Abstract: Las resistencias bacterianas a antimicrobianos representan uno de los principales problemas en la actualidad, encontrándose dentro de las principales causas de muerte en todo el mundo. Latinoamérica y Argentina, lejos de ser una excepción a dichos números, presentan año tras año mayor incidencia de infecciones por gérmenes resistentes. Cada día, se conocen con mejor y mayor detalle los mecanismos de resistencia que presentan tanto los Bacilos Gram Negativos como algunos Cocos Positivos. Sin embargo, limitar el problema al sobreuso de antimicrobianos en medicina clínica es solo una pequeña parte de este fenómeno. Así la sobreutilización de los mismos para maximizar los beneficios en pesca, ganadería y cultivo, se han transformado en los principales pilares que sostienen esta problemática. Desde la Atención Primaria de la Salud, consideramos fundamental conocer todos los aspectos que forman parte de esto, para intentar mitigar el daño que las resistencias bacterianas generan en el mundo. Argentina se transformó en el primer país de la región del continente en contar con una Ley de Prevención y Control de la Resistencia a los Antimicrobianos. Sostenemos que es de marcada importancia, que se fomenten más y mejores políticas sanitarias de orden público para poder así enfrentar este creciente desafío.

152. Prevalence and predictors of medication for opioid use disorder among reproductive-aged women

Abstract: Background: Women of reproductive age would benefit from treatment of opioid use disorder (OUD) prior to pregnancy to improve maternal and infant outcomes. In this study, we aimed to identify the prevalence of medication for OUD (MOUD) and characterize correlates of MOUD receipt among 12–49-year-old women with OUD seeking treatment in publicly funded substance use disorder treatment programs at the time of their first treatment episode. Methods: This cross-sectional study explores the demographic and clinical characteristics of women of reproductive age with OUD receiving publicly funded substance use treatment services. We used data from the concatenated 2015–2021 Treatment Episode Data Set–Admissions (TEDS-A), which documents demographic and clinical characteristics of patient admissions to publicly funded substance use treatment services in the United States. Results: In the sample of females aged 12–49 with no prior treatment admissions and primary OUD (n=325,512), 40.53% received MOUD (n=131,930), including 39.40% of non-pregnant women (n=115,315) and 52.79% of pregnant women (n=8423). Pregnant women had significantly higher odds of receiving MOUD (aOR = 2.42, 95%CI: 2.30, 2.54) compared to non-pregnant women. Non-white race, treatment setting, and treatment self-referral were also associated with higher levels of MOUD. Conclusions: We identified a significant unmet need among both pregnant and non-pregnant women with OUD seeking care in publicly funded treatment clinics. While women who are pregnant are significantly more likely to receive evidence-based treatment with MOUD, still 47.21% of pregnant women did not receive MOUD. All reproductive-aged women with OUD should be offered evidence-based treatment options, including MOUD.

154. Micro-Behavior Encoding for Session-based Recommendation

Abstract: Session-based Recommendation (SR) aims to predict the next item for recommendation based on previously recorded sessions of user interaction. The majority of existing approaches to SR focus on modeling the transition patterns of items. In such models, the so-called micro-behaviors describing how the user locates an item and carries out various activities on it (e.g., click, add-to-cart, and read-comments), are simply ignored. A few recent studies have tried to incorporate the sequential patterns of micro-behaviors into SR models. However, those sequential models still cannot effectively capture all the inherent interdependencies between micro-behavior operations. In this work, we aim to investigate the effects of the micro-behavior information in SR systematically. Specifically, we identify two different patterns of micro-behaviors: "sequential patterns" and "dyadic relational patterns". To build a unified model of user micro-behaviors, we first devise a multigraph to aggregate the sequential patterns from different items via a graph neural network, and then utilize an extended self-attention network to exploit the pair-wise relational patterns of micro-behaviors. Extensive experiments on three public real-world datasets show the superiority of the proposed approach over the state-of-theart baselines and confirm the usefulness of these two different micro-behavior patterns for SR., Comment: Accepted by ICDE 2022

