OBJECTIVE: In this study, different kinds of memory were evaluated using Rey Auditory Verbal Learning (RAVLT) test and were compared between two groups of typical and gifted students using Digit Span test. Finally, we determined if working memory interfered with scores in different Rey stages or not. MATERIAL & METHODS: This study was conducted in Tehran City, Iran in 2013. Scores on RAVLT were compared with WISC- R digit span results in a sample of 148 male students aged 12-14 yr old divided into two groups including 75 students in typical school (IQ ranging between 90 and 110) and 73 gifted students (IQs ranging between 110 and 130). RESULTS: Gifted students obtained higher scores than typical students in both Forward Digit Span (FDS) and Backward Digit Span (BDS) and all 9 stages of RAVLT comparing with typical students (P 0.05). The 14 yr old students in both groups had the highest score. There was a high correlation between FDS and the first stage of RAVLT as well as high correlation between BDS and seventh stage of RAVLT. CONCLUSION: Intelligence has effect on better score of memory and gifted subjects had better scores in memory tests, although the intelligence effect in learning was quantitative rather than qualitative. RAVLT is a comprehensive test, which evaluates short-term memory, working memory and long-term memory and besides Digit span test provides precious information about memory and learning of subjects in order to program different student's educational schedules. Speech comprehension in adverse listening conditions can be effortful even when speech is fully intelligible. Acoustical distortions typically make speech comprehension more effortful, but effort also depends on linguistic aspects of the speech signal, such as its syntactic complexity. In the present study, pupil dilations, and subjective effort ratings were recorded in 20 normal-hearing participants while performing a sentence comprehension task. The sentences were either syntactically simple (subjectfirst sentence structure) or complex (object-first sentence structure) and were presented in two levels of background noise both corresponding to high intelligibility. A digit span and a reading span test were used to assess individual differences in the participants' working memory capacity (WMC). The results showed that the subjectively rated effort was mostly affected by the noise level and less by syntactic complexity. Conversely, pupil dilations increased with syntactic complexity but only showed a small effect of the noise level. Participants with higher WMC showed increased pupil responses in the higher-level noise condition but rated sentence comprehension as being less effortful compared to participants with lower WMC. Overall, the results demonstrate that pupil dilations and subjectively rated effort represent different aspects of effort. An observational study of neuropsychological outcomes at preschool age of tiered lowered oxygen (O2) saturation targets in extremely preterm neonates. We studied 111 three-year-olds born <28 weeks’ gestational age. Fifty-nine participants born in 2009–2010 during a time-limited quality improvement initiative each received three-tiered stratification of oxygen rates (83–93% until age 32 weeks, 85–95% until age 35 weeks, and 95% after age 35 weeks), the TieredO2 group. Comparisons were made with 52 participants born in 2007–2008 when pre-initiative saturation targets were non-tiered at 89–100%, the Non-tieredO2 group. Neuropsychological domains included general intellectual, executive, attention, language, visuoperceptual, visual-motor, and fine and gross motor functioning. Descriptive and inferential analyses were conducted. Group comparisons were not statistically significant. Descriptively, the TieredO2 group had better general intellectual, executive function, visual-motor, and motor performance and the Non-tieredO2 group had better language performance. Cohen’s d and confidence intervals around d were in similar direction and magnitude across measures. A large effect size was found for recall of digits-forward in participants born at 23 and 24 weeks’ gestation, d=0.99 and 1.46, respectively. Better TieredO2 outcomes in all domains except language suggests that the tiered oxygen saturation target method is not harmful and merits further investigation through further studies. Benefit in auditory attention appeared greatest in those born at 23 and 24 weeks. Participants in the tiered oxygen saturation group also had fewer ventilation days and a lower incidence of bronchopulmonary dysplasia, perhaps explanatory for these neuropsychological outcomes at age 3. Girls and women with classical congenital adrenal hyperplasia (CAH) experience elevated androgens prenatally and show increased male-typical development for certain behaviors. Further, individuals with CAH receive glucocorticoid (GC) treatment postnatally, and this GC treatment could have negative cognitive consequences. We investigated two alternative hypotheses, that: (a) early androgen exposure in females with CAH masculinizes (improves) spatial perception and quantitative abilities at which males typically outperform females, or (b) CAH is associated with performance decrements in these domains, perhaps due to reduced short-term-memory (STM). Adolescent and adult individuals with CAH (40 female and 29 male) were compared with relative controls (29 female and 30 male) on spatial perception and quantitative abilities as well as on Digit Span (DS) to