Dracunculiasis is rarely reported in cats, yet over the last few years we have identified two cats with filarioid-like spirurid infections. Case 1 was a 9-year-old cat with pituitary-dependent hyperadrenocorticism from New York tate from which four adult dracunculoid nematodes were isolated from its torso. Based on morphometric characteristics and parasite geographic distribution, the specimens were identified as Dracunculus insignis females; at least one of the females was gravid, suggestive of patent infection. Species identification was confirmed through amplification and sequence analysis of nuclear and mitochondrial loci. Case 2 was a 14-year-old diabetic cat from Massachusetts. Formalin-fixed sections were obtained from a subcutaneous mass excised from the left foreleg. Histopathological examination revealed a large nematode with morphometrical characteristics of Dracunculus, surrounded by lymphocytes and sheets of eosinophils. These two cases appear to be the first published reports of dracunculiasis in domestic cats in the USA, and based on the findings from case 1, D insignis may be the species associated with both infections. People who develop fever, anemia and kidney failure after a blood transfusion may have babesiosis. This infection is caused by a protozoan called Babesis microti, which is transmitted by tick bites. Researchers report six people who were exposed to Babesis microti when they received blood components from a single blood donation by an infected person. Three tested positive for the protozoan and developed babesiosis. All were successfully treated. This infection could be very serious in the elderly and people with suppressed immune systems.

Antifilarial antibody testing has been established as a sensitive and specific method of diagnosing lymphatic filariasis. However, the development of serological responses to specific filarial antigens and their relationship to acquisition of infection is poorly understood. In order to evaluate whether the development of antigen specific antifilarial antibodies precedes microfilaremia and antigenemia, we compared the antibody responses of serum samples collected between 1990 and 1999 from a cohort of 142 Haitian children followed longitudinally. Antigen status was determined using the Og4C3 ELISA and the presence of microfilaremia was detected using microscopy. Antibody responses to Wb123, a Wuchereria bancrofti L3 antigen, were measured using a Luciferase Immunoprecipitation System (LIPS) assay. Antibody responses to Bm14 and Bm33, Brugia malayi antigens and to a major surface protein (WSP) from Wolbachia were analyzed using a multiplex bead assay. Over follow-up, 80 (56%) of the children became antigen-positive and 30 (21%) developed microfilaremia. Detectable antibody responses to Bm14, Bm33, Wb123, and WSP developed in 95%, 100%, 92%, and 29% of children, respectively. With the exception of WSP, the development of antibody responses generally preceded detection of filarial antigen. Our results show that antifilarial antibody responses can serve as an important epidemiological indicator in a sentinel population of young children and thus, may be valuable as tool for surveillance in the context of lymphatic filariasis elimination programs. Background: Morgellons is a poorly characterized constellation of symptoms, with the primary manifestations involving the skin. We conducted an investigation of this unexplained dermopathy to characterize the clinical and epidemiologic features and explore potential etiologies. Methods: A descriptive study was conducted among persons at least 13 years of age and enrolled in Kaiser Permanente Northern California (KPNC) during 2006-2008. A case was defined as the self-reported emergence of fibers or materials from the skin accompanied by skin lesions and/or disturbing skin sensations. We collected detailed epidemiologic data, performed clinical evaluations and geospatial analyses and analyzed materials collected from participants' skin. Results: We identified 115 case-patients. The prevalence was 3.65 (95% CI = 2.98, 4.40) cases per 100,000 enrollees. There was no clustering of cases within the 13-county KPNC catchment area (p = .113). Case-patients had a median age of 52 years (range: 17-93) and were primarily female (77%) and Caucasian (77%). Multi-system complaints were common; 70% reported chronic fatigue and 54% rated their overall health as fair or poor with mean Physical Component Scores and Mental Component Scores of 36.63 (SD = 12.9) and 35.45 (SD = 12.89), respectively. Cognitive deficits were detected in 59% of casepatients and 63% had evidence of clinically significant somatic complaints; 50% had drugs detected in hair samples and 78% reported exposure to solvents. Solar elastosis was the most common histopathologic abnormality (51% of biopsies); skin lesions were most consistent with arthropod bites or chronic excoriations. No parasites or mycobacteria were detected. Most materials collected from participants' skin were composed of cellulose, likely of cotton origin. Conclusions: This unexplained dermopathy was rare among this population of Northern California residents, but associated with significantly reduced health-related quality of life. In recent years, human cyclosporiasis has emerged as an important infection, with large outbreaks in the United States and Canada. Understanding the biology and epidemiology of Cyclospora has been difficult and slow and has been complicated by not knowing the pathogen's origins, animal reservoirs (if any), and relationship to other coccidian parasites. This report provides morphologic and molecular characterization of three parasites isolated from primates and names each isolate: Cyclospora cercopitheci sp.n. for a species recovered from green monkeys, C. colobi sp.n. for a parasite from colobus monkeys, and C. papionis sp.n. for a species infecting baboons. These species, plus C. cayetanensis, which infects humans, increase to four the recognized species of Cyclospora infecting primates. These four species group homogeneously as a single branch intermediate between avian and mammalian Eimeria. Results of our analysis contribute toward clarification of the taxonomic position of Cyclospora and its relationship to other coccidian parasites. [ABSTRACT FROM AUTHOR]

