152. Clinical judgment in nursing diagnoses of chronic kidney patients on hemodialysis.

Objective: To analyze the result of the current state in chronic kidney patients on hemodialysis. Method: quantitative study performed with 25 patients undergoing dialysis therapy in a tertiary hospital through the accomplishment of semi-structured interviews, physical examination and analysis of laboratory test results. In order to analyze the diagnostic inferences, we used the clinical reasoning of Alfaro-LeFevre and then applied the Outcome-Present State Test Model. Results: We inferred 12 nursing diagnoses found in 70% of the sample, where anxiety was the priority diagnosis. In order to minimize changes in the health of the patient, the interventions chosen were: teaching: procedure/treatment; exercise promotion; progressive muscle relaxation; distraction/inattention; emotional support; nutrition control; and improved socialization. Conclusion: the clinical reasoning technique used by this model can contribute to the agility and execution of the nursing process. [ABSTRACT FROM AUTHOR]

- Abstract
Background: The presence of acute renal injury has been associated with increased cardiovascular morbidity and mortality (sudden death and arrhythmias). Objective: To know the frequency of heart rhythm disorders documented by Holter in patients in need of intermittent hemodialysis due to acute renal injury. Material and methods: A prospective observational study was conducted including patients with a diagnosis of acute renal injury who received intermittent hemodialysis; demographic, clinical and laboratory data were collected from 28 patients. Monitoring was carried out through Holter before, during and after the hemodialysis session. Nonparametric statistics were used, with a significant value of p < 0.05. Risks were established with logistic regression. Results: There was a significant difference in supraventricular extra systoles (82.1% vs. 57.1%), ventricular extra systoles (26.7% vs. 3.6%), and supraventricular tachycardia (6.7% vs. 3.6%). The rest of the rhythm alterations without significance. Conclusions: Intermittent hemodialysis is a factor associated with the appearance of heart rhythm disorders. The death of patients with acute renal injury and substitution was related to ventricular extra systole detected by transhemodialysis Holter. [ABSTRACT FROM AUTHOR]

155. Porfiria cutánea tarda vs pseudoporfiria cutánea tarda en un paciente en hemodiálisis. Presentación de un caso.

- Abstract
Porphyria cutanea tarda (PCT) is a relatively common disease. It is the result of the functional deficiency of an enzyme called uroporphyrinogen decarboxylase. In patients on intermittent periodic hemodialysis, it is described a similar clinical situation called late cutaneous pseudoporphyria. The case of a patient in intermittent periodic hemodialysis for 5 years is presented. The patient has C virus liver disease that develops bullous lesions on the skin exposed to the sun, and had an adequate response to the indicated therapy (based on the restriction of iron use, in a more effective use of erythropoietin, periodic phlebotomies of low volume and protection from solar radiation). The lesions disappeared with the medical measures taken to improve the patient's quality of life. [ABSTRACT FROM AUTHOR]

164. Adherencia al tratamiento como pilar fundamental en la calidad de vida en hemodializados

Introducción: La carga farmacológica en pacientes en hemodiálisis supera a otros pacientes crónicos, la no adherencia al tratamiento presenta implicaciones en la calidad de vida, la supervivencia y el coste económico de su tratamiento. El objetivo fue evaluar el puntaje de calidad de vida relacionada con la salud según el tipo de adherencia al tratamiento en pacientes en terapia de hemodiálisis. Métodos: El estudio es de tipo observacional, prospectivo, transversal y descriptivo, el universo de estudio está compuesto por los pacientes que recibieron hemodiálisis en COSSMIL durante el año 2020, la muestra fue de 27 pacientes, la adherencia al tratamiento se midió a través del Test de Morisky-Green y la calidad de vida relacionada con la salud mediante el Kidney Disease Quality of Life de 36 ítems. Resultados: Se hallaron un total de 7 pacientes adherentes al tratamiento (25,9%). En la escala del 1 al 100, el 100% de las puntuaciones del componente mental y 96,3 % del componente físico fueron menores del puntaje referencial de 50. La subescala de carga de enfermedad renal obtuvo puntuaciones de 0. Conclusión: La calidad de vida de estos pacientes se encuentra notablemente afectada, sin diferencias en relación el tipo de adherencia al tratamiento, donde la subescala de carga de enfermedad renal expresa el mayor impacto sobre la calidad de vida, se identifica la necesidad de fortalecer un abordaje integral clínico médico, multidisciplinario y biopsicosocial que incluya estrategias de afrontamiento y apoyo psicosocial para enfrentar los desafíos socioemocionales asociados a la enfermedad renal crónica.

