Abstract

Background Data collected in EHRs have been widely used to identifying specific conditions; however there is still a need for methods to define comorbidities and sources to identify comorbidities burden. We propose an approach to assess comorbidities burden for a specific disease using the literature and EHR data sources in the case of autoimmune diseases in celiac disease (CD). Methods We generated a restricted set of comorbidities using the literature (via the MeSH® co-occurrence file). We extracted the 15 most co-occurring autoimmune diseases of the CD. We used mappings of the comorbidities to EHR terminologies: ICD-10 (billing codes), ATC (drugs) and UMLS (clinical reports). Finally, we extracted the concepts from the different data sources. We evaluated our approach using the correlation between prevalence estimates in our cohort and co-occurrence ranking in the literature. Results We retrieved the comorbidities for 741 patients with CD. 18.1% of patients had at least one of the 15 studied autoimmune disorders. Overall, 79.3% of the mapped concepts were detected only in text, 5.3% only in ICD codes and/or drugs prescriptions, and 15.4% could be found in both sources. Prevalence in our cohort were correlated with literature (Spearman’s coefficient 0.789, p = 0.0005). The three most prevalent comorbidities were thyroiditis 12.6% (95% CI 10.1–14.9), type 1 diabetes 2.3% (95% CI 1.2–3.4) and dermatitis herpetiformis 2.0% (95% CI 1.0–3.0). Conclusion We introduced a process that leveraged the MeSH terminology to identify relevant autoimmune comorbidities of the CD and several data sources from EHRs to phenotype a large population of CD patients. We achieved prevalence estimates comparable to the literature. 162. Long-term follow-up after endoscopic resection for superficial esophageal squamous cell carcinoma: a multicenter Western study.

Abstract

Background: Endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) are the first-line treatments for superficial esophageal squamous cell carcinoma (SCC). This study aimed to compare long-term clinical outcome and oncological clearance between EMR and ESD for the treatment of superficial esophageal SCC.Methods: We conducted a retrospective multicenter study in five French tertiary care hospitals. Patients treated by EMR or ESD for histologically proven superficial esophageal SCC were included consecutively.Results: Resection was performed for 148 tumors (80 EMR, 68 ESD) in 132 patients. The curative resection rate was 21.3 % in the EMR group and 73.5 % in the ESD group (P < 0.001). The recurrence rate was 23.7 % in the EMR group and 2.9 % in the ESD group (P = 0.002). The 5-year recurrence-free survival rate was 73.4 % in the EMR group and 95.2 % in the ESD group (P = 0.002). Independent factors for cancer recurrence were resection by EMR (hazard ratio [HR] 16.89, P = 0.01), tumor infiltration depth ≥ m3 (HR 3.28, P = 0.02), no complementary treatment by chemoradiotherapy (HR 7.04, P = 0.04), and no curative resection (HR 11.75, P = 0.01). Risk of metastasis strongly increased in patients with tumor infiltration depth ≥ m3, and without complementary chemoradiotherapy (P = 0.02).Conclusion: Endoscopic resection of superficial esophageal SCC was safe and efficient. Because it was associated with an increased recurrence-free survival rate, ESD should be preferred over EMR. For tumors with infiltration depths ≥ m3, chemoradiotherapy reduced the risk of nodal or distal metastasis. [ABSTRACT FROM AUTHOR]

