Low flow hypothermic cardiopulmonary bypass, deep hypothermic circulatory arrest, and regional low-flow cerebral perfusion are special techniques used to facilitate complex intracardiac and aortic surgery in neonates and infants. Each carries a risk of cerebral hypoxia and neurologic morbidity. Neurologic monitoring in the form of near-infrared spectroscopy for cerebral oxygenation, transcranial Doppler ultrasound, and the bispectral index electroencephalogram can monitor the brain during these techniques to determine the minimum acceptable bypass flow rates or maximum acceptable duration of deep hypothermic circulatory arrest. The use of this monitoring has the potential to improve long-term neurologic and developmental outcome.

As the number of CHD repairs in adults continues to increase, these operations will be performed in a wider variety of institutions and systems. Unfortunately, not all of these centers will have an optimal environment for correcting CHD in adults. This type of surgery is best accomplished in a facility specifically designed for treating adults with CHD. Optimal care of these patients is provided by cardiologists who are trained and experienced in pediatric and adult cardiology, by surgeons who are trained and experienced in treating CHD, and by anesthesiologists who are experienced in caring for adults with CHD. Whatever the setting, cardiac anesthesiologists involved in these cases must be thoroughly aware of the anesthetic implications for the unique pathophysiology of each patient, and they must not rely on their "usual" expectations of either true pediatric CHD or acquired adult heart disease.

Background A growing number of adults with functional single ventricles are presenting as candidates for first-time and redo-Fontan operations. This study describes the clinical presentation and early operative results of adults who have undergone Fontan modifications. Methods Between July 1995 and April 2003, 23 patients (>18 years old) had Fontan operations. We retrospectively reviewed their perioperative courses. Results Twenty-three Fontan operations (first-time [n = 8] and redo [n = 15]) were performed with no early or late deaths. No patient has required reoperation. One patient has been listed for orthotopic heart transplantation. The overall mean age is 23 years (18 to 41 years); mean follow-up, 30 months; median postoperative hospital stay, 8 days (4 to 34 days); and median duration of chest tube drainage, 4 days (2 to 12 days). The postoperative New York Heart Association (NYHA) functional class was improved in 22 of 23 patients. Eight first-time Fontan operations (7 of 8 nonfenestrated) were performed; lateral tunnel (n = 7) and extracardiac conduit (n = 1). Two patients had preoperative arrhythmias. New onset arrhythmias (ventricular tachycardia and sinus node dysfunction), requiring treatment, occurred in two patients. Fifteen redo-Fontan operations (all nonfenestrated) were performed; lateral tunnel (n = 5) and extracardiac conduit (n = 10). Fifteen patients had preoperative arrhythmias, thirteen of which had intraatrial reentry tachycardia (IART) and required antiarrhythmic medications. Concomitant intraoperative radiofrequency ablation (RFA) (n = 11) and cryoablation (n = 1) procedures were performed. In the immediate postoperative period, there was IART recurrence in five patients (post-RFA [n = 4] and postcryoablation [n = 1]). At latest follow-up, no patient is being treated with antiarrhythmic medications. Two patients had new onset atrial arrhythmias that required treatment. Conclusions The Fontan operation can be performed in adults with minimal morbidity and improved NYHA functional class. New onset arrhythmias requiring treatment are sources of perioperative morbidity. Complete arrhythmia resolution of the preoperative arrhythmia may not be achieved in the immediate postoperative period in redo-Fontan patients. However, modification (intraoperative radiofrequency ablation-right atrial debulking) of the atrial tachycardia circuits in the redo-Fontan patients can result in complete resolution of preoperative atrial tachyarrhythmias at early follow-up.

Adult patients with congenital heart disease are presenting more frequently for cardiac surgery. Frequently, pediatric congenital heart surgeons perform these procedures at pediatric hospitals. Between July 1995 and June 2000, a retrospective review of adult patients (> or = 18 years old) who had undergone cardiothoracic operations was performed. A total of 112 operations were performed and divided into two groups--81 cardiac operations in 79 patients and 31 noncardiac operations in 23 patients. One patient had a cardiac and noncardiac operation performed. The overall early operative mortality was 6% (6/101). There were 3 late deaths. New-onset cardiac arrhythmias requiring treatment were diagnosed after 5/81 (6%) cardiac operations. Six of 79 (7%) patients were diagnosed with postoperative clinical depression. An acceptable mortality can be achieved when adult patients undergo cardiothoracic operations at a pediatric facility. New-onset arrhythmias necessitating treatment are relatively common, and postoperative clinical depression should be anticipated.

