Abstract

Prosociality, conceptualized as a willingness to help, to be fair, and to be friendly to others, is essential to the maintenance of a civil society and has been linked with multiple measures of individual well-being. This study examines how individual, family, and neighborhood factors affect adolescents’ level of prosociality and tests for moderating influences in these relationships. Data for this study come from the 12-year-old cohort of the Project on Human Development in Chicago Neighborhoods (PHDCN) study ( N = 723). Results show that being female and having the ability to rely on family, a teacher/coach, or an adult in the community predicts significantly higher levels of prosociality. In addition, I find that neighborhood collective efficacy moderates the relationship between gender and prosociality, serving as a protective factor in the development of prosociality for boys. The findings of this study are discussed in the context of a sociological view of positive youth development.

Abstract

Phosphor-converted light-emitting diodes (LEDs) are now widely used in all kinds of illumination applications. Their structures are more involved compared with that of chip-only band-gap LEDs. We describe the extra manufacturing steps involved in phosphor-converted LED manufacturing and how these extra features impact the reliability of such devices. Our studies looked at both the short-term and long-term behavior of phosphor-based LEDs. Devices were tested for times ranging from a few hours to 40 000 hours. Some tests were performed at elevated temperatures of up to 70 °C. The light outputs of LEDs were studied as functions of time, temperature, and drive currents of up to 2.5 A. Shifts in chromaticity point of these LEDs were also investigated as functions of drive current and, thus, LED die temperature. These studies lead to several conclusions about the reliability and degradation behavior of phosphor-based LEDs that have been elaborated in this paper.

Abstract

The Aleph Vertex Detector is composed of two complete layers of silicon detectors with double-sided readout providing full information of the impact points of charged particles. Running tests of the detector show that the overall data taking efficiency was equivalent to 86% which corresponds to 200,000 hadronic Zs collected. The cooling system worked properly with temperature sensors measuring a few degrees centigrade difference between inlet and outlet air. On the whole the new Aleph vertex detector works as efficiently as expected.

Abstract

Background and Purpose. Little is known about the methods used to assess the physical fitness of adolescents who are overweight. We investigated the relationship between walk/run performance and cardiorespiratory fitness in adolescents who are overweight. Subjects. Eight African-American adolescents (5 female, 3 male) and 10 Caucasian adolescents (5 female, 5 male) who were overweight (mean age=14.5 years, SD=2.0, range=12-17; mean body mass index [BMI]=42.9 kg/[m.sup.2], SD=11.5) participated in this study. Methods. Subjects performed a 12-minute walk/run test. The distances traveled at both 9 minutes ([D.sub.9]) and 12 minutes ([D.sub.12]) were recorded, and the distance traveled between 9 and 12 minutes ([D.sub.9-12]) was calculated. Subjects also completed a maximal cycle ergometry test, during which peak oxygen uptake (V[O.sub.2]peak), anaerobic threshold (AT), peak power (Wpeak), and power at the anaerobic threshold (WAT) were determined. Body composition was determined by air displacement plethysmography. Results. The mean percentage of body fat was 48.6% (SD=5.3%, range=40.3%-60.4%). Abstract

Cranial radiation therapy (CRT) contributes to cognitive late effects that require rehabilitative interventions. Physical activity (PA) targets CRT-damaged brain mechanisms and cognitive functions. We conducted a pilot RCT to evaluate the feasibility of an innovative home PA intervention program for children treated with CRT for brain tumors. Eligible children 8-17 years, >two years post-CRT, with cognitive late effects were randomized to the Intervention Group (IG), which followed a 12-week home program to increase PA then maintained PA for 12 weeks, or the Control Group (CG), which monitored usual PA for 12 weeks followed by the 12-week program. Psychologists taught Acceptance and Commitment Therapy (ACT) techniques to facilitate PA engagement. Participants used Zamzee accelerometers/website to track moderate-to-vigorous PA (MVPA). Evaluations included plasma/serum circulating growth factors, cardiovascular fitness, cognitive function pre/post the first 12 weeks, and child/parent feasibility questionnaires (1-5 ratings, categories, open-ended questions). Six children (mean age=13.8 years, 10-17; IG=3; CG=3) enrolled and completed the study. Abstract