assess STM and on Vocabulary to assess general intelligence. Females with CAH did not perform better (more male-typical) on spatial perception or quantitative abilities than control females, failing to support the hypothesis of cognitive masculinization. Rather, in the sample as a whole individuals with CAH scored lower on spatial perception ( p ≤ .009), a quantitative composite ( p ≤ .036), and DS ( p ≤ .001), despite no differences in general intelligence. Separate analyses of adolescent and adult participants suggested the spatial and quantitative effects might be present only in adult patients with CAH; however, reduced DS performance was found in patients with CAH regardless of age group. Separate regression analyses showed that DS predicted both spatial perception and quantitative performance (both p ≤ .001), when age, sex, and diagnosis status were controlled. Thus, reduced STM in CAH patients versus controls may have more general cognitive consequences, potentially reducing spatial perception and quantitative skills. Although hyponatremia or other aspects of salt-wasting crises or additional hormone abnormalities cannot be ruled out as potential contributors, elevated GCs appear to be the most likely contributor to reductions in STM. Spatial span tests (SSTs) such as the Corsi Block Test (CBT) and the SST of the Wechsler Memory Scale are widely used to assess deficits in spatial working memory. We conducted three experiments to evaluate the test-retest reliability and clinical sensitivity of a new computerized spatial span test (C-SST) that incorporates psychophysical methods to improve the precision of spatial span measurement. In Experiment 1, we analyzed C-SST test-retest reliability in 49 participants who underwent three test sessions at weekly intervals. Intraclass correlation coefficients (ICC) were higher for a psychophysically derived mean span (MnS) metric (0.83) than for the maximal span and total correct metrics used in traditional spatial-span tests. Response times (ReTs) also showed high ICCs (0.93) that correlated negatively with MnS scores and correlated positively with response-time latencies from other tests of processing speed. Learning effects were insignificant. Experiment 2 examined the performance of Experiment 1 participants when instructed to feign symptoms of traumatic brain injury (TBI): 57% showed abnormal MnS z-scores. A MnS z-score cutoff of 3.0 correctly classified 36% of simulated malingerers and 91% of the subgroup of 11 control participants with abnormal spans. Malingerers also made more substitution errors than control participants with abnormal spans (sensitivity = 43%, specificity = 91%). In addition, malingerers showed no evidence of ReT slowing, in contrast to significant abnormalities seen on other malingered tests of processing speed. As a result, differences between ReT z-scores and z-scores on other processing speed tests showed very high sensitivity and specificity in distinguishing malingering and control participants with either normal or abnormal spans. Experiment 3 examined C-SST performance in a group of patients with predominantly mild TBI: neither MnS nor ReT z-scores showed significant group-level abnormalities. The C-SST improves the reliability and sensitivity of spatial span testing, can accurately detect malingering, and shows that visuospatial working memory is largely preserved in patients with predominantly mild TBI. The purpose of this study was to examine verbal fluency and verbal short-term memory in 12 adults with Down syndrome (DS) and 12 adults with Intellectual Disability (ID) of unspecified origin, matched for receptive vocabulary and chronological age. Participants’ performance was assessed on two conditions of a verbal fluency test, namely, semantic and phonemic fluency. In addition to the total number of correct words produced, the use of clustering and switching strategies was also evaluated. In order to investigate the likely relations of verbal short-term memory with the test variables, a digit span task was administered. According to the results of the study, the two groups differed significantly on the average phonemic cluster size, with the ID group outperforming the DS group. Regarding the within-group comparisons, a relatively similar pattern of results was found for the two groups, indicating higher scores on all the measures involved in the semantic test than the phonemic one. The examination of the relation of verbal short-term memory with the verbal fluency test measures revealed that, in the DS group, only the total words produced in the semantic and the phonemic test and the number of phonemic switches were significantly associated with verbal short-term memory. Additionally, in both groups, word productivity in the semantic and the phonemic test was positively associated with the number of switches. The observed similarities and differences in the performance of the two groups, as well as the associations that emerged among the test variables in each group, are discussed. In a classic paper in intelligence research, Jensen and Figueroa (1975) examined differences between Digit Span Forward (DSF) and Digit Span Backward (DSB), both between groups and between individuals within groups. This paper was the basis for the later development of the Spearman's hypothesis, which states that the stronger the association between General Mental Ability (GMA) and a test, the larger the between-groups differences in test scores. In the current study we re-examine Jensen and Figueroa's results on the basis of a large, nationally representative database. One of our results replicates Jensen and Figueroa earlier results: Consistent with Spearman's hypothesis, we find that the difference between Blacks and White is larger in DSB than in DSF. However, in contrast to the Spearman's hypothesis, we find that the Hispanic–White difference is larger in DSF than in DSB. In addition, in contrast to Jensen and Figueroa (1975), we find that within groups proxies of GMA tend to be more strongly associated with DSF than DSB. [ABSTRACT FROM AUTHOR]

