Bei 15 Patienten mit sinuatrialen (SA) und/oder atrioventrikularen (AV) nodalen Funktionsstorungen wurde die Wirkung einer Glukose (100g)-lnsulin (20E)-Kalium (10 mEq) Infusion (G.I.K., 15 min) durch His-Bundel-Elektrographie untersucht. G.I.K, fuhrte zu einer Beschleunigung der Sinusfrequenz von 58 auf 96/min und zu einer signifikanten Verbesserung der Uberleitung bei SA und AV nodalen Blockierungen. Gleichzeitig wurde eine Erhohung der Plasma-Katecholamie gemessen. Osmotische Ursachen wurden ausgeschlossen. Experimentell wurde die direkte Wirkung von Glukose bei Sauerstoffmangel auf den isolierten SA und AV Knoten von Kaninchen mit intrazellularen Mikroelektroden untersucht. Die hypoxische Depression der nodalen Aktionspotentiale war nach Erhohung der extrazellularen Glukosekonzentration fast vollstandig reversibel (Abb. 1). Danach ist die glukoseinduzierte Verbesserung der nodalen Funktion wahrscheinlich 1. durch direkte Stimulation des „slow inward current“und 2. Indirekt durch gegenregulatorische Stimulation des sympathocoadrenergen Systems bedingt.

Over the past 3–4 decades significant strides have been made in the development of blood pump systems such as total artificial heart replacement (TAH) or ventricular assist devices (VAD). Animals with TAHs have survived for up to 9 months, living and exercising normally, without automatic adaptive control drive systems. A certain physiological control is provided by the animal’s intact peripheral regulation, but the constant cardiac output (CO) of the blood pump system is responsible for the main pathological findings: an increase in venous pressure, enlargement of the liver and mild functional liver disturbances, edema and ascites, and increased circulating blood volume, expressing the need for an adaptive control of the drive system to replace the natural heart’s extremely complex interactive control function. The two basic means by which the heart regulates CO are the intrinsic autoregulation expressed by the Frank-Starling Law and the reflex control of the heart additional to the autonomic nervous system.

Eine auf die ubliche Medikation mit Digitalisglykosiden und Diuretika nicht oder nur unzureichend ansprechende Herzinsuffizienz wird in der Regel als „therapierefraktar“ bezeichnet. Das therapeutisch kaum noch zu beeinflussende Terminalstadium sollte allerdings nur dann angenommen werden, wenn die in Tabelle 1 zusammengestellten diagnostischen und therapeutischen Ursachen einer erfolglosen Behandlung ausgeschlossen werden konnen.

The optimizing of the blood diaphragm thickness by analyzing the results of the fatigue testing was impossible up to now because of the long functional times of pumps even with very thin diaphragms (50-80 microns). 3. First in vivo experiments showed no problems with the blood compatibility, the anatomical fit, the fixation of the valves and the toxicity of the used combination of materials. There was no pump related termination in LVAD as well as in TAH experiments.

In order to investigate the incidence of severe ventricular arrhythmias, the 24-h ambulatory ECG of 55 patients with aortic valve disease (NYHA classes I-IV; stenosis: n = 24; regurgitation: n = 18; mixed lesions: n = 13) without coronary heart disease were evaluated with regard to the number of premature ventricular beats and according to the Lown classification. Ventricular arrhythmias were found in 73% of all patients (Lown classes III-V: 38%; Lown classes IV-V: 20%). A significant relation between the degree of arrhythmias and the gradient or severity of regurgitation of the aortic valve could not be established. However, there was a significant linear correlation between the left ventricular ejection fraction and the degree of arrhythmias (stenosis: r = 0.60; regurgitation: r = -0.72; mixed lesions: r = 0.78; all p less than 0.005). Complex arrhythmias of Lown classes III-IV are rare (less than 10%) in patients with good left ventricular function (LVEF greater than 60%). There was a history of palpitations in 81% of patients with Lown classes III-V. According to these data, ventricular arrhythmias are present in a large number of patients with aortic valve disease. The degree of arrhythmia does not correlate to the transvalvular stenosis or the severity of regurgitation, but is strongly influenced by the myocardial performance. Therefore in patients with aortic valve disease significant arrhythmias are frequently a sign of impaired LV function.