OBJECTIVESedentary children have greater risk of developing abnormalities in glucose homeostasis. We investigated whether interrupting sedentary behavior (sitting) with very short periods of walking would improve glucose metabolism without affecting dietary intake in children with overweight or obesity. We hypothesized that interrupting sitting with short bouts of moderate-intensity walking would decrease insulin area under the curve (AUC) during an oral glucose tolerance test (OGTT) compared with uninterrupted sitting.RESEARCH DESIGN AND METHODSOverweight/obese (BMI ≥85th percentile) children 7–11 years of age underwent two experimental conditions in random order: prolonged sitting (3 h of continuous sitting) and interrupted sitting (3 min of moderate-intensity walking at 80% of ventilatory threshold every 30 min for 3 h). Insulin, C-peptide, and glucose were measured every 30 min for 3 h during an OGTT. Each session was followed by a buffet meal. Primary outcomes were differences in OGTT hormones and substrates and in buffet meal intake by condition.RESULTSAmong 35 children with complete data, mixed-model results identified lower insulin and C-peptide in the interrupted condition (P = 0.007 and P = 0.029, respectively); the intervention reduced insulin AUC by 21% (P < 0.001) and C-peptide AUC 18% (P = 0.001) and improved estimated insulin sensitivity (P = 0.013). Neither buffet total energy intake (1,262 ± 480 vs. 1,260 ± 475 kcal; P = 0.89) nor macronutrient composition of the meal (P values >0.38) differed between conditions significantly.CONCLUSIONSInterrupting sitting with brief moderate-intensity walking improved glucose metabolism without significantly increasing energy intake in children with overweight or obesity. Interrupting sedentary behavior may be a promising intervention strategy for reducing metabolic risk in such children.

Abstract

Objective: The high prevalence of fatigue among persons with traumatic brain injury (TBI) may be related to poor cardiorespiratory fitness observed in this population. Oxygen uptake on-kinetics is a method of assessing cardiorespiratory fitness and may be used to examine performance fatigability (decline in performance during a given activity) in persons with TBI. Purpose: To examine the effect of aerobic exercise training on oxygen uptake on-kinetics during treadmill walking in individuals with TBI. Methods: Seven ambulatory adults with chronic non-penetrating TBI performed short moderate-intensity (3–6 metabolic equivalents) walking bouts on a treadmill, prior to and following an aerobic exercise training program (clinicaltrials.gov: NCT01294332). The 12-week training program consisted of vigorous-intensity exercise on a treadmill for 30 min, 3 times a week. Breath-by-breath pulmonary gas exchange was measured throughout the bouts, and oxygen uptake on-kinetics described the time taken to achieve a steady-state response. Results: Faster oxygen uptake on-kinetics was observed after exercise training, for both the absolute and relative intensity as pre-training. Abstract

Objective. To determine the effects of state legislation requiring patient informed consent prior to medical record abstraction by external researchers for a specific study. Data Sources/Study Setting. Informed consent responses obtained from November 1997 through April 1998 from members of a Minnesota-based IPA model health plan. Study Design. Descriptive case study of consent to gain access to medical records for a pharmaco-epidemiologic study of seizures associated with use of a pain medication that was conducted as part of the FDA&apos;s post-marketing safety surveillance program to evaluate adverse events associated with approved drugs. Data Collection. The informed consent process approved by an institutional review board consisted of three phases: (1) a letter from the health plan&apos;s medical director requesting participation, (2) a second mailing to nonrespondents, and (3) a follow-up telephone call to nonrespondents. Principal Findings. Of 140 Minnesota health plan members asked to participate in the medical records study, 52 percent (73) responded and 19 percent (26) returned a signed consent form authorizing access to their records for the study. For 132 study subjects enrolled in five other health plans in states where study-specific consent was not required, health care providers granted access to patient medical records for 93 percent (123) of the members. Conclusion. Legislation requiring patient informed consent to gain access to medical records for a specific research study was associated with low participation and increased time to complete that observational study. Efforts to protect patient privacy may come into conflict with the ability to produce timely and valid research to safeguard and improve public health. Key Words. Abstract