Background: Malnutrition in chronic kidney patients is frequent, recent studies agree that quality of life decreases and morbidity increases. The evaluation of nutritional status analyzes the individual from biochemical, dietary, anthropometric and clinical aspects, in order to carry out timely nutritional intervention, minimizing the risk of malnutrition. Objective: To determine the evolution of the nutritional status in patients who attended a Guayaquil Hemodialysis Center from January 2017 to February 2020. Methods: Descriptive, correlational study, collected information recorded in 155 medical records of patients over 20 years of age with chronic kidney disease who attended the Guayaquil Hemodialysis Center between January 2017 and February 2020. Data collected from a software for medical records, which records anthropometry, biochemistry, dietetics and clinical. It excluded cancer patients, pregnant women, with incomplete history. The Pearson correlation coefficient test was used to measure the relationship between nutritional status and chronic kidney disease in advanced stages with hemodialysis. Results: Statistical analysis determined the evolution of nutritional status for 3 years, relating variables nutritional status and chronic kidney disease in advanced stages with hemodialysis, associated with age, sex, residence and socioeconomic situation, comparing initial nutritional status, which on average was 25.63 kg. /m2, with a final average of 24.95 kg/m2. Conclusions: It was found that kidney disease occurs regardless of age, with a predominant male sex, indifferent socioeconomic situation. There was no positive relationship between biochemistry and anthropometry. Four of six observed parameters presented improvement, therefore, there was a positive evolution of health within their condition.

- Abstract
O setor de hemodiálise é um ambiente complexo que requer ações que garantam a segurança do paciente que realiza o tratamento de terapia renal substitutiva. Desta forma, este artigo objetiva relatar as intervenções realizadas por acadêmicas de enfermagem sobre aspectos da segurança do paciente em hemodiálise. Trata-se de um estudo descritivo, com abordagem qualitativa do tipo relato de experiência. Realizaram-se intervenções de enfermagem por duas acadêmicas de enfermagem, em clínicas de hemodiálise. As intervenções se deram por meio de educação permanente sobre os registros de enfermagem e educação em saúde sobre o cuidado com acessos vasculares. As educações permanentes foram realizadas com os técnicos de enfermagem sobre a importância e a correta anotação de enfermagem. Enquanto que as educações em saúde foram desenvolvidas para os pacientes com o auxilio de um material educativo e orientações individualizadas. Conclui-se que essas duas intervenções geraram reflexões sobre a segurança do paciente em diálise, bem como aperfeiçoou essa prática por meio de ações simples que impactaram positivamente tanto os profissionais como os pacientes., El sector de la hemodiálisis es un entorno complejo que requiere acciones para garantizar la seguridad de los pacientes sometidos a terapia renal sustitutiva. Así, este artículo tiene como objetivo reportar las intervenciones realizadas por estudiantes de enfermería sobre aspectos de seguridad del paciente en hemodiálisis. Es un estudio descriptivo, con un abordaje cualitativo del tipo relato de experiencia. Las intervenciones de enfermería fueron realizadas por dos estudiantes de enfermería en clínicas de hemodiálisis. Las intervenciones se realizaron a través de la educación permanente sobre los registros de enfermería y la educación para la salud sobre el cuidado con accesos vasculares. Se realizaron capacitaciones permanentes con técnicos de enfermería sobre la importancia y la correcta nota de la enfermería. Mientras que la educación para la salud se desarrolló para los pacientes con la ayuda de material educativo y orientación individualizada. Se concluye que estas dos intervenciones generaron reflexiones sobre la seguridad del paciente en diálisis, además de mejorar esta práctica a través de acciones sencillas que impactaron positivamente tanto a los profesionales como a los pacientes., The hemodialysis sector is a complex environment that requires actions to ensure the safety of patients undergoing renal replacement therapy. Thus, this article aims to report the interventions performed by nursing students on aspects of patient safety in hemodialysis. It is a descriptive study, with a qualitative approach of the experience report type. Nursing interventions were carried out by two nursing students in hemodialysis clinics. The interventions were carried out through permanent education on the nursing records and health education on the care with vascular accesses. Permanent educations were carried out with nursing technicians on the importance and the correct note of nursing. While health education was developed for patients with the aid of educational material and individualized guidance. It is concluded that these two interventions generated reflections on patient safety on dialysis, as well as improved this practice through simple actions that positively impacted both professionals and patients.

