Abstract: Diabetes mellitus in early pregnancy increases the risk in infants of birth defects, such as neural tube defects (NTDs), known as diabetic embryopathy. NTDs are associated with hyperglycemia-induced protein misfolding and Caspase-8-induced programmed cell death. The present study shows that misfolded proteins are ubiquitinylated, suggesting that ubiquitin-proteasomal degradation is impaired. Misfolded proteins form aggregates containing ubiquitin-binding protein p62, suggesting that autophagic-lysosomal clearance is insufficient. Additionally, these aggregates contain the neurodegenerative disease-associated proteins α-Synuclein, Parkin, and Huntingtin (Htt). Aggregation of Htt may lead to formation of a death-inducing signaling complex of Hip1, Hippi, and Caspase-8. Treatment with chemical chaperones, such as sodium 4-phenylbutyrate (PBA), reduces protein aggregation in neural stem cells in vitro and in embryos in vivo. Furthermore, treatment with PBA in vivo decreases NTD rate in the embryos of diabetic mice, as well as Caspase-8 activation and cell death. Enhancing protein folding could be a potential interventional approach to preventing embryonic malformations in diabetic pregnancies.

153. Efficacité d’une intervention de promotion et d’amélioration de l’accessibilité à la vaccination antigrippale pour les soignants d’un centre hospitalier général

Abstract: Introduction La couverture vaccinale antigrippale des soignants en France est particulierement basse (35 %). Notre centre hospitalier souffre egalement d’une faible couverture vaccinale (9 %). Une enquete sur les freins a la vaccination menee en 2018–2019 avait revele que le manque de temps et d’information sur cette vaccination etait des motifs frequents de non-recours parmi les hesitants. Materiels et methodes Une intervention a ete construite et a associe une campagne d’information large et repetee (mailing, affichage de messages humoristiques, document joint aux fiches de paie repondants aux fausses idees et argumentant l’interet de la vaccination, stands d’information), elargissement des modalites de vaccination (vaccination au sein des services par les collegues IDE et medecins, vaccination sans rendez-vous au centre de vaccinations internationales, vaccination sans rendez-vous en medecine du travail dont une tot a la sortie des equipes de nuit), sensibilisation a la vaccination au sein des services par l’EOH avec possibilite de se faire vacciner au decours (y compris a 2 reprises pour les equipes de nuit) et tenue de 2 stands d’information avec offre de vaccination. Resultats La mise en place de ces actions a permis de doubler le nombre d’agents vaccines (432 vs 226 en 2018/2019), soit une couverture vaccinale estimee des soignants de 17 %. La couverture vaccinale etait meilleure chez les cadres de sante (45 %) et les medecins (23 %) que chez les aides-soignants (11 %), les ASH (15 %), les IDE (19 %) et les sages-femmes (18 %). La couverture differait aussi entre les poles : de 36 % pole medico-technique, 33 % pole medecine, 27 % pole urgence-rea, a 22 % pole chirurgie, 17 % pole geriatrie et 4 % pole psychiatrie. Parmi les 432 personnes s’etant fait vaccinees cette annee, 20 % avaient deja ete vaccines mais pas l’annee precedente et 20 % n’avaient jamais ete vaccines. La vaccination par les pairs (dans ¾ des cas par un/e IDE) directement au sein du service a ete largement plebiscite (66 % des vaccines) et les offres complementaires ont permises de realiser pres d’une vaccination sur 4 (equipe mobile 12 % pour 25 interventions aupres de 205 agents, CVI 6 %, service de sante au travail 10 %). Les interventions de l’EOH aupres des equipes de nuit n’ont permis de vacciner que 10 agents et l’ouverture de la consultation en medecine du travail en sortie d’equipe de nuit que 3 agents. Conclusion La mise en place d’une sensibilisation du personnel a l’importance de la vaccination antigrippale et la facilitation de l’acces au vaccin, en particulier par la vaccination au sein des services, a permis de doubler la couverture vaccinale dans un CHG ayant une couverture historique tres basse. Ces resultats invitent a lutter contre les freins a la vaccination par les pairs dans les services et a en faciliter l’organisation.