The capacity to keep multiple items in short-term memory (STM) improves over childhood and provides the foundation for the development of multiple cognitive abilities. The goal of this study was to measure the extent to which age differences in STM capacity are related to differences in task engagement during encoding. Children (n = 69, mean age = 10.5 years) and adults (n = 54, mean age = 27.5 years) performed two STM tasks: the forward digit span test from the Wechsler Intelligence Scale for Children (WISC) and a novel eyetracking digit span task designed to overload STM capacity. Building on prior research showing that task-evoked pupil dilation can be used as a real-time index of task engagement, we measured changes in pupil dilation while participants encoded long sequences of digits for subsequent recall. As expected, adults outperformed children on both STM tasks. We found similar patterns of pupil dilation while children and adults listened to the first six digits on our STM Overload task, after which the adults’ pupils continued to dilate and the children’s began to constrict, suggesting that the children had reached their cognitive limits and that they had begun to disengage attention from the task. Indeed, the point at which pupil dilation peaked at encoding was a significant predictor of WISC forward span, and this relationship held even after partialing out recall performance on the STM Overload task. These findings indicate that sustained task engagement at encoding is an important component of the development of STM.

Historically, Digit Span has been regarded as a relatively poor indicator of general intellectual functioning (g). In fact, Wechsler (1958) contended that beyond an average level of Digit Span performance, there was little benefit to possessing a greater memory span. Although Wechsler's position does not appear to have ever been tested empirically, it does appear to have become clinical lore. Consequently, the purpose of this investigation was to test Wechsler's contention on the Wechsler Adult Intelligence Scale-Fourth Edition normative sample (N = 1,800; ages: 16 - 69). Based on linear and nonlinear contrast analyses of means, as well as linear and nonlinear bifactor model analyses, all 3 Digit Span indicators (LDSF, LDSB, and LDSS) were found to exhibit primarily linear associations with FSIQ/g. Thus, the commonly held position that Digit Span performance beyond an average level is not indicative of greater intellectual functioning was not supported. The Wechsler Adult Intelligence Scale (WAIS) is one of the most frequently used tests among psychologists. In the fourth edition of the test (WAIS-IV), the subtests Digit Span and Letter-Number Sequencing are expanded for better measurement of working memory (WM). However, it is not clear whether the new extended tasks contribute sufficient complexity to be sensitive measures of manipulation WM, nor do we know to what degree WM capacity differs between the visual and the auditory modality because the WAIS-IV only tests the auditory modality. Performance by a mixed sample of 226 patients referred for neuropsychological examination on the Digit Span and Letter-Number Sequencing subtests from the WAIS-IV and on Spatial Span from the Wechsler Memory Scale-Third Edition was analyzed in two confirmatory factor analyses to investigate whether a unitary WM model or divisions based on modality or level/complexity best fit the data. The modality model showed the best fit when analyzing summed scores for each task as well as scores for the longest span. The clinician is advised to apply tests with higher manipulation load and to consider testing visual span as well before drawing conclusions about impaired WM from the WAIS-IV. Traditional accounts of verbal short-term memory explain differences in performance for different types of verbal material by reference to inherent characteristics of the verbal items making up memory sequences. The role of previous experience with sequences of different types is ostensibly controlled for either by deliberate exclusion or by presenting multiple trials constructed from different random permutations. We cast doubt on this general approach in a detailed analysis of the basis for the robust finding that short-term memory for digit sequences is superior to that for other sequences of verbal material. Specifically, we show across four experiments that this advantage is not due to inherent characteristics of digits as verbal items, nor are individual digits within sequences better remembered than other types of individual verbal items. Rather, the advantage for digit sequences stems from the increased frequency, compared to other verbal material, with which digits appear in random sequences in natural language, and furthermore, relatively frequent digit sequences support better short-term serial recall than less frequent ones. We also provide