Abstract: The series of upgrades to the Large Hadron Collider, culminating in the High Luminosity Large Hadron Collider, will enable a significant expansion of the physics program of the CMS experiment. However, the accelerator upgrades will also make the experimental conditions more challenging, with implications for detector operations, triggering, and data analysis. The luminosity of the proton-proton collisions is expected to exceed $2-3\times10^{34}$~cm$^{-2}$s$^{-1}$ for Run 3 (starting in 2022), and it will be at least $5\times10^{34}$~cm$^{-2}$s$^{-1}$ when the High Luminosity Large Hadron Collider is completed for Run 4. These conditions will affect muon triggering, identification, and measurement, which are critical capabilities of the experiment. To address these challenges, additional muon detectors are being installed in the CMS endcaps, based on Gas Electron Multiplier technology. For this purpose, 161 large triple-Gas Electron Multiplier detectors have been constructed and tested. Installation of these devices began in 2019 with the GE1/1 station and will be followed by two additional stations, GE2/1 and ME0, to be installed in 2023 and 2026, respectively. The assembly and quality control of the GE1/1 detectors were distributed across several production sites around the world. We motivate and discuss the quality control procedures that were developed to standardize the performance of the detectors, and we present the final results of the production. Out of 161 detectors produced, 156 detectors passed all tests, and 144 detectors are now installed in the CMS experiment. The various visual inspections, gas tightness tests, intrinsic noise rate characterizations, and effective gas gain and response uniformity tests allowed the project to achieve this high success rate., Comment: 45 pages, 39 figures, submitted to Nuclear Instruments and Methods in Physics Research Section A

Abstract: Abstract Aim of this study was to assess the impact of virtual monoenergetic images (VMI) on dental implant artifacts in photon-counting detector computed tomography (PCD-CT) compared to standard reconstructed polychromatic images (PI). 30 scans with extensive (≥ 5 dental implants) dental implant-associated artifacts were retrospectively analyzed. Scans were acquired during clinical routine on a PCD-CT. VMI were reconstructed for 100–190 keV (10 keV steps) and compared to PI. Artifact extent and assessment of adjacent soft tissue were rated using a 5-point Likert grading scale for qualitative assessment. Quantitative assessment was performed using ROIs in most pronounced hypodense and hyperdense artifacts, artifact-impaired soft tissue, artifact-free fat and muscle tissue. A corrected attenuation was calculated as difference between artifact-impaired tissue and tissue without artifacts. Abstract: Abstract Background Health systems resilience (HSR) research is a rapidly expanding field, in which key concepts are discussed and theoretical frameworks are emerging with vibrant debate. Fragile and conflict-affected settings (FCAS) are contexts exposed to compounding stressors, for which resilience is an important characteristic. However, only limited evidence has been generated in such settings. We conducted a scoping review to: (a) identify the conceptual frameworks of HSR used in the analysis of shocks and stressors in FCAS; (b) describe the representation of different actors involved in health care governance and service provision in these settings; and (c) identify health systems operations as they relate to absorption, adaptation, and transformation in FCAS. Methods We used standard, extensive search methods. The search captured studies published between 2006 and January 2022. We included all peer reviewed and grey literature that adopted a HSR lens in the analysis of health responses to crises. Thematic analysis using both inductive and deductive approaches was conducted, adopting frameworks related to resilience characteristics identified by Kruk et al., and the resilience capacities described by Blanchet et al. Results Thirty-seven studies met our inclusion criteria. The governance-centred, capacity-oriented framework for HSR emerged as the most frequently used lens of analysis to describe the health responses to conflict and chronic violence specifically. Most studies focused on public health systems’ resilience analysis, while the private health sector is only examined in complementarity with the former. Communities are minimally represented, despite their widely acknowledged role in supporting HSR. The documentation of operations enacting HSR in FCAS is focused on absorption and adaptation, while transformation is seldom described. Absorptive, adaptive, and transformative interventions are described across seven different domains: safety and security, society, health system governance, stocks and supplies, built environment, health care workforce, and health care services. Conclusions Our review findings suggest that the governance-centred framework can be useful to better understand HSR in FCAS. Future HSR research should document adaptive and transformative strategies that advance HSR, particularly in relation to actions intended to promote the safety and security of health systems, the built environment for health, and the adoption of a social justice lens.