163. Comment réséquer les polypes colorectaux de petite taille ?

Abstract

Résumé: Les polypes de petite taille (< 10 mm) sont les lésions les plus fréquemment rencontrées lors d'une coloscopie de dépistage. On distingue les micropolypes ou « diminutive polyps (< 5 mm), des petits polypes ou « small polyps » (6-9 mm) dont le risque de dégénérescence dans la littérature est estimé à 0,5-2 %. Malgré l'amélioration des techniques de caractérisation endoscopique optique, les polypes de petite taille doivent faire l'objet d'une résection endoscopique. Les stratégies de « resect and discard » (résection-destruction) ou de « predict and leave behind » (diagnostic sans résection), stratégies sans analyse histologique, doivent être réservées aux centres experts présentant un degré de confiance élevé en prédiction histologique fondée sur l'analyse optique. Il existe de nombreuses techniques endoscopiques pour l'exérèse de ces polypes, dont l'efficacité est variable. On estime que les taux de résections incomplètes varient de 7 % à 10 %, et qu'elles sont responsables d'environ 10 % à 27 % des cas de cancer d'intervalle. La société européenne d'endoscopie digestive (ESGE) a établi en 2012 des recommandations pour la prise en charge des polypes de petite taille, mises à jour en 2017. L'exérèse à l'anse froide est la technique de référence pour ces lésions, car elle est associée à un taux élevé de résection complète, à une meilleure analyse histologique et à un faible taux de complication. En cas de positionnement difficile, l'utilisation de la pince à biopsie froide pourra être utilisée pour les très petits polypes (< 4 mm). L'utilisation de la pince à biopsie chaude doit être proscrite car elle a un faible taux d'ablation complète et augmente le risque de complication post-exérèse. Polyps smaller than 10 mm are the most common lesions encountered during screening colonoscopy and are associated with low risk of colorectal cancer. These lesions are classified according to their size as diminutive polyps (< 5 mm) or small polyps (6-9 mm). Despite improvement in real time characterisation techniques, small polyps must be endoscopically resected. Strategies such as "resect and discard" or "diagnosed and leave behind" are dedicated to expert centres with a high degree of confidence in optical diagnosis. Many endoscopic techniques are available for polyps resection, whose effectiveness is variable in the literature. Incomplete resection rates range from 7% to 10% and are responsible for up to 27% of interval cancer. The European Society of Digestive Endoscopy established in 2012 (updated in 2017) recommendations for the resection of small polyps. Cold snare resection is the gold standard for these polyps because it is associated with a high rate of complete resection, a better histological analysis and a low rate of complications. In case of difficult positioning, the use of the cold biopsy forceps can be used for very small polyps (<4 mm). The use of hot biopsy forceps should be proscribed as it is associated with a low rate of complete resection and post resection complications. [ABSTRACT FROM AUTHOR]

164. Rôle du réseau PRED-IdF, premier réseau régional spécialisé dans la prise en charge et suivi des patients présentant une prédisposition héréditaire aux cancers digestifs

Abstract

Les patients présentant une prédisposition génétique aux cancers digestifs sont à haut risque de développer une néoplasie et doivent être suivis au sein de réseaux spécialisés. Le réseau PRED-IdF a été mis en place par l’INCa en 2009 pour l’ensemble de la région Île-de-France et comporte depuis 2012 (déploiement du dispositif national d’oncogénétique) 7 centres experts dans la prise en charge des prédispositions génétiques aux cancers digestifs. L’objectif de ce réseau est de proposer aux patients une prise en charge et un suivi multidisciplinaire spécifique à chaque syndrome de prédisposition. Tout patient présentant une suspicion de syndrome de prédisposition peut être adressé à l’un des centres du réseau pour un avis spécialisé. Les principales missions du réseau incluent l’établissement d’un plan personnalisé de suivi (PPS) et sa coordination, la mise en place d’activité de recours et d’expertise pour les cas difficiles et le développement de l’activité recherche. Depuis 2009, 3384 patients ont été inclus dans le réseau dont 1925 pour syndrome de Lynch et 539 pour polypose adénomateuse familiale (mutations APCet MUTYH), représentant ainsi 72,8 % de la cohorte PRED-IdF. L’impact bénéfique du réseau a été récemment démontré pour les patients suivis pour syndrome de Lynch, marqué par une diminution de la survenue de cancers colorectaux après inclusion. Le réseau s’implique aussi dans de nombreux projets de recherche, allant du développement de collaborations en recherche fondamentale à la participation aux études contrôlées randomisées. L’objectif à terme, étant de proposer une médecine personnalisée à l’ensemble des patients inclus dans le réseau.