Objective: To compare the perioperative outcome of patients ≥13 years old undergoing surgery for congenital heart disease in a children's hospital by a dedicated congenital heart surgery and anesthesia team with procedure-matched younger control patients. Design: Retrospective medical record review study. From October 1997 to July 2000, medical records of all patients >12 years old requiring cardiopulmonary bypass were reviewed. A control group of patients ≤5 years old was reviewed, and 2 patients were matched to each older patient by diagnosis and surgical procedure. Data are reported as mean ± SD. Older (study) patients were compared with younger (control) patients using t -test or chi square, with p ≤ 0.05 significant. Setting: Medical school–affiliated tertiary-care children's hospital. Participants: Patients undergoing congenital heart surgery. Measurements and Main Results: The study group (older patients) comprised 85 patients, and the control group (younger patients) comprised 170 patients. There were no intraoperative deaths. All major complications—cardiopulmonary resuscitation, neurologic injury, massive hemorrhage with sternotomy, femoral cannulation for emergent bypass, and severe episodes of hypotension on induction of anesthesia—occurred in older patients undergoing repeat sternotomy. Conclusion: Mortality and major morbidity were low in both groups; however, all major intraoperative incidents occurred in older repeat sternotomy patients, suggesting increased perioperative risk for adverse outcomes in these patients. Incorrect positioning of central venous catheters (CVC) in infants and children may lead to serious complications such as perforation of the heart or great vessels. CVC position is not usually assessed until the first postoperative chest radiograph, potentially leaving malposition undetected for several hours. We studied a series of 452 right internal jugular and subclavian catheter placements in infants and children undergoing surgery for congenital heart disease, and measured the distance from the skin insertion site to the radiographic junction of the superior vena cava and right atrium (RA). Based on these data, the following formulae predict that a CVC will be positioned above the RA 97% of the time: correct length of insertion (cm) = (height in cm/10) - 1 for patientsor =100 cm in height, and (height in cm/10) - 2 for patients100 cm in height. Weight-based recommendations were also developed which predict placement of CVC above the RA 98% of the time.This study assessed central venous catheter placement in 452 infants and children undergoing cardiac surgery. Simple, clinically useful guidelines based on height and weight were developed to prevent malposition of these catheters, which may cause serious complications such as perforation of the heart or great vessels.

UNLABELLED We compared the ventilation and pulmonary mechanics produced by a new anesthesia ventilator (NAD 6000) using a circle system with that produced by a critical care ventilator (Servo 900C) using a nonrebreathing circuit in infants with congenital heart disease. Twenty patients, aged 1 day to 7 mo, weighing 2.1 to 4.6 kg, were studied. The NAD 6000 had improved alveolar ventilation: PaCO(2) 43 +/- 8 vs 47 +/- 5 mm Hg (P = 0.005), end-tidal CO(2) 34 +/- 7 vs 37 +/- 5 mm Hg (P = 0.042); larger inspired tidal volumes 12.9 +/- 2.8 vs 11.3 +/- 2.2 mL/kg (P < 0.001), but with higher mean airway pressures 9.7 +/- 1.6 vs 8.6 +/- 1.3 cm H(2)O (P < 0.001). These differences in ventilation and airway pressures were not clinically significant. Although there were differences in observed ventilatory variables, both machines provided adequate ventilation when set in the volume control mode. IMPLICATIONS We compared two ventilators for use in infants. Twenty infants undergoing surgery for congenital heart defects were randomized to receive ventilation first with one ventilator, then with the other. Although there were differences in observed ventilatory variables, both machines provided adequate ventilation when set in the volume control mode.

UNLABELLED We compared the ability of the NAD 6000 (North American Drager, Telford, PA) and the Servo 900C (Siemens-Elema AB, Solna, Sweden) anesthesia ventilators to maintain precise delivery of small tidal volumes (V(t)) and positive end-expiratory pressure using an infant test lung model. A variety of ventilator and lung model settings were selected to test clinical conditions simulating normal and extremely compromised lung function. ECENT IMPROVEMENTS in the instruments for R and the technique of video-assisted thoracoscopy in adults have resulted in increased use of thoracoscopy.l Reported advantages of thoracoscopy include smaller thoracic incisions, reduced postoperative pain, and faster postoperative recovery.2 The descriptions of anesthetic techniques used for thoracoscopy pertain only to adults.2-4 Because thoracoscopy is now used in the pediatric population,5,6 the detailed anesthetic management for thoracoscopy in nine pediatric patients, with emphasis on methods of one-lung ventilation in children, is reported.