Introduction: During the last few years, physical activity has become a therapeutic tool and a protective mechanism for the elderly; it reduces cardiovascular risk in patients undergoing different types of dialysis and receiving palliative care. Objective: The aim of this study was to establish the inclusion criteria for a therapeutic physical activity program through the use of the PAR-Q & YOU Questionnaire with elderly patients treated with different types of hemodialysis, and to relate it with cardiovascular risk factors. Methods: Pearson's chi-squared test was used to determine the possible association among variables, considering the medical history and discomfort caused by physical activity against the results of PAR-Q & YOU. Binary logistic regression was used with the variables in the chi-squared test. Conclusions: Through logistic regression, we found that cardiovascular history was 10.44 times more significant to establish the relevance of the PAR-Q & YOU as a basic assessment instrument for the inclusion in a physical activity program which is part of a physiotherapy-led renal rehabilitation.

169. Factors associated with hepatitis C in dialysed patients

Introduction: in dialyzed patients, it has been observed a high incidence of hepatitis since the beginning of dialysis as a substitute treatment for renal function. In hemodialysis a prevalence of 13% is observed, with a high variability (from one to 70%). Objective: to determine the prevalence of hepatitis C and its associated factors in dialyzed patients. Method: a descriptive, cross-sectional study was performed; the universe of study consisted of the 169 patients who were under dialysis treatment. The variables studied were age, sex, time of dialysis, history of renal transplantation and dialysis method. Results: a prevalence of 36,7% was observed among the patients studied. As for sex the male (37,5%) was the most affected and by age group from 25 to 34 years (55,6%). Of 91,3% was the prevalence among patients with more than three years of dialysis treatment, of 78,6% among those who reported antecedents of renal transplantation and 38,7% in the case of patients receiving hemodialysis as treatment renal replacement. Conclusions: the time of treatment in hemodialysis was the factor of greatest association with hepatitis C infection in dialyzed patients.

170. Peritonitis in peritoneal dialysis. Epidemiology, risk factors, inclusion of BACTEC™ in traditional culture systems, and long-term mortality

Introduction: Peritonitis is the most common complication in peritoneal dialysis (PD), with an effect on morbidity and mortality. The aim of this study was to analyze epidemiology, risk factors, implementation of the BACTEC™ blood culture system and mortality of peritoneal dialysis-associated peritonitis. Methods: In this retrospective, cohort study, all the patients who started PD between 1992 and 2017 at Hospital Privado Universitario de Córdoba (Córdoba Private Medical College Hospital) were included. Results: The total number of patients was 159, 63 (39.62%) of which had suffered from peritonitis at least once and 96 (60.38%) had never had it. The global peritonitis rate was 0.37 episodes per patient-year. The risk factors for peritonitis were the following: a history of hemodialysis before PD (OR=3.18; CI 95%=1.41-7.14; p=0.0051) and hypoalbuminemia (OR=3.10; CI 95%=1.36-7.06; p=0.0071). The implementation of the BACTEC™ system increased the percentage of positive blood cultures (from 73.55% to 96.55%; p=0.0076). The most frequent isolates were MSSA (23.66 %) and GNB (20.61 %). Differences were found between the 1992-2006 and 2006-2017 periods in the Candida isolates (1.49 % and 9.38 %; p=0.0448) and the “other bacteria” group (1.49 % and 9.38 %; p=0.0448). Although peritonitis patients showed a higher mortality rate (55.56 % versus 38.58 %; p=0.0480), the multivariate analysis revealed that this condition was not a mortality independent risk factor. Conclusion: The risk factors for peritonitis were hemodialysis prior to PD and hypoalbuminemia. The use of the BACTEC™ system increased the percentage of positive blood cultures. After 2006 the number of isolates from the Candida and the “other bacteria” groups was higher. Peritonitis was not a long-term mortality independent risk factor.