Abstract: An increasing number of large-scale multi-modal research initiatives has been conducted in the typically developing population, as well as in psychiatric cohorts. Missing data is a common problem in such datasets due to the difficulty of assessing multiple measures on a large number of participants. The consequences of missing data accumulate when researchers aim to explore relationships between multiple measures. Here we aim to evaluate different imputation strategies to fill in missing values in clinical data from a large (total N=764) and deeply characterised (i.e. range of clinical and cognitive instruments administered) sample of N=453 autistic individuals and N=311 control individuals recruited as part of the EU-AIMS Longitudinal European Autism Project (LEAP) consortium. In particular we consider a total of 160 clinical measures divided in 15 overlapping subsets of participants. We use two simple but common univariate strategies, mean and median imputation, as well as a Round Robin regression approach involving four independent multivariate regression models including a linear model, Bayesian Ridge regression, as well as several non-linear models, Decision Trees, Extra Trees and K-Neighbours regression. We evaluate the models using the traditional mean square error towards removed available data, and consider in addition the KL divergence between the observed and the imputed distributions. We show that all of the multivariate approaches tested provide a substantial improvement compared to typical univariate approaches. Further, our analyses reveal that across all 15 data-subsets tested, an Extra Trees regression approach provided the best global results. This allows the selection of a unique model to impute missing data for the LEAP project and deliver a fixed set of imputed clinical data to be used by researchers working with the LEAP dataset in the future., Comment: 22 pages, 3 figures, 3 tables

Abstract: Background Health challenges like coronavirus disease 2019 (COVID-19) are becoming increasingly complex, transnational, and unpredictable. Studying health system responses to the COVID-19 pandemic is an opportunity to enhance our understanding of health system resilience and establish a clearer link between theoretical concepts and practical ideas on how to build resilience. Methods This narrative literature review aims to address four questions using a health system resilience framework: (i) What do we understand about the dimensions of resilience? (ii) What aspects of the resilience dimensions remain uncertain? (iii) What aspects of the resilience dimensions are missing from the COVID-19 discussions? and (iv) What has COVID-19 taught us about resilience that is missing from the framework? A scientific literature database search was conducted in December 2020 and in April 2022 to identify publications that discussed health system resilience in relation to COVID-19, excluding articles on psychological and other types of resilience. A total of 63 publications were included. Results There is good understanding around information sharing, flexibility and good leadership, learning, maintaining essential services, and the need for legitimate, interdependent systems. Decision-making, localized trust, influences on interdependence, and transformation remain uncertain. Vertical interdependence, monitoring risks beyond the health system, and consequences of changes on the system were not discussed. Teamwork, actor legitimacy, values, inclusivity, trans-sectoral resilience, and the role of the private sector are identified as lessons from COVID-19 that should be further explored for health system resilience. Conclusion Knowledge of health system resilience has continued to cohere following the pandemic. The eventual consequences of system changes and the resilience of subsystems are underexplored. Through governance, the concept of health system resilience can be linked to wider issues raised by the pandemic, like inclusivity. Our findings show the utility of resilience theory for strengthening health systems for crises and the benefit of continuing to refine existing resilience theory.