Abstract: BACKGROUND Autophagy is highly active in neuroepithelial cells of the developing neuroepithelium, and impaired autophagy leads to neural tube defects (NTDs). We have demonstrated that maternal diabetes induces NTDs, and that impaired autophagy and consequent cellular imbalance, including the endoplasmic reticulum (ER), where critical events occur leading to the induction of diabetic embryopathy. Because the mammalian target of rapamycin (mTOR) pathway suppresses autophagy, we hypothesize that p70S6K1 (70 kDa ribosomal protein S6 kinase 1), a major downstream effector of mTOR, mediates the inhibitory effect of maternal diabetes on autophagy in the developing neuroepithelium. OBJECTIVE We investigated whether p70S6K1 mediates the inhibitory effect of maternal diabetes on autophagy during neurulation. We also examined if p70S6K1 deficiency restores autophagy and thus relieves ER stress and inhibits maternal diabetes-induced apoptosis, which leads to reduction in NTD incidence in diabetic embryopathy. STUDY DESIGN Female p70S6K1 heterogeneous knockout (p70S6K1+/-) mice were bred with male p70S6K1 heterogeneous knockout (p70S6K1+/-) mice to generate wild type (WT), p70S6K1+/- and p70S6K1 knockout (p70S6K1-/-) embryos. Embryos at embryonic day 8.5 (E8.5) were harvested for the assessment of indices of autophagy, ER stress and apoptosis. NTDs incidence was determined in E10.5 embryos. For in vitro studies, siRNA knockdown of p70S6K1 in C17.2 mouse neural stem cells were used to determine the effect of p70S6K1 deficiency on autophagy impairment and ER stress under high glucose conditions. RESULTS Knockout of the Rps6kb1 gene, which encodes for p70S6K1, ameliorated maternal diabetes-induced NTDs and restored autophagosome formation in neuroepithelial cells suppressed by maternal diabetes. Maternal diabetes-suppressed conversion of LC3-I (Microtubule-associated protein 1A/1B-light chain 3) to LC3-II, an index of autophagic activity, in neurulation stage embryos was abrogated in the absence of p70S6K1. p70S6K1 knockdown in neural stem cells also restored autophagosome formation and the conversion of LC3-I to LC3-II. The activation of the major unfolded protein response (UPR), indicated by phosphorylation of IRE1α, PERK and eIF2α, and the increase of the endoplasmic reticulum (ER) stress marker, CHOP, were induced by maternal diabetes in vivo and high glucose in vitro. UPR and ER stress induced by maternal diabetes or high glucose were diminished by Rps6kb1deletionor p70S6K1 knockdown, respectively. Rps6kb1knockoutblocked maternal diabetes-induced caspase cleavage and neuroepithelial cell apoptosis. The SOD memetic Tempol abolished high glucose-induced p70S6K1 activation. CONCLUSION We revealed the critical involvement of p70S6K1 in the pathogenesis of diabetic embryopathy.

Abstract: Tracker group of the CMS collaboration: et al., A new CMS tracker detector will be installed for operation at the High Luminosity LHC (HL-LHC). This detector comprises modules with two closely spaced parallel sensor plates and front-end ASICs capable of transmitting tracking information to the CMS Level-1 (L1) trigger at the 40 MHz beam crossing rate. The inclusion of tracking information in the L1 trigger decision will be essential for selecting events of interest efficiently at the HL-LHC. The CMS Binary Chip (CBC) has been designed to read out and correlate hits from pairs of tracker sensors, forming so-called track stubs. For the first time, a prototype irradiated module and a full-sized module, both equipped with the version 2 of the CBC, have been operated in test beam facilities. The efficiency of the stub finding logic of the modules for various angles of incidence has been studied. The ability of the modules to reject tracks with transverse momentum less than 2 GeV has been demonstrated. For modules built with irradiated sensors, no significant drop in the stub finding performance has been observed. 156. Endometrium-free uterine closure technique and abnormal placental implantation in subsequent pregnancies

