Army are absolutely exposed to strenuous physical conditions and strengthening exercises as well as or harder than the athletes do. We presented three patients; one with giant aortic aneurysm at level of Valsalva sinus of aorta and two with aortic aneurysm at ascending aorta. First patient underwent surgical treatment whereas other two patients were followed medically. Isometric type of exercise and training activities were probably the main triggering factor for initiation and progression of aortic aneurysm in those subjects. Echocardiography is widely available and is the choice of method in preparticipation screening of certain military forces and diagnosing of aortic aneurysm. Also routine cardiac examination of military personnel in Army at a few years' periods may allow detection of those patients. [ABSTRACT FROM AUTHOR]

Familial hypercholesterolemia is a common autosomal dominant inherited condition. A mutation in the gene encoding the low density lipoprotein receptor, the apolipoprotein B gene, or one of three genes encoding the PCSK9 protein causes familial hypercholesterolemia. These mutations cause lifelong high levels of LDL cholesterol and early onset cardiovascular diseases. If left untreated, heterozygous patients have a significantly increased risk of early onset cardiovascular disease compared to those who do not have the mutations. In our study, it is aimed to contribute to the literature by comparing the cardiovascular status of patients with familial hypercholesterolemia in the pediatric age group followed up in our hospital with age, cholesterol levels and risk factors. In our study, 32 patients between the ages of 5-18 who were followed-up with the diagnosis of familial hypercholesterolemia in Hacettepe University Faculty of Medicine, Department of Pediatrics, Division of Pediatric Metabolism and Nutrition, and Division of Pediatric Cardiology, with total and LDL cholesterol levels above 95th percentile and 35 healthy control group with similar demographic characteristics and normal total and LDL cholesterol levels were included. When the patient group was compared with each other by ECHO, ECG, exercise test and carotid USG, it was observed that 3 patients had moderate mitral regurgitation, 3 patients had aortic regurgitation, 2 patients had aortic stenosis, and 1 patient had aortic stenosis and insufficiency. Interventricular septum and left ventricular posterior Wall thickness, left ventricular volume, and left ventricular mass index (99 ± 25.65 and 70.96 ± 18.12 g / m2, respectively, p = 0.015) in homozygous patients were found to be higher than heterozygous patients. When patients with a blood LDL cholesterol level below 200 mg / dL and those with 200 mg / dL and above were compared, the aortic STJ diameter measurements of patients with high LDL cholesterol levels (16.47 ± 2.58 and 19.65 ± 3, respectively, 20 mm p = 0.030), and IVCT measurements (75.60 ± 3.65 and 70.14 ± 9.72 ms, respectively, p = 0.024) were significantly longer. All global longitudinal strain measurements of homozygous FH patients were found to be lower than heterozygous patients, but only a statistically significant difference was found in GLPS-LAX measurement (15.70 ± 2.40% and 20.31 ± 3.26%, respectively, p = 0.028). Eccentric hypertrophy was detected in 2 patients and concentric remodeling in 3 patients. Ventricular geometry of 27 patients was normal. Exercise test was terminated due to exhaustion before reaching the target heart rate in 2 (28.6%) of 7 patients with homozygous FH and in 3 (12.5%) of 24 patients with heterozygous FH. There was no significant difference between carotid USG measurements. When the patient group and the healthy control group were compared in terms of ECG data, it was found that the P wave dispersion, PR dispersion and QT dispersion measurements of the patient group were significantly higher than the control group. QTc time measurements of the patient group were also significantly longer than the control group (416.96 ± 22.12 and 406.35 ± 17.28 ms, respectively, p = 0.023). As a result; familial hypercholesterolemia is a condition that has adverse effects on the cardiovascular system. Early detection of these patients with FH screening and early initiation of strict blood LDL cholesterol control will positively affect cardiovascular results. These patients should be under cardiology follow-up and ECHO and strain measurements should be done routinely. Thus, subclinical dysfunctions can be detected earlier. Çalışmamızda hastanemizde takip edilen çocuk yaş grubundaki ailesel hiperkolesterolemi tanılı hastaların kardiyovasküler durumunun yaş, kolesterol düzeyleri ve risk faktörleri ile olan ilişkilerinin kendi içinde karşılaştırılması ile literatüre katkı sağlamak amaçlanmaktadır. Çalışmamıza Hacettepe Üniversitesi Tıp Fakültesi, Çocuk Sağlığı ve Hastalıkları Anabilim Dalı, Çocuk Metabolizma Hastalıkları ve Beslenme Bilim Dalı ve Çocuk Kardiyoloji Bilim Dalı’nda ailesel hiperkolesterolemi tanısıyla takipli 5-18 yaş arasında, total ve LDL kolesterol düzeyleri 95 persentil üzeri olan 32 hasta ile aynı demografik özelliklere sahip total ve LDL kolesterol düzeyleri normal olan 35 kişilik sağlıklı kontrol grubu dahil edildi. Hasta grubu birbirleriyle EKO, EKG, efor testi ve karotis USG ile karşılaştırılarak değerlendirildiğinde 3 hastada orta derece mitral yetmezlik, 3 hastada aort yetmezliği, 2 hastada aort darlığı ve 1 hastada aort darlığı ve yetmezliği birlikte olduğu gözlendi. Homozigot hastaların interventriküler septum ve sol ventrikül posterior duvar kalınlıkları, sol ventrikül içi hacimleri ve sol ventrikül kütle indeksleri (sırasıyla 99±25,65 ve 70,96±18,12 g/m2 p=0,015) heterozigot hastalarına göre daha yüksek bulundu. Kan LDL kolesterol düzeyi 200 mg/dL’nin altında olan hastalar ile 200 mg/dL ve üzeri olan hastalar karşılaştırıldığında, LDL kolesterol düzeyi yüksek olan hastaların aort STB çapı ölçümlerinin (sırasıyla 16,47±2,58 ve 19,65±3,20 mm p=0,030) anlamlı derecede daha düşük, İVKZ ölçümlerinin ise (sırasıyla 75,60±3,65 ve 70,14±9,72 ms p=0,024) anlamlı derecede daha uzun olduğu gözlendi. Homozigot AH hastalarının tüm global longitüdinal strain ölçümleri heterozigot hastalara göre daha düşük bulundu, ancak yalnız GLPS-LAX ölçümünde istatistiksel olarak anlamlı fark saptandı (sırasıyla %15,70±2,40 ve %20,31±3,26 p=0,028). 2 hastada eksentrik hipertrofi, 3 hastada konsentrik remodelling tespit edildi. 27 hastanın ventrikül geometrisi normaldi. Homozigot AH tanılı 7 hastanın 2 (%28,6)’sinde ve heterozigot AH tanılı 24 hastanın 3 (%12,5)’ünde efor testi, hedef kalp hızına ulaşılamadan bitkinlik nedeniyle sonlandırıldı. Karotis USG ölçümleri arasında anlamlı bir fark bulunmadı. Hasta grubu ve sağlıklı kontrol grubu EKG verileri açısından karşılaştırılarak değerlendirildiğinde hasta grubun P dalga dispersiyonu, PR dispersiyonu ve QT dispersiyonu ölçümlerinin kontrol grubuna göre anlamlı derecede yüksek olduğu bulundu. Hasta grubun QTc süresi ölçümleri de kontrol grubuna göre anlamlı derecede daha uzundu (sırasıyla 416,96±22,12 ve 406,35±17,28 ms p=0,023). Sonuç olarak; ailesel hiperkolesterolemi, kardiyovasküler sistem üzerinde olumsuz etkileri olan bir durumdur. AH taraması ile bu hastaların erken tespit edilmesi ve sıkı kan LDL kolesterolü kontrolüne erken başlanması, kardiyovasküler sonuçlarını olumlu yönde etkileyecektir. Bu hastalar kardiyoloji takibinde olmalıdır ve EKO ile strain ölçümleri rutin yapılmalıdır. Böylece subklinik fonksiyon bozuklukları daha erken tespit edilebilir.