Context: The risk of acquiring babesiosis by blood transfusion is largely unknown since in areas where it is endemic it is often an asymptomatic infection.Objective: To investigate and treat a cluster of blood transfusion-associated babesiosis cases.Design: Case series and epidemiologic investigation.Setting: Urban inner-city hospital.Patients: Six persons who received Babesia microti-infected blood components from a donor.Main Outcome Measure: Diagnosis and successful therapy of babesiosis following transfusion.Results: Six individuals (1 adult, 1 child, and 4 neonates) were exposed to products from a single blood donation by an asymptomatic Babesia-infected donor. Three of the 6 exposed patients became parasitemic. Polymerase chain reaction testing, animal inoculation studies, and indirect immunofluorescent antibody testing were used to confirm the presence of Babesia microti in the donor's blood and to establish the presence of infection in 3 of the 6 recipients. The 3 infected recipients and 1 additional recipient were treated without incident.Conclusion: Physicians should consider babesiosis in the differential diagnosis of a febrile hemolytic disorder after blood transfusion. Necropsy of an adult, wild-born squirrel monkey (Saimiri sciureus) revealed 30 to 40 esophageal nematodes. The anterior end of each nematode was firmly attached to the mucosa, with the remaining portion free in the lumen. Histologic changes were limited to the superficial mucosa, where the epithelium adjacent to the nematode was hyperplastic and increased keratin formed a bridge over the body of the parasite. The nematode was identified as belonging to the genus Spirura. The number of parasites in this animal was considered significant and contributed to the death of the animal. [ABSTRACT FROM AUTHOR]

Cutaneous invasion by Dirofilaria (Nochtiella) repens is a very rare disorder in Belgium. A diagnosis of D. repens infection was macic in a Belgian woman. Itching and dermal linear infiltrations developed also in the patient's husband and baby, but for them a parasitologic diagnosis was not obtained. How this family acquired dirofilariasis is unclear. No evidence of this infection could be found in the family pet dog. Both husband and wife traveled to endemic regions, but the baby had never left Belgium. Infected mosquitoes might have been brought with the luggage or might have escaped from the international airport in the vicinity of the family home.

Objective: To characterize the etiologic agent (WA1) of the first reported case of babesiosis acquired in Washington State.Design: Case report, and serologic, molecular, and epizootiologic studies.Setting: South-central Washington State.Patient: A 41-year-old immunocompetent man with an intact spleen who developed a moderately severe case of babesiosis.Measurements: Serum specimens from the patient were assayed by indirect immunofluorescent antibody (IFA) testing for reactivity with seven Babesia species and with WA1, which was propagated in hamsters inoculated with his blood. A Babesia-specific, ribosomal-DNA (rDNA) probe was hybridized to Southern blots of restriction-endonuclease-digested preparations of DNA from WA1, Babesia microti, and Babesia gibsoni. Serum specimens from 83 family members and neighbors were assayed for IFA reactivity with WA1 and B. microti. Small mammals and ticks were examined for Babesia infection.Results: The patient's serum had very strong IFA reactivity with WA1, strong reactivity with B. gibsoni (which infects dogs), but only weak reactivity with B. microti. DNA hybridization patterns with the rDNA probe clearly differentiated WA1 from B. gibsoni and B. microti. Four of the patient's neighbors had IFA titers to WA1 of 256. The tick vector and animal reservoir of WA1 have not yet been identified, despite trapping 83 mammals and collecting 235 ticks.Conclusions: WA1 is morphologically indistinguishable but antigenically and genotypically distinct from B. microti. Some patients elsewhere who were assumed to have been infected with B. microti may have been infected with WA1. Improved serodiagnostic and molecular techniques are needed for characterizing Babesia species and elucidating the epidemiology of babesiosis, an emergent zoonosis. [ABSTRACT FROM AUTHOR]

Most cases of babesiosis reported in the United States have been tickborne and caused by Babesia microti, the etiologic agent of all previously described transfusion-transmitted cases. A 76-year-old man with the first recognized case of transfusion-transmitted infection with the recently identified WAl-type Babesia parasite is described. The subject received multiple blood transfusions in 1994. Indirect immunofluorescent antibody testing of serum from 57 blood donors implicated a 34-year-old man (WA1 titer, 1:65,536) whose donation had been used for packed red cells. Isolates of the organisms that infected the recipient and the donor, both of whom were spleen-intact residents of Washington State, were obtained by hamster inoculation. The DNA sequence of a 536-bp region of the nuclear small subunit—rRNA gene of both isolates was identical to that of WA1 (isolated in 1991 from the index WA1 case-patient). Antifilarial IgG4 is a marker of active filarial infection; however, 10% of microfilaremic persons may have low levels of antifilarial IgG4. To gain insight into how persons with microfilaremia with low antifilarial IgG4 levels (<10 µ/mL) differ from those with high levels (>170 µg/mL), total IgG4 and IgE and filaria-specific IgE, IgG1, and IgG2 were measured by ELISA in serum samples collected from 85 microfilaremic Haitians. Persons with lower levels of antifilarial IgG4 had significantly lower total IgG4 and total IgE (P < .01), lower levels of antifilarial IgG1, IgG2, and IgE (P < .01, = .03, and < .01, respectively), and higher antigenemia and microfilaremia (P < .01) than did persons with higher levels of antifilarial IgG4. Increased antigen loads in microfilaremic persons may be associated with a down-regulation of antibody production, which extends to all isotypes. [ABSTRACT FROM PUBLISHER]