165. Complications of coeliac disease

Abstract

Within the past 20 years the spectrum of complications of coeliac disease (CD) has been considerably extended. Besides the classic complications, autoimmune diseases and osteopenia, numerous forms of CD non-responsive to a gluten-free diet have been recently identified. Among the non-responsive CD, the majority of patients presents as long term responders. However a small subset of CD patients becomes refractory to a gluten-free diet with persistent malabsorption and intestinal villous atrophy. Whereas refractory coeliac disease type I (RCDI) is hardly distinguishable from active CD, the type II (RCDII) has a severe clinical presentation and a very poor prognosis. Enteropathy Associated T cell Lymphoma (EATL) is even more aggressive with a five year survival of 20%. Classic adriamycin-based chemotherapy is poorly efficient in the lymphomatous complications of CD and current therapeutic strategies focus on more intensive regimen with autologous or allogenic stem cell transplantation. Notable pathogenic advances let us to test targeted therapy both in low (RCDII) and high grade lymphomatous (EATL) complications associated with CD.

166. Refractory Celiac Disease

Abstract

Refractory celiac disease (RCD) refers to persistence of malnutrition and intestinal villous atrophy for more than 1 to 2 years despite strict gluten-free diet in patients with celiac disease. Diagnosis remains difficult and impacts treatment and follow-up. RCD has been subdivided into 2 subgroups according to the normal (RCDI) or abnormal phenotype of intraepithelial lymphocytes (IELs) (RCDII). RCDII is considered as a low-grade intraepithelial lymphoma and has a poor prognosis due to gastrointestinal and extraintestinal dissemination of the abnormal IELs, and high risk of overt lymphoma.

Abstract

Résumé: L'hypersensibilité au gluten non cœliaque est un syndrome imputé à l'ingestion de gluten, distinct de la maladie cœliaque et l'allergie au blé. Elle se manifeste par des symptômes digestifs et extradigestifs semblables à ceux observés au cours de la maladie cœliaque et du syndrome de l'intestin irritable. En revanche, on ne trouve chez ces malades ni lésions intestinales à type d'atrophie villositaire, ni anticorps anti-transglutaminase ou anti-endomysium. La physiopathologie de la sensibilité au gluten non cœliaque n'est pas connue et est controversée. L'imputabilité du rôle du gluten reste à démontrer, car d'autres protéines du blé (les inhibiteurs de l'alpha-amylase et la trypsine) ou les FODMAPs pourraient être responsables des symptômes. L'hypersensibilité au gluten est un diagnostic d'exclusion après avoir éliminé une maladie cœliaque et une allergie au blé. À ce jour, aucun biomarqueur spécifique de l'hypersensibilité au gluten n'a été identifié. Le traitement repose sur le régime sans gluten comme dans la maladie cœliaque ou les allergies au blé. ..LAN-EN Gluten related disorders include celiac disease, wheat allergy and non-celiac gluten sensitivity. Non celiac gluten sensitivity is characterized by intestinal and extra-intestinal symptoms that occur after gluten ingestion, disappear with gluten withdrawal from diet and relapse following gluten challenge. These manifestations are similar to those occuring in celiac disease and irritable bowel syndrom. However, patients with non-celiac gluten sensitivity do not suffer from lesions in the duodenal mucosa and do not develop tissue transglutaminase or endomysium antibodies. The pathogenesis is not yet well understood and the role of gluten is still controversial, with studies suggesting that FODMAPs or other wheat proteins could be responsible for the symptoms. The diagnosis is made after exclusion of celiac disease and wheat allergy. There are no laboratory markers specific to gluten sensitivity. Treatment consists in avoiding gluten to improve symptoms. Abstract