The inotropic and electrophysiological effects of plasma obtained from patients and experimental dogs during cardiogenic shock following acute myocardial infarction were studied. Changes in the isometric contraction and the intracellular action potential were determined in isolated papillary muscles of rabbits. Control plasma collected from normal subjects produced no significant changes in the contraction or the electrical parameters. Plasma from shock patients decreased peak force by 42% and the maximum rate of force development by 38% in comparison to control values; the time to peak of contraction, the relaxation time and the action potential parameters were not significantly altered. Corresponding results were obtained with plasma from dogs before and during experimental cardiogenic shock. Biochemical determinations failed to identify a single specific "myocardial depressant factor" in the plasma of patients and dogs with cardiogenic shock. The results suggest that (1) various humoral factors released during cardiogenic shock may depress the contractile function of cardiac muscle and (2) that the observed negative intropic effect is not due to electrical changes in the cell membrane.

The effect of butylated hydroxyanisole (BHA) administration on the hepatic monooxygenase system of nuclear and microsomal fraction was investigated in male mice. Addition of BHA to the diet significantly lowered the content of cytochrome P-450 in liver nuclei and increased the specific activity of NADPH-cytochrome c reductase and the content of cytochrome b5 in liver microsomes. Incubation of benzo[a]pyrene (BP) with liver nuclei from BHA-fed mice resulted in inhibition of binding of BP metabolites to nuclear macromolecules by 50% compared with control. However, there was no effect of BHA on the binding of BP metabolites to macromolecules when BP was incubated with added DNA and liver microsomes from BHA-fed mice. It has been postulated that modification of nuclear monooxygenases by BHA may play a role in the inhibitory effect of BHA on BP carcinogenesis.

Klinisch wurden bei einem akuten Myokardinfarkt insbesondere wahrend der ersten Stunden folgende supraventrikularen Rhythmusstorungen beobachtet: 1. Bradykardien mit Dysfunktion des Sinusknotens und AV-Knotens bei etwa 40% der Patienten und 2. atriale Tachykardien mit Vorhofflimmern und Flattern bei 20—30% der Falle (Jewitt et al., 1969). Als mogliche pathophysiologische Mechanismen werden eine erhohte Aktivierung des parasympathicotonen und sympathicotonen Nervensystems sowie eine direkte ischamische Schadigung von supraventrikularen Myokardarealen diskutiert (Corr and Gillis, 1974; James, 1961).

Nach umfangreichen wissenschaftlichen Arbeiten mit dem extrakorporalen Kreislauf in den Jahren 1951 bis 1957 lag als Fortsetzung die Entwicklung eines kunstlichen Herzens nahe, wenngleich die alternative Herztransplantation auch von wissenschaftlichem Interesse war.

The first report about the atrio-aortic left ventricular assist device (AA-LVAD) was given by Kusserow [8, 9]. It was followed by the substantial work of Liotta and De Bakey [10, 11, 12, 13, 14, 15]. Kirsch et al. [7] showed the efficacy of a temporary AA-LVAD in severe myocardial infarction in the dog. Pierce et al. [23, 24] and Vadot et al. [32] later applied continuously working pumps. Subsequently Takano et al. [29, 30] constructed a complicated six-chamber bypass pump with peristaltic action. Finally Takagi et al. [27, 28] implanted two blood pumps in parallel which were activated alternately to achieve a better blood flow from the atrium.