Document Development Process The Society of Pediatric Cardiology Training Program Directors (SPCTPD) board assembled a Steering Committee that nominated 2 chairs, 1 SPCTPD Steering Committee member, and 6 additional members from a wide range of program sizes, geographic regions, and subspecialty focuses. Membership of this writing group reflected the diverse backgrounds of the physicians who currently direct pediatric cardiac critical care management, including pediatric cardiology, critical care medicine, and anesthesiology. Representatives from the American College of Cardiology (ACC), American Academy of Pediatrics (AAP), and American Heart Association (AHA) participated. The Steering Committee member was added to provide perspective to each Task Force as a “nonexpert” in that field. Relationships with industry and other entities were not deemed relevant to the creation of a general cardiology training statement; however, employment and affiliation information for authors and peer reviewers are provided in Appendixes 1 and 2, respectively, along with disclosure reporting categories. The writing committee developed the document, approved it for review by individuals selected by the participating organizations (Appendix 2), and addressed their comments. The final document was approved by the SPCTPD, AAP, and AHA in February 2015 and approved by the ACC, as well as endorsed by the Pediatric Cardiac Intensive Care Society, in March 2015. Objective: This study was undertaken to describe the combined measurement of cerebral blood flow velocity and cerebral oxygen saturation as a guide to bypass flow rate for regional low-flow perfusion during neonatal aortic arch reconstruction. Methods: Data were prospectively collected from 34 patients undergoing neonatal aortic arch reconstruction with regional low-flow perfusion. Cerebral oxygen saturation and blood flow velocity were measured by near-infrared spectroscopy and transcranial Doppler ultrasonography, respectively, throughout cardiopulmonary bypass. After cooling to 17°C to 22°C, baseline values of cerebral oxygen saturation and blood flow velocity were recorded during full-flow bypass. Regional low-flow perfusion was instituted for aortic arch reconstruction, and bypass flow rate was adjusted to maintain cerebral oxygen saturations and blood flow velocities within 10% of baseline recorded during cold full-flow bypass. Cerebral oxygen saturations and blood flow velocities were recorded again after repair during full-flow hypothermic bypass. Bypass flow during regional low-flow perfusion was recorded, as were arterial pressure and blood gas data. One-way repeated measures analysis of variance was used to determine differences in values during regional low-flow perfusion relative to baseline and after perfusion. Results: A mean bypass flow of 63 mL/(kg × min) was required to maintain cerebral oxygen saturations and blood flow velocities within 10% of baseline. Mean arterial pressure had a poor correlation with the required bypass flow rate ( r 2 = 0.006 by linear regression analysis). Fourteen of 34 patients had a cerebral oxygen saturation of 95% during regional low-flow perfusion, placing them at risk for cerebral hyperperfusion if the cerebral oxygen saturation had been used alone to guide bypass flow. Pressure was detected in the umbilical or femoral artery catheter (mean 12 mm Hg) in all patients during regional low-flow perfusion. Conclusions: Cerebral blood flow velocity, as determined by transcranial Doppler ultrasonography, adds valuable information to cerebral oxygen saturation data in guiding bypass flow during regional low-flow perfusion. Its most important use may be prevention of cerebral hyperperfusion during periods with high near-infrared spectroscopic saturation values. J Thorac Cardiovasc Surg 2003;125:491-9

Objective The aim of this study was to measure cerebral oxygenation, cerebral blood volume index, and cerebral blood flow velocity values in both cerebral hemispheres before, during, and after regional low-flow cerebral perfusion for neonatal aortic arch reconstruction and to test the hypothesis that cerebral blood volume index measured by near infrared spectroscopy correlates with cerebral blood flow velocity measured by transcranial Doppler ultrasonography. Methods Bilateral near infrared spectroscopy and transcranial Doppler ultrasonography sensors were placed, and values were recorded immediately before, during, and after regional low-flow cerebral perfusion at 18°C. Cerebral oxygen saturations, cerebral blood flow velocities, and cerebral blood volume index values were compared by Mann-Whitney U test. Correlations between values of cerebral blood volume index and cerebral blood flow velocity were tested with Spearman rank order correlation. Results Twenty patients were studied. Median cerebral oxygen saturations for the right and left sides were 95% and 95% before regional low-flow cerebral perfusion, 95% and 87% during regional low-flow cerebral perfusion ( P = .054), and 93% and 94% after regional low-flow cerebral perfusion. Median cerebral blood flow velocity values did not change during regional low-flow cerebral perfusion. Cerebral blood volume index exhibited a poor correlation with cerebral blood flow velocity. Conclusions Regional low-flow cerebral perfusion provides comparable blood flows and oxygenation to both cerebral hemispheres. Transcranial Doppler ultrasonography is recommended as a corroborative method with near-infrared spectroscopy to guide flow during regional low-flow cerebral perfusion, because cerebral blood volume index does not correlate with cerebral blood flow velocity.
