158. Vector-Indistinguishability: Location Dependency Based Privacy Protection for Successive Location Data

Abstract

With the wide use of GPS enabled devices and Location-Based Services, location privacy has become an increasingly worrying challenge to our community. Existing approaches provide solid bases for addressing this challenge. However, they mainly focus on independent location perturbation or whole trajectory perturbation with little attention to location dependency based perturbation between two successive locations. This may result in that perturbed successive locations lose mutual distance and direction dependency, which in turn can lead to significant data utility loss. To address this problem, we propose a new location dependency based privacy notion named as vector-indistinguishability (vector-ind). Vector-ind defines a vector to represent the dependency relationship between two successive locations. Correspondingly, it consists of distance-indistinguishability for distance dependency and direction-indistinguishability for direction dependency. Noise generation for applying Differential Privacy is based on the distance and direction, hence can reflect the location dependency to ensure data utility after perturbation. We also present four mechanisms to achieve vector-ind notion. Finally, we evaluate the empirical privacy and utility of vector-ind in real-world GPS data to demonstrate how our proposed mechanisms for vector-ind can protect location privacy with high data utility in successive location data.

Abstract

Objective: To identify the clinical and urodynamic risk factors associated with the development of recurrent febrile urinary tract infections (FUTIs) in children with neurogenic bladder (NB) who perform clean intermittent catheterization (CIC). Methods: Children with NB receiving CIC were prospectively enrolled from January to December 2019 and followed up prospectively for 2 years. All data were compared between occasional (0–1 FUTI) and recurrent FUTIs (≥2 FUTI) groups. In addition, the risk factors for recurrent FUTIs in children were evaluated. Results: Complete data from 321 children were analyzed. Occasional FUTIs occurred in 223 patients, and 98 patients experienced recurrent FUTIs. Univariate and multivariate analyses, showed late‐initiation and low‐frequency CIC, vesicoureteral reflux (VUR), small bladder capacity and low compliance, and detrusor overactivity were associated with an increased risk of recurrent FUTIs. Children with high‐grade VUR (grades IV‒V) had a higher risk of recurrent FUTIs than those with low‐grade VUR (grades I‒III) (odds ratio [OR]: 26.95 vs. OR: 4.78, p < 0.001). Conclusions: Our study suggests that late‐initiation and low‐frequency CIC, VUR, small bladder capacity and low compliance, and detrusor overactivity were associated with recurrent FUTIs in patients with NB. Abstract

The study aimed to investigate the short‐term outcomes of hospitalized patients with chronic liver disease (CLDs) and assess the prognostic impact of predisposition and precipitants, which currently remains unclear. The study included 3970 hospitalized patients with CLDs from two prospective longitudinal multicenter studies (NCT02457637 and NCT03641872) conducted in highly endemic hepatitis B virus (HBV) areas. Competing risk analysis was used to evaluate the effect of predispositions, including the etiology and severity of CLDs and precipitants; on sequential 28, 90, and 365‐day liver transplantation (LT)‐free mortality. Among all enrolled patients, 76.8% of adverse outcomes (including death and LT) within one year occurred within 90 days. Compared with alcoholic etiology, the association of HBV etiology with poorer outcomes was remarkably on the 28thday (hazard ratio [HR], 1.81; 95% confidence interval [CI], 1.07–3.06; p= 0.026); however, and diminished or became insignificant at 90 days and 365 days. Cirrhosis increased the adjusted risk for 365‐day (HR, 1.50; CI, 1.13–1.99; p= 0.004) LT‐free mortality when compared with noncirrhosis. In patients with cirrhosis, prior decompensation (PD) independently increased the adjusted risk of 365‐day LT‐free mortality by 1.25‐fold (p= 0.021); however, it did not increase the risk for 90‐day mortality. Neither the category nor the number of precipitants influenced the adjusted risk of 28 or 90‐day LT‐free mortality. The 90‐day outcome should be considered a significant endpoint for evaluating the short‐term prognosis of hospitalized patients with CLD. Predisposing factors, other than etiology, mainly affected the delayed (365‐day) outcome. Timely effective therapy for CLD etiology, especially antiviral treatments for HBV, and post‐discharge long‐term surveillance monitoring in cirrhotic patients undergoing PD are suggested to enhance disease management and reduce mortality.