corpus-based computational support for the argument that performance in a short-term memory setting is a function of basic associative learning processes operating on the linguistic experience of the rememberer. The experimental and computational results raise questions not only about the role played by measurement of digit span in cognition generally, but also about the way in which long-term memory processes impact on short-term memory functioning. The authors investigated the parameters for predicting epileptiform abnormalities in a group of children diagnosed with attention-deficit/hyperactivity disorder (ADHD). The sample consisted of 148 subjects aged between 6 and 13 (8.76 ± 1.26; 25.7% female) years. Subtypes of ADHD and comorbid psychiatric disorders were defined according to DSM-IV criteria. The Wechsler Intelligence Scale for Children–Revised was applied to all patients. Most of the subjects (89.2%) had wakefulness and sleep electroencephalography examinations lasting about one hour. The authors found out that the coexistence of speech sound disorder (odds ratio [OR] 3.90, 95% confidence interval [CI]: 1.61-9.48) and higher Digit Span test performance (OR 1.24, 95% CI: 1.06-1.44) predicted the presence of accompanying epileptiform abnormalities. The prevalence of epileptiform abnormalities was 26.4%, and they were frequently localized in the frontal (41%) and centrotemporal (28.2%) regions. Higher percentage of speech sound disorder co-occurrence (64%) in subjects with rolandic spikes suggests that epileptiform abnormalities associated with ADHD can be determined genetically at least in some cases. Introduction Many authors have suggested that a perceptual auditory disorder involving temporal processing is the primary cause of Specific Language Impairment (SLI). The aim of this study was to compare the performance of children with and without SLI on a temporal processing task controlling for the confounding of verbal short-term memory and working memory. Method Thirty participants with SLI aged 6 years were selected, along with 30 age- and gender-matched participants with typical language development. The Adaptive Test of Temporal Resolution (ATTR) was used to evaluate temporal resolution ability (an aspect of temporal processing), and the digit span subtest of the Wechsler Intelligence Scale for Children was used to evaluate auditory short-term memory and working memory. Results The analysis of covariance showed that children with SLI performed significantly worse than children with typical language development on the temporal resolution task (ATTR), even when controlling for short-term memory and working memory. Statistically significant correlations between ATTR and digit span were found for the group of children with SLI but not for the children with typical language development. Conclusion Children with SLI showed significantly worse temporal resolution ability than their peers with typical language development. Working memory (WM) is central to the acquisition of knowledge and skills throughout childhood and adolescence. While numerous behavioral and task-based functional magnetic resonance imaging (fMRI) studies have examined WM development, few have used resting-state fMRI (R-fMRI). Here, we present a systematic R-fMRI examination of age-related differences in the neural indices of verbal WM performance in a cross-sectional pediatric sample (ages: 7–17; n = 68), using data-driven approaches. Verbal WM capacity was measured with the digit span task, a commonly used educational and clinical assessment. We found distinct neural indices of digit span forward (DSF) and backward (DSB) performance, reflecting their unique neuropsychological demands. Regardless of age, DSB performance was related to intrinsic properties of brain areas previously implicated in attention and cognitive control, while DSF performance was related to areas less commonly implicated in verbal WM storage (precuneus, lateral visual areas). From a developmental perspective, DSF exhibited more robust age-related differences in brain–behavior relationships than DSB, and implicated a broader range of networks (ventral attention, default, somatomotor, limbic networks) – including a number of regions not commonly associated with verbal WM (angular gyrus, subcallosum). The purpose of the study was to determine whether perceptual masking or cognitive processing accounts for a decline in working memory performance in the presence of competing speech. The types and patterns of errors made on the backward digit span in quiet and multitalker babble at -5 dB signal-to-noise ratio (SNR) were analyzed. The errors were classified into two categories: item (if digits that were not presented in a list were repeated) and order (if correct digits were repeated but in an incorrect order). Fifty fi ve children with normal hearing were included. All the children were aged between 7 years and 10 years. Repeated measures of analysis of variance (RM-ANOVA) revealed the main effects for error type and digit span length. In terms of listening condition interaction, it was found that the order errors occurred more frequently than item errors in the degraded listening condition compared to quiet. In addition, children had more difficulty recalling the correct order of intermediate items, supporting strong primacy and recency effects. Decline in children's working memory performance was not primarily related to perceptual difficulties alone. The majority of errors was