Objective: Peripartum cardiomyopathy (PPCM) has an unknown etiology and it is one of the complications exposing a life-threatening risk for pregnancy potentially. In our case report, we aimed to discuss the management of PPCM case developing at postpartum period. Case: In this case report, we analyzed a case which delivered at 37 weeks of gestation and had complaints of dyspnea, orthopnea and swelling feet. When severe left ventricle systolic dysfunction was observed in the examinations performed, other diagnoses were ruled out and the patient was diagnosed with PPCM. Conclusion: Peripartum cardiomyopathy should be considered certainly in the differential diagnosis of a patient who refers with the complaints of shortness of breath and swelling feet which are common during pregnancy. Long period of professionally training may induce adaptive changes in left ventricle (LV) dimensions in young sportsmen. Those changes may differ according to the type and frequency of training. In this study we aimed to evaluate the echocardiographic left ventricular parameters according the type of training especially in adult trainers. We retrospectively evaluated the parameters of LV recorded during the outpatient echocardiographic examination of adult sportsmen. Study groups were designed as the subjects who train sportive activity at least 2 or three times a week for a period of 1 hour and but differ in type of exercise content. Clinical features (age, weight, height, body mass index, systolic and diastolic blood pressure), end-diastolic and end-systolic diameters of LV, left atrium (LA), LV mass and mass index, LV ejection fraction (EF) were compared among groups. Age, weight, and height, BMI were not different among groups. Parameters of left ventricle were not different among groups. However left atrial diameter (35.9±2.0 vs 36.8±2.1, p=0.046) was significantly increased in isometric typed trainers. Diastolic diastolic pressure (72.4±3.6 vs 77.2±4.2, p=0.000) was significantly lowered in strength typed sportmen. Regular sports training may not induce pronounced changes in LV dimensions except physiologically left atrial dilatation observed in adult strength sports trainers. Strength sports training was favorably lowered diastolic blood pressures in adults. Regular exercise training combined strength and endurance sports would provide the maximum gain and healthier life for adults. [ABSTRACT FROM AUTHOR]