Abstract Background Physical activity is being studied as a breast cancer prevention strategy. Women at risk of breast cancer report interest in lifestyle modification, but recruitment to randomized physical activity intervention studies is challenging. Methods We conducted an analysis of recruitment techniques used for a prospective, randomized pilot study of physical activity in women at risk of breast cancer. We evaluated differences in proportion of eligible patients, enrolled patients, and successful patients identified by each individual recruitment method. The Fisher-Freeman-Halton test (an extension of Fisher's exact test from 2 × 2 tables to general row by column tables) was used to compare the success of different recruitment strategies. Results We received 352 inquiries from women interested in participating, of whom 171 (54%) were eligible. Ninety-nine women completed a baseline activity evaluation, and 58 (34% of eligible; 16% of total inquiries) were randomized. Recruitment methods fell into three broad categories: media techniques, direct contact with potential participants, and contacts with health care providers. Recruitment strategies differed significantly in their ability to identify eligible women (p = 0.01), and women who subsequently enrolled in the study (p = 0.02). Conclusion Recruitment techniques had varying success. Our data illustrate the challenges in recruiting to behavior modification studies, and provide useful information for tailoring future recruitment efforts for lifestyle intervention trials. Abstract

Limited data suggest that interrupting sedentary behaviors with activity improves metabolic parameters in adults.We tested whether interrupting sitting with short, moderate-intensity walking bouts improved glucose tolerance in children.Participants underwent two experimental conditions in random order on different days: continuous sitting for 3 hours or sitting interrupted by walking (3 min of moderate-intensity walking every 30 min). Insulin, C-peptide, glucose, and free fatty acids were measured every 30 minutes for 3 hours during an oral glucose tolerance test. Area under the curve (AUC) was calculated from hormone and substrate measurements. Children were given a buffet meal after each condition.The study was conducted at the National Institutes of Health Hatfield Clinical Research Center.Twenty-eight normal-weight 7-11 year olds participated.Patterns of substrate/hormone secretion and AUC, as well as energy intake, were examined by experimental condition.Interrupting sitting resulted in a 32% lower insulin AUC (P.001), 17% lower C-peptide AUC (P.001), and 7% lower glucose AUC (P = .018) vs continuous sitting. Mixed model results indicated that insulin (P = .036) and free fatty acid concentrations (P = .009) were significantly lower in the interrupted vs the continuous sitting condition. Lunchtime buffet meal energy intake did not significantly differ between the conditions (975 ± 387 vs 963 ± 309 kcal; P = .85).Interrupting sedentary time with brief moderate-intensity walking improved short-term metabolic function in non-overweight children without increasing subsequent energy intake. These findings suggest that interrupting sedentary behavior may be a promising prevention strategy for reducing cardiometabolic risk in children.

Abstract

OBJECTIVE: To determine if, in patients with interstitial lung disease (ILD), fatigue might be lessened after vigorous aerobic exercise. METHODS: 13 physically inactive patients (5 men and 8 women; age 57.2 ± 9.1 years, BMI 28.2 ± 4.6 kgm(−2)) with ILD of heterogeneous etiology and able to walk on a motor driven treadmill without physical limitation were enrolled. Subjects underwent cardiopulmonary exercise (CPET) and 6-min walk (6MWT) tests and completed Fatigue Severity Scale and Human Activity Profile questionnaires before and after an aerobic exercise-training regimen. The training regimen required participation in at least 24 of 30 prescribed aerobic exercise training sessions at a target heart rate of 70–80% of the heart rate reserve, 30 min per session, 3 times per week for 10 weeks. RESULTS: After training, a 55% (p < 0.001) increase in time to anaerobic threshold on the CPET, and an 11% (p = 0.045) reduction in performance fatigability index (PFI), calculated from the performance on the 6MWT were observed. Distance walked on the 6MWT (6MWD) increased by 49.7 ± 46.9 m (p = 0.002). Significant improvements in scores on the Fatigue Severity Scale (p = 0.046) and Human Activity Profile (AAS p = 0.024; MAS p = 0.029) were also observed. No adverse events related to the training regimen were noted. CONCLUSION: After training, the decrease in fatigability appeared to result in increased 6MWD and was associated with physical activity. Since significant declines in 6MWD may be a marker for impending mortality in ILD, a better understanding of the etiological state of fatigue in patients with ILD and its reversal might provide fundamental insight into disease progression and even survival.