Abstract: Abnormal placentation can result in massive hemorrhage, which is the leading cause of severe maternal morbidities and mortality in its management. Over the past 50 years, the incidence of placenta previa (PP), abnormal implantation of the placenta, and cesarean scar pregnancy have continued to rise. This coincides with the well-documented parallel rise in the rate of cesarean deliveries, the performance of multiple repeat cesarean deliveries and the adoption of newer uterine closure techniques. However, no studies have examined the role of uterine closure techniques in abnormal placentation in women with a history of a prior cesarean delivery.To assess the practicality of one specific uterine closure technique at cesarean delivery and to evaluate the relationship between previous cesarean delivery and subsequent development of abnormal implantation of the placenta, as well as neonatal and other perioperative outcomes after receiving an endometrium-free uterine closure technique.This retrospective observational study considered cesarean deliveries (In contrast to published data, independent of the number of repeat cesarean deliveries, the presence of 26 (3.1%) PPs and of 366 (43.8%) anterior placentas, there were no patients with abnormal implantation of the placenta in a cesarean scar, neither prenatally nor at delivery. Maternal hemorrhage, postoperative and neonatal complications did not reach clinical significance. The statistical analysis revealed that, when compared with women who had fewer repeat cesarean deliveries using endometrium-free uterine closure technique, those with the most had a lesser risk of forming PP and less blood loss, as measured by both hematocrit and hemoglobin evaluation.In this retrospective cohort study, the exclusion of the endometrium during the endometrium-free uterine closure technique was associated with fewer placental abnormalities in subsequent pregnancies and reduced life-threatening maternal morbidity for future cesarean deliveries.

Abstract: Background: Atrial fibrillation (AF) burden and duration appear to be related to stroke risk. A wearable consumer electronic device could provide long-term assessment of these measures inexpensively and noninvasively. This study compares the accuracy of an AF-sensing watch (AFSW; Apple Watch with KardiaBand) with simultaneous recordings from an insertable cardiac monitor (ICM; Reveal LINQ). Methods: SmartRhythm 2.0, a convolutional neural network, was trained on anonymized data of heart rate, activity level, and ECGs from 7500 AliveCor users. The network was validated on data collected in 24 patients with ICMs and a history of paroxysmal AF who simultaneously wore the AFSW with SmartRhythm 0.1 software. The primary outcome was sensitivity of the AFSW for AF episodes ≥1 hour. Secondary end points included sensitivity of the AFSW for detection of AF by subject and sensitivity for total AF duration across all subjects. Subjects with >50% false-positive AF episodes on ICM were excluded. Results: We analyzed 31 348.9 hours (mean (SD), 11.3 (4.4) hours/day) of simultaneous AFSW and ICM recordings in 24 patients. The ICM detected 82 episodes of AF ≥1 hour while the AFSW was worn, with a total duration of 1127.1 hours. Of these, the SmartRhythm 2.0 neural network detected 80 episodes (episode sensitivity, 97.5%) with a total duration of 1101.1 hours (duration sensitivity, 97.7%). Three of the 18 subjects with AF ≥1 hour had AF only when the watch was not being worn (patient sensitivity, 83.3%; or 100% during time worn). Positive predictive value for AF episodes was 39.9%. Conclusions: An AFSW is highly sensitive for detection of AF and assessment of AF duration in an ambulatory population when compared with an ICM. Such devices may represent an inexpensive, noninvasive approach to long-term AF surveillance and management.