169. Differential Responses of Soil Nitrogen Forms to Climate Warming in Castanopsis hystrix and Quercus aliena Forests of China

Abstract: Climate warming impacts soil nitrogen cycling in forest ecosystems, thus influencing their productivity, but this has not yet been sufficiently studied. Experiments commenced in January 2012 in a subtropical Castanopsis hystrix Hook. f. and Thomson ex A. DC. plantation and in May 2011 in a temperate Quercus aliena Blume forest, China. Four treatments were established comprising trenching, artificial warming (up to 2 °C), artificial warming + trenching, and untreated control plots. The plots were 2 × 3 m in size. In 2021 and 2022, soil nitrogen mineralization, soil nutrient availability, fine root biomass and microbial biomass were measured at 0–20 cm soil depth in 6 replicate plots per treatment. Warming significantly increased soil temperature in both forests. In the C. hystrix plantation, warming significantly increased available phosphorus (AP) and fine root biomass (FRB), but it did not affect soil microbial biomass carbon (MBC), microbial biomass nitrogen (MBN), microbial biomass phosphorus (MBP) and their ratios. Warming depressed the net mineralization rate (NMR) and net ammonification rate (NAR) of the C. hystrix plantation, probably because the competition for nitrogen uptake by fine roots and microorganisms increased, thus decreasing substrates for nitrogen mineralization and ammonification processes. Trenching and warming + trenching increased the net nitrification rate (NNR), which might be related to decreased NH4+-N absorption of trees in the trenched plots and the increased microbial activity involved in soil nitrification. In the Q. aliena forest, warming significantly increased NH4+-N, MBC/MBN, Root C/N, Root N/P, and decreased pH, MBN, MBN/MBP and Root P; and there was no effect of trenching. Notably, the NAR, NNR and NMR were largely unaffected by long-term warming. We attributed this to the negative effect of increasing NH4+-N and decreasing MBN/MBP offsetting the positive effect of soil warming. This study highlights the vulnerability of subtropical forest stands to long-term warming due to decreased soil N mineralization and increased NO3−-N leaching. In contrast, the soil N cycle in the temperate forest was more resilient to a decade of continuous warming.

172. Unlocking Opportunity, Advancing Equity: Improving College Outcomes for Marginalized Students

Abstract: Empirical evidence unambiguously asserts that a college education provides robust benefits to both individuals and societies. However, disparities in college enrollment and completion persist and limit opportunities for marginalized students to enjoy the advantages a college education affords, impeding progress toward equitable outcomes for all. The three chapters in this dissertation employ quantitative methods to evaluate policies and programs that aim to improve college access and success for marginalized students. The first chapter evaluates efforts to increase college access for rural students and revitalize rural economies. Results suggest a strong positive effect of branch campus openings on local economic activity. The second chapter estimates the differential impact of changes to the federal student loan policy on Historically Black Colleges and Universities (HBCUs) relative to other institutions. Results suggest that HBCUs are more responsive to these policy changes and experienced a disproportionately negative effect on enrollment when access to loans was restricted. The third chapter estimates the impact of a virtual college coaching intervention designed to help low-income, first-generation, and racial/ethnic minority students persist to degree completion. Results suggest that coaching has a positive effect on persistence to the second year, but that effect is not sustained beyond the intervention period. Together, these chapters provide evidence that informs efforts to unlock opportunity for all students and advance equity in college access and completion. Abstract: Realising the full potential for educational technologies to improve the student experience is a challenge for higher education institutions. Although academic resistance is often blamed for poor dissemination of teaching technologies, recent literature has more satisfactorily framed the issue in terms of intransigent organisational culture. Features of higher education organisational culture that are conducive to innovation include senior executive support, freedom to experiment and permission to fail and interdisciplinary and cross-institutional engagement. One approach to changing organisational culture is to establish new organisational structures embodying positive traits. This study qualitatively evaluated a technology in teaching innovators group established in 2018 in a science, health and engineering faculty of an Australian university; it provides evidence that the group improved the culture of teaching technology innovation by creating a safe space for innovation, which members then disseminated into their departmental communities. The study also found that the group created a forum for teaching technology innovators to engage with stakeholders in the university's learning and teaching and information and communication technology units, leading to improved outcomes for the university's learning environment.