Purpose: The aim of this prospective study was to evaluate the feasibility, tolerance and performance of virtual enteroscopy (VE) using carbon dioxide for small-bowel distension in patients with suspected small-bowel tumours (SBTs).Patients and Methods: After IRB approval, 17 patients with suspected SBTs were prospectively included. Radiation dose was compared to 34 matched patients (2 for 1) for age, gender and body weight, who had undergone CT-enteroclysis with neutral contrast (CTE). Performance of VE was evaluated through comparison with the current standard of reference, including surgery and/or enteroscopy and/or follow-up.Results: Tolerance was excellent in 16/17 patients (94%). The radiation dose was lower for VE than for CTE (533 ± 282 vs. 974 ± 505 mGy.cm; p = 0.002). With VE, a total of 25 polyps >5 mm in size were depicted in 12/17 patients. On a per-lesion analysis, sensitivity and positive predictive value of VE were 92.0% and 92.0%, respectively. On a per-segment analysis VE had a sensitivity and specificity of 95.0% and 87.0%, respectively.Conclusion: Our preliminary study suggests that VE is a feasible and well-tolerated technique with high sensitivity and specificity for the diagnosis of SBT.Key Points: • Virtual enteroscopy is feasible and well tolerated. • Virtual enteroscopy appears to be accurate for detection of small-bowel tumours. • Sensitivity and PPV of virtual enteroscopy is 92.0% and 92.0%. • Radiation dose is lower with virtual enteroscopy compared to MDCT-enteroclysis. ObjectivesPrimary GI T-cell lymphoproliferative diseases (T-LPD) are heterogeneous entities, which raise difficult diagnosis and therapeutic challenges. We have recently provided evidences that lymphomas complicating coeliac disease (CD) arise from innate-like lymphocytes, which may carry NK receptors (NKRs).DesignNKRs expression was compared by flow cytometry in intraepithelial lymphocytes (IEL) from CD, type I or type II refractory CD (RCD). NKp46 was next assessed by immunohistochemistry in paraffin-embedded biopsies from 204 patients with CD, RCDI, RCDII or GI T-cell lymphomas and from a validation cohort of 61 patients. The cytotoxic properties of an anti-NKp46 monoclonal antibody conjugated to pyrrolobenzodiazepine (PBD) was tested ex vivoin human primary tumour cells isolated from fresh duodenal biopsies.ResultsNKp46 (but not CD94, NKG2A, NKG2C, NKG2D) was significantly more expressed by malignant RCDII IEL than by normal IEL in CD and RCDI. In paraffin biopsies, detection of >25 NKp46+ IEL per 100 epithelial cells discriminated RCDII from CD and RCDI. NKp46 was also detected in enteropathy-associated T-cell lymphomas (EATL, 24/29) and in monomorphic epitheliotropic intestinal T-cell lymphomas (MEITL, 4/4) but not in indolent T-LPD (0/15). Treatment with anti-NKp46-PBD could efficiently and selectively kill human NKp46+ primary IEL ex vivo.ConclusionNKp46 is a novel biomarker useful for diagnosis and therapeutic stratification of GI T-LPD. Strong preclinical rationale identifies anti-NKp46-PBD as a promising therapy for RCDII, EATL and MEITL.

Abstract

Celiac disease is a chronic inflammation of the small intestine with villous atrophy that can become refractory to a gluten-free diet. Two categories of refractory celiac disease can be distinguished by the phenotype of intraepithelial lymphocytes and the status of TRGgenes. Their distinction is important because 30% to 50% of type II but only 0% to 14% of type I evolve to an aggressive enteropathy-associated T-cell lymphoma and therefore require intensive treatment. Currently, differential diagnosis integrates immunohistochemistry, immunophenotyping, and TRGclonality analyses, but each has limitations. A single-tube multiplex TRGPCR (ECN) was prospectively compared to an in-house two-tube TRGPCR (N2T) in 73 samples, including 67 cryopreserved intestine tissues. Thirteen formalin-fixed, paraffin-embedded (FFPE) samples were also analyzed retrospectively. The ECN PCR had comparable efficiency to detect major clonal rearrangements in highly infiltrated tissues from T-cell lymphoproliferative disorders and type II refractory celiac disease and to detect the persistence of minor clones in type II refractory celiac disease follow-up samples. The ECN PCR abolished the risk of amplification of false-positive weak clonal rearrangements in cryopreserved specimens and allowed improved detection of clonal rearrangements in DNA from FFPE samples. The ECN PCR allows robust assessment of cryopreserved and FFPE digestive tissues at diagnosis and follow-up of enteropathies with villous atrophy, thus guiding therapeutic management.

