156. Cost-effectiveness of a physician-nurse supplementary triage assessment team at an academic tertiary care emergency department

Abstract

ABSTRACTObjectiveThe purpose of this study was to evaluate the cost-effectiveness of physician-nurse supplementary triage assistance team (MDRNSTAT) from a hospital and patient perspective.MethodsThis was a cost-effectiveness evaluation of a cluster randomized control trial comparing the MDRNSTAT with nurse-only triage in the emergency department (ED) between the hours of 0800 and 1500. Cost was MDRNSTAT salary. Revenue was from Ontario’s Pay-for-Results and patient volume-case mix payment programs. The incremental cost-effectiveness ratio was based on MDRNSTAT cost and three consequence assessments: 1) per additional patient-seen; 2) per physician initial assessment (PIA) hour saved; and 3) per ED length of stay (EDLOS) hour saved. Patient opportunity cost was determined. Patient satisfaction was quantified by a cost-benefit ratio. A sensitivity analysis extrapolating MDRNSTAT to different working hours, salary, and willingness-to-pay data was performed.ResultsThe added cost of the MDRNSTAT was $3,597.27 [$1,729.47 to 8] per additional patient-seen, $75.37 [$67.99 to $105.30] per PIA hour saved, and $112.99 [$74.68 to $251.43] per EDLOS hour saved. From the hospital perspective, the cost-benefit ratio was 38.6 [19.0 to 8] and net present value of –$447,996 [–$435,646 to –$459,900]. For patients, the cost-benefit ratio for satisfaction was 2.8 [2.3 to 4.6]. If MDRNSTAT performance were consistently implemented from noon to midnight, it would be more cost-effective.ConclusionsThe MDRNSTAT is not a cost-effective daytime strategy but appears to be more feasible during time periods with higher patient volume, such as late morning to evening.

Abstract

Background: The 'Act FAST' campaign is a major international stroke prevention program aiming to raise knowledge and awareness of stroke signs and symptoms and encouraging calling emergency medical service (EMS) providers as soon as symptoms are recognized.1 We aim to evaluate the impact of the campaign by analyzing EMS stroke calls in Qatar as studies on the impact of such initiative on behavioral response at onset are limited.2 Methods: This is a retrospective time series evaluation utilizing monthly Hamad Medical Corporation (HMC) EMS call data for strokes in Qatar. HMC Ambulance Service (HMCAS) uses the Medical Priority Dispatch System (MPDS) and International Academies of Emergency Dispatch (IAED) protocols. Emergency calls are thoroughly analyzed for stroke diagnosis. We will evaluate pre- and post-campaign the relative change in stroke calls volume and classify them into true stroke and stroke mimic calls based on Emergency Department and Stroke Unit data, and make a comparison in total proportion of true stroke calls. We will perform multivariable regression to measure the effect of campaign exposure on the volume of stroke-related calls and also describe any significant association between specific patient characteristics with increased ambulance usage, after controlling for confounders. Anticipated results: Following the signs and symptoms of the stroke awareness campaign and suggested behavioral response at stroke onset, a relative increase in the number of stroke calls to EMS is expected as reported by Bray et al., following a similar campaign in Australia3. Conclusion: The impact of the Act FAST stroke campaign on the population of Qatar will be determined by evaluating a relative change in stroke calls post-campaign and misclassifications. The percentage change in false alarms and calls without stroke specific symptoms in a population group will help identify campaign related contributing factors thereby providing evidence to tailor future campaigns. 167. Comparison of surgical lasers and conventional methods in skin incisions

Abstract

Steel scalpel, electrocautery, CO2 laser used in a continuous wave mode (CW) and rapid superpulse mode (RSP), and contact Nd:YAG laser were tested on pig skin incisions. Speed of incision and histological changes near the wounds were examined. Light microscopical observations were made on postoperative day 0 using standard Van Gieson stain. Width of the scar on postoperative day 14 was also measured. Steel scalpel produced the least pathological changes in the skin, followed by RSP. Electrocautery did not differ significantly from the CO2 lasers in this respect on postoperative day 0. The damage was larger after contact Nd:YAG laser. The situation was essentially similar on postoperative day 14. The width of the scar was narrowest after steel scalpel and widest after contact Nd:YAG laser (p<0.01; Nd:YAG vs. other methods). Electrocautery and the two CO2 lasers produced equal scarring. However, electrocautery was significantly faster than any of the lasers (p<0.001).

