Abstract

Objective Differences/disorders of sex development (DSDs) are rare, congenital conditions involving discordance between chromosomes, gonads, and phenotypic sex and are often diagnosed in infancy. A key subset of parents of children newly diagnosed with a DSD experience clinically elevated distress. The present study examines the relationship between perinatal factors (i.e. gestational age, delivery method) and trajectories of parental adjustment. Methods Parent participants (mothers = 37; fathers = 27) completed measures at baseline, 6- and 12-month follow-up. Multilevel linear regression controlled for clustering of the data at three levels (i.e. time point, parent, and family) and examined the relationship between perinatal factors and trajectories of depressive and anxious symptoms. Two-way interactions between perinatal factors and parent type were evaluated. Results Overall depressive and anxious symptoms decreased over time. There were significant interactions between gestational age and parent type for depressive and anxious symptoms, with younger gestational age having a stronger negative effect on mothers vs. fathers. There was a significant interaction between time and gestational age for depressive symptoms, with 36 weeks' gestational age demonstrating a higher overall trajectory of depressive symptoms across time compared to 38 and 40 weeks. Findings for the delivery method were not significant. Conclusions Findings uniquely demonstrated younger gestational age was associated with increased depressive symptoms, particularly for mothers compared to fathers. Thus, a more premature birth may predispose parents of infants with DSD to distress. Psychosocial providers should contextualize early diagnosis-related discussions within stressful birth experiences when providing support. [ABSTRACT FROM AUTHOR]

Abstract

Introduction: Human Immunodeficiency Virus (HIV) is a retrovirus that targets the CD4+ of human T-lymphocyte cells of the immune system. The weakening immune system causes susceptibility to multiple infective diseases and cancers. This study is aimed to determinant the knowledge regarding HIV/AIDS and to identify the associated factors of the HIV/AIDS knowledge among the urban and rural Malaysian young people. Methods: Data was collected using Computer Assisted Telephone Interviewing (CATI) method. HIV knowledge was assessed using the UNGASS indicators, which contain five questions on HIV prevention and transmission. There were two questions regarding the association between HIV transmission with sexual practice and behaviour. The remainder was one question, each related to insect bite, meal sharing, and knowledge about the physical appearance of someone with HIV infection. Respondents who correctly answer all five questions were considered to have adequate knowledge about HIV/AIDS. Results: The result from a questionnaire revealed the prevalence of the young people HIV/AIDS knowledge in urban area was 14.7% (95% CI: 9.96, 21.28) whereas in rural area was 10.9% (95% CI: 6.83, 16.89) in 2020. The results revealed significant difference of misconceptions on healthy-looking person have HIV where urban was 71.7% (95% CI: 66.46, 76.37) and rural was 59.8% (95% CI: 56.05, 63.41). Furthermore, there are significant difference of misconceptions on person get HIV by sharing food with someone who is infected where as urban was 64.8% (95% CI: 60.48, 68.98) while rural was 52.6% (95% CI: 48.67, 56.50). Conclusion: The findings from this survey have important implications for the development of primary HIV/AIDS prevention programs and HIV educational campaigns to increase knowledge and dispel misconceptions about HIV.

Abstract

Background and challenges to implementation Second-Hand Smoking (SHS) exposure is common in restaurants and other enclosed places where smoking is permitted as some people failed to notice that restaurant is a public place. Tobacco free environment policy is one of the way to reduce the SHS exposure. This study aimed to determine the level of knowledge, attitude and practice towards tobacco free environment among restaurant workers and owners and the factors affecting thereof. Intervention or response This was a cross-sectional study conducted among the restaurant workers and owners irrespective of gender. A total of 301 respondents had participated in this study. Data was collected by assisted self-administered questionnaires. Data on current tobacco control law, preventive knowledge and attitude, support and implementation of tobacco free restaurant were collected. Data entry and analysis were done by using SPSS version 22.0. Results and lessons learnt The mean (SD) age of the restaurant workers and owners was 26.0(8.9) years with minimum age 15 and maximum 74 years old. Most of the respondents (47.2%) had good level of knowledge on Tobacco Control Law in Malaysia and 36.2% of them had excellent knowledge on SHS. Besides, 30.2% of them had good attitude towards tobacco free restaurants. Moreover, less than two-fifth (14.6%) of them were implementing 100% tobacco free restaurant. About half of the respondents (48.6%) had good support towards tobacco free restaurants. The support towards tobacco free restaurants was highest among Iban, respondent with secondary educational level non-smokers, having good knowledge on tobacco control law and SHS and having excellent level of attitude towards tobacco free restaurants (p< 0.05). Conclusions and key recommendations Although the results did not depict national scenarios, however, the findings could be important in helping the effort on implementation of tobacco free restaurants. The findings of the results could also be utilised by policy makers for planning the implementation of tobacco free policy in restaurants in Malaysia.

Abstract

Objective: Verbal autopsy (VA) through face-to-face interviews with caregivers is a way to determine cause of death without medical certification. In Malaysia, the use of VA has improved mortality statistics. However, during the coronavirus disease 2019 (COVID-19) pandemic, face-to-face interviews were delayed, reducing VA data collection and affecting data for mortality surveillance. This study aims to investigate the feasibility and acceptability of conducting VA interviews via telephone calls, and the quality of the data gathered. Methods: The study was conducted in Malaysia from September to October 2020 using a cross-sectional design. Participants were health-care workers from established VA teams across the country. They conducted VA interviews via telephone and provided feedback through a customized online form. Data collected from the form were used to assess the feasibility, acceptability and quality of the telephone interviews using IBM SPSS version 23. Results: Responses were received from 113 participants. There were 74 (65.5%) successful interviews, representing 91% of the 81 cases who were able to be contacted. More than two thirds of health-care workers provided positive feedback on the telephone interview method for themselves and the interviewees. Only 10.8% of causes of death were unusable. Discussion: This study provides preliminary evidence that VA via telephone interview is feasible, acceptable and can be used as an alternative to face-to-face interviews without affecting data quality. During times when face-to-face interviews are not advisable, VA telephone interviews can be used for data collection for mortality surveillance.

184. 'We Spend More Time with the Children Than They Do?…:' Education, Care and the Work of Foreign Domestic Workers in Hong Kong

Abstract

Despite the ongoing global financial crisis, there is an increasing deployment of migrant workers across the globe, and in Hong Kong the foreign domestic worker occupies a ubiquitous presence in the lives of many families. Seven domestic workers from Indonesia, the Philippines, and Thailand were interviewed to gain insight into their role in providing education and care for children in Hong Kong. This exploratory study focuses on the educational and care aspects of the work of foreign domestic workers. It considers the nature of their work and their relationship with the mother of the family. Findings reveal the difficulties of maintaining coordinated education and care of children in a culture where the dynamics of the unequal power relations are clearly demarcated. We raise new possibilities for interdisciplinary work to re-theorise the ways in which foreign domestic workers act as educators of young children and we call for new methodologies to enable their potential.