Methods: 60 chronic hemodialysis patients and 40 patients with normal kidney function were studied and their PTH levels were measured for both assays. Results: In patients population with normal kidney function the average on PTH was 51.8 and 45.6 pg/ml with second and third generation assays respectively. In chronic hemodialysis patients the average PTH was 193.9 and 137.1 pg/ml with second and third generation assays respectively. The difference between assays was 11.3% and 29.3% in patients with normal kidney function and in hemodialysis patients respectively. Third generation assay caused a variation in the amount of patients that fall over seve ral PTH ranges according to KDIGO guidelines, for a lesser value of 2 times the reference upper limit: it changes from 20 to 25 patients, between 2 and 9 times: it changes from 31 to 32 patients, and more than 9 times: it changes from 9 to 3 patients. Conclusions: When PTH concentration increases the difference between both assays also increases, for this reason we cannot use them indiscriminately in a chronic hemodialysis patient population. With third generation assays 11 patients (18.3%) changed their classification according to KDIGO guidelines, which will result in a change of treatment.

Introduction: Dynamometry is a cheap, simple and easily reproduced method to assess muscle strength (MS), which, like other processes of obtaining measurements, displays high sensitivity and specificity for the early detection of any change in the nutritional status. Objectives: To evaluate factors concerning the reduction in MS in patients undergoing hemodialysis (HD). Methods: A single center, crosssectional study was carried out. The following clinical variables were analyzed: sex, age, blood pressure, weight, significant weight loss within the last 36 months, body mass index (BMI), weight gain in between dialysis sessions, hemodialysis treatment time, diabetes, energy and protein intake. Other factors taken into account were: laboratory findings (hemoglobin, hepatogram, electrolyte panel, calcium test, phosphate test, lipid profile, protein analysis, transferrin saturation) and hand grip strength (before and after dialysis, on both arms) measured through dynamometry. The patients' muscle strength values were compared to those of healthy subjects. Values below the 10th percentile were regarded as abnormal. Introduction: Peritoneal dialysis is under-used in many countries. Usually the admission of acute patients to renal replacement therapy are directed to HD. Urgent start of peritoneal dialysis offers a safe and efficient option to avoid the use of central venous catheters and to increase peritoneal dialysis use. Material and methods: It is a retrospective, observational study comparing patients who started urgent peritoneal dialysis with those that started dialysis in the conventional way, after 4 weeks. Results: 12 peritoneal dialysis patients were analyzed. Six of them under urgent start and the other six received the conventional procedure- Follow-up period was shorter for patients receiving urgent start. There were not significant differences between both groups concerning: age, sex, initial volume, KT/V, albumin, urea, calcium, phosphorus, ferritin, PCR and type of catheter. PTH was significantly lower in patients who received urgent start PD (P≤0.047). Regarding complications: two patients presented leakage of peritoneal fluid and one patient had wall bruising, in the urgent start group. Complications were solved with adequate treatments and did not cause change of methods. Discussion: Urgent start to peritoneal dialysis is a safe method and does not involve major complications. It is a strategy to decrease the use of hemodialysis catheters and to increase number of patients on dialysis peritoneal programs.

Objetivo: Describir y comparar la percepción del comportamiento del cuidado de enfermería que tienen los pacientes en hemodiálisis y diálisis peritoneal en una unidad renal de Bogotá. Material y Método: Estudio descriptivo comparativo realizado con 92 pacientes en terapia renal distribuidos en dos grupos: hemodiálisis (50) diálisis peritoneal (42). Se utilizó el instrumento comportamiento profesional de cuidado de Horner. Para el análisis se acudió a estadística descriptiva, medidas de tendencia central y de dispersión; e inferencial mediante pruebas no paramétricas para comparación de grupos. Resultados: Se obtuvo alto puntaje en los comportamientos de cuidado brindados por enfermería en los dos grupos de pacientes. Los dos grupos perciben en su orden atributos de interacción familia –paciente, cortesía, relación y compromiso, en el cuidado brindado por enfermería. Es ligeramente mayor la percepción dada por los que se encuentran en diálisis peritoneal, sin ser estadísticamente significativa la diferencia frente a los de hemodiálisis Conclusiones: Los pacientes en terapia renal de hemodiálisis y diálisis peritoneal reconocen altos comportamientos de cuidado de enfermería. Se requiere mantener el componente técnico y el conocimiento teórico propios del quehacer en las unidades renales, y fortalecer las relaciones de confianza, respeto y empatía que la enfermera(o) puede brindar de acuerdo a sus condiciones y las ofrecidas por la institución.