related to the maintenance of sequential order information, which suggests that reduced performance in competing speech may result from increased cognitive processing demands in noise. Based on an analysis of ten normative studies of digit span covering the years 1923 to 2008, Gignac concluded that there are no secular trends on this measure. Temporal correlations involving the subset of studies reporting performance on digit span backwards (DSB) and forwards (DSF) separately revealed modest to large magnitude effects (− .57 and .45 respectively), which were dismissed on the basis that they are non-significant and also go in opposing directions. This finding is potentially substantive as it is predicted by the co-occurrence model, which posits that more g -loaded and also heritable IQ measures should be more sensitive to the effects of so-called dysgenic fertility (the negative association between completed fertility and IQ), whereas less g -loaded and more environmentally influenced measures should be more sensitive to environmental improvements. As DSB is both more g- loaded and more heritable than DSF, this model might account for Gignac's findings. Gignac's data are reanalyzed using weighted least squares regressions, yielding consistently large-magnitude and also significant β values (DSB = − .554, DSF = .580). When rescaled in terms of IQ losses and gains, average DSB performance has declined by − 1.34 points over 85 years, or − .16 points per decade, whereas average DSF performance has increased by 6.50 points in total, or .77 points per decade. The tablet computer initiates an important step toward computerized administration of neuropsychological tests. Because of its lack of standardization, the Corsi Block-Tapping Task could benefit from advantages inherent to computerization. This task, which requires reproduction of a sequence of movements by tapping blocks as demonstrated by an examiner, is widely used as a representative of visuospatial attention and working memory. The aim was to validate a computerized version of the Corsi Task (e-Corsi) by comparing recall accuracy to that on the standard task. Forty university students (Mage = 22.9 years,SD = 2.7 years; 20 female) performed the standard Corsi Task and the e-Corsi on an iPad 3. Results showed higher accuracy in forward reproduction on the standard Corsi compared with the e-Corsi, whereas backward performance was comparable. These divergent performance patterns on the 2 versions (small-to-medium effect sizes) are explained as a result of motor priming and interference effects. This finding implies that computerization has serious consequences for the cognitive concepts that the Corsi Task is assumed to assess. Hence, whereas the e-Corsi was shown to be useful with respect to administration and registration, these findings also stress the need for reconsideration of the underlying theoretical concepts of this task. This study was done to determine the effect of the music and motoric movement as an intervention to increase attention among third grader elementary school in Semarang. Quasi Experimental with nonrandomized pretest-posttest control group design was used to achieve the aim of study. Wechsler Intelligence Scale for Children (WISC) – IV digit spansub-test (forward backward)was used to measure students attention. Subjects were divided into three groups, first group wastreated with music intervention involved 28 students, second group was treated with music and motoric movement intervention involved 25 students, and 25 students in the control group. The results showed that theattention in group 1 was significantly increased ( MD = -1321, p <0.05), in group 2 was not significantly increased ( MD = -0280; p > 0.05), while subject in control group experienced with decreased attention ( MD = -0443, p <0.05). Music intervention could increase attention by 11.7%, music and motoric movement intervention only increased attention by0.5%, and subjects experienced a significant decrease by 13% in control group.This finding is strikingly unique and perhaps will promote the notion that “two heads are not always better than one”, but clearly better than nothing. Within the framework of the working memory model proposed by A. Baddeley and G. Hitch, a dual-task paradigm has been suggested to evaluate the capacity to perform simultaneously two concurrent tasks. This capacity is assumed to reflect the functioning of the central executive component, which appears to be impaired in patients with dysexecutive syndrome. The present study extends the investigation of an index ("mu"), which is supposed to indicate the capacity of coordination of concurrent auditory digit span and tracking tasks, by testing the influence of training on the performance in the dual task. The presentation of the same digit sequence lists or always-different lists did not differently affect the performance. The span length affected the mu values. The improved performance in the tasks under the dual condition closely resembled the improvement in the single-task performance. So, although training improved performance in the single and dual conditions, especially for the tracking component, the mu values remained stable throughout the sessions when the single tasks were performed first. Conversely, training improved the capacity of dual-task coordination throughout the sessions when dual task was performed first, addressing the issue of the contribution of the within-session practice to the mu index.

