Abstract

Africa contains more human genetic variation than any other continent, but the majority of the population-scale analyses of the African peoples have focused on just two of the four major linguistic groups, the Niger-Congo and Afro-Asiatic, leaving the Nilo-Saharan and Khoisan populations under-represented. In order to assess genetic variation and signatures of selection within a Nilo-Saharan population and between the Nilo-Saharan and Niger-Congo and Afro-Asiatic, we sequenced 50 genomes from the Nilo-Saharan Lugbara population of North-West Uganda and 250 genomes from 6 previously unsequenced Niger-Congo populations. We compared these data to data from a further 16 Eurasian and African populations including the Gumuz, another putative Nilo-Saharan population from Ethiopia. Of the 21 million variants identified in the Nilo-Saharan population, 3.57 million (17%) were not represented in dbSNP and included predicted non-synonymous mutations with possible phenotypic effects. We found greater genetic differentiation between the Nilo-Saharan Lugbara and Gumuz populations than between any two Afro-Asiatic or Niger-Congo populations. F3 tests showed that Gumuz contributed a genetic component to most Niger-Congo B populations whereas Lugabara did not. We scanned the genomes of the Lugbara for evidence of selective sweeps. We found selective sweeps at four loci (SLC24A5, SNX13, TYRP1, and UVRAG) associated with skin pigmentation, three of which already have been reported to be under selection. These selective sweeps point toward adaptations to the intense UV radiation of the Sahel.

Abstract

The Parc National du Banc d’Arguin in Mauritania hosts the largest concentrations of coastal waterbirds along the East Atlantic Flyway. In spite of this importance, a review of the changes in the numbers of waterbirds in the area is lacking since the first complete count in 1980. Here we analysed the seven complete waterbird counts made since then, and the additional yearly counts made in one subunit (Iwik region) since 2003. We present evidence for changes in the community composition of waterbirds over the past four decades. Total waterbird numbers showed a decrease between 1980 and 2017, with only Great White Pelican Pelecanus onocrotalus showing a significant increase in numbers. Five species showed significant declines: Long-tailed Cormorant Phalacrocorax africanus, Red Knot Calidris canutus, Bar-tailed Godwit Limosa lapponica, Eurasian Curlew Numenius arquata, and Western Marsh Harrier Circus aeruginosus. In the remaining species, the variation in numbers between counts was too large, and the number of complete counts too small, for trends to be detected. The yearly counts at Iwik region also showed sharp decreases in the numbers of Red Knot, Bar-tailed Godwit, and Marsh Harrier, but not of Long-tailed Cormorant and Eurasian Curlew. A multivariate analysis revealed a significant change in species composition over time, which was caused mainly by changes in the species depending on the intertidal mudflats for feeding (generally in decline) vs. the species depending on fish and crustaceans in the sublittoral and offshore zones (often showing increases).

Abstract

The aim of this study was to compare the ultrasound-guided technique with that of anatomical landmarks for the insertion of central catheters tunneled into the internal jugular vein in hemodialysis. This was an observational prospective and randomized single-blind study conducted over a period of 6 months from the 01st October 2019 to the 20th march 2020 at the national hemodialysis center of the Donka National Hospital in Conakry. The study included 50 patients: 25 patients in the “echo guided” group and 25 patients in the “anatomical landmarks” group. We observed an application success rate of 96% in the “echo guided” group versus 84% in the “anatomical landmarks” group (p=0.145). More complications were observed in the “anatomical landmarks” group (12 cases) than in the “echo guided” group (4 cases). The mean number of attempted punctures in the “echo guided” group was 1.96 ± 1.09 while the mean number of puncture attempts in the “anatomical landmarks” group was 3 ± 1.44 (p=0.009). The mean duration of puncture in the “echo guided” group was 3.92 ± 1.29 seconds while it was 10.88 ± 2.79 seconds in the “anatomical landmarks” group (p=0.000). The mean time for catheter placement was 38.20 ± 5.70 minutes in the “echo guided” group versus 51.24 ± 7.65 minutes in the “anatomical landmarks” group (p=0.031). Our study showed that the ultrasound-guided technique was superior to the anatomical landmark technique used for catheterization of the internal jugular vein in hemodialysis. [ABSTRACT FROM AUTHOR]