Objetivo: Evaluar si la visualización de películas de humor durante las sesiones de hemodiálisis mejora el bienestar subjetivo y el sentido del humor, al tiempo que disminuye los niveles de estrés / ansiedad y depresión, en pacientes con hemodiálisis. Material y Método: Se realizó un estudio pre-post intervención con un grupo de control no equivalente, en pacientes con enfermedad renal crónica en hemodiálisis, en dos unidades de hemodiálisis de Diaverum Clinics. El estudio incluyó a 34 pacientes en el grupo de intervención y 33 pacientes en el grupo de control. La intervención consistió en la visualización de películas de humor durante diez sesiones de hemodiálisis en un período de cuatro semanas. El grupo control vio películas neutrales. Los datos se recogieron mediante un cuestionario de información sociodemográfica y de salud (edad, sexo, nacionalidad, educación, ocupación, estado civil, duración de la sesión de diálisis, presencia de hipertensión y diabetes); la escala subjetiva de felicidad; la escala de satisfacción general con la vida; la escala de depresión, ansiedad y estrés (DASS-21) y la escala multidimensional de sentido del humor. Se emplearon pruebas t-Student y pruebas t para muestras pareadas. La respuesta al cambio se evaluó utilizando: el porcentaje de cambio, el efecto pico y el efecto base, el tamaño del efecto y la media de respuesta estandarizada. Resultados: Los niveles de felicidad subjetiva y de sentido del humor aumentaron en el grupo de intervención, pero no hubo aumento significativo en la satisfacción general con la vida.

Objetivo: Evaluar la calidad de vida relacionada a la salud (CVRS), describir sus dimensiones y explorar su probable asociación con la hemoglobina, albumina sérica y adherencia al tratamiento, en pacientes en hemodiálisis crónica de la Red Asistencial de Huánuco – EsSalud. Métodos: Estudio observacional descriptivo, prospectivo, serie de casos. Se evaluó la CVRS en 39 pacientes en hemodiálisis crónica utilizando el cuestionario KDQOL-SF. Se recogieron datos sociodemográficos, clínicos y laboratoriales. Se evaluó la adherencia al tratamiento. Se exploraron asociaciones bivariadas. Resultados: La media de edad fue 51.44±17.81 años. El 59 % no fue adherente. En el análisis CVRS, el puntaje total fue 54.14±12.14; las dimensiones con las puntuaciones más altas fueron síntomas/problemas (78.47±6.91) y actitud del personal de hemodiálisis (75.64±5.44) salud mental (71.79±8.75) y con las puntuaciones más bajas función cognitiva (28.93±9.75) rol físico (23.07±5.53) y calidad de relaciones sociales (20.00±8.89). Se encontró relación entre el nivel de hemoglobina y el rol emocional (rs=0.326 p=0.042); los no adherentes tienen mayores puntuaciones en las dimensiones salud general (51.73±16.96 vs 39.06±15.72 p=0.023), PCS (52.22±17.28 vs 40.35±14.44 p=0.030) y la función social (73.91±20.26 vs 53.12±33.38 p=0.020). No se halló relación significativa respecto a la albúmina. Conclusiones: La población estudiada tiene mejor CVRS en las dimensiones síntomas y problemas, actitud del personal de hemodiálisis, salud mental y peor CVRS función cognitiva, rol físico y calidad de relaciones sociales. A mayor hemoglobina el puntaje en el rol emocional aumenta. Además el ser no adherente está relacionado con mejores puntajes en salud general, PCS y función social.