Background: Previous studies have suggested that cancers of the breast and prostate cluster in families and that the presence of both diseases in a family may be associated with increased risk of breast cancer. Purpose: Our purpose was to evaluate whether 1) prostate cancer aggregates in families with postmenopausal breast cancer, 2) families with cancers of the breast and prostate are the same ones as families with cancers of the breast and ovary, and 3) a family history of prostate cancer is associated with increased risk of postmenopausal breast cancer. Methods: We analyzed data from a large prospective cohort study of Iowa women that were (at baseline) aged 55-69 years in 1986. At the third follow-up survey in 1992, self-reported data on family history of breast, ovarian, and prostate cancers in parents and siblings were provided by 30 883 women. Additional information was collected to ascertain whether the age-of-onset of breast cancer in mothers or sisters was before or after the age of 45 years. Cancer occurrence was documented using the State Health Registry of Iowa. Results: History of prostate cancer in their father or a brother was reported by 3384 (11.0%) of the women, and a total of 4090 women (13.2%) reported breast cancer in their mother or a sister. Abstract

Background Although the Centers for Disease Control and Prevention does not recommend routine oropharyngeal and anorectal screening for Chlamydia trachomatis and Neisseria gonorrhoeae in the general population, they do recommend it for men who have sex with men. However, risk-based extragenital screening of men may not have been adopted at all college health centers, and existing research has not focused on the college population. Methods We examined health records of men at a college health center in a large urban university over 6 years to evaluate effectiveness of C. trachomatis and N. gonorrhoeae screening. We also evaluated the proportion of C. trachomatis and N. gonorrhoeae infections that would have been missed if risk-based extragenital screening were not performed. Decisions to screen at extragenital sites were based on patient-reported risk behavior. Results For 4093 male college students screened, 7.6% of the screening visits used extragenital screening in response to self-reported risk behaviors. The case positivity rate for C. trachomatis was 3.1% with urogenital-only screening and 3.7% with risk-prompted extragenital screening. The case positivity rate for N. gonorrhoeae was 0.7% with urogenital-only screening and 3.3% with risk-prompted extragenital screening. If the college health center had relied solely on urogenital screening rather than risk-based extragenital screening, 26.4% of C. trachomatis infections and 63.2% of N. gonorrhoeae infections would have been missed. Conclusions One out of four C. trachomatis infections and 2 of 3 N. gonorrhoeae infections would have been missed without extragenital screening in this analysis of college men. This study reinforces Centers for Disease Control and Prevention recommendations for risk-based extragenital screening and is the first report to focus on college men. Because guidelines exist only for men, future studies should focus on extragenital screening in college women to build evidence for another group of patients that may benefit from this practice, given the high risk in young adults.