The measure "mu", proposed as an index of the ability to coordinate concurrent box-crossing (BC) and digit-span (DS) tasks in the dual task (DT), should reflect the capacity of the executive component of the working memory system. We investigated the effect of practice in BC and of a change in the digit span on mu by adding previous practice trials in BC and diminishing, maintaining or increasing the digit sequence length. The mu behavior was evaluated throughout three trials of the test. Reported strategies in digit tasks were also analyzed. Subjects with diminished span showed the best performance in DT due to a stable performance in DS and BC in the single- and dual-task conditions. These subjects also showed a more stable performance throughout trials. Subjects with diminished span tended to employ effortless strategies, whereas subjects with increased span employed effort-requiring strategies and showed the lowest means of mu. Subjects with initial practice trials showed the best performance in BC and the most differentiated performance between the single- and dual-task conditions in BC. The correlation coefficient between the mu values obtained in the first and second trials was 0.814 for subjects with diminished span and practice trials in BC. It seems that the within-session practice in BC and the performance variability in DS affect the reliability of the index mu. To control these factors we propose the introduction of previous practice trials in BC and a modification of the current method to determine the digit sequence length. This proposal should contribute to the development of a more reliable method to evaluate the executive capacity of coordination in the dual-task paradigm.

Research consistently shows that individuals high in impulsivity are at increased risk for excessive alcohol use and alcohol-related problems including alcohol use disorders (AUDs). Recent theorizing posits that working memory (WM) ability might moderate this association, but extant studies have suffered from methodological shortcomings, particularly mischaracterizing WM as a single, unitary construct and using only cross-sectional designs. This paper reports two studies that attempted to replicate and extend previous investigations of the relationship between WM, impulsivity, and alcohol involvement using two independent samples. Study 1 used a large (N=489 at baseline), prospective cohort of college students at high and low risk for AUD to investigate interactions between WM capacity and impulsivity on cross-sectional and prospective alcohol involvement. Study 2 used a large (N=420), cross-sectional sample of participants in an alcohol challenge study to investigate similar interactions between WM updating and impulsivity on recent alcohol involvement. Whereas Study 1 found that WM capacity moderates the relationship between some measures of impulsivity and alcohol involvement, with effects prospectively predicting alcohol involvement for up to three years, Study 2 did not find similar moderation effects when using measures of WM updating. Introduction:N-back tasks are commonly used in functional neuroimaging studies to identify the neural mechanisms supporting working memory (WM). Despite widespread use, the clinical utility of these tasks is not well specified. This study compared N-back performance during functional magnetic resonance imaging (fMRI) with task data acquired outside of the scanner as a measure of reliability across environment. N-back task validity was examined in relation to performance and rater-based measures used clinically to assess working memory.Method:Forty-three healthy adults completed verbal and object N-back tasks during fMRI scanning and outside the scanner. Task difficulty was varied parametrically (0, 1, and 2-back conditions). Order of N-back task completion was stratified by modality (verbal/object) and environment. Participants completed the Digit Span (DS) and provided self-ratings using the Behavior Rating Inventory of Executive Function (BRIEF-WM).Results:Mean verbal and object N-back accuracy was above 95% across load conditions; task difficulty was effectively manipulated across load conditions. Performance accuracy did not significantly differ by environment. N-back reaction time was slower during fMRI (F= 6.52,p= .01, ηp2= .13); participants were faster when initially completing tasks outside the scanner (ηp2= .10–.15). Verbal 2-back accuracy was significantly related to DS performance (r= .36,p= .02). N-back performance was not related to BRIEF-WM.Conclusions:Our results provide evidence for reliability of N-back accuracy during fMRI scanning; however, reliability of reaction time data is affected by order of task presentation. Data regarding construct validity are inconsistent and emphasize the need to consider clinical utility of behavioral measures in the design and interpretation of functional neuroimaging studies. A 42-year-old man suffered damage to the left supra-sylvian areas due to a stroke and presented with verbal short-term memory (STM) deficits. He occasionally could not recall even a single syllable that he had heard one second before. A study of mismatch negativity using magnetoencephalography suggested that the duration of auditory sensory (echoic) memory traces was reduced on the affected side of the brain. His maximum digit span was four with auditory presentation (equivalent to the 1st percentile for normal subjects), whereas it was up to six with visual presentation (almost within the normal range). He