Material y Método: Estudio observacional, longitudinal, retrospectivo en 157 pacientes en hemodiálisis afiliados al Instituto Mexicano del Seguro Social en Guerrero, atendidos en 5 unidades privadas y 2 públicas. Se recolectaron variables socio-demográficas, mediciones de laboratorio, evolución de la enfermedad, complicaciones propias de la enfermedad y del tratamiento dialítico. Resultados: Los eventos adversos se presentaron en el 73% de la población estudiada, la principal causa etiológica fue Diabetes mellitus tipo 2 (DM2) e hipertensión arterial. Los eventos adversos más frecuentes fueron: hipotensión (35,5%) infección de angioacceso (24%), cefalea (22,3%), crisis hipertensiva (14,5%), mareos (9,9%), escalofríos (9,9%), y trombosis de la fístula arterovenosa (9,9%). Los factores que se asociaron a su desarrollo fueron: pacientes mayores de 65 años, (OR=6,859 IC 95%; 1,55-30,35), ser obeso, (OR=1,70, IC 95%: 1,60-4,81), e hipoalbuminemia (OR=0.251, IC 95%: 0,160-0,593). Conclusión: Los pacientes mayores de 65 años, obesos, con hipertensión diastólica e hipoalbuminemia tienen mayor probabilidad de desarrollar eventos adversos durante el periodo de hemodiálisis.

Introducción: El cambio de modalidad de dialítica evidencia un acontecimiento de gran estrés e incertidumbre cuando no es una transferencia deseada, y el cual se describe como un proceso de transición, donde la persona se mueve de un estado de la vida a otro, implicando el dominio de nuevos roles, conocimientos y habilidades para regresar al estado de equilibrio previo al cambio. Objetivos: Describir la experiencia de los pacientes que transitan de diálisis peritoneal a hemodiálisis. Material y Método: Estudio cualitativo, descriptivo, de orientación fenomenológica. Nueve informantes elegidos por muestreo de conveniencia fueron sometidos a entrevista semiestructurada en el domicilio. El desarrollo analítico resultó de un proceso deductivo-inductivo del que surgieron los ejes temáticos en los que se articuló la vivencia de los participantes. Se obtuvo la autorización del Comité Ético y los hallazgos fueron confirmados por los informantes. Resultados: Emergieron 3 temas principales tales como, el significado de la experiencia de transición, las fases del proceso de la transición, y los cambios del ser en transición como fueron las modificaciones físicas, psíquicas, sociales y familiares. Conclusiones: La transición de diálisis peritoneal a hemodiálisis se revela como una vivencia vulnerable que denota cambios en el estado de salud y en las relaciones de roles y habilidades, asimismo, requiere cambio en los patrones de comportamiento de los participantes y, por tanto, resulta en la necesidad de redefinir un sentido de sí mismos.

La enfermedad renal crónica es una patología de alto costo, disminuye la calidad de vida, especialmente, la de los pacientes en hemodiálisis, es importante implementar estrategias para reducir su impacto clínico y económico. El ejercicio intradiálisis ha mostrado la reducción en la mortalidad por cualquier causa y mejoría en la calidad de vida. Por medio de esta revisión de la literatura disponible en las bases de datos Pubmed, OVID, VHL, Clinical Key y LILACS, se busca determinar la seguridad del ejercicio intradiálisis, se han revisado las características de la población intervenida, los tipos de intervención y los eventos adversos. Se incluyeron 8 artículos de la búsqueda sistemática y 15 más por búsqueda manual. Se encontró que la población era en su mayoría adulta (con edades entre los 19 a 88 años) y solo un estudio reportó una población más joven (edades entre los 9,1 a 24,2 años); la proporción entre géneros fue similar. Se encontró que la intervención más frecuente fue el ejercicio cardiovascular realizado con sistemas de pedales, de intensidades bajas a moderadas, con una frecuencia de 2 a 3 sesiones semanales durante mínimo 2 meses. Algunos estudios no reportaron efectos adversos, pero los que sí, mencionan complicaciones en el sistema cardiovascular (hipotensión/ hipertensión) y en otras se manifiesta lesiones músculo esqueléticas (poco frecuentes), pero ninguna considerada severa o frecuente. Con la información disponible se concluyó que el ejercicio intrahemodiálisis parece ser una intervención segura.