Abstract: For sets $A, B\subset \mathbb N$, their sumset is $A + B := \{a+b: a\in A, b\in B\}$. If we cannot write a set $C$ as $C = A+B$ with $|A|, |B|\geq 2$, then we say that $C$ is $\textit{irreducible}$. The question of whether a given set $C$ is irreducible arises naturally in additive combinatorics. Equivalently, we can formulate this question as one about the irreducibility of boolean polynomials, which has been discussed in previous work by K. H. Kim and F. W. Roush (2005) and Y. Shitov (2014). We prove results about the irreducibility of polynomials and power series over the max-min semiring, a natural generalization of the boolean polynomials. We use combinatorial and probabilistic methods to prove that almost all polynomials are irreducible over the max-min semiring, generalizing work of Y. Shitov (2014) and proving a 2011 conjecture by D. L. Applegate, M. Le Brun, and N. J. A. Sloane. Furthermore, we use measure-theoretic methods and apply Borel's result on normal numbers to prove that almost all power series are asymptotically irreducible over the max-min semiring. Abstract: Models designed for intelligent process automation are required to be capable of grounding user interface elements. This task of interface element grounding is centred on linking instructions in natural language to their target referents. Even though BERT and similar pre-trained language models have excelled in several NLP tasks, their use has not been widely explored for the UI grounding domain. This work concentrates on testing and probing the grounding abilities of three different transformer-based models: BERT, RoBERTa and LayoutLM. Our primary focus is on these models' spatial reasoning skills, given their importance in this domain. We observe that LayoutLM has a promising advantage for applications in this domain, even though it was created for a different original purpose (representing scanned documents): the learned spatial features appear to be transferable to the UI grounding setting, especially as they demonstrate the ability to discriminate between target directions in natural language instructions., Comment: *Equal contribution

Abstract: Driven by the need to capture users' evolving interests and optimize their long-term experiences, more and more recommender systems have started to model recommendation as a Markov decision process and employ reinforcement learning to address the problem. Shouldn't research on the fairness of recommender systems follow the same trend from static evaluation and one-shot intervention to dynamic monitoring and non-stop control? In this paper, we portray the recent developments in recommender systems first and then discuss how fairness could be baked into the reinforcement learning techniques for recommendation. 193. Low frequency attenuation of sound with acoustic metamaterials

Abstract: The work presented within this thesis pertains to the use of acoustic metamaterials for low frequency absorption and attenuation of sound. Acoustic metamaterials are structures composed of periodic and sub-wavelength locally resonant unit cells and are typically an order of magnitude or smaller than the associated wavelength of the frequency they are designed to manipulate. All naturally occurring materials have both positive mass density and bulk modulus; acoustic metamaterials have the ability to artificially manipulate these properties to have negative effective quantities dependant on the frequency of the incident acoustic wave. Within this thesis, a general analytical method based upon the linear superposition of terms derived with the transfer matrix method are used to derive simple analytical expressions for complex acoustical systems. Good agreement is found when in comparison to results produced using the transfer matrix method and numerically. One limitation with the effective property models presented is the inability to capture the evanescent coupling between Helmholtz resonators. The mechanism to achieve perfect absorption for one port systems has been explored with the development of single frequency and broadband frequency perfect absorbing acoustic metamaterial unit cells. Through the use of Helmholtz resonators with porous inclusions within their cavities, one port perfect absorbers have been developed that obtain perfect absorption at 290 Hz using a single Helmholtz resonator with a sample thickness of 1/28 the wavelength, and over a broadband frequency range between 275 and 625 Hz using a system of three Helmholtz resonators with a sample thickness of 1/10 the wavelength. In a two port system, Helmholtz resonators with porous inclusions within the cavity have been utilised to achieve perfect absorption at a single frequency and over a broadband frequency range. The single frequency perfect absorber has a sample thickness of 1/16 the wavelength at a frequency of 300 Hz. The broadband perfect absorber presented within this chapter exhibited perfect absorption at 312, 426 and 576 Hz, with a sample thickness of 1/8 the wavelength at the lowest critically coupled frequency. A method of simplification has been proposed for impedance terms derived by the transfer matrix method. This has been applied to the case of a serial array of $M$ coupled identical Helmholtz resonators. The simplification method is reliant upon the use of an impedance contrast to create a small order term which can be used in the Taylor expansion of the impedance expressions. By utilising the leading order term from the Taylor series expansions, simple expressions were found composed of polynomials of the same order as $M$. It was also found that the resonant frequencies of the modelled systems can be obtained through the solution of the polynomials present within the numerator of the impedance approximations. Finally, an ideal analytical model has been developed to model the acoustic attenuation achieved by periodic arrays of non-rigidly backed perforations acting as sound-soft scatterers. It has been shown that periodic arrays of sound soft scatterers produce a low frequency band gap from 0 Hz to a frequency determined by the geometry of the perforations and the unit cell length. Acoustic waves within the frequency range of the bandgap become evanescent, achieving large amounts of attenuation for finite systems, and no wave propagation for infinite systems. To confirm the existence of the low frequency band gap, an experimental setup to investigate the sound propagation in an open ended perforated pipe was designed. The experimental transmission loss highlights that periodically arranged holes in the rigid pipe create a low frequency band gap. The band gap produced experimentally matches the predictions obtained for the simplified numerical model in which the perforations were idealised with an acoustical soft backed elliptical geometry in the 3D finite element model.

