Abstract

Bronchiolitis obliterans syndrome is the main limitation for long‐term survival after lung transplantation. Some specific B cell populations are associated with long‐term graft acceptance. We aimed to monitor the B cell profile during early development of bronchiolitis obliterans syndrome after lung transplantation. The B cell longitudinal profile was analyzed in peripheral blood mononuclear cells from patients with bronchiolitis obliterans syndrome and patients who remained stable over 3 years of follow‐up. CD24hiCD38hitransitional B cells were increased in stable patients only, and reached a peak 24 months after transplantation, whereas they remained unchanged in patients who developed a bronchiolitis obliterans syndrome. These CD24hiCD38hitransitional B cells specifically secrete IL‐10 and express CD9. Thus, patients with a total CD9+B cell frequency below 6.6% displayed significantly higher incidence of bronchiolitis obliterans syndrome (AUC = 0.836, PPV = 0.75, NPV = 1). These data are the first to associate IL‐10‐secreting CD24hiCD38hitransitional B cells expressing CD9 with better allograft outcome in lung transplant recipients. CD9‐expressing B cells appear as a contributor to a favorable environment essential for the maintenance of long‐term stable graft function and as a new predictive biomarker of bronchiolitis obliterans syndrome–free survival. In lung transplant patients with bronchiolitis obliterans syndrome and patients who remained stable over 3 years of follow‐up, IL‐10–secreting CD24hiCD38hi transitional B cells expressing CD9 are associated with better allograft outcome, suggesting CD9‐expressing B cells as a new predictive biomarker of bronchiolitis obliterans syndrome–free survival.

Abstract

In order to determine whether the quantity of bacteria in saliva and the degree of periodontal infection influence the speed of de novo plaque formation in periodontitis patients, 23 patients rinsed with 10 ml sterile saline. All teeth were then supragingivally and professionally cleaned, after which the patients were not allowed to undertake any form of oral hygiene for 24 hours. After this period, the rinsing procedure was repeated and the amount of de novo plaque was assessed. Three months after the initial periodontal therapy was completed the experiment was repeated. Both before and after treatment, the periodontal parameters were evaluated. The results showed that the quantity of the de novo plaque in healthy areas was less than in infected areas. In order to determine the influence of the quantity of bacteria in the saliva, only the areas which were already healthy before the treatment and the same areas after treatment were inspected. It was concluded that the degree of periodontal infection was the most important parameter for the degree of de novo plaque formation, but that the number of bacteria in the saliva also played a role.

Abstract

This study was designed as an external evaluation of the Steyerberg score in the prediction of different categories of postoperative mortality after esophagectomy on a large nationwide database of thoracic surgeons. Data collection was obtained from the Epithor national database encompassing the majority of thoracic procedures performed in France. We retrospectively compared the predicted to the observed postoperative 30-day (30DM), 90-day (90DM) and in-hospital mortality (IHM) rate in each decile of equal patient. Patients included in the study were operated for an esophageal cancer and Gastroesophageal junction (GEJ). Steyerberg score was determined according to its logarithmic formula obtained from a sum score including age, comorbidities, neoadjuvant treatment and hospital volume. Deviation of observed from theoretically expected number of deaths was investigated using the calibration test of Hosmer-Lemeshow. Discrimination of the score was determined using the measure of the area under the receiver operating characteristic curve (AUC) of each category of mortality. Over a 9-year period, 1039 consecutive patients underwent an esophagectomy over 42 centers. Among them, 18 centers were considered as intermediate or high-volume institutions, and 24 were low-volume institutions. There were 841 males (81%) with a mean age of 62.3 ± 10 years. Preoperative treatment was allocated to 420 patients (40%). Numbers of comorbidity was: 1 in 261 patients (25%), 2 in 264 patients (25%), 3 in 383 patients (36%) and 4 in 5 patients (1%). The 30DM, 90DM and IHM rate were, respectively, 5.6%, 9.2% and 9.6%. The main causes of postoperative deaths were related to pulmonary complications (44%), complications of the gastric interposition (28%), cardiologic and thromboembolism events (10%). For 30DM, there were significant differences between predicted/observed mortalities in four deciles, whereas there was no significant difference for 90DM and for IHM. In term of calibration, there was a fair agreement of the Steyerberg score with observed 30DM. Predictions were above 20% for seven deciles. Calibration seemed more adequate for 90DM and for IHM. Predictions were above 20% for only three deciles but deviations were not significant. In terms of discrimination, for the 30DM the Steyerberg score overpredicted, the observed mortality rate and AUC was 0.64 (CI 95%: 0.57-0.71). For the 90DM, AUC indicated 0.63 (CI 95%: 0.57-0.68). For the IHM, AUC indicated 0.63 (CI 95%: 0.58-0.68). Steyerberg scoring system seems to be a moderate risk score of the prediction of the IHM and 90DM. This score appears to have a fair discrimination for the 30DM. Nevertheless, because of its simplicity, we believe that this simple predictive score is relevant and transportable to others institution performing such surgery for benchmarking purposes. Abstract