Background: The person with chronic kidney disease (CKD) on hemodialysis has a complex drug therapy regimen to follow, both in the control of the symptoms caused by the disease and in the control of comorbidities. Objective: To assess the level of adherence to the drug treatment in CKD patients on a hemodialysis program. Methodology: Quantitative, descriptive-correlational and cross-sectional study, based on the application of the Measure of Treatment Adherence scale (MTA), to a sample of 101 people with CKD, undergoing hemodialysis. Results: The sample had a mean score of adherence to the drug treatment regimen of 5.32 ± 0.47, with a maximum value of 6. The dichotomous pattern of the scale, based on the median, classifies 82.18% of the sample (n = 83) as adherent and 17.82% (n = 18) as non-adherent. A statistically significant relationship of adherence was found as a function of some clinical variables: number of pathologies and daily medication. Conclusion: The percentage of non-adherents to the drug therapy regimen calls for improvement actions and highlights the importance of continuous assessment of adherence levels., Marco contextual: Una persona con enfermedad renal crónica (ERC) en hemodiálisis tiene que seguir un complejo régimen de terapia farmacológica, tanto en el control de los síntomas causados por la enfermedad como en el control de las comorbilidades. Objetivo: Evaluar el nivel de adhesión al régimen terapéutico de medicamentos de la persona con ERC en programa de hemodiálisis. Metodología: Estudio cuantitativo, descriptivo-correlacional y transversal, basado en la aplicación de la Escala de Medición de Adhesión a los Tratamientos (MAT) a una muestra de 101 personas com ERC, en programa de hemodiálisis. Resultados: La muestra reveló una puntuación media de adherencia al régimen terapéutico de medicamentos de 5,32 ± 0,47, para un valor máximo de 6. El patrón dicotómico de la escala, basado en la mediana, clasificó al 82,18% de la muestra (n = 92) como adherente y al 17,82% (n = 18) como no adherente. Se encontró una relación estadísticamente significativa de la adherencia según algunas variables clínicas: número de patologías y medicación diaria. Conclusión: El porcentaje de no adherentes al régimen farmacoterapéutico reclama acciones de mejora y pone de manifiesto la importancia de la evaluación continua de los niveles de adherencia, Enquadramento: A pessoa com doença renal crónica (DRC) em hemodiálise apresenta um regime terapêutico medicamentoso complexo a seguir, seja no controlo da sintomatologia provocada pela mesma, seja no controlo de comorbilidades. 183. Qualidade de vida do cuidador familiar de paciente em hemodiálise

Resumo Objetivo Analisar a qualidade de vida do cuidador familiar do paciente em hemodiálise e a existência de associação com suas características clínicas e sociodemográficas. Métodos Estudo transversal, realizado com 75 cuidadores familiares em dois centros de diálise. A qualidade de vida do cuidador foi medida por meio do instrumento World Health Organization Quality of Life – BREF, e sua associação com as características sociodemográficas foi avaliada pelo teste de Kruskal-Wallis (nível de significância igual a 5%). Resultados A maioria dos cuidadores familiares era mulher (84%), cônjuge (41,3%), filho (38,7%), do lar (38,7%) e possuía renda familiar de um a três salários mínimos (85,4%). A variável trabalhar fora de casa teve influência positiva na qualidade de vida geral (p=0,014), nos domínios psicológicos (p=0,009) e nas relações sociais (p≤0,001). Ser do sexo masculino (p=0,016), não ter doenças (p=0,002) e não usar medicamentos (p=0,007) interferiram positivamente no domínio físico. A ausência de ajuda financeira de outras pessoas e/ou familiares influenciou de forma negativa nos domínios das relações sociais (p=0,050) e meio ambiente (p=0,001). Conclusão O cuidador familiar do sexo masculino, ou que trabalha fora de casa ou recebe ajuda financeira de outras pessoas e/ou familiares apresentaram melhores níveis de qualidade de vida. Em contrapartida, as condições clínicas tais como fazer uso de algum medicamento ou ter doenças influenciaram de forma negativa nos níveis de qualidade de vida. Resumen Objetivo Analizar la calidad de vida de los cuidadores familiares de pacientes en hemodiálisis y la existencia de asociación con las características demográficas y clínicas. Métodos Estudio transversal, realizado con 75 cuidadores familiares en dos centros de diálisis. La calidad de vida de los cuidadores fue medida a través del instrumento World Health Organization Quality of Life – BREF, y su relación con las características sociodemográficas fue evaluada mediante la prueba de Kruskal-Wallis (nivel de significación igual a 5 %). Resultados La mayoría de los cuidadores familiares era mujer (84 %), cónyuge (41,3 %), hijo (38,7 %), del hogar (38,7 %) y poseía ingresos familiares de uno a tres salarios mínimos (85,4 %). La variable trabajar fuera de casa tuvo influencia positiva en la calidad de vida general (p=0,014), en los dominios psicológicos (p=0,009) y en las relaciones sociales (p≤0,001). Ser de sexo masculino (p=0,016), no tener enfermedades (p=0,002) y no usar medicamentos (p=0,007) interfirieron positivamente en el dominio físico. La ausencia de ayuda financiera de otras personas o familiares influyó de manera negativa en los dominios de las relaciones sociales (p=0,050) y del medio ambiente (p=0,001). Conclusión Los cuidadores familiares de sexo masculino, o que trabajan fuera de casa o reciben ayuda financiera de otras personas o familiares presentaron mejores niveles de calidad de vida. En contraste, las condiciones clínicas, tales como hacer uso de algún medicamento o padecer enfermedades, influyeron de forma negativa en los niveles de calidad de vida. Abstract Objective To analyze the quality of life of family caregivers of patients under hemodialysis and the existence of an association with their sociodemographic and clinics characteristics. Methods This is a cross-sectional study conducted with 75 family caregivers in two hemodialysis centers. Caregivers’ quality of life was measured using the World Health Organization Quality of Life – BREF instrument, and its association with sociodemographic characteristics was assessed using the Kruskal-Wallis test (significance level equal to 5%). Results Most family caregivers were women (84%), spouses (41.3%), children (38.7%), housewives (38.7%) and had a family income of one to three minimum wages (85.4 %). The variable working outside the home had a positive influence on overall quality of life (p=0.014), psychological domains (p=0.009) and social relationships (p≤0.001). Being male (p=0.016), not having diseases (p=0.002) and not using medications (p=0.007) positively interfered with the physical domain. The absence of financial help from other people and/or family members had a negative influence on the social relationships (p=0.050) and environment (p=0.001) domains. Conclusion Male family caregivers, or those who work outside the home or receive financial help from other people and/or family members had better quality of life levels. On the other hand, clinical conditions such as using a drug or having diseases have negatively influenced quality of life levels.