Abstract

Background 1) To identify socio-demographic factors associated with parental “no-intent” for their 13–17 year old unvaccinated daughter to receive the human papillomavirus (HPV) vaccine series within the next twelve months, 2) to describe patterns in “no-intent” by socio-demographic factors, and 3) to identify socio-demographic factors associated with parental reasons for “no-intent”. Methods Data from 2008–2012 National Immunization Survey - Teen (NIS - Teen) were examined in this study. Parents with “no-intent” to vaccinate their daughters were asked to identify reasons for their decision. All responses were categorized into five domains identified as barriers to receive the HPV vaccine series: 1) Safety and Effectiveness Concerns; 2) Systemic Barriers; 3) Vaccine Misinformation; 4) Lack of Knowledge about the Vaccine; and 5) Socio-cultural Barriers. Multivariable logistic regression models were performed to address the study objectives. Results Number of people in the household, household income, mother’s age, education, health insurance, recommendation of a health care provider, and the survey year were significantly associated with parental “no-intent”. Race/ethnicity, mother’s education, marital status, recommendation of a health care provider, household income, age of the unvaccinated daughter, and the survey year, were significantly associated with one or more domains identified as barriers to receive the HPV vaccine. Conclusions This study identified sub-groups of parents across different socio-demographic factors with “no-intent” for their adolescent daughters to receive the HPV vaccine. Developing strategies that target educational tools towards the identified sub-groups of parents about the purpose, safety, and efficacy of the HPV vaccine, and HPV infection, may help increase HPV vaccine acceptance, initiation and completion rates.

Abstract

Background: In a population health partnership, shareable clinical decision support (CDS) can reduce service duplication and promote patient wellness by presenting consistent information to all members of a cross-functional, distributed care team. However, existing health IT standards present challenges in the exchange of CDS outputs from multiple systems. Objectives: To exchange and reconcile CDS outputs across systems in a partnership between an integrated health system (Deaconess Health System) and a population health services organization (Evolent Health). Methods: We developed a bi-directional HL7-based interface for CDS outputs between Deaconess’s Electronic Health Record (EHR) and Evolent’s population health services platorm (PHSP). The mapping of CDS logic between systems enabled this interface to automatically reconcile inconsistent CDS outputs. Fifteen quality measure (QM)- based CDS rules to identify care gaps were selected for this initiative. These care gaps remind Deaconess’s clinicians to provide care or documentation necessary for gap closure, and also guide Evolent’s care management services. Results: Two months after launch, Deaconess reconciled 14,040 care gaps from Evolent using data only available in the EHR. Additionally, 1,047 care gaps were resolved due to patient refusal or clinical inappropriateness, and 246 gaps were closed by services or prescriptions provided during clinical encounters. Conclusions: We implemented an HL7-based interface to exchange and reconcile a large volume of CDS outputs between a health system EHR and a PHSP. Future direction is to standardize the linkage between a CDS rule and its reference QM by universal identifiers and a taxonomy of variations.

Abstract

Pulmonary hypertension (PH) restricts the ability to engage in physical activity and decreases longevity. We examined the impact of aerobic exercise training on function and quality of life in patients with World Health Organization group 1 PH.Patients were randomized to a 10-week education only (EDU) or education/exercise combined (EXE) group. The exercise program consisted of 24-30 sessions of treadmill walking for 30-45 min per session at 70% to 80% of heart rate reserve. Outcome variables included changes in 6-min walk test (6MWT) distance, time to exercise intolerance, peak work rate (WR) from a cardiopulmonary treadmill test, and quality-of-life measures, including the Short Form Health Survey, version 2 (SF-36v2) and Cambridge Pulmonary Hypertension Outcome Review (CAMPHOR).Data are presented as mean SD. Twenty-three women (age, 54 11 years; BMI, 31 7 kg/m 2 ) were randomized to the EDU (n 5 13) or EXE (n 5 10) groups. Following 10 weeks of intervention, patients in the EXE group demonstrated an improvement in 6MWT distance (56 45 m; P 5 .002), increased time to exercise intolerance (1.9 1.3 min; P 5 .001), and peak WR (26 23 W; P 5 .004). Additionally, the EXE group scored significantly ( P , .050) better on six of the eight scales on SF-36v2, and fi ve of the six scales on CAMPHOR. In contrast, no significant improvement was observed for any of the outcome measures following EDU. No adverse events were noted in either group.Ten weeks of brisk treadmill walking improved 6MWT distance, cardiorespiratory function, and patient-reported quality of life in female patients with group 1 PH.