Abstract

Introduction. Epileptic seizures and epilepsy are part of daily clinical practice in neurology. Yet, the number of false positive diagnoses is surprisingly high. Almost one out of every five patients treated for epilepsy does not really have this diagnosis, which is a high percentage bearing in mind the social and medical consequences that being diagnosed with epilepsy entails. Aims. To summarise the most important diagnostic challenges in epilepsy, to describe possible sources of diagnostic error and to offer advice on how to avoid them. Development. Epilepsy is characterised by a tendency to suffer unprovoked epileptic seizures. The greatest obstacle when it comes to diagnosing a case of epilepsy is the fact that epileptic seizures are transient phenomena that occur relatively infrequently and the physician who must carry out the diagnosis will rarely see them. Moreover, there are other clinical events, such as syncopes or non-epileptic seizures, that may be similar to epileptic seizures in appearance and, consequently, can be mistaken for them. Finally, when interpreting the two most important complementary diagnostic techniques in epileptology, the electroencephalogram and magnetic resonance imaging of the brain, the most common errors must be taken into account in order to prevent mistaken diagnoses. Conclusions. The diagnosis of epilepsy is a challenge and must be based on a detailed and specific medical record. If there are any reasonable doubts, from the outset, about the diagnosis of epilepsy or if the patient does not respond well to the antiepileptic treatment, we recommend referring the patient to a specialised centre to establish a definitive diagnosis.

Abstract

During 15 August through 30 September 2006, ground and aircraft measurements were obtained from a multi-national group of students and scientists in Senegal. Key measurements were aimed at investigating and understanding precipitation processes, thermodynamic and dynamic environmental conditions, cloud, aerosol and microphysical processes and spaceborne sensors (TRMM, CloudSat/Calipso) validation. Ground and aircraft instruments include: ground based polarimetric radar, disdrometer measurements, a course and a high-density rain gauge network, surface chemical measurements, a 10 m flux tower, broadband IR, solar and microwave measurements, rawinsonde and radiosonde measurements, FA-20 dropsonde, in situ microphysics and cloud radar measurements. Highlights during SOP3 include ground and aircraft measurements of squall lines, African Easterly Waves (AEWs), Saharan Air Layer advances into Senegal, and aircraft measurements of AEWs -- including the perturbation that became Hurricane Isaac.

Abstract

In this study, we investigate changes in temperature and precipitation extremes over West and Central Africa (hereafter, WAF domain) as a function of global mean temperature with a focus on the implications of global warming of 1.5 degrees C and 2 degrees C according the Paris Agreement. We applied a scaling approach to capture changes in climate extremes with increase in global mean temperature in several subregions within the WAF domain: Western Sahel, Central Sahel, Eastern Sahel, Guinea Coast and Central Africa including Congo Basin. While there are several uncertainties and large ensemble spread in the projections of temperature and precipitation indices, most models show high-impact changes in climate extremes at subregional scale. At these smaller scales, temperature increases within the WAF domain are projected to be higher than the global mean temperature increase (at 1.5 degrees C and at 2 degrees C) and heat waves are expected to be more frequent and of longer duration. The most intense warming is observed over the drier regions of the Sahel, in the central Sahel and particularly in the eastern Sahel, where the precipitation and the soil moisture anomalies have the highest probability of projected increase at a global warming of 1.5 degrees C. Over the wetter regions of the Guinea Coast and Central Africa, models project a weak change in total precipitation and a decrease of the length of wet spells, while these two regions have the highest increase of heavy rainfall in the WAF domain at a global warming of 1.5 degrees C. Western Sahel is projected by 80% of the models to experience the strongest drying with a significant increase in the length of dry spells and a decrease in the standardized precipitation evapotranspiration index. This study suggests that the 'dry gets drier, wet gets wetter' paradigm is not valid within the WAF domain.

Abstract

In addition to the thousands of deaths due the unprecedented ebola outbreak that stroke West Africa (2014-2016), national health systems in affected countries were deeply challenged impacting a number of diseases control programs. Here we describe the case of Human African Trypanosomiasis (HAT), a deadly neglected tropical disease due to a trypanosome transmitted by tsetse flies for which no vaccine nor chemoprophylaxis exists. Data are presented for the disease focus of Boffa in Guinea where a pilot elimination project combining medical screening and vector control was launched in 2012. During ebola, HAT active screening activities were postponed and passive surveillance also was progressively impaired. However, tsetse control using small insecticide impregnated targets could be maintained. The over two years disruption of screening activities led to a dramatic increase of HAT prevalence, from 0.7% in 2013 (21/2885) to 2% (69/3448) in 2016, reaching epidemic levels (>5%) in some villages. In deep contrast, control levels reached in 2013 (0.1%; 7/6564) were maintained in areas covered with impregnated targets as no cases were found in 2016 (0/799). In Boffa, ebola has thus incidentally provided a unique framework to assess the impact of current HAT control strategies. A first lesson is that the “screen and treat” strategy is fragile as rapid bursts of the disease may occur in case of disruption. A second lesson is that vector control reducing human-tsetse contacts, even implemented alone, is effective in providing a good level of protection against infection. This advocates for a greater attention being paid to the combination of tsetse control together with medical activities in aiming to reach the HAT elimination objective in Africa.