Abstract: Pregnancies complicated by preexisting maternal diabetes mellitus are associated with a higher risk of birth defects in infants, known as diabetic embryopathy. The common defects seen in the central nervous system result from failure of neural tube closure. The formation of neural tube defects (NTDs) is associated with excessive programmed cell death (apoptosis) in the neuroepithelium under hyperglycemia-induced intracellular stress conditions. The early cellular response to hyperglycemia remains to be identified. We hypothesize that hyperglycemia may disturb intracellular calcium (Ca(2+)) homeostasis, which perturbs organelle function and apoptotic regulation, resulting in increased apoptosis and embryonic NTDs. In an animal model of diabetic embryopathy, we performed Ca(2+) imaging and observed significant increases in intracellular Ca(2+) ([Ca(2+)](i)) in the embryonic neural epithelium. Blocking T-type Ca(2+) channels with mibefradil, but not L-type with verapamil, significantly blunted the increases in [Ca(2+)](i), implicating an involvement of channel type-dependent Ca(2+) influx in hyperglycemia-perturbed Ca(2+) homeostasis. Treatment of diabetic pregnant mice with mibefradil during neurulation significantly reduced NTD rates in the embryos. This effect was associated with decreases in apoptosis, alleviation of endoplasmic reticulum stress, and increases of anti-apoptotic factors. Taken together, our data suggest an important role of Ca(2+) influx in hyperglycemia-induced NTDs and of T-type Ca(2+) channels as a potential target to prevent birth defects in diabetic pregnancies.

Erratic insulin patterns (4) in lactating women with type 1 diabetes may [...], Objective To describe glycemia and insulin administration in lactating women with type 1 diabetes and compare their energy intake, lactation onset, and postpartum weight to lactating women without diabetes. Design A prospective observational study conducted at 2, 3, 6, 14, 42, and 84 days postpartum. Insulin dosage and glucose levels after fasting and at 80 and 110 minutes after eating (by measuring capillary blood glucose concentrations and glycated hemoglobin [Hb[A.sub.1c]] values) are described for women with type 1 diabetes. Dietary intake, weight, and lactation onset for women with type 1 diabetes are compared with those without the disease. Subjects/setting Fourteen lactating women with type 1 diabetes and 25 women without diabetes (control subjects) were recruited from private obstetrician offices and high-risk prenatal clinics at 3 major medical centers. Statistical analyses A repeated-measures 2-factor analysis of variance was used to determine group, time, and interaction effects between women with type 1 diabetes and the control group at 2, 3, 7, 14, 42, and 84 days postpartum. Results Seven women with type 1 diabetes breast-fed through 84 days postpartum, although they perceived later onset of milk and had fasting and postprandial hyperglycemia. Their Hb[A.sub.1c] levels increased by 20%, confirming relative hyperglycemia. After adjustment for prepregnancy weight, women with type 1 diabetes consumed 7 kcal fewer per kilogram per day than control subjects. Average insulin dose was returned to prepregnancy levels of 45.3±3.6 U/d (least square means±standard error of the mean) by 1 week. Application Achieving desired metabolic control during the establishment of lactation is difficult. A better understanding of energy and insulin needs and their relationship to lactation in women with type 1 diabetes is needed.

Abstract: Many biomarker-based diagnostic methods are inhibited by nontarget molecules in patient samples, necessitating biomarker extraction before detection. We have developed a simple device that purifies RNA, DNA, or protein biomarkers from complex biological samples without robotics or fluid pumping. The device design is based on functionalized magnetic beads, which capture biomarkers and remove background biomolecules by magnetically transferring the beads through processing solutions arrayed within small-diameter tubing. The process was automated by wrapping the tubing around a disc-like cassette and rotating it past a magnet using a programmable motor. This device recovered biomarkers at ~80% of the operator-dependent extraction method published previously. The device was validated by extracting biomarkers from a panel of surrogate patient samples containing clinically relevant concentrations of (1) influenza A RNA in nasal swabs, (2) Escherichia coli DNA in urine, (3) Mycobacterium tuberculosis DNA in sputum, and (4) Plasmodium falciparum protein and DNA in blood. The device successfully extracted each biomarker type from samples representing low levels of clinically relevant infectivity (i.e., 7.3 copies/µL of influenza A RNA, 405 copies/µL of E. coli DNA, 0.22 copies/µL of TB DNA, 167 copies/µL of malaria parasite DNA, and 2.7 pM of malaria parasite protein).