Phosphate (PO4) is a major component of most fertilizers, and when erosion and runoff occur, large amounts of it enter the water bodies, causing several problems such as eutrophication. Feitsui reservoir, the primary source of water supply to Taipei, reported half of the reservoir's pollutants from nonpoint-source pollution. The value of the PO4 in the water body fluctuates in highly nonlinear and stochastic patterns. However, conventional modeling techniques are no longer sufficiently effective in predicting accurately such stochastic patterns in the concentrations of PO4 in water. Therefore, this study proposes different machine learning algorithms: the artificial neural network (ANN), support vector machine (SVM), random forest (RF), and boosted trees (BT) to predict the concentration of PO4. Monthly measured data between 1986 and 2014 were used to train and test the accuracy of these models. The performances of these models were examined using different statistical indices. Hyperparameters optimization such as cross-validation was performed to enhance the precision of the models. Five water quality parameters were used as input to the proposed models. Different input combinations were explored to optimize the precision. The findings revealed that ANN outperformed the other three models to capture the changes in the concentrations of PO4 with high precision where RMSE is equal to 1.199, MAE is equal to 0.858, and R2 is equal to 0.979, MSE is equal to 1.439, and finally, CC is equal to 0.9909. The developed model could be used as a reliable means for managing eutrophication problems.

Abstract

Thrombotic obstruction of aortic bioprostheses is rare. Few cases have been reported involving the use of the Carpentier-Edwards (CE) prosthesis, the Hancock bioprosthesis, or the Medtronic Intact porcine valve. Thrombolytic therapy for mechanical valve thrombosis has been used frequently even though it is known to carry a high risk of embolism and recurrence. However, the use of this therapy was reported for the first time only recently, in a case of acute aortic thrombosis which occurred 3 1/2 months after bioprosthesis insertion. We report a case of late progressive thrombotic obstruction of a CE aortic valve 3 years after insertion. The case was successfully treated with coumadin therapy, as confirmed by serial Doppler echocardiographic examinations and a 3-year follow-up.

Abstract

BACKGROUND, AIMS: The aim of our study was to investigate the patterns of several metalloproteinases (MMP-1, MMP-2 and MT1-MMP) mRNAs expression using a semi-quantitative reverse transcriptase-polymerase chain reaction (RT-PCR) and to correlate them with clinical parameters and bacteriological diagnosis in healthy versus diseased human gingiva. METHODS: To identify the cell origin of MMP production, in situ hybridization (ISH) was also performed for the MMPs on the same samples. 17 gingival biopsies were collected (13 affected by advanced periodontitis and 4 healthy used as controls) and plaque index, gingival index, pocket depth and bleeding on probing were measured. Subgingival microbial samples were also collected to be analysed by a DNA probe technique. The biopsies were processed both for RT-PCR and ISH. We also investigated a model for bacterial induced MMP expression in human gingival fibroblasts (HGF) infected by Eikenella corrodens. RESULTS: We found an expression of the mRNA encoding MMP-1 only in diseased gingiva but at low levels relative to beta-actin (mean+/-SD: diseased versus healthy: 0.013+/-0.024 versus 0). Although the frequencies and levels of mRNA encoding for MMP-2 or MT1-MMP are not significantly different between each group (mean+/-SD: 0.329+/-0.344 versus 0.137+/-0.219 for MMP-2; 0.485+/-0.374 versus 0.466+/-0.296 for MT1-MMP), using ISH, we observed an expression of both mRNAs in fibroblasts of pathological specimens at sites that histologically showed signs of chronic inflammation and connective tissue remodelling. In vitro infection of HGF by Eikenella corrodens stimulated 3-fold the production of the mRNA encoding MMP-2 while other mRNAs remained unchanged. CONCLUSION: Our results did not reveal significant differences in the expression of mRNAs encoding for the MMPs between healthy and periodontitis-affected patients, reflecting the great heterogeneity in the periodontal status of individuals. However, they indicate that gingival fibroblasts are an active source of MMP-2 production in response to a periopathogen.

180. Genotypic variation in potassium uptake in dryland cotton

Abstract

The variation in potassium (K) uptake was studied by the analysis of leaf K concentration in cotton varieties grown under conditions of terminal drought. Youngest fully-expanded leaves (3rd- 4th from apex of main stem) were used for the analysis of leaf K concentration and physiological parameters (photosynthesis, stomatal conductance, leaf water content, etc). Sampling was performed several years at two different times during the cycle in two different experimental sites. Significant variation in leaf K concentration was recorded among genotypes sampled at flowering and fruiting stages. Leaf K concentration decreased from flowering to boll opening ranging from 2.3-1.36% to 1.29-0.85% and critical leaf K concentrations (below 1.7%K) were recorded at times of high nutrient requirement. Terminal drought imposed a severe restriction over seedcotton yield which varied from 574 up to 1612 kg ha-1 depending on years and location. Boll production and seedcotton yield was positively correlated with leaf K concentration only in 1998. Stomatal conductance and specific leaf weight were the physiological traits more consistently related to leaf K concentration. The strong effect of water stress plant growth and yield components might have shaded any K effect. Environmental factors (drought intensity, soil properties) and genotypic intrinsic traits (variation in K use efficiency) may have interacted to provide a complex response. Genetic correlation for leaf K concentration was found when analyzing sampling dates in each site but it did not occur among sites. Under controlled conditions, differences in root K uptake were found in cultivars and the number of secondary roots was associated with greater shoot K contents. Conditions of diminished K availability and root uptake in drying soils might have affected plant nutrition at times of high requirements.