Abstract: BackgroundAlthough family caregivers play a critical role in care delivery, research has shown that they face significant physical, emotional, and informational challenges. One promising avenue to address some of caregivers’ unmet needs is via the design of digital technologies that support caregivers’ complex portfolio of responsibilities. Augmented reality (AR) applications, specifically, offer new affordances to aid caregivers as they perform care tasks in the home. ObjectiveThis study explored how AR might assist family caregivers with the delivery of home-based cancer care. The specific objectives were to shed light on challenges caregivers face where AR might help, investigate opportunities for AR to support caregivers, and understand the risks of AR exacerbating caregiver burdens. MethodsWe conducted a qualitative video elicitation study with clinicians and caregivers. We created 3 video elicitations that offer ways in which AR might support caregivers as they perform often high-stakes, unfamiliar, and anxiety-inducing tasks in postsurgical cancer care: wound care, drain care, and rehabilitative exercise. The elicitations show functional AR applications built using Unity Technologies software and Microsoft Hololens2. Using elicitations enabled us to avoid rediscovering known usability issues with current AR technologies, allowing us to focus on high-level, substantive feedback on potential future roles for AR in caregiving. Moreover, it enabled nonintrusive exploration of the inherently sensitive in-home cancer care context. ResultsWe recruited 22 participants for our study: 15 clinicians (eg, oncologists and nurses) and 7 family caregivers. Our findings shed light on clinicians’ and caregivers’ perceptions of current information and communication challenges caregivers face as they perform important physical care tasks as part of cancer treatment plans. Most significant was the need to provide better and ongoing support for execution of caregiving tasks in situ, when and where the tasks need to be performed. Such support needs to be tailored to the specific needs of the patient, to the stress-impaired capacities of the caregiver, and to the time-constrained communication availability of clinicians. We uncover opportunities for AR technologies to potentially increase caregiver confidence and reduce anxiety by supporting the capture and review of images and videos and by improving communication with clinicians. However, our findings also suggest ways in which, if not deployed carefully, AR technologies might exacerbate caregivers’ already significant burdens. ConclusionsThese findings can inform both the design of future AR devices, software, and applications and the design of caregiver support interventions based on already available technology and processes. Our study suggests that AR technologies and the affordances they provide (eg, tailored support, enhanced monitoring and task accuracy, and improved communications) should be considered as a part of an integrated care journey involving multiple stakeholders, changing information needs, and different communication channels that blend in-person and internet-based synchronous and asynchronous care, illness, and recovery.