Abstract: The effects of hair dressing effluent on soil chemical properties, germination and growth of maize and cowpea were investigated in pot experiment. The experiment was laid out in a completely randomized design of 3 effluent treatments (100 ml, 200 ml and 400 ml) and control with 5 replications at University of Port Harcourt Centre for Ecological Studies. Results showed that pH ranged between 6.8±0.2 – 7.5±0.4 for both control and treatments, total organic carbon (TOC) increased from 1.56±0.4% in control to 1.95±0.09 - 2.55±0.05% in treatments. Oil and grease increased from 24.20±2.64 ppm in control to 48.70±4.10 - 129.80±3.00 ppm in treatments. Germination percentage were 98% and 48% in control for maize and cowpea respectively and ranged between 84 – 88% and 12 – 32% in treatments for maize and cowpea respectively. Shoot lengths were 27.6±5.8 cm and 20.4±1.4 cm in control for maize and cowpea respectively and ranged between 29.1±4.1 – 32.2±1.0 cm and 18.5±1.7 – 19.8±7.8 cm in treatments for maize and cowpea respectively. Fresh weights were 100.4±7.5 g and 52.3±8.7 g in control for maize and cowpea respectively while it ranged between 97.8±2.9 g – 111.0±6.4 g and 42.0±5.1 – 58.2±4.0 g in treatments for maize and cowpea respectively. In conclusion, hair dressing effluent had no effect on soil pH but increased soil TOC and O & G. Abstract: In 2017 a new pixel detector was installed in the CMS detector. This so-called Phase-1 pixel detector features four barrel layers in the central region and three disks per end in the forward regions. The upgraded pixel detector requires an upgraded data acquisition (DAQ) system to accept a new data format and larger event sizes. A new DAQ and control system has been developed based on a combination of custom and commercial microTCA parts. Custom mezzanine cards on standard carrier cards provide a front-end driver for readout, and two types of front-end controller for configuration and the distribution of clock and trigger signals. Before the installation of the detector the DAQ system underwent a series of integration tests, including readout of the pilot pixel detector, which was constructed with prototype Phase-1 electronics and operated in CMS from 2015 to 2016, quality assurance of the CMS Phase-1 detector during its assembly, and testing with the CMS Central DAQ. This paper describes the Phase-1 pixel DAQ and control system, along with the integration tests and results. Abstract: Silva-Tamayo et al. (2017) study the Chattian to Langhian carbonate succession of the Siamana Formation in the Cocinetas Basin (La Guajira, Colombia). They identify a change in carbonate factory from mixed photozoan-heterozoan and photozoan associations dominated by corals in the Chattian-early Burdigalian to a heterozoan rhodalgal association in the late Burdigalian-Langhian. To validate the regional scale of this shift in carbonate-producing biota along the southeastern Circum-Caribbean realm, Silva-Tamayo et al. compare the Siamana Formation with the San Luis carbonate succession in the Falcon Basin (NE Venezuela) and the Perla carbonates in the Urumaco Trough (Gulf of Venezuela). Referring to Albert-Villanueva (2016) they state that, as in the case of the Siamana Formation, the carbonates of the San Luis Formation also recorded a change in carbonate-producing biota, from a photozoan/heterozoan carbonate factory in the late Oligocene-early Miocene to a heterozoan/rhodalgal carbonate factory in the middle Miocene. Notwithstanding, Albert-Villanueva (2016) interprets the carbonate units cropping out in the Falcon Basin (San Luis and Churuguara formations) as early Miocene in age, and the passage from photozoan to heterozoan carbonate factory as a lateral change of facies within the lower Miocene carbonate platforms of the Falcon Basin.