Objective: Non-dipper hypertension is associated with increased cardiovascular morbidity and mortality. The purpose of this study was to evaluate the association of non-dipping hypertension with aortic diameter in patients with metabolic syndrome. Materials and Methods: This study included 70 hypertensive patients with metabolic syndrome. These patients were evaluated with 24-h blood pressure Holter monitoring and divided into two groups of 35 patients each. Aortic diameter was measured by M-mode and two-dimensional echocardiography. These parameters were compared between the two groups. Results: In the dipper group, there were 26 female and 9 male patients with a mean age of 55±11 years. In the non-dipper group, there were 25 female and 10 male patients with a mean age of 56±11 years. No significant difference was found between the two groups in terms of basic characteristics. Non-dipper hypertensive patients had a higher thoracic aortic diameter value than dipper patients (35.6±2.4 and 33.23±1.1, p<0.01). Conclusion: These findings suggest that thoracic aortic diameter value is higher in patients with non-dipper hypertension. [ABSTRACT FROM AUTHOR]

Amaç: Koroner arter hastalığı tüm dünyada ve ülkemizde ölümün en önemli sebebidir. Koroner arter by-pass cerrahisi uygulanan hastaların risk düzeyi arttıkça postoperatif komplikasyonlarda artmaktadır. Bu çalışmanın amacı; koroner arter by-pass cerrahisi uygulanan hastalarda postoperatif erken dönemde oluşan kardiyak komplikasyonların tanı ve tedavisinde ekokardiyografinin rolünü araştırmaktır. Gereç ve Yöntem: Bu amaçla Haziran 2016 – Haziran 2017 tarihleri arasında kliniğimizde koroner arter by-pass cerrahisi uygulanan 53 hasta postoperatif dönemde 1 (bir) hafta boyunca günlük ekokardiyografik değerlendirme ile takip edildi.Bulgular: Hastalarda en az drenaj 300 cc iken en fazla 1200 cc (ortalama 460 cc) oldu. Perikardiyal efüzyon 1.günde 28 hastada, 2.günde 6 hastada, 3. günde ise sadece 1 hastada tespit edildi. İki hastada postop 0. gün kalbin ön yüzünde ciddi bası oluşturan ve hemodinamiyi etkileyen perikardiyal tamponat vardı. Bu hastalar yeniden operasyona alınarak hemodinamisi stabil hale getirildi.Sonuç: Koroner bypass yapılan hastaların Yoğun Bakım ünitelerindeki takibinde Ekokardiografinin rutin olarak kullanılmasının hastanede kalış süresini, morbiditeyi ve mortaliteyi önemli ölçüde azaltabileceği kanaatindeyiz., Purpose: Coronary artery disease is the leading cause of death all over the world and in our country. As the level of risk increases in the patients undergone Coronary artery bypass surgery, postoperative complications increases too. The aim of this study is to determine the role of echocardiography in the diagnosis and treatment of early postoperative complications in patients undergoing coronary artery bypass surgery. Materials and Methods: 53 patients undergone coronary artery bypass surgery between June 2016 and June 2017 were followed by daily echocardiographic evaluation during the postoperative period for one week.Results: The minimum drainage in patients was 300 cc and the maximum was 1200 cc (average 460 cc). Pericardial effusion was detected in 28 patients on day 1, 6 patients on day 2, and only 1 patient on day 3. Two patients had pericardial tamponade, which caused significant pressure on the anterior surface of the postop and day 0 and affected the hemodynamics. These patients were re-operated. Hemodynamics stabilized.Conclusion: We believe that the routine use of Echocardiography in the follow-up of intensive care units of patients with coronary bypass may significantly reduce the length of hospital stay, morbidity and mortality.