Die medikamentose Behandlung von hamodynamisch relevanten Bradykardien mit Sympathicomimetica gilt als unsicher, da bei einzelnen Patienten das Auftreten ernster Komplikationen, insbesondere von Adams-Stokes-Anfallen nicht verhindert werden kann (Torresani u. Mitarb., 1969; Scott u. Mitarb., 1969). Bei den meisten kardiologi-schen Zentren hat sich deshalb die Schrittmachertherapie als Behandlung der Wahl durchgesetzt (Irnich, 1974). Dagegen werden Sympathicomimetica neben dem Atropin noch immer kurzfristig bei akuten Bradykardien zur Uberbruckung bis zur Schrittma-cherbehandlung eingesetzt oder werden auch langerfristig gegeben, falls der Patient die Schrittmachertherapie ablehnt. Experimentell ist fur den Nachweis einer klinisch relevanten Wirkung nicht eine Beschleunigung der Kammerfrequenz unter physiologischen Bedingungen, sondern unter den pathologischen Voraussetzungen der Bradykardie entscheidend.

A nonlinear mathematical model of the short-term behavior of the systemic vascular dynamics has been developed for the design of adaptive total artificial heart (TAH) controllers and their optimization. A three-step identification scheme was applied in connection with measurements observed on TAH calves to estimate the unknown model parameters. It was proved that all model parameters are identifiable. A good representation of the real system dynamics was obtained, and the robust identifiability of the total compliance of the system was established, which implies an estimate with a low standard deviation. >

The development of a total artificial heart system in Berlin for more than 20 years now has been a challenge for the group of medical and technical scientists under the leadership of Professor Bucherl at the Free University of Berlin. Over the years, the main problems have changed according to the state of the development — expressed by the survival times of the experimental animals — from medical to technical fields and vice versa [1].

Bei 489 Patienten (393 m, 98 w) zwischen 31 und 75 Jahren wurden 1980 und 1981 1315 ACVB (Durchschnitt 2,7/Pat.) angelegt; zusatzlicher Herzklappenersatz bei 27 Patienten. Nach 5 Jahren waren 79,6% der Pat. im NYHA-Stad. I und II. Die Schmerzbefreiung zum NYHA-Std. I war bei den 37,7% Mannern signifikant (P < 0,05) hoher als bei den 15,6% Frauen. Gesamtletalitat der Bypasspat.: 14,5%, 3.9% davon perioperativ. Die 5-Jahres-Uberlebensrate der Manner war mit 87,0% signifikant hoher als die der Frauen mit 71,9% (P < 0,05). Nach einer durchschnittlichen Rehabilitation von 6,5 Monaten nahmen 38,0% die Arbeit wieder auf. Damit rechtfertig eine deutlich verbesserte Lebensqualitat die Indikation zur Bypassoperation.

Klinisch wird bei vielen Bradyarrhythmien eine gesteigerte Empfindlichkeit fur die Wirkung von Digitalis oder ein erhohter Vagotonus beobachtet (Schamroth u. Mitarb., 1966; Scherlag u. Mitarb., 1971). Insbesondere konnen bei frischem Hinterwandinfarkt schwere vagotone Komplikationen bis zur Asystolie auftreten, wobei bereits geringe Glykosiddosen zu einer bedrohlichen Verlangsamung der Sinusfrequenz und der atrioventrikularen Uberleitung fuhren konnen (James, 1961).

Aside from the medical requirements a material has to fulfill to qualify as biocompatible, for the bioengineer, who is involved in the design, construction and fabrication of prosthetic devices, the mechanical, physical and chemical properties of the material as well as its possible manufacturing procedures and, last but not least, its biostability are of the same importance. In the very aggressive biological environment, the material should behave inertly, showing no surface erosions, molecular chain disruptions, uptake of low molecular weight biological materials, local chemical imbalances, tendency to calcification or negative changes of mechanical properties. With regard to this aspect, different polyurethanes have been evaluated in-vitro and in-vivo as basic materials and already fabricated devices (blood pumps and heart valves). The comparison of the results stresses the necessity of further efforts to achieve a standardized test protocol for the prove of the biostability of polymers, containing appropriate, well-defined in-vitro and in-vivo test methods to establish the necessary data basa for the materials' clinical approval.