Abstract

International audience; BACKGROUND: Observational studies showed that the profile of infective endocarditis (IE) significantly changed over the past decades. However, most studies involved referral centers. We conducted a population-based study to control for this referral bias. The objective was to update the description of characteristics of IE in France and to compare the profile of community-acquired versus healthcare-associated IE. METHODS: A prospective population-based observational study conducted in all medical facilities from 7 French regions (32% of French individuals aged ≥18 years) identified 497 adults with Duke-Li-definite IE who were first admitted to the hospital in 2008. Main measures included age-standardized and sex-standardized incidence of IE and multivariate Cox regression analysis for risk factors of in-hospital death. RESULTS: The age-standardized and sex-standardized annual incidence of IE was 33.8 (95% confidence interval [CI], 30.8-36.9) cases per million inhabitants. The incidence was highest in men aged 75-79 years. A majority of patients had no previously known heart disease. Staphylococci were the most common causal agents, accounting for 36.2% of cases (Staphylococcus aureus, 26.6%; coagulase-negative staphylococci, 9.7%). Healthcare-associated IE represented 26.7% of all cases and exhibited a clinical pattern significantly different from that of community-acquired IE. S. aureus as the causal agent of IE was the most important factor associated with in-hospital death in community-acquired IE (hazard ratio [HR], 2.82 [95% CI, 1.72-4.61]) and the single factor in healthcare-associated IE (HR, 2.54 [95% CI, 1.33-4.85]). CONCLUSIONS: S. aureus became both the leading cause and the most important prognostic factor of IE, and healthcare-associated IE appeared as a major subgroup of the disease.

Abstract

International audience; Background & Aims: Refractory celiac disease (RCD) was recently subdivided into 2 subtypes (RCD I and 11) based on a normal or abnormal phenotype of intraepithelial lymphocytes (IELs), respectively. It is not clear, however, if these 2 entities differ in their presentation at diagnosis or long-term outcome. We compared the clinical and biological characteristics of RCD I and RCD IT at diagnosis, the risk of developing an overt lymphoma, and the predictive factors of survival. Methods: Medical files of 14 patients with RCD I and 43 with RCD II were analyzed retrospectively. Predictive factors of overt lymphoma and survival were studied in univariate and multivariate analyses. Results: At diagnosis, malnutrition, ulcerative jejunitis, and lymphocytic gastritis were more common m patients with RCD II than RCD I (P < .05). Overt lymphomas occurred in 2 patients with RCD I and 16 with RCD II. In the univariate analysis, abnormal IEL phenotype and increased age at diagnosis of RCD were predictive factors for overt lymphoma. Abnormal TEL phenotype (P < .01), clonality (P = .01), and overt lymphoma. (P = .001) predicted short survival time. Only abnormal TEL phenotype (P = .03) and overt lymphoma (P = .04) were predictive in the multivariate analysis. The 5-year survival rate was 93% in patients with RCD I and 44% with RCD II. Conclusions: RCD II has a much more severe presentation and prognosis than patients with RCD I

Abstract

International audience; Celiac disease (CD) is an enteropathy resulting from an abnormal immune response to gluten-derived peptides in genetically susceptible individuals. This immune response is initiated by intestinal transport of intact peptide 31-49 (p31-49) and 33-mer gliadin peptides through an unknown mechanism. We show that the transferrin receptor CD71 is responsible for apical to basal retrotranscytosis of gliadin peptides, a process during which p31-49 and 33-mer peptides are protected from degradation. In patients with active CD, CD71 is overexpressed in the intestinal epithelium and colocalizes with immunoglobulin (Ig) A. Intestinal transport of intact p31-49 and 33-mer peptides was blocked by polymeric and secretory IgA (SIgA) and by soluble CD71 receptors, pointing to a role of SIgA-gliadin complexes in this abnormal intestinal transport. This retrotranscytosis of SIgA-gliadin complexes may promote the entry of harmful gliadin peptides into the intestinal mucosa, thereby triggering an immune response and perpetuating intestinal inflammation. Our findings strongly implicate CD71 in the pathogenesis of CD.