simply showed partial recall in the digit span task, and there was no self correction or incorrect reproduction. From these findings, reduced echoic memory was thought to have affected his verbal short-term retention. Thus, the impairment of verbal short-term memory observed in this patient was "pure auditory" unlike previously reported patients with deficits of the phonological short-term store (STS), which is the next higher-order memory system. We report this case to present physiological and behavioral data suggesting impaired short-term storage of verbal information, and to demonstrate the influence of deterioration of echoic memory on verbal STM. ABSTRACT Fluctuations in mental status are 1 of the core diagnostic criteria for dementia with Lewy bodies (DLB) and are thought to reflect variability in daytime alertness. Previous attempts to study fluctuations have been limited to caregiver reports, observer rating scales, short segments of electroencephalography, or motor-dependent, reaction time tests. Concordance among such measures is often poor, and fluctuations remain difficult to quantify. We compared fluctuations in cognition and alertness in patients with DLB (n = 13) and idiopathic Parkinson's disease (PD) (n = 64), a condition associated with deficits in daytime alertness. We systematically and repeatedly collected cognitive and physiologic measures during a 48-hour inpatient protocol in a sound-attenuated sleep laboratory in a geriatric hospital. Cognitive fluctuations were analyzed using coefficients of variation (COVs) derived from performance on a bedside examination familiar to clinicians (digit span). Alertness fluctuations were assessed objectively using COVs from the polysomnographically-based Maintenance of Wakefulness Test. Despite predictably lower mean digit span performances, DLB patients demonstrated significantly greater cognitive fluctuations than PD patients ( P < 0.001), even when groups were matched on general cognitive impairment. There were no group differences in alertness fluctuations, although DLB patients were less alert than PD patients not receiving dopaminergics. The prevailing assumption that fluctuations in cognition in DLB are reflected in fluctuations in daytime alertness was not supported by objective, physiological measurements. Fluctuating mental status in DLB patients can be detected with repeated administration of a simple bedside exam that can be adapted to a clinic setting. © 2013 International Parkinson and Movement Disorder Society. Abstract: Homesigns are communication systems created by deaf individuals without access to conventional linguistic input. To investigate how homesign gestures for number function in short-term memory compared to homesign gestures for objects, actions, or attributes, we conducted memory span tasks with adult homesigners in Nicaragua, and with comparison groups of unschooled hearing Spanish speakers and deaf Nicaraguan Sign Language signers. There was no difference between groups in recall of gestures or words for objects, actions or attributes; homesign gestures therefore can function as word units in short-term memory. However, homesigners showed poorer recall of numbers than the other groups. Unlike the other groups, increasing the numerical value of the to-be-remembered quantities negatively affected recall in homesigners, but not controls. When developed without linguistic input, gestures for number do not seem to function as summaries of the cardinal values of the sets (four), but rather as indexes of items within a set (one–one–one–one). Cognitive deficits are highly prevalent in alcohol-dependent (AD) patients and may have a detrimental impact on treatment response and treatment outcome. Enhancing cognitive functions may improve treatment success. Modafinil is a promising compound in this respect. Therefore, a randomized double-blind placebo-controlled trial was conducted with modafinil (300 mg/d) or placebo in 83 AD patients for 10 weeks. Various cognitive functions (digit span task, Tower of London task, Stroop task) were measured at baseline, during and after treatment. Compared to placebo, modafinil improved verbal short-term memory (number of forward digit spans) (p=0.030), but modafinil exerted a negative effect on the working memory score of the digit span task (p=0.003). However, subgroup analyses revealed that modafinil did improve both working memory and verbal short-term memory in AD patients with a poor working memory ability at baseline (25% worst performers), whereas no significant treatment effect of modafinil was found on these two dependent variables in patients with good working memory skills at baseline (25% best performers). No effect of modafinil was found on measures of planning (Tower of London task) and selective attention (Stroop task). Musical ability has been found to be associated with an enhancement of verbal working memory. In this study, we investigated whether this effect would generalize to visual-spatial working memory as would be expected if the effect were driven by general intelligence. We administered the WAIS-III Digit Span; the WMS-III Spatial Span; and the Musical Ear Test (MET), a forced-choice same/different listening task measuring musical ability, to non-musicians, amateur musicians, and expert musicians. Expert musicians significantly outperformed non-musicians on the Digit Span. Additionally, Digit Span Forward scores were found to be correlated with MET total scores and with scores on the rhythm subtest of the MET.