Abstract: Abstract In degraded ecosystems, soil microbial communities (SMCs) may influence the outcomes of ecological restoration. Restoration practices can affect SMCs, though it is unclear how variation in the onset of restoration activities in woodlands affects SMCs, how those SMCs influence the performance of hard‐to‐establish woodland forbs, and how different woodland forbs shape SMCs. In this study, we quantified soil properties and species abundances in an oak woodland restoration chronosequence (young, intermediate, and old restorations). We measured the growth of three woodland forb species when inoculated with live whole‐soil from young, intermediate, or old restorations. We used DNA metabarcoding to characterize SMCs of each inoculum treatment and the soil after conditioning by each plant species. Our goals were to (1) understand how time since the onset of restoration affected soil abiotic properties, plant communities, and SMCs in a restoration chronosequence, (2) test growth responses of three forb species to whole‐soil inoculum from restoration sites, and (3) characterize changes in SMCs before and after conditioning by each forb species. Younger restored woodlands had greater fire‐sensitive tree species and lower concentrations of soil phosphorous than intermediate or older restored woodlands. Bacterial and fungal soil communities varied significantly among sites. Forbs exhibited the greatest growth in soil from the young restoration. Each forb species developed a unique soil microbial community. Our results highlight how restoration practices affect SMCs, which can in turn affect the growth of hard‐to‐establish forb species. Our results also highlight that the choice of forb species can alter SMCs, which could have long‐term potential consequences for restoration success.

Abstract: Background Pre-existing cardiovascular disease (CVD) or cardiovascular risk factors have been associated with an increased risk of complications following hospitalisation with COVID-19, but their impact on the rate of recovery following discharge is not known.Objectives To determine whether the rate of patient-perceived recovery following hospitalisation with COVID-19 was affected by the presence of CVD or cardiovascular risk factors.Methods In a multicentre prospective cohort study, patients were recruited following discharge from the hospital with COVID-19 undertaking two comprehensive assessments at 5 months and 12 months. Patients were stratified by the presence of either CVD or cardiovascular risk factors prior to hospitalisation with COVID-19 and compared with controls with neither. Full recovery was determined by the response to a patient-perceived evaluation of full recovery from COVID-19 in the context of physical, physiological and cognitive determinants of health.Results From a total population of 2545 patients (38.8% women), 472 (18.5%) and 1355 (53.2%) had CVD or cardiovascular risk factors, respectively. Compared with controls (n=718), patients with CVD and cardiovascular risk factors were older and more likely to have had severe COVID-19. Full recovery was significantly lower at 12 months in patients with CVD (adjusted OR (aOR) 0.62, 95% CI 0.43 to 0.89) and cardiovascular risk factors (aOR 0.66, 95% CI 0.50 to 0.86).Conclusion Patients with CVD or cardiovascular risk factors had a delayed recovery at 12 months following hospitalisation with COVID-19. Targeted interventions to reduce the impact of COVID-19 in patients with cardiovascular disease remain an unmet need.Trail registration number ISRCTN10980107.

198. Exploring the role of service dogs for Canadian military Veterans experiencing suicidality

Abstract: Abstract Despite ample anecdotal evidence, there are limited meaningful studies speaking to the important role of the human-animal bond (HAB) in reducing suicidality. However, research is increasingly showing the viability of service dogs (SDs) as a complementary approach for military Veterans suffering from post-traumatic stress disorder (PTSD) and substance use harms – two of the strongest indicators of suicidality across any population. An original, exploratory study completed in 2020 focused on how SDs supported Canadian Veterans living with PTSD and substance use concerns. From this work, a secondary analysis was then undertaken: 28 transcripts were examined through thematic analysis to explore the experiences of the Veterans who were identified as being at high risk for suicide to better understand how SDs may assist with their suicidality. Our methodological approach for the secondary analysis employed affective coding to discover how the social support system enabled by the SDs reduced experiences of loneliness and hopelessness, as well as symptoms of PTSD, depression, and substance use concerns that are commonly associated with suicidality. The SDs were reported by the Veterans as being a catalyst in reducing self-harm and suicidality, as the HAB provided a unique and necessary form of social support for Veterans that was distinct from what other human-human interactions could provide. While acknowledgement of how context specificity and the lived experience of each individual remains crucial for making sense of suicidality, the significant finding from this research has been the identification of the critical impact that SDs have in the lives of Veterans when it comes to preventing suicide. The SD has been explained as a bridge to improve Veterans’ overall quality of life and reduce markers commonly recognized as precursors to suicide – a finding that may be critical in helping reduce future suicide risk among military Veterans, and warrants further investigation.