Abstract

2017 marks the 25th anniversary of the International Liaison Committee on Resuscitation (ILCOR). ILCOR was formed in 1992 to create a forum for collaboration among principal resuscitation councils worldwide. Since then, ILCOR has established and distinguished itself for its pioneering vision and leadership in resuscitation science. By systematically assessing the evidence for resuscitation standards and guidelines and by identifying national and regional differences, ILCOR reached consensus on international resuscitation guidelines in 2000, and on international science and treatment recommendations in 2005, 2010 and 2015. However, local variation and contextualization of guidelines are evident by subtle differences in regional and national resuscitation guidelines. ILCOR's efforts to date have enhanced international cooperation, and progressively more transparent and systematic collection and analysis of pertinent scientific evidence. Going forward, this sets the stage for ILCOR to pursue its vision to save more lives globally through resuscitation.

Abstract

Background: Cardiopulmonary resuscitation (CPR) improves cardiac arrest survival. Cough CPR, percussion pacing and precordial thump have been reported as alternative CPR techniques. We aimed to summarise in a systematic review the effectiveness of these alternative CPR techniques.Methods: We searched Ovid MEDLINE, EMBASE and the Cochrane Library on 24/08/2020. We included randomised controlled trials, observational studies and case series with five or more patients. Two reviewers independently reviewed title and abstracts to identify studies for full-text review, and reviewed bibliographies and 'related articles' (using PubMed) of full-texts for further eligible studies. We extracted data and performed risk-of-bias assessments on studies included in the systematic review. We summarised data in a narrative synthesis, and used GRADE to assess evidence certainty.Results: We included 23 studies (cough CPR n = 4, percussion pacing n = 4, precordial thump n = 16; one study studied two interventions). Only two (both precordial thump) had a comparator group ('standard' CPR). For all techniques evidence certainty was very low. Available evidence suggests that precordial thump does not improve survival to hospital discharge in out-of-hospital cardiac arrest. The review did not find evidence that cough CPR or percussion pacing improve clinical outcomes following cardiac arrest.Conclusion: Cough CPR, percussion pacing and precordial thump should not be routinely used in established cardiac arrest. In specific inpatient, monitored settings cough CPR (in conscious patients) or percussion pacing may be attempted at the onset of a potential lethal arrhythmia. Abstract

Background: A decision system in the ambulance allowing alternative pathways to alternate healthcare providers has been developed for older patients in Stockholm, Sweden. However, subsequent healthcare resource use resulting from these pathways has not yet been addressed. The aim of this study was therefore to describe patient pathways, healthcare utilisation and costs following ambulance transportation to alternative healthcare providers.Methods: The design of this study was descriptive and observational. Data from a previous RCT, where a decision system in the ambulance enabled alternative healthcare pathways to alternate healthcare providers were linked to register data. The receiving providers were: primary acute care centre or secondary geriatric ward, both located at the same community hospital, or the conventional pathway to the emergency department at an acute hospital. Resource use over 10 days, subsequent to assessment with the decision system, was mapped in terms of healthcare pathways, utilisation and costs for the 98 included cases.Results: Almost 90% were transported to the acute care centre or geriatric ward. The vast majority arriving to the geriatric ward stayed there until the end of follow-up or until discharged, whereas patients conveyed to the acute care centre to a large extent were admitted to hospital. The median patient had 6 hospital days, 2 outpatient visits and costed roughly 4000 euros over the 10-day period. Arrival destination geriatric ward indicated the longest hospital stay and the emergency department the shortest. However, the cost for the 10-day period was lower for cases arriving to the geriatric ward than for those arriving to the emergency department.Conclusions: The findings support the appropriateness of admittance directly to secondary geriatric care for older adults. However, patients conveyed to the acute care centre ought to be studied in more detail with regards to appropriate level of care. [ABSTRACT FROM AUTHOR]

Abstract

2017 marks the 25th anniversary of the International Liaison Committee on Resuscitation (ILCOR). ILCOR was formed in 1992 to create a forum for collaboration among principal resuscitation councils worldwide. Since then, ILCOR has established and distinguished itself for its pioneering vision and leadership in resuscitation science. By systematically assessing the evidence for resuscitation standards and guidelines and by identifying national and regional differences, ILCOR reached consensus on international resuscitation guidelines in 2000, and on international science and treatment recommendations in 2005, 2010 and 2015. However, local variation and contextualization of guidelines are evident by subtle differences in regional and national resuscitation guidelines. ILCOR's efforts to date have enhanced international cooperation, and progressively more transparent and systematic collection and analysis of pertinent scientific evidence. Going forward, this sets the stage for ILCOR to pursue its vision to save more lives globally through resuscitation. [ABSTRACT FROM AUTHOR]