Amaç: Bu çalışmada hastanemizde izlenen prematürelerde gebelik haftasına göre patent duktus arteriosus (PDA) sıklığının, kendiliğinden kapanma oranlarının ve kapanmayanlarda oral tedavi etkinliğinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Yenidoğan yoğun bakım ünitemizde 2013-2016 yılları arasında PDA tanısıyla izlenen hastalar retrospektif olarak tarandı. Kardiyak incelemede yalnızca PDA tanısı konulan 231 hasta, gebelik haftasına göre gruplara ayrılarak ekokardiyografik ölçümleri karşılaştırıldı. Ekokardiyografik değerlendirme doğumdan sonra ortalama 4.6±15 günlük iken uygulandı. Hastaların demografik verilerin yanı sıra PDA boyutu, sol atrium/aorta oranı, PDA gradienti, sol ventrikül fonksiyonları değerlendirildi. Bulgular: Yenidoğan yoğun bakım ünitemize izlenen hastalar arasında PDA sıklığı %10,8 (231/2127 hasta) idi. Hastaların 41’inde (%17.7) anlamlı PDA vardı, bu hastalarda duktus çapı ortalamaları 2.5±0.5 ve sol atrium/aorta oranı 1.37±0.15 bulundu. Kapanma olmayan ve hemodinamik olarak anlamlı PDA’sı olan 8 (%19.5) hastaya oral ibuprofen verildi. Kapatma verilen hastaların tümünün gestasyonel yaşı

199. Echocardiographic evaluation of patients with subacute sclerosing panencephalitis.

Objective: Subacute sclerosing panencephalitis is a slowly progressive, inflammatory and neurodegenerative disease caused by virus infection in the central nervous system. Since there are a limited number of studies in the literature evaluating the cardiovascular functions of patients with SSPE, the present study evaluates the patients with SSPE using tissue Doppler echocardiography and compares them between the control group in order to shed some light on the subject. Methods: The study is a prospective observational study. 49 patients (17 female, 32 male) with SSPE were included in the study. Patients were divided into two groups: Stage 2 (n=29) and Stage 3 (n=20). Echocardiographic data were compared with a control group of 26 which is the same average age. All children underwent a detailed echocardiography, which contained an M-mode, pulse Doppler and tissue Doppler imaging. Results: Sinus tachycardia ( >100 beats/min in children) was detected in nineteen (38.7%) patients. There were not significant differences between parameters of systolic and diastolic function of the heart. Stage 2 group, EF: 69.9±6.4; SF: 39.2±5.58; and MPI (mitral): 0.38±0.03 and MPI (tricuspid): 0.39±0.10. And in the Stage 3 group, EF: 68.5±7.0, SF: 37.8±5.34, MPI (mitral): 0.37±0.09 and MPI (tricuspid): 0.38±0.12. In the control group EF:70.96±5.54; SF:39.96±5.05 and MPI(mitral): 0.35±0.06 MPI (tricuspid): 0.36±0.04 and statistically meaningful differences were not found between patients and control groups (p >0.05). Conclusion: Cardiac functions may be preserved and cardiac functions constitute no significant risks of mortality in the advanced stages of patients with Subacute sclerosing panencephalitis, which is a group of chronic and bedridden patients. 200. Frequency of Left Ventricular Hypertrophy, Mitral Annular Calsification and Aortic Calcification in Ischemic Stroke Subgroups.

- Abstract
Objective: We sought to determine whether each of mitral annular calcification (MAC), left ventricular hypertrophy (LVH) and aortic calcification (AC) contribute to the increased risk of acute ischemic stroke in patients with cryptogenic stroke. Material and Methods: Three hundred and thirty patients with acute ischemic stroke were evaluated with transthoracic echocardiography for MAC, LVH and AC. and these echocardiography findings were analyzed for any correlation with stroke etiology. Results: Three hundred and thirty patients with acute ischemic stroke were classified according to the etiology as cardioembolic (n=83), atherothrombotic (n=48), lacunar (n=49), cryptogenic (n=125), multiple etiological groups (n=25). Twenty (24.1%) patients with cardioembolic, 7 (14.6%) patients with atherotrombotic, 8 (16.3%) patients with lacunar, 20 (16%) patients with cryptogenic, 5 (8.3%) patients with multiple etiological factor had MAC (p>0.05). Sixty-four (77.1%) patients with cardioembolic, 41 (85.4%) patients with atherotrombotic, 41 (83.7%) patients with lacunar, 95 (76%) of patients with cryptogenic, 2 (8%) patients with multiple etiological factors had LVH (p>0.05). Forty-two (50.6%) patients with cardioembolic, 29 (60.4%) of patients with atherothrombotic, 31 (63.3%) of patients with lacunar, 76 (60.8%) patients with cryptogenic, 15 (10.6%) patients with multiple etiological factor had AC (p>0.05). Conclusion: This study shows that there is no association between the presence of mitral annular calcification, left ventricular hypertrophy and aortic calcification with cryptogenic stroke.