199. Patient Abuse, Neglect, and Exploitation: Why Physicians Need to Be Trauma-Informed

Abstract: Introduction Many people experience trauma, and its cumulative effects throughout the life span can alter health, development, and well-being. Despite this, few publications focusing on interpersonal trauma include a holistic understanding of the nature and widespread exposure of trauma experiences for patients. We developed an educational resource to teach residents about identifying and intervening with patients who experience trauma across the life span using a trauma-informed care (TIC) perspective. Methods We created a 4-hour educational session for residents that included didactics, a virtual visit with a domestic violence shelter, a discussion with a person who had experienced trauma, and role-playing. A pretest/posttest retrospective survey assessed resident confidence level in identifying and intervening with patients who may have experienced trauma. We used the Wilcoxon signed rank test to compare pretest and posttest scores and the Kruskal-Wallis test to compare responses by residency type and year. Free-text questions were analyzed for thematic content. Results During the 2021–2022 academic year, 72 of 90 residents (80%) from four residency programs attended and evaluated the session. More than 90% of respondents reported the session met their educational needs and provided them with new ideas, information, and practical suggestions to use in their clinical endeavors. The results demonstrated significantly increased confidence on most of the metrics measured. Discussion This session significantly improved residents' confidence in identifying and intervening with patients who have had trauma experiences using a TIC perspective, which may lead them to provide improved patient care to those who have experienced trauma.

200. Image quality of abdominal photon-counting CT with reduced contrast media dose: Evaluation of reduced contrast media protocols during the COVID19 pandemic supply shortage

Abstract: Rationale and objectives: Aim of this study was to assess the impact of contrast media dose (CMD) reduction on diagnostic quality of photon-counting detector CT (PCD-CT) and energy-integrating detector CT (EID-CT). Methods: CT scans of the abdominal region with differing CMD acquired in portal venous phase on a PCD-CT were included and compared to EID-CT scans. Diagnostic quality and contrast intensity were rated. Additionally, readers had to assign the scans to reduced or regular CMD. Regions-of-interest (ROIs) were placed in defined segments of portal vein, inferior vena cava, liver, spleen, kidneys, abdominal aorta and muscular tissue. Signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were calculated. Results: Overall 158 CT scans performed on a PCD-CT and 68 examinations on an EID-CT were analyzed. Overall diagnostic quality showed no significant differences for PCD-CT with standard CMD which scored a median 5 (IQR:5-5) and PCD-CT with 70% CMD scoring 5 (4–5). (For PCD-CT, 71.69% of the examinations with reduced CMD were assigned to regular CMD by the readers, for EID-CT 9.09%. Averaged for all measurements SNR for 50% CMD was reduced by 19% in PCD-CT (EID-CT 34%) and CNR by 48% (EID-CT 56%). Virtual monoenergetic images (VMI)50keV for PCD-CT images acquired with 50% CMD showed an increase in SNR by 72% and CNR by 153%. Conclusions: Diagnostic interpretability of PCD-CT examinations with reduction of up to 50% CMD is maintained. PCD-CT deducted scans especially with 70% CMD were often not recognized as CMD reduced scans. Compared to EID-CT less decline in SNR and CNR is observed for CMD reduced PCD-CT images. Employing VMI50keV for CMD-reduced PCD-CT images compensated for the effects.