Objective: Identify the adverse events occurring in hemodialysis units of the Federal District public network. Methods: This is a descriptive study with a quantitative approach, carried out in Brasília / DF. Data were collected in three public hospitals of the Health's Secretary of the Federal District in the year 2017. These units, together, performed in the year 2017, 1770 hemodialysis sessions in patients with CKD per month. A total of 152 patient charts were analyzed to record adverse events. The data were organized in an Excel spreadsheet, and descriptive statistics were used for the analysis. Results: Adverse events with the highest number of records are related to vascular access to hemodialysis, they are: bleeding, double-catheter secretion lumen, inadequate blood flow, and infection or signs of vascular access infection. As for the person responsible for the registration, 76.9% of the records were made by nursing technicians, 16.6% by nurses and 6.5% by other health team professionals. Conclusion: It is concluded in this study that hemodialysis is a hospital sector with a great risk potential for the occurrence of adverse events, this occurs for several reasons such as: it is a complex procedure, using high technology, chronicity characteristic of chronic renal disease, the high use of medications. Strategies need to be taken in order to optimize the functioning of these vascular accesses, because the quality of dialysis depends on them, consequently the quality of life of the patient with chronic renal disease in dialysis treatment. - Abstract
Foundation: chronic renal failure has a negative impact in the quality of life and constitutes a morbidity predictor in patients with periodic hemodialysis. Objective: to evaluate the influence of quality of life related to health in the morbidity of patients in intermittent periodic hemodialysis. Methods: a descriptive study was done in patients with three or more time of intermittent periodic hemodialysis at the Nephrology service of the Military Hospital Dr. Carlos J. Finlay. Havana, in a 12 month follow up period. The variables analyzed were: age, sex, time with the substituting renal function treatment, morbidity rate, type of morbidity in hemodialysis and hospital admission rate. A questionnaire for quality of life in patients with renal disease (version 1.3) was applied. Results: patients under 60 years old predominated, of high schooling level, with blood hypertension mainly due to renal failure. The most frequent morbidities were: chills, arterial Hypotension, and complications related to vascular access. The admission rate was low. The summary physical component had the lower punctuation followed by the summary mental component. Among these and age there was found an inversely proportional correlation, so as the morbidity rate and the summary physical component. Conclusion: the results obtained are related to preceding studies. The physical component summary results with higher involvement, mainly in the older adult, considering the negative relation with the morbidity rate and hospital admission. [ABSTRACT FROM AUTHOR]
